The study sought to assess and evaluate occupational h...INTRODUCTION Occupational safety and health (OSH) is generally the anticipation, recognition, evaluation, and control of hazards arising in or from the workplace. The study sought to assess and evaluate occupational health and safety hazards experienced among health workers in the Bono region of Ghana. METHODOLOGY The study was descriptive cross-sectional quantitative study. Data was from two hundred (200) health workers and was analyzed using the binary logistic regression analysis. RESULTS The findings from the study show that risk factors associated with biological hazards were clinical staff [OR = 2.487 (1.146 - 5.397), p = 0.021], poor maintenance of hospital items [OR = 0.446 (0.240 - 0.831), p = 0.011], assault (verbal) abuse [OR = 2.581 (1.317 - 5.059), p = 0.006] and extreme pressure from work [OR = 2.975 (1.519 - 5.829), p = 0.001]. Non-biological hazards were associated with being single [OR = 0.499 (0.263 - 0.947), p = 0.034], being verbally assaulted [OR = 3.581 (1.865 - 6.876), p CONCLUSION Risk factors related with biological hazards include poor maintenance of hospital items and extreme pressure from work whereas non-biological hazards were associated with being single, being verbally assaulted. Clinical healthcare providers are more vulnerable to occupational health and safety hazards. The study recommends the provision of strategic policies to promote and protect the workers’ health based on the development of the epidemiological profile of health, needs to be readjusted and strengthened.展开更多

Food safety and hygiene practices require a multisectoral approach including food, water, sanitation, waste management, transport, education, trade, policies and programs that enable safe food preparation, storage and...Food safety and hygiene practices require a multisectoral approach including food, water, sanitation, waste management, transport, education, trade, policies and programs that enable safe food preparation, storage and service. Unsafe food can cause illness keeping people from achieving their full potential and death. This was a descriptive study that uses a mixed method approach to derive insights into the characteristics of food vendors related to demography, knowledge, practices, infrastructure, compliance and recommendation for a policymaking framework. Using the Lemeshows’ sample size formula, 473 vendors from formal (restaurants) and informal (cookri-baffa/table top) sites were interviewed and observed. We found from discussions that respondents had a good understanding on how to keep food safe. However, observed practices were poor 93% handled food with their bare hands, 83% did not cover their hair, and 76% did not wear an apron whilst handling, preparing or serving food, 61% did not keep their finger nails clean or short and 57% did not wash their hand before preparing or serving food. Over half (51%) had access to a toilet but 32% reported their use required payment and emphasized their poor condition/inadequate management. Nearly half (47%) of the vending sites did not have a handwashing facility, with soap and water available. Only 7% reported having any authority oversight of food safety. Food safety and hygiene practices in most cookri shops and restaurants was ‘poor’ with very limited surveillance system in place by competent authorities for compliance of food operators. Hand washing, clean surroundings, and covered food were the most common and emphasized practices to mitigate the risks associated with unsafe food.展开更多
Objectives:This study was conducted to investigate the current status of handoffs,perception of patient safety culture,and degrees of handoff evaluation in small and medium-sized hospitals and identified factors that ...Objectives:This study was conducted to investigate the current status of handoffs,perception of patient safety culture,and degrees of handoff evaluation in small and medium-sized hospitals and identified factors that make a difference in handoff evaluation.Methods:This is a descriptive study.425 nurses who work at small and medium-sized hospitals in South Korea were included in our study.They completed a set of self-reporting questionnaires that evaluated demographic data,handoff-related characteristics,perception of patient safety culture,and handoff evaluation.Results:Results showed that the overall score of awareness of a patient safety culture was 3.65±0.45,the level was moderate.The score of handoff evaluation was 5.24±0.85.Most nurses experienced errors in handoff and most nurses had no guidelines and checklist in the ward.Handoff evaluation differed significantly according to the level of education,work patterns,duration of hospital employment,handoff method,degree of satisfaction with the current handoff method,errors occurring at the time of handoff,handoff guidelines,and appropriateness of handoff education time(P<0.05).Conclusion:For handoff improvement,guidelines and standards should be established.It is necessary to develop a structured handoff education system.And formal handoff education should be implemented to spread knowledge uniformly.展开更多

