To further investigate the one-dimensional(1D)rheological consolidation mechanism of double-layered soil,the fractional derivative Merchant model(FDMM)and the non-Darcian flow model with the non-Newtonian index are respectively introduced to describe the deformation of viscoelastic soil and the flow of pore water in the process of consolidation.Accordingly,an 1D rheological consolidation equation of double-layered soil is obtained,and its numerical analysis is performed by the implicit finite difference method.In order to verify its validity,the numerical solutions by the present method for some simplified cases are compared with the results in the related literature.Then,the influence of the revelent parameters on the rheological consolidation of double-layered soil are investigated.Numerical results indicate that the parameters of non-Darcian flow and FDMM of the first soil layer greatly influence the consolidation rate of double-layered soil.As the decrease of relative compressibility or the increase of relative permeability between the lower soil and the upper soil,the dissipation rate of excess pore water pressure and the settlement rate of the ground will be accelerated.Increasing the relative thickness of soil layer with high permeability or low compressibility will also accelerate the consolidation rate of double-layered soil. 展开更多

Similar to the method of continuum mechanics, the variation of the price of index futures is viewed to be continuous and regular. According to the characteristic of index futures, a basic equation of price of index futures was established. It is a differential equation, its solution shows that the relation between time and price forms a logarithmic circle. If the time is thought of as the probability of its corresponding price, then such a relation is perfectly coincided with the main assumption of the famous formula of option pricing, based on statistical theory, established by Black and Scholes, winner of 1997 Nobel’ prize on economy. In that formula, the probability of price of basic assets (they stand for index futures here) is assummed to be a logarithmic normal distribution. This agreement shows that the same result may be obtained by two analytic methods with different bases. However, the result, given by assumption by Black_Scholes, is derived from the solution of the differential equation. 展开更多

Olfactory ensheathing cells(OECs)are promising seed cells for nerve regeneration.However,their application is limited by the hypoxic environment usually present at the site of injury.Exosomes derived from human umbilical cord mesenchymal stem cells have the potential to regulate the pathological processes that occur in response to hypoxia.The ability of OECs to migrate is unknown,especially in hypoxic conditions,and the effect of OECs combined with exosomes on peripheral nerve repair is not clear.Better understanding of these issues will enable the potential of OECs for the treatment of nerve injury to be addressed.In this study,OECs were acquired from the olfactory bulb of Sprague Dawley rats.Human umbilical cord mesenchymal stem cell-derived exosomes(0–400μg/mL)were cultured with OECs for 12–48 hours.After culture with 400μg/mL exosomes for 24 hours,the viability and proliferation of OECs were significantly increased.We observed changes to OECs subjected to hypoxia for 24 hours and treatment with exosomes.Exosomes significantly promoted the survival and migration of OECs in hypoxic conditions,and effectively increased brain-derived neurotrophic factor gene expression,protein levels and secretion.Finally,using a 12 mm left sciatic nerve defect rat model,we confirmed that OECs and exosomes can synergistically promote motor and sensory function of the injured sciatic nerve.These findings show that application of OECs and exosomes can promote nerve regeneration and functional recovery.This study was approved by the Institutional Ethical Committee of the Air Force Medical University,China(approval No.IACUC-20181004)on October 7,2018;and collection and use of human umbilical cord specimens was approved by the Ethics Committee of the Linyi People’s Hospital,China(approval No.30054)on May 20,2019. 展开更多

An empiric equation for the calculation of ΔfHθm(g) was worked out. The calculated values of standard formation enthalpy based on this model are excellently consistent with those from experimental for 55 organic compounds. 人工冻土分数阶导数应力-应变指数模型参数确定及验证

