Many people with hypertension have poorly controlled blood pressure and up to half of the adults with hypertension are unaware of their hypertensive status due to factors that bother on poor management and poor screening approaches. The implication is that people who have poor access to healthcare especially those in the rural communities are at increased risk of cardiovascular complications and all-cause mortality. Unfortunately, not much has been done to ascertain the burden of undiagnosed hypertension and associated risk factors in rural communities in Nigeria. Methods: We conducted a community-based cross-sectional study in a rural community in Imo State, Nigeria, on burden of undiagnosed hypertension with participants recruited via a multi-stage sampling method. An interviewer-administered questionnaire was used, and standardized instruments were applied to obtain, process and analyze the data. Tests of association between the independent variables and outcome were conducted using logistic regression. P-value of Results: A total of 380 adults participated in the study. The mean age was 44.2 years. The prevalence of undiagnosed hypertension was 35.8%. Logistic regression revealed that age, with the respondents in the age groups 26 - 35 years (OR = 10.647, 1.910 - 59.345, p-value = 0.007), 36 - 45 (OR = 3.680, 1.263 - 10.723, p-value = 0.017), 46 - 55 years (OR = 2.737, 1.114 - 6.727, p-value = 0.039), 56 - 65 years old (OR = 3.384, 1.610 - 7.115, p-value = 0.001);and being married (OR = 3.846, 1.118 - 13.233, p-value = 0.033), were independent risk factors for undiagnosed hypertension. Conclusion: The prevalence of undiagnosed hypertension in the rural population of South-East Nigeria is high. Younger age (26 - 35 years) had the highest odds of risk for occurrence of hypertension. Also being married was identified as a risk factor for undiagnosed hypertension.展开更多
Hypertension(HTN)is one of the most common chronic diseases affecting over 30%of the adult population globally,with a growing incidence rate.This review assesses the relationship between health literacy(HL)interventio...Hypertension(HTN)is one of the most common chronic diseases affecting over 30%of the adult population globally,with a growing incidence rate.This review assesses the relationship between health literacy(HL)intervention and hypertension(HTN)self-management among people with HTN.The study design was a systematic review of empirical research articles using a well-defined strategy.Online journals were accessed through databases such as PubMed,CINAHL,Google Scholar,ProQuest,Global Health,WHOLIS,Embase,and EbscoHost,spanning from January 2016 to December 2022 as the scope of the study.Articles selected for inclusion were those published in English during the specified time frame and adhering to Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines and Sample,Phenomenon of Interest,Design,Evaluation,Research Type framework for systematic review,focusing on criteria related to the sample,the phenomenon of interest,study design,evaluation,and research type.Out of 180 studies initially identified in the database search,20 studies were ultimately included in the review.The findings were reported based on these five emerging themes:Relationship between HL and HTN self-management;Effect of HL intervention on HTN self-management;Factors predicting self-care behaviors among HTN patients with low HL;Effect of HL and self-management efficacy on health-related quality of life(HRQoL);and level of self-care practices.This review highlights a relationship between HL,self-efficacy,self-care,and HRQoL,underscoring the necessity for further well-designed intervention studies focused on enhancing HL in individuals with HTN in Nigeria to enhance their quality of life.展开更多
Objective:To explore the effects of health education and chronic disease management nursing in elderly community patients with hypertension,in order to provide scientific evidence for improving the health management l...Objective:To explore the effects of health education and chronic disease management nursing in elderly community patients with hypertension,in order to provide scientific evidence for improving the health management level of these patients.Methods:Sixty-four elderly hypertension patients treated at this hospital between March 2022 and March 2024 were selected and randomly divided into two groups,with 32 patients in each group.One group received conventional management,designated as the control group,while the other group received a combined management strategy involving health education and chronic disease management,designated as the experimental group.The study compared the management outcomes of the two groups to evaluate the value of the combined management approach in elderly hypertensive patients in the community.Results:The study found that the experimental group showed significantly lower systolic blood pressure(SBP),diastolic blood pressure(DBP),and scores on the Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS)compared to the control group,with statistically significant differences(P<0.05).Additionally,the experimental group demonstrated significantly higher scores in disease cognition levels regarding awareness of normal blood pressure ranges,prevention of complications,identification of high-risk factors,and healthy lifestyle practices,with statistically significant differences(P<0.05).Moreover,the experimental group showed significantly better rates of self-management behaviors,such as quitting smoking and alcohol,self-monitoring of blood pressure,dietary control,regular medication adherence,and consistent exercise,compared to the control group,with statistically significant differences(P<0.05).Conclusion:This study indicates that a combined management model integrating health education and chronic disease management effectively improves the emotional state of elderly hypertensive patients in the community,significantly enhances their disease cognition levels,and boosts their self-management abilities.Furthermore,this model can effectively lower patients’blood pressure,thereby achieving better health management outcomes for elderly hypertensive patients in the community.展开更多