The perception of patient safety culture varies among health care staff. Health care staff (n = 1023) working in medical, surgical or mixed medical-surgical health care divisions answered the 51 items (14 dimensions) Swedish Hospital Survey on Patient Safety Culture (S-HSOPSC). Respondents with a managerial function scored higher than non-managers for 11 of 14 dimensions, indicating patient safety culture strengths for a majority of dimensions. Enrolled nurses and staff with experience > 10 years also scored high for several dimensions. The 12 dimensions and sample characteristics explained 49% and 26% of the variance for the outcome dimensions Overall Perceptions of Safety and Frequency of Incident Reporting, respectively. RNs, ENs and physicians have different views on patient safety culture. Hospital Management Support and Organisational Learning is some important factors influencing patient safety culture. Bridging the gap in health care staff’s perceptions of safety in order to improve patient safety is of utmost importance. The aim of the study is to measure physician and nurse awareness about four selected patient safety indicators by authors and events reported about these relevant indicators in the hospital. The study uses standardized four patientsafety indicators like "needle sticks, cut wounds, dressing allergy, infections indicators". Cross section study was conducted through three month period in 2011-2012 based on voluntary response to the questionnaire that intend to measure knowledge about four health indicators. Studypopulations consisted of accessible sample of 146 different specialty physicians and 108 nurses present on duty during survey period. The association between the patient safety indicators and events reported about indicators in questionswere analyzed. Meanpatient safety knowledge questionnaire scores of health staff (nurse and physician) for needle sticks, cut wounds, dressing allergy, infections indicators were 47.13(11.8), 39.04(14.5), 38.02(10.5), 39.72(9.7), respectively. Significant statistical differences were also found between the frequency of events reported according to department and patient safety indicators (F = 8.34; p 〈 0.05) Measuring patient safety culture via safety indicators is essential in improving patient safety. Higher culture of patient safety has been shown to be associated with better patient outcomes, and the implementation of improvements in organizational and safety culture enhances quality in healthcare. <strong>Aim:</strong> The main aim of this study is to assess the perception of nurses regarding patient safety among nurses at prince sultan military medical city in Saudi Arabia. <strong>Materials and Methods:</strong> Cross-sectional design was applied in this study among nurses who are working at prince sultan military medical city in Riyadh, Saudi Arabia. Sample was 215 nurses, selected by convenience sampling method. The Hospital Survey on Patient Safety Culture (HSOPS) survey was used in the current study to collect data. Data have been collected within seven weeks. Statistical package for social sciences has been used to analyze data using descriptive statistics and inferential statistics such as Independent sample t test and One-Way ANOVA. <strong>Results and </strong><strong>Conclusion:</strong> The results can only be useful if data analysis is described. The highest domain of their perception was illustrated in “Hospital management support for patient safety” with a mean of 3.68 out of 5.0, while, the lowest mean for their perception was in the domain of “Teamwork across hospital units” with a mean 2.62. The total mean of nurses’ perception regarding patient safety was 3.19 out of 5.0. Also, there was a significant difference in the mean level of nurses’ perception of patient safety with regard to nurses’ gender. Encouraging patients’ safety culture among nurses is strongly recommended through effective communication and providing feedback.展开更多