人工冻土可看成是理想固体和理想流体以某种比例进行的勾兑,其力学特性既不符合胡克定律,也不遵守牛顿黏性定律,而是遵守介于它们之间的某种关系,分数阶导数能够很好地描述这种勾兑效应。对合肥膨胀土在不同冻结温度下进行单轴压缩试验... 人工冻土可看成是理想固体和理想流体以某种比例进行的勾兑,其力学特性既不符合胡克定律,也不遵守牛顿黏性定律,而是遵守介于它们之间的某种关系,分数阶导数能够很好地描述这种勾兑效应。对合肥膨胀土在不同冻结温度下进行单轴压缩试验,得到冻结温度对应力-应变的影响规律。将分数阶导数引入指数模型,将它改进为人工冻土单轴压缩下应力-应变分数阶指数模型。通过对建立的模型两边取自然对数,得到不同温度下应力-应变线性方程组,求解建立方程组确定出分数阶导数模型参数。为进一步验证所建立模型的适用性,引用一组南京冻结粉质黏土三轴剪切试验,在分数阶系数中考虑围压的影响,将它改进为能考虑围压影响的应力-应变分数阶指数模型。将改进的人工冻土应力-应变分数阶指数模型的计算结果与试验结果进行比较,结果表明:计算结果与试验结果吻合度较高,能准确地预测单轴压缩、三轴压缩剪切应力-应变曲线的变化趋势。改进后的分数阶导数模型参数较少且有明确的物理意义,便于工程应用。当前的模型仅仅适用于应变硬化型,为了能进一步描述应变软化型的力学特性,下一步将在模型中考虑损伤进而建立应变软化型分数阶指数模型。同时,如何在模型中反映冻土的结构性与各向异性对应力-应变的影响也将是下一步的研究内容。 展开更多

基于绿被覆盖率上推的天然草地冠层SPAD值高光谱反演方法

叶绿素是评估草地光合作用能力和生理状况的重要指标。高光谱遥感包含丰富的光谱信息,已经成为无损估算草地叶绿素含量的重要手段。然而,通常获取的草地冠层高光谱数据与实测叶片的叶绿素含量存在尺度不匹配问题,导致高光谱反演叶绿素... 叶绿素是评估草地光合作用能力和生理状况的重要指标。高光谱遥感包含丰富的光谱信息,已经成为无损估算草地叶绿素含量的重要手段。然而,通常获取的草地冠层高光谱数据与实测叶片的叶绿素含量存在尺度不匹配问题,导致高光谱反演叶绿素的精度低。为此,提出基于绿被覆盖率上推的天然草地冠层叶绿素高光谱反演方法。以内蒙古呼伦贝尔典型天然草地为研究对象,利用ASD高光谱仪获取的地面冠层高光谱数据、SPAD叶绿素仪采集的叶片叶绿素相对含量实测值和样方手机数码相片,以绿被覆盖率为媒介,将叶片尺度的叶绿素相对含量实测值上推到冠层尺度。结果表明植被指数与SPAD相关性(-0.74~0.76)普遍高于场平均法上推SPAD(-0.63~0.50)。通过原始冠层光谱反射率、一阶导数光谱和42种常用叶绿素光谱指数构建基于绿被覆盖率上推的天然草地冠层叶绿素高光谱反演模型(SPAD_cover)。单变量的最优草地冠层叶绿素反演模型R^(2)=0.689,RMSE=2.714,RPD=1.752;多元线性逐步回归的最优草地冠层叶绿素反演模型R^(2)=0.833,RMSE=2.019,RPD=2.354。实验表明基于绿被覆盖率将草地叶片叶绿素含量实测值上推至冠层尺度,可以有效提高天然草地冠层叶绿素高光谱反演精度。 展开更多