BACKGROUND Postpartum hypertension poses a considerable health risk.Despite research on gestational hypertension,comprehensive studies focusing on postpartum hypertension in communities are limited.Understanding its p...BACKGROUND Postpartum hypertension poses a considerable health risk.Despite research on gestational hypertension,comprehensive studies focusing on postpartum hypertension in communities are limited.Understanding its prevalence and associated risk factors is crucial for effective prevention and management.AIM To provide insights for postpartum hypertension’s prevention and management.METHODS In total,3297 women who gave birth between June 2021 and December 2022 in Xuhui District,Shanghai were selected.Blood pressure was measured thrice within one month post-delivery during home visits.Eighty-six women with hypertension were followed up for four months to analyze hypertension persistence and its related risk factors.A predictive model for persistent postpartum hypertension was established and verified using the Nomo diagram model.RESULTS Hypertension prevalence 1 month post-delivery was 2.61%(86/3297).Among the 86 pregnant women,32(37.21%)had persistent hypertension at four months post-delivery.Multivariate logistic regression analysis revealed that older age[odds ratio(OR)=1.212;95%confidence interval(CI):1.065–1.380]and higher pre-pregnancy body mass index(BMI)(OR=1.188;95%CI:1.006–1.404)were associated with hypertension(OR=10.781;95%CI:1.006–1.404)during pregnancy.A 95%CI of 1.243–93.480 is a risk factor for persistent postpartum hypertension.The Nomograph model accurately predicted the risk of persistent postpartum hypertension,demonstrating high precision.CONCLUSION In Xuhui,older age,higher pre-pregnancy BMI,and gestational hypertension are risk factors for persistent postpartum hypertension.Our prediction model can identify high-risk individuals,thereby improving patient quality of life.展开更多

Materials and Methods: The study was phenomenological of the factors, carried out in Haut Katanga. All health facilities (FOSAs) having integrated the prevention of transmission of HIV infection from mother to child (PMTCT) were included in the study. The collection was carried out through individual semi-structured interviews with PMTCT focal points, mentor mothers and people living with HIV (PLWHA) cared for in some of these health facilities (FOSAs). The number of participants was determined by the saturation of responses. Nvivo v.11 software was used for the analyses. The data of each woman was handled confidentially. The authorization of the ethics committee of the University of Kinshasa ESP/CE091/2015, the free and written consent, was obtained before collecting the information. Results: It was observed the socio-cultural factors of success and failure of the interventions: the social fear generated by contradictory messages in the sensitization of the community;the lack of an obvious strategy for the involvement of the partner;the weak coordination of community care activities between the central office of the health zone;the FOSA and the community worker: low interest in community care evidenced by weak accountability of FOSAs, community and program providers in this regard;that relates to community activities;coordination of care between care units;overload of the staff in charge of activities within the health structure and their low motivation;the low communication time devoted to people living with HIV in the FOSAs. Conclusion: The study shows that interventions that can improve the quality and outcomes of prevention of mother-to-child transmission of HIV (PMTCT) services can be directly linked to the program itself, as well as come from another or the community, which generally remains the weak link in which efforts are even less noticeable, at least as far as PMTCT is concerned. Integrating care data for the mother-child couple improve the continuum of services between the different care units as well as the quality of data management.展开更多

Methods: A total of 90 patients with gestational hypertension who received antenatal care and gave birth in our hospital between July 2014 and July 2016 were collected and divided into control group and observation group according to random number table, 45 cases in each group. Control group of patients received routine therapy, observation group of patients received routine therapy + nutrition combined with exercise intervention, and the ischemic hypoxic injury index expression in placenta tissue and serum angiogenesis factor levels before delivery were compared between two groups of patients. Results: After intervention, ischemic hypoxic injury index NO level in placental grinding fluid of observation group was higher than that of control group while ET-1, HIF-1α, Bax, Caspase-3 and MDA levels were lower than those of control group;serum angiogenesis factors TGFβ1, HGF, bFGF, VEGF and Ang-2 levels were significantly higher than those of control group while sFlt-1 level was lower than that of control group. Conclusion: The combination of medical nutrition and exercise can effectively reduce the placental ischemic hypoxic injury and reduce the angiogenesis in patients with gestational hypertension.展开更多
Objective:To explore the therapeutic effects of community health management and nursing strategies for elderly hypertensive patients.Methods:A total of 64 elderly hypertensive patients who were treated in our hospital...Objective:To explore the therapeutic effects of community health management and nursing strategies for elderly hypertensive patients.Methods:A total of 64 elderly hypertensive patients who were treated in our hospital from March 2020 to March 2021 were selected.The control group took conventional care and guidance.The research group carried out community health management and nursing strategy guidance on the basis of the control group.Then compare the blood pressure levels of the two groups of patients before and after nursing and the patients’satisfaction with nursing.Results:Through comparison,it can be seen that the diastolic and systolic blood pressure levels of the study group and the control group are not significantly different before nursing.After nursing,the diastolic blood pressure of the patients in the study group was 81.22.1 mmHg and the systolic blood pressure was 126.58.7 mmHg.The diastolic blood pressure of the control group was 90.55.4 mmHg and the systolic blood pressure was 136.412.9 mmHg.There are obvious differences in the comparison of the two sets of data.By comparing the two groups of patients’satisfactions with nursing care,it can be seen that among the 32 patients in the study group:31 were very satisfied and basically satisfied,with a satisfaction rate of 96.87%.Among the 32 patients in the control group,28 were very satisfied and basically satisfied,with a satisfaction rate of 87.5%.The data of the two groups of patients are clearly comparable.Conclusion:Through community health management and nursing strategies,the satisfaction and treatment effect of elderly hypertensive patients can be improved,thereby contributing to the recovery of patients.展开更多