Background: Compulsive behavior, dyskinesias, motor fluctuations, and hallucinations are common Parkinson’s disease (PD) medication side effects. These are yet to be examined in relation to race and level of educatio...Background: Compulsive behavior, dyskinesias, motor fluctuations, and hallucinations are common Parkinson’s disease (PD) medication side effects. These are yet to be examined in relation to race and level of education. The goal of this analysis was to identify socioeconomic or clinical variables that are associated with compulsive behavior, dyskinesias, motor fluctuations, and hallucinations in patients in a safety-net hospital. Methods: A movement disorder patient database containing 452 patients with idiopathic PD was analyzed for differences in PD medication side effects using univariate and multivariate logistic regression analysis. Race, sex, and level of education were evaluated as possible confounders. Results: A greater proportion of the patients in this study were Caucasian males. The only variable associated with compulsive behavior was age, with higher age having a protective effect (p = 0.0336). Disease duration (defined as time since the onset of symptoms), diagnosis duration (time since formal diagnosis), and level of education were significantly associated with dyskinesia inunivariate analysis (p =< 0.0001, <0.0001, 0.1236 respectively). However, diagnosis duration was the only variable significantly associated with dyskinesia in multivariate analysis (p = 0.0038), in addition to a borderline significant association when comparing individuals with graduate degree to those who had completed high school education or less (p = 0.0599), with a protective effect of higher education. Disease duration, diagnosis duration, and use of monoamineoxidase inhibitors were also significantly associated with motor fluctuations in the univariate analysis, while only diagnosis duration was significantly associated with motor fluctuations in multivariate analysis (p = 0.0035) with longer diagnosis duration associated with higher risk of motor fluctuations. Age, disease duration, and diagnosis duration were associated with an increased risk of hallucinations in univariate analysis (p =< 0.0001, <0.0001, <0.0001 respectively), but age and disease duration were the only variables associated with hallucinations in multivariate analysis (p = 0.0009, 0.1196 respectively). Race was not associated with a higher risk of compulsive behavior, dyskinesias, motor fluctuations, or hallucinations. Conclusion: Compulsive behavior, dyskinesias, motor fluctuations, and hallucinations in our PD population may be associated with differences in socioeconomic status and access to care, but not with differences in race.展开更多
Objectives:A good patient safety culture(PSC)is linked to a reduced risk of patient problems and minimal undesirable occurrences.This study investigated the PSC levels from nurses'perspectives during the COVID-19 ...Objectives:A good patient safety culture(PSC)is linked to a reduced risk of patient problems and minimal undesirable occurrences.This study investigated the PSC levels from nurses'perspectives during the COVID-19 pandemic.Methods:A descriptive cross-sectional design was applied.The Hospital Survey on Patient Safety Culture(HSOPSC)questionnaire was administered to 315 nurses working at 2 major hospitals in Jeddah,Saudi Arabia.The data were analyzed using descriptive statistics,a t-test,and a one-way ANOVA test.The statistical significance of the correlation was determined at the 0.05 level.Results:PSC was rated as medium overall according to the nurses,with a weighted mean of 2.88–0.76 and a relative weight of 57.57%.In addition,all PSC composites were rated from medium to high,except organizational learning,which was rated low.The correlation between sociodemographic variables as well as PSC levels was investigated using the t-test and one-way ANOVA test.The association is statistically significant when P≤0.05.The findings revealed a statistically significant correlation between nurse nationality(t=-4.399,P=0.000),age(F=7.917,P=0.000),experience in years(F=3.760,P=0.024),and hospital(t=-0.401,P=0.689).Conclusions:The nurses in this study had a medium overall PSC level,and all PSC composites ranged from a medium to a high level,except organizational learning,which had a low level.In addition,the findings showed that there is a significant relationship between PSC levels,nurses'nationalities,experience in years,and the hospital itself.展开更多

To improve patient safety reporting, the present paper suggests an ontological representation of patient safety knowledge. Design/methodology/approach: We propose a framework for constructing an ontological knowledge base of patient safety. The present paper describes our design, implementation,and evaluation of the ontology at its initial stage. Findings: We describe the design and initial outcomes of the ontology implementation. The evaluation results demonstrate the clinical validity of the ontology by a self-developed survey measurement. Research limitations: The proposed ontology was developed and evaluated using a small number of information sources. Presently, US data are used, but they are not essential for the ultimate structure of the ontology.Practical implications: The goal of improving