dNLR和LIPI对靶向药物联合免疫检查点抑制剂治疗晚期肾癌预后的影响

目的探讨衍生的中性粒细胞淋巴细胞比值(dNLR)和肺免疫预后指数(LIPI)对靶向药物联合免疫检查点抑制剂(ICIs)治疗晚期肾癌患者预后的影响。方法回顾性收集2020年3月至2024年2月于新疆医科大学附属肿瘤医院接受靶向药物联合ICIs治疗的65... 目的探讨衍生的中性粒细胞淋巴细胞比值(dNLR)和肺免疫预后指数(LIPI)对靶向药物联合免疫检查点抑制剂(ICIs)治疗晚期肾癌患者预后的影响。方法回顾性收集2020年3月至2024年2月于新疆医科大学附属肿瘤医院接受靶向药物联合ICIs治疗的65例晚期肾癌患者的临床资料。根据治疗前血常规结果计算出dNLR值,采用受试者工作特征曲线(ROC)确定最佳dNLR截断值,根据截断值将患者分为低dNLR水平组(n=36)和高dNLR水平组(n=29)。根据治疗前乳酸脱氢酶(LDH)值和dNLR值算出LIPI评分,将患者分为LIPI优组(n=44)和LIPI中/差组(n=21)。使用Kaplan-Meier方法进行生存分析,并进行Log-Rank检验,采用Cox风险比例回归模型对预后影响因素进行分析。结果dNLR预测晚期肾癌患者预后的最佳截断值为2.57,曲线下面积(AUC)为0.729。低dNLR水平组患者生存率显著高于高dNLR水平组(P<0.05);LIPI优组生存率显著高于LIPI中/差组(P<0.05)。单因素生存分析结果显示,国际转移性肾细胞癌数据库联盟危险分层(IMDC)评分、体重指数(BMI)、dNLR、LDH、LIPI、淋巴结转移与患者预后均具有相关性(P<0.05)。多因素Cox生存分析结果显示,IMDC评分、dNLR、LIPI为患者预后的独立影响因素(HR>1,P<0.05)。结论dNLR水平和LIPI评分与接受靶向药物联合ICIs治疗晚期肾癌患者的预后相关,治疗前高dNLR水平及中差LIPI评分提示晚期肾癌患者的预后较差。 展开更多

炎症衍生指标和胱抑素C对子痫前期的临床意义

目的探讨炎症衍生指标[衍生中性粒细胞/淋巴细胞比值(dNLR)和系统性炎症反应指数(SIRI)]和胱抑素C(Cys C)对子痫前期的预测价值。方法选取2019年1月—2022年11月上海交通大学医学院附属国际和平妇幼保健院子痫前期患者194例(观察组)和... 目的探讨炎症衍生指标[衍生中性粒细胞/淋巴细胞比值(dNLR)和系统性炎症反应指数(SIRI)]和胱抑素C(Cys C)对子痫前期的预测价值。方法选取2019年1月—2022年11月上海交通大学医学院附属国际和平妇幼保健院子痫前期患者194例(观察组)和健康孕妇152名(对照组)。分析dNLR、SIRI、CysC与子痫前期的关系。结果观察组CysC水平和白细胞计数、中性粒细胞计数、单核细胞计数、dNLR、SIRI高于对照组(P<0.01)。观察组和对照组之间淋巴细胞计数、血小板计数、血小板平均体积差异均无统计学意义(P>0.05)。Cys C、dNLR分别与收缩压和舒张压呈正相关(P<0.01)。SIRI与舒张压呈正相关(P<0.05)。Cys C、dNLR、SIRI联合检测预测子痫前期的曲线下面积为0.852[95%可信区间(CI)值为0.812~0.892,P<0.01],敏感性和特异性分别为61.8%和92.7%。Cys C≥0.905 mg·L^(-1)、dNLR≥2.950、SIRI≥2.315均为子痫前期的独立危险因素(P<0.05)。根据CysC、dNLR、SIRI最佳临界值对观察组进行二次分组,各高水平组与各低水平组比较妊娠结局差异均无统计学意义(P>0.05)。结论Cys C、dNLR、SIRI是子痫前期的独立危险因素,且具有较高的敏感性和特异性,对子痫前期的预测和评估有一定价值。 展开更多

