The results show that: the lightning and thunderstorm showed great regional differences in the spring of 2013 in Sichuan Province and the thunderstorm activity period was not the same in different areas. Long term outcomes for patients treated with half-fluence photodynamic therapy for chronic central serous chorioretinopathy: a case series 被引量:3

A case series was used to evaluate the efficacy of halffluence photodynamic therapy(PDT) for chronic central serous chorioretinopathy(CSCR). Patients were treated with standard-dose verteporfin and half-fluence PDT. Totally 13 eyes from 11 patients were included. The mean patient age was 52.0 y. There was a mean reduction in central retinal thickness of 107.0 microns. Totally 7/13 eyes(53.8%) achieved resolution of subretinal fluid(SRF) on optical coherence tomography(OCT) scan after 1 treatment with PDT. Four eyes had further treatment with PDT; of these 1 eye achieved resolution of SRF. Seven of the 13 eyes(53.8%) achieved an improvement of more than 5 ETDRS letters. One patient experienced acute macula oedema 1 d post PDT treatment. These results support the hypothesis that half-fluence PDT can have a positive effect in chronic CSCR for a gain in visual acuity and reduction in sub-retinal fluid. Acute macula oedema is a rare but potential adverse effect of half-fluence PDT. 展开更多

An Algorithm for the Reconstruction of Entrance Beam Fluence from Virtual Patient Exit Electronic Portal Images

The problem of determining the in vivo dosimetry for patients undergoing radiation treatment has been an area of interest since the development of the field. More recent methods of measurement employ Electronic Portal... The problem of determining the in vivo dosimetry for patients undergoing radiation treatment has been an area of interest since the development of the field. More recent methods of measurement employ Electronic Portal Image Devices (EPID), or dosimeter arrays, for entrance or exit fluence determination. The more recent methods of in vivo dosimetry make use of detector arrays and reconstruction techniques to determine dose throughout the patient volume. One method uses an array of ion chambers located upstream of the patient. This requires a special hardware device and places an additional attenuator in the beam path, which may not be desirable. An alternative to this approach is to use the existing EPID, which is part of most modern linear accelerators, to image the patient using the treatment beam. Methods exist to deconvolve the detector function of the EPID using a series of weighted exponentials [1]. Additionally, this method has been extended to the deconvolution of the patient scatter in order to determine in vivo dosimetry. The method developed here intends to use EPID images and an iterative deconvolution algorithm to reconstruct the impinging primary fluence on the patient. This primary fluence may then be employed, using treatment time volumetric imaging, to determine dose through the entire patient volume. Presented in this paper is the initial discussion of the algorithm, and a theoretical evaluation of its efficacy using montecarlo derived virtual fluence measurements. The results presented here indicate an agreement of 1% dose difference within 95% the field area receiving 10% of the entrance fluence for a set of sample highly modulated fields. These results warrant continued investigation in applying this algorithm to clinical patient treatments. 展开更多

An Automatic Approach for Satisfying Dose-Volume Constraints in Linear Fluence Map Optimization for IMPT

Prescriptions for radiation therapy are given in terms of dose-volume constraints (DVCs). Solving the fluence map optimization (FMO) problem while satisfying DVCs often requires a tedious trial-and-error for selecting appropriate dose control parameters on various organs. In this paper, we propose an iterative approach to satisfy DVCs using a multi-objective linear programming (LP) model for solving beamlet intensities. This algorithm, starting from arbitrary initial parameter values, gradually updates the values through an iterative solution process toward optimal solution. This method finds appropriate parameter values through the trade-off between OAR sparing and target coverage to improve the solution. We compared the plan quality and the satisfaction of the DVCs by the proposed algorithm with two nonlinear approaches: a nonlinear FMO model solved by using the L-BFGS algorithm and another approach solved by a commercial treatment planning system (Eclipse 8.9). We retrospectively selected from our institutional database five patients with lung cancer and one patient with prostate cancer for this study. Numerical results show that our approach successfully improved target coverage to meet the DVCs, while trying to keep corresponding OAR DVCs satisfied. The LBFGS algorithm for solving the nonlinear FMO model successfully satisfied the DVCs in three out of five test cases. However, there is no recourse in the nonlinear FMO model for correcting unsatisfied DVCs other than manually changing some parameter values through trial and error to derive a solution that more closely meets the DVC requirements. The LP-based heuristic algorithm outperformed the current treatment planning system in terms of DVC satisfaction. A major strength of the LP-based heuristic approach is that it is not sensitive to the starting condition. 展开更多

