Evaluation of the effects of health education interventions for hypertensive patients based on the health belief model 被引量:1

BACKGROUND Hypertension is a major risk factor for cardiovascular disease and stroke,and its prevalence is increasing worldwide.Health education interventions based on the health belief model(HBM)can improve the knowledge,attitudes,and behaviors of patients with hypertension and help them control their blood pressure.AIM To evaluate the effects of health education interventions based on the HBM in patients with hypertension in China.METHODS Between 2021 and 2023,140 patients with hypertension were randomly assigned to either the intervention or control group.The intervention group received health education based on the HBM,including lectures,brochures,videos,and counseling sessions,whereas the control group received routine care.Outcomes were measured at baseline,three months,and six months after the intervention and included blood pressure,medication adherence,self-efficacy,and perceived benefits,barriers,susceptibility,and severity.RESULTS The intervention group had significantly lower systolic blood pressure[mean difference(MD):-8.2 mmHg,P<0.001]and diastolic blood pressure(MD:-5.1 mmHg,P=0.002)compared to the control group at six months.The intervention group also had higher medication adherence(MD:1.8,P<0.001),self-efficacy(MD:12.4,P<0.001),perceived benefits(MD:3.2,P<0.001),lower perceived barriers(MD:-2.6,P=0.001),higher perceived susceptibility(MD:2.8,P=0.002),and higher perceived severity(MD:3.1,P<0.001)than the control group at six months.CONCLUSION Health education interventions based on the HBM effectively improve blood pressure control and health beliefs in patients with hypertension and should be implemented in clinical practice and community settings. 展开更多

BACKGROUND Vestibular dysfunction(VH)is a common concomitant symptom of late peri-pheral vestibular lesions,which can be trauma,poisoning,infection,heredity,and neurodegeneration,but about 50%of the causes are unknown.The study uses the information-motivation-behavioral skills(IMB)model for health education,effectively improve the quality of life,increase their self-confidence,reduce anxiety and depression,and effectively improve the psychological state of patients.AIM To explore the effect of health education based on the IMB model on the degree of vertigo,disability,anxiety and depression in patients with unilateral vestibular hypofunction.METHODS The clinical data of 80 patients with unilateral vestibular hypofunction from January 2019 to December 2021 were selected as the retrospective research objects,and they were divided into the control group and the observation group with 40 cases in each group according to different nursing methods.Among them,the control group was given routine nursing health education and guidance,and the observation group was given health education and guidance based on the IMB model.The changes in self-efficacy,anxiety and depression,and quality of life of patients with unilateral VH were compared between the two groups.RESULTS There was no significant difference in General Self-Efficacy Scale(GSES)scale scores between the two groups of patients before nursing(P>0.05),which was comparable;after nursing,the GSES scale scores of the two groups were higher than those before nursing.The nursing group was higher than the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the scores of Hospital Anxiety and Depression Scale(HADS)and anxiety and depression subscales between the two groups before nursing(P>0.05).After nursing,the HADS score,anxiety,and depression subscale scores of the two groups of patients were lower than those before nursing,and the nursing group was lower than the control group,and the difference was statistically significant(P<0.05).After nursing,the Dizziness Handicap Inventory(DHI)scale and DHI-P,DHI-E and DHI-F scores in the two groups were decreased,and the scores in the nursing group were lower than those in the control group,and the difference was statistically significant(P<0.05).CONCLUSION Health education based on the IMB model can effectively improve patients'quality of life,increase self-efficacy of patients with unilateral vestibular hypofunction,enhance patients'confidence,enable patients to resume normal work and life as soon as possible,reduce patients'anxiety and depression,and effectively improve patients'psychological status. 展开更多

Objective:To observe the status quo of patients’psychological distress,and to explore the effect of Internet+health education model(IHEM)on patients who experienced psychological distress during their first hemodialysis treatment,with the goal of reducing their psychological distress and improving their quality of life.Methods:IHEM was conducted on 120 first-time hemodialysis patients for 3 months while a distress thermometer and a list of questionnaires were used to screen patients and provide corresponding psychological intervention.The incidence rate of psychological distress was analyzed statistically to explore the difference in psychological distress at various periods.Results:The incidence rate(score≥4)of psychological distress in first-time hemodialysis patients was 46.67%,and their distress was mainly rooted in physical,emotional,practical problems(economy,time,and energy),etc.Through IHEM,the psychological distress scores of the patients decreased to 3.29±1.02 at one month after their discharge,and the incidence rate was 32.14%;the psychological distress scores of the patients were 2.29±1.02 at 3 months after their discharge,and the incidence rate was 21.14%.The difference before and after the intervention was statistically significant(P<0.05).Conclusion:A psychological distress thermometer can timely detect the degree and causes of psychological distress among first-time hemodialysis patients,and the use of IHEM may significantly alleviate the psychological distress among hemodialysis patients. 展开更多