目的探讨三酰甘油-葡萄糖指数(triglyceride-glucose index,TyG)、脑源性神经营养因子(brain-derived neurotrophic factor,BDNF)对非糖尿病维持性血液透析(maintenance hemodialysis,MHD)患者的肌肉减少症的诊断价值。方法选择2021年1... 目的探讨三酰甘油-葡萄糖指数(triglyceride-glucose index,TyG)、脑源性神经营养因子(brain-derived neurotrophic factor,BDNF)对非糖尿病维持性血液透析(maintenance hemodialysis,MHD)患者的肌肉减少症的诊断价值。方法选择2021年1月—2023年1月南京中医药大学附属南京医院肾内科收治的MHD患者,根据2019年亚洲肌少症工作组(Asian Working Group for Sarcopenia,AWGS)修订的诊断标准统计肌肉减少症患病情况,检测血清三酰甘油、葡萄糖、BDNF水平,计算TyG。Pearson分析TyG、BDNF与骨骼肌质量指数(skeletal muscle mass index,SMI)、握力、步速的相关性。多因素Logistic回归分析影响MHD患者合并肌肉减少症的因素,受试者工作特征(ROC)曲线TyG、BDNF诊断MHD患者合并肌肉减少症的价值。结果126例MHD患者中39例(30.95%)患者合并肌肉减少症,与非肌肉减少症组比较,肌肉减少症组患者TyG较高、血清BDNF水平偏低(t=7.974、14.699,均P<0.001)。肌肉减少症组患者TyG与SMI、握力、步速呈负相关(r=-0.365、-0.319、-0.452,均P<0.001),BDNF与SMI、握力、步速呈正相关(r=0.326、0.275、0.395,P<0.001、0.003、P<0.001)。年龄偏大(OR=1.861,95%CI:1.040~3.330,P=0.019)、高TyG(OR=2.050,95%CI:1.049~4.008,P=0.015)是MHD患者合并肌肉减少症的危险因素,高白蛋白(OR=0.694,95%CI:0.515~0.935,P=0.008)、高BDNF(OR=0.630,95%CI:0.430~0.923,P=0.010)是MHD患者合并肌肉减少症的保护因素。TyG、BDNF诊断MHD患者合并肌肉减少症的曲线下面积为0.740(95%CI:0.654~0.814,P<0.001)、0.727(95%CI:0.641~0.803,P<0.001),联合TyG、BDNF诊断MHD患者合并肌肉减少症的曲线下面积为0.939(95%CI:0.882~0.974,P<0.001),高于单独诊断(Z=4.075、4.241,P均<0.001)。结论MHD患者TyG值增加,血清BDNF水平降低与合并肌肉减少症有关,联合TyG和BDNF可提高对MHD患者肌肉减少症风险评估价值。 展开更多

Estimation of Daily Global Solar Radiation with Different Sunshine-Based Models for Some Burundian Stations