AIM:To compare the efficacy of low-fluence photodynamic therapy(PDT) combinations in the treatment of age-related macular degeneration(AMD).· METHODS:Forty-five previously untreated eyes of 45 patients with exudative AMD whose best-corrected visual acuity(BCVA) was ≥0.3(Snellen) were enrolled.15 patients in Group I underwent low-fluence PDT(25J/cm2-300mW/cm2-83sec) and intravitreal pegaptanib combination,15 patients in Group II underwent PDT(50J/cm2-600mW/cm2-83sec) and intravitreal pegaptanib combination while,15 patients in Group III underwent intravitreal pegaptanib monotherapy.Complete ophthalmologic examinations were performed in pre and post treatment visits,and the results were statistically analised.A clinical activity score(CAS) was calculated by using changes in lesion size,amount of hemorrhage,staining pattern in FA and OCT measurement of intra/subretinal fluid.≤3 logMAR lines of decrease in BCVA and decrease in CAS were considered as successful treatment.· RESULTS:The mean age of 19 female(42.2%) and 26 male(57.8%) patients was(72.82±8.02) years.Mean follow-up was(13.93±5.87) months.Lesion type was occult in 28 eyes(62.2%).Treatment success rates according to BCVA assessments were 86.7%,80%,60% and mean BCVA decrease were 0.3,1.0,2.2 logMAR lines in Group I,II and III,respectively(P >0.05).According to the changes in central macular thickness and CAS,no difference was found among the study groups(P =0.850 and P =0.811,respectively).Patients treated with combination regimens had lower intravitreal injection frequencies(P =0.015).· CONCLUSION:Combination regimen with intravitreal pegaptanib and low-fluence PDT seems to be safe and effective in stabilizing the clinical activity and BCVA in exudative AMD.· 展开更多

第三代SiC半导体探测器具有体积小、响应时间快、中子/伽马(n/γ)甄别容易等优点,广泛应用于反应堆堆芯剂量监测。本文针对自研的第三代SiC半导体探测器,采用电子束蒸发真空镀膜的技术将中子转换层材料^(6)LiF(^(6)Li丰度为95%)喷镀到Si... 第三代SiC半导体探测器具有体积小、响应时间快、中子/伽马(n/γ)甄别容易等优点,广泛应用于反应堆堆芯剂量监测。本文针对自研的第三代SiC半导体探测器,采用电子束蒸发真空镀膜的技术将中子转换层材料^(6)LiF(^(6)Li丰度为95%)喷镀到SiC基底上,厚度为25μm,实现了中子转换层厚度优化。利用^(241)Amα放射源(活度9.37×10^(3)Bq)开展α粒子响应信号幅度的测量,并在^(137)Csγ放射源(活度6.23×10^(7)Bq)环境下开展γ射线的响应测试。另外,在标准辐射场系统中进行了SiC探测器的中子注量率响应线性度测量、γ剂量率响应线性度测量以及中子注量率响应线性标定。结果表明:该探测器在1×10^(3)~1×10^(6)cm^(−2)·s^(−1)中子注量率范围内线性响应拟合R^(2)=0.9969,具有良好的线性响应,n/γ剂量响应范围为0.005~20 Gy∙h^(−1),可用于核电现场反应堆中子和γ剂量的实时、精确监测。 展开更多