Methods A total of 309 newborns and their families were recruited in Xuhui district, Shanghai. They were asked to fill out the baseline questionnaires. The newborns' intelligence quotients were measured by Developmental Screening Test for Child Under Six, and physical examinations were conducted at the same time. The newborns were randomly assigned to intervention group (156 cases) and control group (153 cases). The infants and their families were followed up every 6 months. A questionnaire of follow-up was completed by face-to-face interview at the communities and infant intelligence quotients were measured and had their physical check-ups again at the end of the 6th month. Results After 6 months' intervention, the means of MI and DQ in the intervention group were respectively 5.96 and 9.80 higher than those in control group after adjustment of the baseline scores. Conclusion Early intelligence education in the community may promote the intelligence development of infants.展开更多
Background:The widely accepted view that portal hypertension(PHT)is a con-traindication to hepatectomy for patients with hepatocellular carcinoma(HCC)is being increasingly challenged.The long-term survival outcomes an...Background:The widely accepted view that portal hypertension(PHT)is a con-traindication to hepatectomy for patients with hepatocellular carcinoma(HCC)is being increasingly challenged.The long-term survival outcomes and safety of partial hepatectomy versus interventional treatment using ablation with or without pre-ablation transarterial chemoembolization(TACE)in patients with HBV-related HCC within the Milan criteria and with clinically significant PHT were compared in this study.Methods:This open-label randomized clinical trial was conducted on consecu-tive patients with clinically PHT and hepatitis B virus(HBV)-related HCC with tumors which were within the Milan criteria.These patients were randomized 1:1 to receive either partial hepatectomy or interventional treatment between December 2012 and June 2018.The primary endpoint was overall survival(OS);secondary endpoints included recurrence-free survival(RFS)and therapeutic safety.Results:Each of the 2 groups had 80 patients.The 1-,3-and 5-year OS rates in the partial hepatectomy group and the interventional treatment group were 95.0%,86.2%,69.5%versus 93.8%,77.5%,64.9%,respectively(P=0.325).The corresponding RFS rates were 78.8%,55.0%,46.2%versus 71.3%,52.5%,45.0%,respectively(P=0.783).The partial hepatectomy group had a higher compli-cation rate compared to the interventional group(67.5%vs.20%,P<0.001).However,the differences were mainly in Clavien-Dindo Grade Ⅰ complications(P<0.001),while not significant in Grade Ⅱ/Ⅲ/Ⅳ/Ⅴ(All P>0.05).Conclusions:This study shows that partial hepatectomy treatment did not meet prespecified significance for improved OS and RFS compared to interventional treatment for patients with HBV-related HCC within the Milan criteria and with clinically significant PHT.However,partial hepatectomy is still a safe procedure and should be considered as a treatment option rather than a contraindication.展开更多

A 65-year-old male with recently diagnosed atrial septal defect was referred to our centre for decompensated right heart failure with rest and exercise induced dispnea and severe pulmonary hypertension. Right heart catheterization confirmed a mean pulmonary pressure of about 55 mmHg and a Qp/Qs of 2.7. An occlusion test with a compliant large balloon demonstrated partial fall of pulmonary arterial pressure. The implantation of a home-made fenestrated Amplatzer ASD Occluder (ASO) was planned in order to decrease left-to-right shunt and promote further decrease of pulmonary arterial pressure in the long-term. Thus, by means of mechanical intracardiac echocardiography study with a 9F 9 MHz Ultralce catheter (Boston Scientific Corp.), we selected a 34 mm ASO for implantation. Four millimeter fenestration was made inflating a 4 mm non-compliant coronary balloon throughout the waist of the ASO, which was successfully implanted under intmcardiac echocardiography. After six months, a decrease of pulmonary arterial pressure to 24 mmHg and full compensated right heart failure was observed on transthoracic echocardiography and clinical examination. This case suggests that Wanscatheter closure with home-made fenestrated ASD in elderly patients with severe pulmonary hypertension is feasible.展开更多

Results: Prevalence of hypertension of rural and urban residents over 18 is 27.44% in Hubei province. There are many differences among prevalence of hypertension of male and female, distribution of BMI of rural and urban residents and prevalence of hypertension and so on, and it has statistical significance (P-value < 0.05 averagely);logistic regressive analysis result shows that different age, gender, education level, dieting habits (high salt and high oil), family per capita monthly income, BMI have statistical significance on the prevalence of hypertension in urban and rural residents of Hubei Province. Conclusions: The prevalence of hypertension in the residents of five cities and prefectures in Hubei Province is on the trend of rising in ladder form and at a much earlier age. The health education, monitoring and intervention of chronic diseases need to be widely carried out, with emphasis on the intervention of the residents’ eating habits of high salt and oil, smoking, drinking and other bad lifestyle.展开更多

Approximately 80% of these NCD-related deaths occur in LMICs. A quasi-experimental study utilizing a non-equivalent pre-and post-test was conducted from May 2022 to March 2023 with 370 study participants. Multistage cluster and random sampling were used to select ten community units, and therefore, 150 CHVs were chosen for the control unit, and 150 were used to form the interventional group. Data was collected from the KOBO app. Six (6) homogenous FGDs comprised ten members, and 10 KII were conducted across study sites. Quantitative data was analyzed using SPSS version 28.0, and qualitative data was audio-recorded, transcribed, and analyzed via N-Vivo 12. The study shows that 59.3% of respondents have minimal information, and 92.7% (n = 139) have no clear understanding of NCDs, with a pre-intervention capacity of 48.8%. Independent sample t-test showed a significant difference in capacity from a pre-intervention average of 48.75 (SD ± 5.7)%, which increased to 68.28 (SD ± 7.6)%, p < 0.001. A well-designed community interventional model plays a pivotal role in grassroots healthcare delivery but requires optimization for NCD management.展开更多