Sunshine duration (S) based empirical equations have been employed in this study to estimate the daily global solar radiation on a horizontal surface (G) for six meteorological stations in Burundi. Those equations include the Ångström-Prescott linear model and four amongst its derivatives, i.e. logarithmic, exponential, power and quadratic functions. Monthly mean values of daily global solar radiation and sunshine duration data for a period of 20 to 23 years, from the Geographical Institute of Burundi (IGEBU), have been used. For any of the six stations, ten single or double linear regressions have been developed from the above-said five functions, to relate in terms of monthly mean values, the daily clearness index () to each of the next two kinds of relative sunshine duration (RSD): and . In those ratios, G<sub>0</sub>, S<sub>0 </sub>and stand for the extraterrestrial daily solar radiation on a horizontal surface, the day length and the modified day length taking into account the natural site’s horizon, respectively. According to the calculated mean values of the clearness index and the RSD, each station experiences a high number of fairly clear (or partially cloudy) days. Estimated values of the dependent variable (y) in each developed linear regression, have been compared to measured values in terms of the coefficients of correlation (R) and of determination (R<sub>2</sub>), the mean bias error (MBE), the root mean square error (RMSE) and the t-statistics. Mean values of these statistical indicators have been used to rank, according to decreasing performance level, firstly the ten developed equations per station on account of the overall six stations, secondly the six stations on account of the overall ten equations. Nevertheless, the obtained values of those indicators lay in the next ranges for all the developed sixty equations:;;;, with . These results lead to assert that any of the sixty developed linear regressions (and thus equations in terms of and ), fits very adequately measured data, and should be used to estimate monthly average daily global solar radiation with sunshine duration for the relevant station. It is also found that using as RSD, is slightly more advantageous than using for estimating the monthly average daily clearness index, . 目的探究冠状动脉造影微循环阻力指数(coronary angiography-derived index of microcirculatory resistance,caIMR)对急性ST段抬高型心肌梗死(ST-segment elevation myocardial infarction,STEMI)患者行急诊经皮冠状动脉介入(percutane... 目的探究冠状动脉造影微循环阻力指数(coronary angiography-derived index of microcirculatory resistance,caIMR)对急性ST段抬高型心肌梗死(ST-segment elevation myocardial infarction,STEMI)患者行急诊经皮冠状动脉介入(percutaneous coronary intervention,PCI)术后发生主要心血管不良事件(major adverse cardiovascular events,MACE)的预测价值。方法连续纳入2019年9月~2022年3月在徐州医科大学附属医院诊断为STEMI的患者541例。使用FlashAngio系统(苏州润迈德医疗科技有限公司)计算caIMR。按照住院或随访期间MACE发生与否将患者分为MACE组和非MACE组,MACE定义为全因死亡、心力衰竭再入院、非计划性血运重建。采用COX回归分析、受试者工作特征(receiver operating characteristics,ROC)曲线、Kaplan-Meier生存曲线探究caIMR对STEMI患者急诊PCI术后预后的评估价值。结果随访时间为1年,其中发生MACE 61例(11.28%)。与非MACE组比较,MACE组患者有着更高的caIMR。多因素COX分析表明,caIMR是MACE的独立危险因素。ROC曲线分析结果显示,caIMR预测MACE的曲线下面积0.688,最佳截断值为25.3 U。caIMR加入到临床危险因素模型中后显著增加了模型的判别和重新分类能力;进一步将患者分为低caIMR组(caIMR<25 U,n=377)和高caIMR组(caIMR≥25 U,n=164)。Kaplan-Meier曲线显示caIMR≥25 U患者预后更差。结论caIMR是STEMI患者行PCI术后预后不良的独立危险因素,caIMR≥25 U患者预后更差。 展开更多

This paper discusses the existence and multiplicity of positive solutions for a class of singular boundary value problems of Hadamard fractional differential systems involving the p-Laplacian operator. First, for the sake of overcoming the singularity, sequences of approximate solutions to the boundary value problem are obtained by applying the fixed point index theory on the cone. Next, it is demonstrated that these sequences of approximate solutions are uniformly bounded and equicontinuous. The main results are then established through the Ascoli-Arzelà theorem. 【目的】草地光谱反射特征是运用遥感数据研究草地植被理化特性和植被分类的基础,对不同草地植物光谱反射特征开展分析研究,为草地植被的定量遥感与准确识别提供理论参考与技术支持。【方法】采用便携式地物光谱仪,于2021年7-8月在四川... 【目的】草地光谱反射特征是运用遥感数据研究草地植被理化特性和植被分类的基础,对不同草地植物光谱反射特征开展分析研究,为草地植被的定量遥感与准确识别提供理论参考与技术支持。【方法】采用便携式地物光谱仪,于2021年7-8月在四川省甘孜州雅江县对川西北8种高寒草甸常见植物蕨麻(Argentina anserina)、高山嵩草(Kobresia pygmaea)、花葶驴蹄草(Caltha scaposa)、藏蒲公英(Taraxacum tibetanum)、广叶橐吾(Ligularia euryphylla)、淡黄香青(Anaphalis flavescens)、圆穗蓼(Polygonum macrophyllum)、黄花棘豆(Oxytropis ochrocephala)进行了野外光谱测量,利用一阶导数、二阶导数、光谱去包络线以及植被指数法对原始光谱数据进行了处理,揭示了8种高寒草甸植被光谱反射特性。【结果】相对于不同草地植物的绿峰位置、绿峰幅值、红谷位置、红谷幅值、红边幅值、红边面积、吸收峰面积、吸收峰对称度等光谱特征参量,高光谱植被指数更容易区分此8种草地植物,8种植物的NDVI和mSR705指数具有一致性,指数由大到小依次为:藏蒲公英>黄花棘豆>高山嵩草>蕨麻>圆穗蓼>花葶驴蹄草>广叶橐吾>淡黄香青,且各植物mSR705指数值差异明显,易于相互区分。【结论】采用不同方法对草地植物光谱反射特征开展分析,可有效的区分不同的草地植被,为高光谱遥感开展草地资源植被分类、草原基况调查和动态监测等提供参考。 展开更多