patient safety can be aided through investigating patient safety reports and providing actionable knowledge to clinical practitioners.As such, constructing a domain specific ontology for patient safety reports serves as a cornerstone in information collection and text mining methods.Originality/value: The use of ontologies provides abstracted representation of semantic information and enables a wealth of applications in a reporting system. This paper describes how these four types of hazard can be incorporated into a design methodology approach, including the process by which the relevant mechanical rock mass parameters can be recognised for modelling and hence predictive purposes. The purpose of this study was to assess the culture of...Establishing a culture of patient safety can be effective in reducing the incidence of medical errors and solving concerns of safety inadequacy in health systems. The purpose of this study was to assess the culture of patient safety in the selected hospitals, and compare the results with published reports of AHRQ. This study was approved by the Ethical Committee of BPUMS. The subjects signed the informed consent form to participle in the study. Confidentiality was maintained throughout the study reports. Cross-sectional study was conducted in 2012;the study sample was composed of 364 staffs working at two selected hospitals affiliated to Bushehr University of Medical Sciences. Hospital Survey on Patient Safety Culture was used to collect data. Descriptive statistical analysis was used to analyze the data. No reports of events in both studied hospitals and benchmark were accounted for the most of the reported errors, although this indicator in studied hospitals was nearly 23% higher than that of the benchmark report. The highest patient safety grade in studied hospitals and benchmark was “acceptable” and “very good”, respectively. The highest percentage of positive response to patient safety dimension was organization learning and then teamwork within units in studied hospitals. Teamwork within units also was the highest average percent in benchmark report. Non-punitive response to errors had the lowest positive percentage of participant responses in both studies. To achieve the patient safety culture, we do not need to blame individual and apply punitive approach when errors occur. This makes person accept responsibility for their actions honestly and report errors in a timely manner to prevent reoccurrence of similar errors.展开更多

However, observed practices were poor 93% handled food with their bare hands, 83% did not cover their hair, and 76% did not wear an apron whilst handling, preparing or serving food, 61% did not keep their finger nails clean or short and 57% did not wash their hand before preparing or serving food. Over half (51%) had access to a toilet but 32% reported their use required payment and emphasized their poor condition/inadequate management. Nearly half (47%) of the vending sites did not have a handwashing facility, with soap and water available. Only 7% reported having any authority oversight of food safety. Food safety and hygiene practices in most cookri shops and restaurants was ‘poor’ with very limited surveillance system in place by competent authorities for compliance of food operators. Hand washing, clean surroundings, and covered food were the most common and emphasized practices to mitigate the risks associated with unsafe food.

文摘Objectives:This study was conducted to investigate the current status of handoffs,perception of patient safety culture,and degrees of handoff evaluation in small and medium-sized hospitals and identified factors that make a difference in handoff evaluation.Methods:This is a descriptive study.425 nurses who work at small and medium-sized hospitals in South Korea were included in our study.They completed a set of self-reporting questionnaires that evaluated demographic data,handoff-related characteristics,perception of patient safety culture,and handoff evaluation.Results:Results showed that the overall score of awareness of a patient safety culture was 3.65±0.45,the level was moderate.The score of handoff evaluation was 5.24±0.85.Most nurses experienced errors in handoff and most nurses had no guidelines and checklist in the ward.Handoff evaluation differed significantly according to the level of education,work patterns,duration of hospital employment,handoff method,degree of satisfaction with the current handoff method,errors occurring at the time of handoff,handoff guidelines,and appropriateness of handoff education time(P<0.05).Conclusion:For handoff improvement,guidelines and standards should be established.It is necessary to develop a structured handoff education system.And formal handoff education should be implemented to spread knowledge uniformly.