This study aims to evaluate the effectiveness of general practitioners (GP) management for Chinese elderly hypertension, and explore reasonable and effective blood pressure management pattern. Methods: We searched the published literature for randomized controlled trials designed to improve blood pressure with community care management delivered by general practitioners or nurses, compared with usual care. Major outcome measures were systolic and diastolic blood pressure;the percentage of patients whose blood pressures are under control with community care management and Revman 5.3 was used in this study. Results: Pooled data from all 13 researches showed a lower outcome diastolic blood pressure (SMD = −0.95, 95% CI (−1.23, −0.67)) and systolic blood pressure (SMD = −1.17, 95% CI (−1.52, −0.81)) respectively in favor of community management. Pooled data from all 11 researches showed a higher percentage of patients with blood pressure under control in favor of community management (OR = 3.85, 95% CI (1.58, 9.37)) and the difference between treatment group and control group on blood pressure control ratio had statistical significance (Z = 2.97, P Conclusions: General practitioners’ management in Chinese elderly hypertension is effective, and complies with the advanced hypertension management guidelines. BACKGROUND Renal sympathetic denervation(RSD)provides a minimally invasive interventional treatment modality for patients with resistant hypertension.However,the post-operative outcomes remain a key area of investigat...BACKGROUND Renal sympathetic denervation(RSD)provides a minimally invasive interventional treatment modality for patients with resistant hypertension.However,the post-operative outcomes remain a key area of investigation since its earliest clinical trials.AIM To evaluate patient outcomes after RSD intervention among peer-reviewed patient cases.METHODS A systematic review of literature on MEDLINE,Google Scholar,and the Cochrane Database of Systematic Reviews for RSD case studies to assess post-operative hypertension readings and medical management.RESULTS Among 51 RSD cases,the post-operative RSD patients report an apparent reduction with a mean number of 3.1 antihypertensive medications.The mean systolic arterial blood pressure 1 year following RSD was 136.0 mmHg(95%CI:118.7-153.3).CONCLUSION The apparent improvements in office systolic blood pressure after 12 month postoperative RSD can support the therapeutic potential of this intervention for blood pressure reduction.Additional studies which utilized a uniform methodology for blood pressure measurement can further support the findings of this systematic review.展开更多

Methods: A randomized controlled field intervention trial was used in this study, patients with essential hypertension treated in community health services were subjects investigated, and psychological intervention should be carried out based on pharmaceutical services, to evaluate the effect of psychological intervention model based on pharmaceutical care on blood pressure control and overall health improvement in patients with essential hypertension. Results: Before treatment, systolic and diastolic blood pressure had no significant difference between two groups, P > 0.05. After treatment, SBP and DBP in two groups were significantly decreased compared to that before treatment in the same group. After treatment, SBP and DBP in the intervention group were significantly decreased compared with normal group, P 0.05). After treatment, the scores of the physiological health subscale and the total scale of the control group, the scores of each subscale and the total amount of the intervention group were significantly higher than those before the same group, the difference was statistically significant (P Conclusion: Under the background of the new round of medical and health system reforms, this model will help health workers and community residents establish new types of doctor-patient relationship and improve the quality of life of chronic diseases such as hypertension.展开更多
The aim of this paper is to detect, prevent and resolve DRP (drug-related problems) and NOM (negative outcomes associated with medication) in hospitalized patients with DM2 (type 2 diabetes) with HTN (hypertens...The aim of this paper is to detect, prevent and resolve DRP (drug-related problems) and NOM (negative outcomes associated with medication) in hospitalized patients with DM2 (type 2 diabetes) with HTN (hypertension) in a tertiary care clinic. Descriptive cross-sectional interventional study is used. DTM (drug therapy monitoring) was conducted in 73 patients using data obtained from clinical histories and interviews. NOM were detected based on symptoms and laboratory test results. The statistical significance was 0.05. It can be found that 23 DRP were detected, primarily in the category "likelihood of adverse effects" (30.43%) causing NOM in the "non-quantitative safety problem" category. The NOM detected were related to safety (62%), effectiveness (24.5%) and necessity (13.5%). Of the 68.57% of pharmacist interventions accepted, 48.57% were resolved and 20% were not resolved. A simple linear correlation (r = -0.34) analysis indicated a weak association between patient age and severity ofNOM. DTM made it'possible to detect suspected DRP and NOM, which were then prevented or resolved, improving the control of HTN and DM2 and helping ensure better drug therapy outcomes for patients.展开更多
文摘Background: Cardiovascular diseases are the leading cause of death globally and hypertension is a major contributor to this burden. Many people with hypertension have poorly controlled blood pressure and up to half of the adults with hypertension are unaware of their hypertensive status due to factors that bother on poor management and poor screening approaches. The implication is that people who have poor access to healthcare especially those in the rural communities are at increased risk of cardiovascular complications and all-cause mortality. Unfortunately, not much has been done to ascertain the burden of undiagnosed hypertension and associated risk factors in rural communities in Nigeria. Methods: We conducted a community-based cross-sectional study in a rural community in Imo State, Nigeria, on burden of undiagnosed hypertension with participants recruited via a multi-stage sampling method. An interviewer-administered questionnaire was used, and standardized instruments were applied to obtain, process and analyze the data. Tests of association between the independent variables and outcome were conducted using logistic regression. P-value of Results: A total of 380 adults participated in the study. The mean age was 44.2 years. The prevalence of undiagnosed hypertension was 35.8%. Logistic regression revealed that age, with the respondents in the age groups 26 - 35 years (OR = 10.647, 1.910 - 59.345, p-value = 0.007), 36 - 45 (OR = 3.680, 1.263 - 10.723, p-value = 0.017), 46 - 55 years (OR = 2.737, 1.114 - 6.727, p-value = 0.039), 56 - 65 years old (OR = 3.384, 1.610 - 7.115, p-value = 0.001);and being married (OR = 3.846, 1.118 - 13.233, p-value = 0.033), were independent risk factors for undiagnosed hypertension. Conclusion: The prevalence of undiagnosed hypertension in the rural population of South-East Nigeria is high. Younger age (26 - 35 years) had the highest odds of risk for occurrence of hypertension. Also being married was identified as a risk factor for undiagnosed hypertension.