目的:探究血常规衍生炎症指标及协同刺激分子B7家族在系统性红斑狼疮患者中的表达。方法:选取2019年6月—2022年10月当阳市人民医院收治的164例系统性红斑狼疮患者及在本院体检的健康志愿者41例。系统性红斑狼疮疾病活动度指数(SLEDAI)... 目的:探究血常规衍生炎症指标及协同刺激分子B7家族在系统性红斑狼疮患者中的表达。方法:选取2019年6月—2022年10月当阳市人民医院收治的164例系统性红斑狼疮患者及在本院体检的健康志愿者41例。系统性红斑狼疮疾病活动度指数(SLEDAI)评分≥15分患者为A组(n=41),SLEDAI评分10~14分患者为B组(n=41),SLEDAI评分5~9分患者为C组(n=41),SLEDAI评分0~4分患者为D组(n=41),体检健康者为E组(n=41)。比较五组的血常规衍生炎症指标[中性粒细胞/淋巴细胞比值(NLR)、血小板/淋巴细胞比值(PLR)、中性粒细胞和单核细胞之和/淋巴细胞比值(NMLR)及衍生的中性粒细胞/淋巴细胞比值(dNLR)]及协同刺激分子B7家族,采用Pearson相关性分析血常规衍生炎症指标与协同刺激分子B7家族的关系。结果:A组、B组、C组及D组的血常规衍生炎症指标均显著高于E组,A组、B组及C组均显著高于D组,A组及B组均显著高于C组,A组均显著高于B组,差异均有统计学意义(P<0.05);A组、B组、C组及D组的协同刺激分子B7家族均显著低于E组,A组、B组及C组均显著均低于D组,A组及B组均显著低于C组,A组均显著低于B组,差异均有统计学意义(P<0.05);Pearson相关性分析显示,系统性红斑狼疮患者NLR、PLR、NMLR、dNLR与协同刺激分子B7家族3(B7-H3)、协同刺激分子B7家族4(B7-H4)均呈负相关(P<0.05)。结论:血常规衍生炎症指标及协同刺激分子B7家族在系统性红斑狼疮患者中的表达显著异常,且上述指标之间有密切的关系。 展开更多

