The Global Strategy for Maternal, Infant and Young Child Feeding (MI- YCF) Strategy developed by WHO/UNICEF in 2002 was to revitalize world attention to the impact of feeding practices on the nutritional status, growth, development, health and survival of infants and children. The data for this case study was collected through key informant interviews, observations and review of Save the Children nutrition programme reports and surveys. This information was then organized to produce a detailed description of the maternal, infant and young child nutrition programme in Habaswein and Wajir South districts or sub-counties. The maternal and infant young child nutrition (MIYCN) programme was launched in Habaswein and Wajir South districts in January 2012. The MIYCN programme followed recommendations of a Knowledge, Practice and Coverage (KPC) survey report in July 2012. To date, the programme has formed 48 Mother-to-mother Support Groups (MTMSGs), which are actively promoting the uptake of the recommended MIYCN practices in the community. MIYCN indicators have been markedly improved between July 2011 and February 2013 when surveys were conducted. The uptake of kitchen gardening has picked up significantly at Meri site and some mothers now have a changed attitude towards unskilled home deliveries and are conducting referrals for skilled births. The marked improvements in the performance of MIYCN indicators between July 2011 and February 2013 inWajir South and Habaswein districts can be partly attributed to the MIYCN programme established in January 2012. However, a randomized community trial is still required to provide conclusive results on the impact of care support groups on maternal, infant and young child feeding in this region.展开更多
There is a long-standing controversy of what triggered the extinction at the Permian-Triassic boundary, the most severe mass extinction in the geologic record, including flood basaltic volcanism and/or bolide impact h...There is a long-standing controversy of what triggered the extinction at the Permian-Triassic boundary, the most severe mass extinction in the geologic record, including flood basaltic volcanism and/or bolide impact hypothesis. In order to clarify various pieces of evidence for the mass extinction event at the Permian-Triassic boundary, some researchers from some laboratories throughout the world have made a comprehensive study on a group of samples from the Meishan area of China. Some fresh core samples from the Permian-Triassic boundary in the Meishan area were analyzed in this study. The results showed that there is no Ir anomaly. Moreover, the PGEs patterns of those samples show obvious differentiation characteristics, that is different from the case encountered in meteorites. So no evidence supports the hypothesis of extraterrestrial impact. In contrast, the PGEs patterns are similar to those of Siberian and Emeishan basalts, which indicates that those PGEs are derived mainly from the basalts, lending a support to the correlation between mass extinction at the Permian-Triassic boundary and flood basaltic volcanism. This study has also confirmed the results for samples from section C prior to the analysis of the samples.展开更多

The regions these city groups lie in usually havewell-developed economy, dense population, and are regional politics and culture centers. Some groups lie in theregions with high level of earthquake activity, such as the Surrounding Capital City Group with the centers of Bei-jing and Tianjin. Once a large earthquake occurs, its influence will spread to very extensive region and its disasterwill be tremendous too. So earthquake resistance and disaster mitigation of city group will be very significant issue.The cities in a group have close distance with each other; they can carry out unified preparation for disaster as onewhole and reduce the heavy load of single city before an earthquake, and have an advantage of prompt mutual-aidafter an earthquake because of close distance. It is especially significant to mitigate the lose of lives. One importantprecondition is that all the cities in one group cannot be exposed to the same level of destroy during one earth-quake. So the division of city group in the region with dense cities distribution shall be very significant to theemergent mutual-aid in early time after a large earthquake. AIM To evaluate the influence of creatinine methodology on the performance of chronic kidney disease(CKD)-Epidemiology Collaboration Group-calculated estimated glomerular filtration rate(CKD-EPI-eGFR) for CKD diagnosi...AIM To evaluate the influence of creatinine methodology on the performance of chronic kidney disease(CKD)-Epidemiology Collaboration Group-calculated estimated glomerular filtration rate(CKD-EPI-eGFR) for CKD diagnosis/staging in a large cohort of diabetic patients. METHODS Fasting blood samples were taken from diabetic patients attending our clinic for their regular annual examination, including laboratory measurement of serum creatinine and eGFR.RESULTS Our results indicated an overall excellent agreement in CKD staging(kappa = 0.918) between the Jaffé serum creatinine-and enzymatic serum creatinine-based CKDEPI-eGFR, with 9% of discordant cases. As compared to the enzymatic creatinine, the majority of discordances(8%) were positive, i.e., associated with the more advanced CKD stage re-classification, whereas only 1% of cases were negatively discordant if Jaffé creatinine was used for eGFR calculation. A minor proportion of the discordant cases(3.5%) were re-classified into clinically relevant CKD stage indicating mildly to moderately decreased kidney function(< 60 m L/min per 1.73 m^2). Significant acute and chronic hyperglycaemia, assessedas plasma glucose and Hb A1 c levels far above the recommended glycaemic goals, was associated with positively discordant cases. Due to a very low frequency, positive discordance is not likely to present a great burden for the health-care providers, while intensified medical care may actually be beneficial for the small number of discordant patients. On the other hand, a very low proportion of negatively discordant cases(1%) at the 60 m L/min per 1.73 m^2 eGFR level indicate a negligible possibility to miss the CKD diagnosis, which could be the most prominent clinical problem affecting patient care, considering high risk of CKD for adverse patient outcomes. CONCLUSION This study indicate that compensated Jaffé creatinine procedure, in spite of the glucose-dependent bias, is not inferior to enzymatic creatinine in CKD diagnosis/staging and therefore may provide a reliable and cost-effective tool for the renal function assessment in diabetic patients.展开更多