文摘Many hospital patients are affected by adverse events. Managers are important when improving safety. The perception of patient safety culture varies among health care staff. Health care staff (n = 1023) working in medical, surgical or mixed medical-surgical health care divisions answered the 51 items (14 dimensions) Swedish Hospital Survey on Patient Safety Culture (S-HSOPSC). Respondents with a managerial function scored higher than non-managers for 11 of 14 dimensions, indicating patient safety culture strengths for a majority of dimensions. Enrolled nurses and staff with experience > 10 years also scored high for several dimensions. The 12 dimensions and sample characteristics explained 49% and 26% of the variance for the outcome dimensions Overall Perceptions of Safety and Frequency of Incident Reporting, respectively. RNs, ENs and physicians have different views on patient safety culture. Hospital Management Support and Organisational Learning is some important factors influencing patient safety culture. Bridging the gap in health care staff’s perceptions of safety in order to improve patient safety is of utmost importance. Managers have the responsibility to foster patient safety culture at their workplace and can thus benefit from results when improving safety for patients.

文摘Patient safety is an important component of risk management in hospitals. The aim of the study is to measure physician and nurse awareness about four selected patient safety indicators by authors and events reported about these relevant indicators in the hospital. The study uses standardized four patientsafety indicators like "needle sticks, cut wounds, dressing allergy, infections indicators". Cross section study was conducted through three month period in 2011-2012 based on voluntary response to the questionnaire that intend to measure knowledge about four health indicators. Studypopulations consisted of accessible sample of 146 different specialty physicians and 108 nurses present on duty during survey period. The association between the patient safety indicators and events reported about indicators in questionswere analyzed. Meanpatient safety knowledge questionnaire scores of health staff (nurse and physician) for needle sticks, cut wounds, dressing allergy, infections indicators were 47.13(11.8), 39.04(14.5), 38.02(10.5), 39.72(9.7), respectively. Significant statistical differences were also found between the frequency of events reported according to department and patient safety indicators (F = 8.34; p 〈 0.05) Measuring patient safety culture via safety indicators is essential in improving patient safety. This matter is perfectly influence the financial management of the hospital.

文摘<strong>Background:</strong> The absence of patient safety culture among nurses has been linked to increased errors and higher level of mortality and morbidity among patients. Higher culture of patient safety has been shown to be associated with better patient outcomes, and the implementation of improvements in organizational and safety culture enhances quality in healthcare. <strong>Aim:</strong> The main aim of this study is to assess the perception of nurses regarding patient safety among nurses at prince sultan military medical city in Saudi Arabia. <strong>Materials and Methods:</strong> Cross-sectional design was applied in this study among nurses who are working at prince sultan military medical city in Riyadh, Saudi Arabia. Sample was 215 nurses, selected by convenience sampling method. The Hospital Survey on Patient Safety Culture (HSOPS) survey was used in the current study to collect data. Data have been collected within seven weeks. Statistical package for social sciences has been used to analyze data using descriptive statistics and inferential statistics such as Independent sample t test and One-Way ANOVA. <strong>Results and </strong><strong>Conclusion:</strong> The results can only be useful if data analysis is described. The highest domain of their perception was illustrated in “Hospital management support for patient safety” with a mean of 3.68 out of 5.0, while, the lowest mean for their perception was in the domain of “Teamwork across hospital units” with a mean 2.62. The total mean of nurses’ perception regarding patient safety was 3.19 out of 5.0. Also, there was a significant difference in the mean level of nurses’ perception of patient safety with regard to nurses’ gender. Encouraging patients’ safety culture among nurses is strongly recommended through effective communication and providing feedback.