文摘Hypertension(HTN)is one of the most common chronic diseases affecting over 30%of the adult population globally,with a growing incidence rate.This review assesses the relationship between health literacy(HL)intervention and hypertension(HTN)self-management among people with HTN.The study design was a systematic review of empirical research articles using a well-defined strategy.Online journals were accessed through databases such as PubMed,CINAHL,Google Scholar,ProQuest,Global Health,WHOLIS,Embase,and EbscoHost,spanning from January 2016 to December 2022 as the scope of the study.Articles selected for inclusion were those published in English during the specified time frame and adhering to Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines and Sample,Phenomenon of Interest,Design,Evaluation,Research Type framework for systematic review,focusing on criteria related to the sample,the phenomenon of interest,study design,evaluation,and research type.Out of 180 studies initially identified in the database search,20 studies were ultimately included in the review.The findings were reported based on these five emerging themes:Relationship between HL and HTN self-management;Effect of HL intervention on HTN self-management;Factors predicting self-care behaviors among HTN patients with low HL;Effect of HL and self-management efficacy on health-related quality of life(HRQoL);and level of self-care practices.This review highlights a relationship between HL,self-efficacy,self-care,and HRQoL,underscoring the necessity for further well-designed intervention studies focused on enhancing HL in individuals with HTN in Nigeria to enhance their quality of life.

文摘Objective:To explore the effects of health education and chronic disease management nursing in elderly community patients with hypertension,in order to provide scientific evidence for improving the health management level of these patients.Methods:Sixty-four elderly hypertension patients treated at this hospital between March 2022 and March 2024 were selected and randomly divided into two groups,with 32 patients in each group.One group received conventional management,designated as the control group,while the other group received a combined management strategy involving health education and chronic disease management,designated as the experimental group.The study compared the management outcomes of the two groups to evaluate the value of the combined management approach in elderly hypertensive patients in the community.Results:The study found that the experimental group showed significantly lower systolic blood pressure(SBP),diastolic blood pressure(DBP),and scores on the Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS)compared to the control group,with statistically significant differences(P<0.05).Additionally,the experimental group demonstrated significantly higher scores in disease cognition levels regarding awareness of normal blood pressure ranges,prevention of complications,identification of high-risk factors,and healthy lifestyle practices,with statistically significant differences(P<0.05).Moreover,the experimental group showed significantly better rates of self-management behaviors,such as quitting smoking and alcohol,self-monitoring of blood pressure,dietary control,regular medication adherence,and consistent exercise,compared to the control group,with statistically significant differences(P<0.05).Conclusion:This study indicates that a combined management model integrating health education and chronic disease management effectively improves the emotional state of elderly hypertensive patients in the community,significantly enhances their disease cognition levels,and boosts their self-management abilities.Furthermore,this model can effectively lower patients’blood pressure,thereby achieving better health management outcomes for elderly hypertensive patients in the community.
文摘Objective:To explore the effect of community pharmacy services on rational medication use in elderly patients with hypertension and diabetes.Methods:Between November 2022 and December 2023,80 elderly patients with hypertension and diabetes were selected and randomly divided into a control group(routine medication guidance)and an observation group(community pharmacy services),with 40 subjects each.The medication effect scores,blood pressure,blood sugar levels,and quality of life scores before and after intervention were compared between the two groups.Results:Comparison of pharmaceutical knowledge,medication compliance,and safe medication behavior scores showed that the observation group had higher scores as compared to the control group(P<0.05);blood pressure(systolic blood pressure,diastolic blood pressure,heart rate)and blood sugar(fasting blood glucose,2 h postprandial blood glucose,glycated hemoglobin)index levels were compared,and the observation group’s index levels were lower than those of the control group(P<0.05);the scores of physical health,mental health,social relationships,and environment in the observation group were higher than those in the control group(P<0.05).Conclusion:Community pharmacy services improved the rational medication effect of elderly patients with hypertension and diabetes,and improved their blood pressure,blood sugar control levels,and quality of life.
文摘BACKGROUND Postpartum hypertension poses a considerable health risk.Despite research on gestational hypertension,comprehensive studies focusing on postpartum hypertension in communities are limited.Understanding its prevalence and associated risk factors is crucial for effective prevention and management.AIM To provide insights for postpartum hypertension’s prevention and management.METHODS In total,3297 women who gave birth between June 2021 and December 2022 in Xuhui District,Shanghai were selected.Blood pressure was measured thrice within one month post-delivery during home visits.Eighty-six women with hypertension were followed up for four months to analyze hypertension persistence and its related risk factors.A predictive model for persistent postpartum hypertension was established and verified using the Nomo diagram model.RESULTS Hypertension prevalence 1 month post-delivery was 2.61%(86/3297).Among the 86 pregnant women,32(37.21%)had persistent hypertension at four months post-delivery.Multivariate logistic regression analysis revealed that older age[odds ratio(OR)=1.212;95%confidence interval(CI):1.065–1.380]and higher pre-pregnancy body mass index(BMI)(OR=1.188;95%CI:1.006–1.404)were associated with hypertension(OR=10.781;95%CI:1.006–1.404)during pregnancy.A 95%CI of 1.243–93.480 is a risk factor for persistent postpartum hypertension.The Nomograph model accurately predicted the risk of persistent postpartum hypertension,demonstrating high precision.CONCLUSION In Xuhui,older age,higher pre-pregnancy BMI,and gestational hypertension are risk factors for persistent postpartum hypertension.Our prediction model can identify high-risk individuals,thereby improving patient quality of life.