白细胞衍生性指标在儿童肺炎严重程度及预后评估中的应用观察

目的 分析白细胞衍生性指标在儿童肺炎严重程度及预后评估中的应用价值。方法 选取2022年1月至2023年1月本院收治的60例肺炎患儿,根据肺炎严重指数(PSI)评分将其分为低危组(25例)、中危组(21例)和高危组(14例)。采集患儿静脉血,检测3组... 目的 分析白细胞衍生性指标在儿童肺炎严重程度及预后评估中的应用价值。方法 选取2022年1月至2023年1月本院收治的60例肺炎患儿,根据肺炎严重指数(PSI)评分将其分为低危组(25例)、中危组(21例)和高危组(14例)。采集患儿静脉血,检测3组患儿外周血乳酸脱氢酶(LDH)、降钙素原(PCT)、白细胞介素-6(IL-6)水平、C-反应蛋白(CRP)及血常规,计算中性粒细胞与淋巴细胞比值(NLR)、血小板与淋巴细胞比值(PLR)、淋巴细胞与单核细胞比值(LMR)。采用Pearson相关系数分析上述指标与临床肺部感染评分(CPIS)之间的相关性。于出院时评估患儿预后效果,并根据评估结果将患儿分为预后良好组(47例)和预后不良组(13例),比较2组上述指标差异;采用多因素回归分析上述指标与儿童肺炎严重程度相关性;并应用受试者工作特征(ROC)曲线分析LDH、IL-6、CRP、NLR、PLR在儿童肺炎预后评估中的价值。结果 中、高危组LDH、PCT、IL-6、CRP、NLR、PLR、MLR及CPIS评分均高于低危组(P<0.05),高危组LDH、PCT、IL-6、CRP、NLR、PLR、MLR及CPIS评分均高于中危组(P<0.05);肺炎患儿血清LDH、PCT、IL-6、CRP、NLR、PLR、MLR水平与CPIS评分均呈正相关(r=0.625、0.429、0.358、0.413、0.676、0.465、0.645,P<0.05);预后良好组LDH、PCT、IL-6、CRP、NLR、PLR、MLR水平均低于预后不良组(P<0.01);多因素回归分析显示,LDH、IL-6、CRP、NLR以及PLR水平升高是影响儿童肺炎预后的独立危险因素(P<0.05);LDH、IL-6、CRP、NLR、PLR及联合检测在预测肺炎患儿预后方面效能较高,其中LDH预测效能最高。结论 LDH、PCT、IL-6、CRP、NLR、PLR、MLR水平与肺炎患儿病情严重程度呈正相关,LDH、IL-6、CRP、NLR、PLR水平升高是影响肺炎患儿预后的独立危险因素,应早期监测上述指标,以改善患儿预后。 展开更多

目的评估CT血流储备分数(CT-FFR)与冠状动脉周围脂肪衰减指数(FAI)联合临床及冠状动脉CT血管造影(CCTA)特征预测主动脉瓣置换术(AVR)术后发生主要不良心血管事件(MACE)的价值。方法回顾性分析139例接受AVR的主动脉瓣狭窄患者,根据随访... 目的评估CT血流储备分数(CT-FFR)与冠状动脉周围脂肪衰减指数(FAI)联合临床及冠状动脉CT血管造影(CCTA)特征预测主动脉瓣置换术(AVR)术后发生主要不良心血管事件(MACE)的价值。方法回顾性分析139例接受AVR的主动脉瓣狭窄患者,根据随访中是否发生MACE将其分为MACE组与非MACE组;以Cox比例风险回归分析临床、CCTA及冠状动脉CT-FFR、FAI,筛选术后发生MACE的独立预测因素,基于临床、CCTA特征,以及CT-FFR及右冠状动脉(RCA)FAI建立嵌套模型。绘制受试者工作特征(ROC)曲线,计算曲线下面积(AUC)及Harrell C指数,评估各模型诊断效能及其拟合优度。结果MACE组22例、无MACE组117例。CT-FFR(HR=3.683)及RCA-FAI(HR=3.261)均为AVR术后发生MACE的独立预测因素。临床模型、临床+CCTA、临床+CCTA+CT-FFR模型及临床+CCTA+CT-FFR+RCA-FAI模型预测AVR术后MACE的AUC分别为0.636、0.730、0.758及0.817,C指数分别为0.614、0.707、0.733及0.782;其中,临床+CCTA+CT-FFR+RCA-FAI模型预测结果与实际结果的一致性最高、拟合优度最佳。结论CT-FFR及RCA-FAI联合临床及CCTA特征能有效预测AVR术后MACE。 展开更多