辐照诱发电荷耦合器件(CCD)图像传感器性能退化与CCD片上放大器中的金属氧化物半导体(MOS)管性能退化密切相关。本文以国产CCD片上放大器中的MOS管为研究对象,开展了3种不同注量的质子辐照实验,测试了辐照前后性能参数的退化规律。采用S... 辐照诱发电荷耦合器件(CCD)图像传感器性能退化与CCD片上放大器中的金属氧化物半导体(MOS)管性能退化密切相关。本文以国产CCD片上放大器中的MOS管为研究对象,开展了3种不同注量的质子辐照实验,测试了辐照前后性能参数的退化规律。采用SENTAURUS TCAD软件建立了CCD片上放大器中MOS管的工艺模型,在SiO2层引入固定正电荷,在Si/SiO2界面处引入界面态进行了辐射效应仿真模拟,仿真结果与辐照实验测试结果吻合较好。结合辐照实验和仿真模拟结果,对CCD片上放大器中MOS管的质子辐照损伤效应进行了深入分析,结果表明:阈值电压与饱和输出电流等辐照特性参数的变化主要由辐射感生氧化层陷阱电荷和界面态导致,辐照感生陷阱电荷量与质子辐照注量大小相关,辐照注量越大特性参数变化越大。最后开展了室温退火测试,测试结果表明:随着退火时间的延长,阈值电压与饱和电流出现一定程度的恢复。 展开更多

针对高层建筑深基坑支护结构位移监测时,位移轨迹、位移速率以及位移时间变化监测准确性较差问题,本文研究了高层建筑深基坑支护结构位移动态监测方法。对支护结构位移影响因素进行具体分析,生成了影响指标;再对基坑数据进行采集,建立... 针对高层建筑深基坑支护结构位移监测时,位移轨迹、位移速率以及位移时间变化监测准确性较差问题,本文研究了高层建筑深基坑支护结构位移动态监测方法。对支护结构位移影响因素进行具体分析,生成了影响指标;再对基坑数据进行采集,建立数据集并进行降维处理;计算获取目标函数,结合影响指标建立时间序列模型,依据对模型的计算建立动态变量矩阵;通过对矩阵的计算获取动态监测数据的统计量,完成支护结构的动态监测。研究结果表明:运用该方法进行监测时,位移移动轨迹监测误差为0.1,位移速率保持在0.9 mm/d以下,且与实际位移速率基本一致,纵向位移量达到202 mm,且与实际沉降量一致。本文方法能够有效应用于高层建筑深基坑支护结构的位移动态监测,为保障高层建筑的稳定性和安全性提供重要的技术支持。 展开更多

目的目的探讨磷脂酰肌醇-4,5-二磷酸3-激酶催化亚单位β(PIK3CB)蛋白在结直肠癌组织中的表达及预后影响因素分析。方法方法选取153例结直肠癌患者,取结直肠癌组织及相应癌旁组织,采用免疫组化法检测PIK3CB蛋白表达情况,比较不同临床特... 目的目的探讨磷脂酰肌醇-4,5-二磷酸3-激酶催化亚单位β(PIK3CB)蛋白在结直肠癌组织中的表达及预后影响因素分析。方法方法选取153例结直肠癌患者,取结直肠癌组织及相应癌旁组织,采用免疫组化法检测PIK3CB蛋白表达情况,比较不同临床特征结直肠癌患者结直肠癌组织中PIK3CB蛋白的表达情况。结直肠癌患者预后的影响因素采用二元Logistic回归分析。结果结果结直肠癌组织中PIK3CB蛋白阳性表达率为75.16%,明显高于癌旁组织的26.14%,差异有统计学意义(P﹤0.01)。分化程度为低中分化、TNM分期为Ⅲ期、有淋巴转移、有远处转移结直肠癌患者结直肠癌组织中PIK3CB阳性表达率分别高于分化程度为高分化、TNM分期为Ⅰ~Ⅱ期、无淋巴结转移、无远处转移患者,差异均有统计学意义(P﹤0.05)。截至随访结束,153例结直肠癌患者中,预后不良41例,预后良好112例。二元Logistic回归分析结果显示,TNM分期为Ⅲ期、淋巴结转移及PIK3CB阳性表达均是结直肠癌患者预后不良的独立危险因素(P﹤0.05)。结论结论PIK3CB蛋白在结直肠癌组织中阳性表达率较高,且PIK3CB阳性表达是结直肠癌患者预后不良的独立危险因素。 展开更多