文摘Background: Compulsive behavior, dyskinesias, motor fluctuations, and hallucinations are common Parkinson’s disease (PD) medication side effects. These are yet to be examined in relation to race and level of education. The goal of this analysis was to identify socioeconomic or clinical variables that are associated with compulsive behavior, dyskinesias, motor fluctuations, and hallucinations in patients in a safety-net hospital. Methods: A movement disorder patient database containing 452 patients with idiopathic PD was analyzed for differences in PD medication side effects using univariate and multivariate logistic regression analysis. Race, sex, and level of education were evaluated as possible confounders. Results: A greater proportion of the patients in this study were Caucasian males. The only variable associated with compulsive behavior was age, with higher age having a protective effect (p = 0.0336). Disease duration (defined as time since the onset of symptoms), diagnosis duration (time since formal diagnosis), and level of education were significantly associated with dyskinesia inunivariate analysis (p =< 0.0001, <0.0001, 0.1236 respectively). However, diagnosis duration was the only variable significantly associated with dyskinesia in multivariate analysis (p = 0.0038), in addition to a borderline significant association when comparing individuals with graduate degree to those who had completed high school education or less (p = 0.0599), with a protective effect of higher education. Disease duration, diagnosis duration, and use of monoamineoxidase inhibitors were also significantly associated with motor fluctuations in the univariate analysis, while only diagnosis duration was significantly associated with motor fluctuations in multivariate analysis (p = 0.0035) with longer diagnosis duration associated with higher risk of motor fluctuations. Age, disease duration, and diagnosis duration were associated with an increased risk of hallucinations in univariate analysis (p =< 0.0001, <0.0001, <0.0001 respectively), but age and disease duration were the only variables associated with hallucinations in multivariate analysis (p = 0.0009, 0.1196 respectively). Race was not associated with a higher risk of compulsive behavior, dyskinesias, motor fluctuations, or hallucinations. Conclusion: Compulsive behavior, dyskinesias, motor fluctuations, and hallucinations in our PD population may be associated with differences in socioeconomic status and access to care, but not with differences in race.
文摘Objectives:A good patient safety culture(PSC)is linked to a reduced risk of patient problems and minimal undesirable occurrences.This study investigated the PSC levels from nurses'perspectives during the COVID-19 pandemic.Methods:A descriptive cross-sectional design was applied.The Hospital Survey on Patient Safety Culture(HSOPSC)questionnaire was administered to 315 nurses working at 2 major hospitals in Jeddah,Saudi Arabia.The data were analyzed using descriptive statistics,a t-test,and a one-way ANOVA test.The statistical significance of the correlation was determined at the 0.05 level.Results:PSC was rated as medium overall according to the nurses,with a weighted mean of 2.88–0.76 and a relative weight of 57.57%.In addition,all PSC composites were rated from medium to high,except organizational learning,which was rated low.The correlation between sociodemographic variables as well as PSC levels was investigated using the t-test and one-way ANOVA test.The association is statistically significant when P≤0.05.The findings revealed a statistically significant correlation between nurse nationality(t=-4.399,P=0.000),age(F=7.917,P=0.000),experience in years(F=3.760,P=0.024),and hospital(t=-0.401,P=0.689).Conclusions:The nurses in this study had a medium overall PSC level,and all PSC composites ranged from a medium to a high level,except organizational learning,which had a low level.In addition,the findings showed that there is a significant relationship between PSC levels,nurses'nationalities,experience in years,and the hospital itself.

文摘Purpose: The current development of patient safety reporting systems is criticized for loss of information and low data quality due to the lack of a uniformed domain knowledge base and text processing functionality. To improve patient safety reporting, the present paper suggests an ontological representation of patient safety knowledge. Design/methodology/approach: We propose a framework for constructing an ontological knowledge base of patient safety. The present paper describes our design, implementation,and evaluation of the ontology at its initial stage. Findings: We describe the design and initial outcomes of the ontology implementation. The evaluation results demonstrate the clinical validity of the ontology by a self-developed survey measurement. Research limitations: The proposed ontology was developed and evaluated using a small number of information sources. Presently, US data are used, but they are not essential for the ultimate structure of the ontology.Practical implications: The goal of improving patient safety can be aided through investigating patient safety reports and providing actionable knowledge to clinical practitioners.As such, constructing a domain specific ontology for patient safety reports serves as a cornerstone in information collection and text mining methods.Originality/value: The use of ontologies provides abstracted representation of semantic information and enables a wealth of applications in a reporting system. Therefore, constructing such a knowledge base is recognized as a high priority in health care.