文摘The objective of the study was to identify the failure factors of community interventions in terms of HIV activities in the province of Haut-Katanga during the year 2016. Materials and Methods: The study was phenomenological of the factors, carried out in Haut Katanga. All health facilities (FOSAs) having integrated the prevention of transmission of HIV infection from mother to child (PMTCT) were included in the study. The collection was carried out through individual semi-structured interviews with PMTCT focal points, mentor mothers and people living with HIV (PLWHA) cared for in some of these health facilities (FOSAs). The number of participants was determined by the saturation of responses. Nvivo v.11 software was used for the analyses. The data of each woman was handled confidentially. The authorization of the ethics committee of the University of Kinshasa ESP/CE091/2015, the free and written consent, was obtained before collecting the information. Results: It was observed the socio-cultural factors of success and failure of the interventions: the social fear generated by contradictory messages in the sensitization of the community;the lack of an obvious strategy for the involvement of the partner;the weak coordination of community care activities between the central office of the health zone;the FOSA and the community worker: low interest in community care evidenced by weak accountability of FOSAs, community and program providers in this regard;that relates to community activities;coordination of care between care units;overload of the staff in charge of activities within the health structure and their low motivation;the low communication time devoted to people living with HIV in the FOSAs. Conclusion: The study shows that interventions that can improve the quality and outcomes of prevention of mother-to-child transmission of HIV (PMTCT) services can be directly linked to the program itself, as well as come from another or the community, which generally remains the weak link in which efforts are even less noticeable, at least as far as PMTCT is concerned. Integrating care data for the mother-child couple improve the continuum of services between the different care units as well as the quality of data management.
文摘Objective: To discuss the effect of medical nutrition combined with exercise intervention on the placental ischemic hypoxic injury and serum angiogenesis factors in patients with gestational hypertension. Methods: A total of 90 patients with gestational hypertension who received antenatal care and gave birth in our hospital between July 2014 and July 2016 were collected and divided into control group and observation group according to random number table, 45 cases in each group. Control group of patients received routine therapy, observation group of patients received routine therapy + nutrition combined with exercise intervention, and the ischemic hypoxic injury index expression in placenta tissue and serum angiogenesis factor levels before delivery were compared between two groups of patients. Results: After intervention, ischemic hypoxic injury index NO level in placental grinding fluid of observation group was higher than that of control group while ET-1, HIF-1α, Bax, Caspase-3 and MDA levels were lower than those of control group;serum angiogenesis factors TGFβ1, HGF, bFGF, VEGF and Ang-2 levels were significantly higher than those of control group while sFlt-1 level was lower than that of control group. Conclusion: The combination of medical nutrition and exercise can effectively reduce the placental ischemic hypoxic injury and reduce the angiogenesis in patients with gestational hypertension.
文摘Objective:To explore the therapeutic effects of community health management and nursing strategies for elderly hypertensive patients.Methods:A total of 64 elderly hypertensive patients who were treated in our hospital from March 2020 to March 2021 were selected.The control group took conventional care and guidance.The research group carried out community health management and nursing strategy guidance on the basis of the control group.Then compare the blood pressure levels of the two groups of patients before and after nursing and the patients’satisfaction with nursing.Results:Through comparison,it can be seen that the diastolic and systolic blood pressure levels of the study group and the control group are not significantly different before nursing.After nursing,the diastolic blood pressure of the patients in the study group was 81.22.1 mmHg and the systolic blood pressure was 126.58.7 mmHg.The diastolic blood pressure of the control group was 90.55.4 mmHg and the systolic blood pressure was 136.412.9 mmHg.There are obvious differences in the comparison of the two sets of data.By comparing the two groups of patients’satisfactions with nursing care,it can be seen that among the 32 patients in the study group:31 were very satisfied and basically satisfied,with a satisfaction rate of 96.87%.Among the 32 patients in the control group,28 were very satisfied and basically satisfied,with a satisfaction rate of 87.5%.The data of the two groups of patients are clearly comparable.Conclusion:Through community health management and nursing strategies,the satisfaction and treatment effect of elderly hypertensive patients can be improved,thereby contributing to the recovery of patients.

文摘Objective To assess the interventions effect on intelligence of the infants through the community. Methods A total of 309 newborns and their families were recruited in Xuhui district, Shanghai. They were asked to fill out the baseline questionnaires. The newborns' intelligence quotients were measured by Developmental Screening Test for Child Under Six, and physical examinations were conducted at the same time. The newborns were randomly assigned to intervention group (156 cases) and control group (153 cases). The infants and their families were followed up every 6 months. A questionnaire of follow-up was completed by face-to-face interview at the communities and infant intelligence quotients were measured and had their physical check-ups again at the end of the 6th month. Results After 6 months' intervention, the means of MI and DQ in the intervention group were respectively 5.96 and 9.80 higher than those in control group after adjustment of the baseline scores. Conclusion Early intelligence education in the community may promote the intelligence development of infants.