蒙特卡罗方差与通量平滑对乳腺癌VMAT放疗计划的剂量学影响

目的探讨容积旋转调强放疗(VMAT)中不同蒙特卡洛方差(MCV)和通量平滑因子(FSF)组合下的物理参数对VMAT剂量学和放射生物学参数的影响。方法选择2021年1月至2022年12月医院行VMAT治疗的12例左侧乳腺癌根治术后患者为研究对象。通过在Mona... 目的探讨容积旋转调强放疗(VMAT)中不同蒙特卡洛方差(MCV)和通量平滑因子(FSF)组合下的物理参数对VMAT剂量学和放射生物学参数的影响。方法选择2021年1月至2022年12月医院行VMAT治疗的12例左侧乳腺癌根治术后患者为研究对象。通过在Monaco 5.11 TPS中选择不同的MCV选项(0.5%、1%、2%)调整剂量计算误差范围,并通过调整FSF(Low、Medium、High)减少强度分布图噪声,每例患者设计9组放疗计划,分别为Low 0.5、Low 1.0、Low 2.0、Medium 0.5、Medium 1.0、Medium 2.0、High 0.5、High 1.0和High 2.0,共108个放疗计划,以系统默认参数(MCV=1%、FSF=Medium)为对照组,其余纳入试验组。通过剂量-体积直方图(DVH)评估患者靶区和危及器官的剂量学参数,并记录子野数、机器跳数(MU)和治疗时间(T)等物理参数。结果在108个放疗计划中,77.7%的计划满足了将95%的处方剂量覆盖95%靶区的计划靶区(PTV)约束条件,其余计划中超过90%靶区接受95%处方剂量。所有试验组的2%PTV所受剂量(D2%)、平均剂量(D_(mean))、适形性指数(CI)和均匀性指数(HI)与对照组比较,差异均有统计学意义(P<0.05)。在心脏、患侧肺和健侧乳腺剂量学参数方面,MCV值变化对剂量影响较小,而FSF值变化对剂量学和物理参数影响较大,差异有统计学意义(P<0.05)。结论对于左侧乳腺癌术后患者进行VMAT治疗时,通过适当调整MCV值和FSF,可优化放疗方案、减少子野数和T,同时保持剂量覆盖范围,从而提高治疗效率和质量,建议采用MCV为1%、FSF为Low和Medium组合,以获得更佳的剂量学和生物学效果。 展开更多

目的 分析经皮肾镜取石术(PCNL)后患者出院准备度现状及其影响因素。方法 2021年3月至2022年10月,采用方便抽样法选取莆田市第一医院收治的128例PCNL后患者作为研究对象。采用一般资料调查表、出院准备度量表(RHDS)、社会支持评定量表(S... 目的 分析经皮肾镜取石术(PCNL)后患者出院准备度现状及其影响因素。方法 2021年3月至2022年10月,采用方便抽样法选取莆田市第一医院收治的128例PCNL后患者作为研究对象。采用一般资料调查表、出院准备度量表(RHDS)、社会支持评定量表(SSRS)、出院指导质量量表(QDTS)对其进行调查。结果 纳入的128例PCNL后患者RHDS得分为(148.57±16.36)分;多元线性回归分析显示,年龄≥60岁、初中及以下文化程度、携带肾造瘘管、社会支持水平低、出院指导质量差是影响PCNL后患者出院准备度的危险因素(P<0.05)。结论 PCNL后患者出院准备度水平尚有提升空间,临床医护人员应加强对高龄、携带肾造瘘管、文化程度及社会支持水平低等人群的关注,注重对其进行疾病健康宣教和出院指导并协助其完善社会支持系统,以提高其出院准备度水平。 展开更多