文摘Knowledge graph technology play a more and more important role in various fields of industry and academia.This paper firstly introduces the general framework of the knowledge graph construction,which includes three stages:information extraction,knowledge fusion and knowledge processing.In order to improve the efficiency of quality and safety supervision of transportation engineering construction,this paper constructs a knowledge graph by acquiring multi-sources heterogeneous data from supervision of transportation engineering quality and safety.It employs a bottom-up construction strategy and some natural language processing methods to solve the problems of the knowledge extraction for transportation engineering construction.We use the entity relation extraction method to extract the entity triples from the multi-sources heterogeneous data,and then employ knowledge inference to complete the edges in the constructed knowledge graph,finally perform quality evaluation to add the valid triples to the knowledge graph for updating.Subgraph matching technology is also exploited to retrieve the constructed knowledge graph for efficiently acquiring the useful knowledge about the quality and safety of transportation engineering projects.The results show that the constructed knowledge graph provides a practical and valuable tool for the quality and safety supervision of transportation engineering construction.
文摘Given the public concern over the safety of genetically modified (GM) technology and products, the article elaborated the safety regulatory and administration on agricultural GMOs in China. China has made a set of laws and regulations of GMO safety management and confirmed competent authorities with clear-cut responsibilities. According to the laws and regulations, GMO products before entering markets have to pass through safety evaluation, get production and processing permission and be labeled correctly. For the importation of GM products, China has set up an import approval system. In addition, China has established technical supportive systems, including safety evaluation specifications, trial specifications and criteria specifications. The existing regulatory system supervises and regulates all activities and work related to agri- cultural GMOs in China.
文摘In order to optimise the safety of underground rock engineering construction and the long-term security of the resultant facilities, it is necessary to have a knowledge of the likely hazards. These risks or hazards fall into the four categories of 'known beforehand and relatively easily addressed', 'known beforehand and not easily addressed', 'not known beforehand and relatively easily addressed', and 'not known beforehand and not easily addressed'. This paper describes how these four types of hazard can be incorporated into a design methodology approach, including the process by which the relevant mechanical rock mass parameters can be recognised for modelling and hence predictive purposes. In particular, there is emphasis on the fact that information and judgement are the keys to safety——whether the hazards are known or unknown before construction proceeds.
文摘Establishing a culture of patient safety can be effective in reducing the incidence of medical errors and solving concerns of safety inadequacy in health systems. The purpose of this study was to assess the culture of patient safety in the selected hospitals, and compare the results with published reports of AHRQ. This study was approved by the Ethical Committee of BPUMS. The subjects signed the informed consent form to participle in the study. Confidentiality was maintained throughout the study reports. Cross-sectional study was conducted in 2012;the study sample was composed of 364 staffs working at two selected hospitals affiliated to Bushehr University of Medical Sciences. Hospital Survey on Patient Safety Culture was used to collect data. Descriptive statistical analysis was used to analyze the data. No reports of events in both studied hospitals and benchmark were accounted for the most of the reported errors, although this indicator in studied hospitals was nearly 23% higher than that of the benchmark report. The highest patient safety grade in studied hospitals and benchmark was “acceptable” and “very good”, respectively. The highest percentage of positive response to patient safety dimension was organization learning and then teamwork within units in studied hospitals. Teamwork within units also was the highest average percent in benchmark report. Non-punitive response to errors had the lowest positive percentage of participant responses in both studies. To achieve the patient safety culture, we do not need to blame individual and apply punitive approach when errors occur. This makes person accept responsibility for their actions honestly and report errors in a timely manner to prevent reoccurrence of similar errors.