文摘Background:The widely accepted view that portal hypertension(PHT)is a con-traindication to hepatectomy for patients with hepatocellular carcinoma(HCC)is being increasingly challenged.The long-term survival outcomes and safety of partial hepatectomy versus interventional treatment using ablation with or without pre-ablation transarterial chemoembolization(TACE)in patients with HBV-related HCC within the Milan criteria and with clinically significant PHT were compared in this study.Methods:This open-label randomized clinical trial was conducted on consecu-tive patients with clinically PHT and hepatitis B virus(HBV)-related HCC with tumors which were within the Milan criteria.These patients were randomized 1:1 to receive either partial hepatectomy or interventional treatment between December 2012 and June 2018.The primary endpoint was overall survival(OS);secondary endpoints included recurrence-free survival(RFS)and therapeutic safety.Results:Each of the 2 groups had 80 patients.The 1-,3-and 5-year OS rates in the partial hepatectomy group and the interventional treatment group were 95.0%,86.2%,69.5%versus 93.8%,77.5%,64.9%,respectively(P=0.325).The corresponding RFS rates were 78.8%,55.0%,46.2%versus 71.3%,52.5%,45.0%,respectively(P=0.783).The partial hepatectomy group had a higher compli-cation rate compared to the interventional group(67.5%vs.20%,P<0.001).However,the differences were mainly in Clavien-Dindo Grade Ⅰ complications(P<0.001),while not significant in Grade Ⅱ/Ⅲ/Ⅳ/Ⅴ(All P>0.05).Conclusions:This study shows that partial hepatectomy treatment did not meet prespecified significance for improved OS and RFS compared to interventional treatment for patients with HBV-related HCC within the Milan criteria and with clinically significant PHT.However,partial hepatectomy is still a safe procedure and should be considered as a treatment option rather than a contraindication.
文摘The standardized hypertension management provided by primary health care workers is an important part of China's recent health care reform efforts. Investigating 5,116 hypertensive patients from a cross-sectional survey conducted by the Chinese Center for Disease Control and Prevention in 2012, this study found that adherence to standardized hypertension management is associated with positive effects on hypertension- related knowledge, healthy lifestyle behavior, antihypertensive medical treatments, and blood pressure control. It will be necessary to provide primary health care workers with sufficient training and reasonable incentives to ensure the implementation and effectiveness of hypertension management.
文摘We report the management of a patient with secundum atrial septal defect (ASD) and severe pulmonary hypertension. A 65-year-old male with recently diagnosed atrial septal defect was referred to our centre for decompensated right heart failure with rest and exercise induced dispnea and severe pulmonary hypertension. Right heart catheterization confirmed a mean pulmonary pressure of about 55 mmHg and a Qp/Qs of 2.7. An occlusion test with a compliant large balloon demonstrated partial fall of pulmonary arterial pressure. The implantation of a home-made fenestrated Amplatzer ASD Occluder (ASO) was planned in order to decrease left-to-right shunt and promote further decrease of pulmonary arterial pressure in the long-term. Thus, by means of mechanical intracardiac echocardiography study with a 9F 9 MHz Ultralce catheter (Boston Scientific Corp.), we selected a 34 mm ASO for implantation. Four millimeter fenestration was made inflating a 4 mm non-compliant coronary balloon throughout the waist of the ASO, which was successfully implanted under intmcardiac echocardiography. After six months, a decrease of pulmonary arterial pressure to 24 mmHg and full compensated right heart failure was observed on transthoracic echocardiography and clinical examination. This case suggests that Wanscatheter closure with home-made fenestrated ASD in elderly patients with severe pulmonary hypertension is feasible.
文摘Objective: To know about the current situation of hypertension in some areas of Hubei Province and analyze the influencing factors. Methods: According to the principle of Stratified sampling, we conducted health examination and questionnaire survey for 1500 residents over the age of 18 at observation point of chronic noninfectious diseases at 10 sub-districts (towns) of Wuhan City, Jingzhou City, Huanggang City, Shiyan City, which used descriptive statistics and logistics to regressively analyze Current situation of hypertension and its influencing factors of residents. Results: Prevalence of hypertension of rural and urban residents over 18 is 27.44% in Hubei province. There are many differences among prevalence of hypertension of male and female, distribution of BMI of rural and urban residents and prevalence of hypertension and so on, and it has statistical significance (P-value < 0.05 averagely);logistic regressive analysis result shows that different age, gender, education level, dieting habits (high salt and high oil), family per capita monthly income, BMI have statistical significance on the prevalence of hypertension in urban and rural residents of Hubei Province. Conclusions: The prevalence of hypertension in the residents of five cities and prefectures in Hubei Province is on the trend of rising in ladder form and at a much earlier age. The health education, monitoring and intervention of chronic diseases need to be widely carried out, with emphasis on the intervention of the residents’ eating habits of high salt and oil, smoking, drinking and other bad lifestyle.
文摘Non-communicable diseases (NCDs) account for 63% of mortalities. Approximately 80% of these NCD-related deaths occur in LMICs. A quasi-experimental study utilizing a non-equivalent pre-and post-test was conducted from May 2022 to March 2023 with 370 study participants. Multistage cluster and random sampling were used to select ten community units, and therefore, 150 CHVs were chosen for the control unit, and 150 were used to form the interventional group. Data was collected from the KOBO app. Six (6) homogenous FGDs comprised ten members, and 10 KII were conducted across study sites. Quantitative data was analyzed using SPSS version 28.0, and qualitative data was audio-recorded, transcribed, and analyzed via N-Vivo 12. The study shows that 59.3% of respondents have minimal information, and 92.7% (n = 139) have no clear understanding of NCDs, with a pre-intervention capacity of 48.8%. Independent sample t-test showed a significant difference in capacity from a pre-intervention average of 48.75 (SD ± 5.7)%, which increased to 68.28 (SD ± 7.6)%, p < 0.001. A well-designed community interventional model plays a pivotal role in grassroots healthcare delivery but requires optimization for NCD management.

文摘Objective:To analyze and provide a comprehensive overview of the knowledge structure and research hotspots of clinical interventions for falls in elderly patients in the community.Methods:The search for publications related to clinical interventions for falls in elderly patients in the community from 2002 to 2022 was conducted on the Web of Science Core Collection(WoSCC)database.VOSviewers,CiteSpace,and the R package“bibliometrix”were used to conduct this bibliometric analysis.Results:2091 articles from 70 countries,primarily the United States and Australia,were included.The number of publications related to clinical interventions for falls in elderly patients is increasing yearly.The main research institutions in this field were the University of Sydney,Harvard University,and the University of California.BioMed Central(BMC)Geriatrics was the most popular journal in this field and Journals of the American Geriatrics Society was the most co-cited journal.These publications came from 8984 authors among which author Lord SR had published the most papers and author Tinetti Me had the most co-citations.The main keywords in this research field were“balance,”“exercise,”and“risk factor.”Conclusion:This was the first bibliometric study that comprehensively summarized the research hot spots and development of clinical interventions for falls in elderly patients in the community.This paper aims to provide a reference for scholars and researchers in this particular field.
文摘Background: Among Chinese adults aged 35 - 75 years, nearly half have hypertension and the prevalence of hypertension is increased with age. But older patients with high systolic pressure and high pulse pressure could not be cured, and will have greater cardiovascular morbidity. This study aims to evaluate the effectiveness of general practitioners (GP) management for Chinese elderly hypertension, and explore reasonable and effective blood pressure management pattern. Methods: We searched the published literature for randomized controlled trials designed to improve blood pressure with community care management delivered by general practitioners or nurses, compared with usual care. Major outcome measures were systolic and diastolic blood pressure;the percentage of patients whose blood pressures are under control with community care management and Revman 5.3 was used in this study. Results: Pooled data from all 13 researches showed a lower outcome diastolic blood pressure (SMD = −0.95, 95% CI (−1.23, −0.67)) and systolic blood pressure (SMD = −1.17, 95% CI (−1.52, −0.81)) respectively in favor of community management. Pooled data from all 11 researches showed a higher percentage of patients with blood pressure under control in favor of community management (OR = 3.85, 95% CI (1.58, 9.37)) and the difference between treatment group and control group on blood pressure control ratio had statistical significance (Z = 2.97, P Conclusions: General practitioners’ management in Chinese elderly hypertension is effective, and complies with the advanced hypertension management guidelines. The multi-patterns are supposed to adopt community blood pressure monitoring and team-based general practitioners.
文摘BACKGROUND Renal sympathetic denervation(RSD)provides a minimally invasive interventional treatment modality for patients with resistant hypertension.However,the post-operative outcomes remain a key area of investigation since its earliest clinical trials.AIM To evaluate patient outcomes after RSD intervention among peer-reviewed patient cases.METHODS A systematic review of literature on MEDLINE,Google Scholar,and the Cochrane Database of Systematic Reviews for RSD case studies to assess post-operative hypertension readings and medical management.RESULTS Among 51 RSD cases,the post-operative RSD patients report an apparent reduction with a mean number of 3.1 antihypertensive medications.The mean systolic arterial blood pressure 1 year following RSD was 136.0 mmHg(95%CI:118.7-153.3).CONCLUSION The apparent improvements in office systolic blood pressure after 12 month postoperative RSD can support the therapeutic potential of this intervention for blood pressure reduction.Additional studies which utilized a uniform methodology for blood pressure measurement can further support the findings of this systematic review.
文摘Objective: To investigate the treatment effect of essential hypertension based on health education. Methods: A randomized controlled field intervention trial was used in this study, patients with essential hypertension treated in community health services were subjects investigated, and psychological intervention should be carried out based on pharmaceutical services, to evaluate the effect of psychological intervention model based on pharmaceutical care on blood pressure control and overall health improvement in patients with essential hypertension. Results: Before treatment, systolic and diastolic blood pressure had no significant difference between two groups, P > 0.05. After treatment, SBP and DBP in two groups were significantly decreased compared to that before treatment in the same group. After treatment, SBP and DBP in the intervention group were significantly decreased compared with normal group, P 0.05). After treatment, the scores of the physiological health subscale and the total scale of the control group, the scores of each subscale and the total amount of the intervention group were significantly higher than those before the same group, the difference was statistically significant (P Conclusion: Under the background of the new round of medical and health system reforms, this model will help health workers and community residents establish new types of doctor-patient relationship and improve the quality of life of chronic diseases such as hypertension.
文摘The aim of this paper is to detect, prevent and resolve DRP (drug-related problems) and NOM (negative outcomes associated with medication) in hospitalized patients with DM2 (type 2 diabetes) with HTN (hypertension) in a tertiary care clinic. Descriptive cross-sectional interventional study is used. DTM (drug therapy monitoring) was conducted in 73 patients using data obtained from clinical histories and interviews. NOM were detected based on symptoms and laboratory test results. The statistical significance was 0.05. It can be found that 23 DRP were detected, primarily in the category "likelihood of adverse effects" (30.43%) causing NOM in the "non-quantitative safety problem" category. The NOM detected were related to safety (62%), effectiveness (24.5%) and necessity (13.5%). Of the 68.57% of pharmacist interventions accepted, 48.57% were resolved and 20% were not resolved. A simple linear correlation (r = -0.34) analysis indicated a weak association between patient age and severity ofNOM. DTM made it'possible to detect suspected DRP and NOM, which were then prevented or resolved, improving the control of HTN and DM2 and helping ensure better drug therapy outcomes for patients.