目的探讨赋能授权联合药物素养构建的健康教育处方对首发心肌梗死(myocardial infarction,MI)患者的药物素养、服药依从性及自我管理情况的影响。方法采用便利抽样法选取2023年6—12月江西省某三级甲等医院心内科首发MI入院治疗的70例... 目的探讨赋能授权联合药物素养构建的健康教育处方对首发心肌梗死(myocardial infarction,MI)患者的药物素养、服药依从性及自我管理情况的影响。方法采用便利抽样法选取2023年6—12月江西省某三级甲等医院心内科首发MI入院治疗的70例患者为研究对象,按随机数字表法分为对照组35例和观察组35例。对照组采取常规健康教育处方,观察组实施赋能授权联合药物素养构建的健康教育处方,评估比较2组患者干预前后的药物素养、服药依从性及自我管理情况。结果干预后,观察组患者的药物素养、服药依从性得分分别为(5.29±0.93)分、(6.51±0.82)分,均优于对照组(4.69±0.96)分、(5.91±1.10)分,差异有统计学意义(P<0.05);观察组患者的自我管理情况总得分为(95.63±7.15)分,明显高于对照组(88.79±6.00)分,差异有统计学意义(P<0.05)。结论赋能授权联合药物素养构建的健康教育处方,有助于改善首发MI患者安全用药情况,提高自我管理水平。 展开更多

社区中老年慢性病患者个体化健康教育干预效果:一项整群随机对照试验

背景慢性病的频发与患者对健康知识及疾病风险认识不足有关,从全国范围来看,较为传统的健康教育方式依然广泛存在于社区卫生服务中,基层医生提供健康教育的形式单一、内容缺乏针对性,居民参与健康教育积极性普遍不足,对慢性病患者实施... 背景慢性病的频发与患者对健康知识及疾病风险认识不足有关,从全国范围来看,较为传统的健康教育方式依然广泛存在于社区卫生服务中,基层医生提供健康教育的形式单一、内容缺乏针对性,居民参与健康教育积极性普遍不足,对慢性病患者实施健康教育质量及效果有待提升。目的探究基于应用信息化知识库模型生成的个体化健康教育干预对社区中老年慢性病群体的影响,为进一步强化社区慢性病治理效果提供参考。方法于2021年选取北京市东城区社区卫生服务中心7390例患有4种慢性病(高血压、糖尿病、冠心病、脑卒中)的50~70岁患者作为研究对象,并进行为期1年的整群随机对照试验。对照组患者采用常规慢性病随访管理策略(保持原有的慢性病基本公共卫生服务项目);干预组患者在常规慢性病随访管理策略的基础上,应用信息化知识库模型生成健康教育指导方案的方式,即添加了健康教育处方指导和个体化健康管理的方式进行随访,每3个月进行1次随访及干预,共持续12个月。在所有患者入组1年后进行终线调查。本研究主要从“慢性病知识知晓率、自我管理态度、自我效能、服药依从性、健康信息化接受程度”等方面来分析两组慢性病患者在基线与终线调查之间数据结果的差异。结果共纳入7390例4种慢性病患者,其中干预组患者3673例,对照组患者3717例。两组慢性病患者年龄分布、性别、文化程度、工作状态比较,差异无统计学意义(P>0.05);两组慢性病患者医疗保障形式比较,差异有统计学意义(P<0.05)。干预组干预后整体疾病知识、慢性病基础知识、糖尿病知识、冠心病知识、脑卒中知识知晓正确率高于组内干预前(P<0.05);干预前后高血压知识知晓正确率比较,差异无统计学意义(P>0.05)。对照组患者干预后整体疾病知识、慢性病基础知识、高血压知识、糖尿病知识、冠心病知识知晓正确率与干预前比较,差异无统计学意义(P>0.05),脑卒中知识知晓正确率低于组内干预前(P<0.05)。干预组干预后自我管理态度问卷、自我效能问卷、服药依从性问卷、健康信息化接受度问卷得分均高于对照组(P<0.05)。干预后干预组自我管理态度问卷、自我效能问卷、服药依从性问卷、健康信息化接受度问卷得分均高于组内干预前(P<0.05)。对照组干预后自我效能问卷、服药依从性问卷得分高于组内干预前(P<0.05);对照组干预后自我管理态度问卷、健康信息化接受度问卷得分与组内干预前比较,差异无统计学意义(P>0.05)。结论从患者对慢性病知识知晓率、自我管理态度、信息化接受度方面可看出干预组患者改善效果明显优于对照组;从患者自我效能与服药依从性角度方面,两组患者在干预后均有提升,干预组效果更为显著。综合研究结果表明,通过信息化知识库模型进行个体化健康教育方式有助于慢性病患者健康素养提升。 展开更多

Public involvement in Ebola Virus Disease(EVD)prevention efforts is key to reducing disease outbreaks.Targeted education through practical health information to particular groups and sub-populations is crucial to controlling the disease.In this paper,we study the dynamics of Ebola virus disease in the presence of public health education with the aim of assessing the role of behavior change induced by health education to the dynamics of an outbreak.The power of behavior change is evident in two outbreaks of EVD that took place in Sudan only 3 years apart.The first occurrence was the first documented outbreak of EVD and produced a significant number of infections.The second outbreak produced far fewer cases,presumably because the population in the region learned from the first outbreak.We derive a system of ordinary differential equations to model these two contrasting behaviors.Since the population in Sudan learned from the first outbreak of EVD and changed their behavior prior to the second outbreak,we use data from these two instances of EVD to estimate parameters relevant to two contrasting behaviors.We then simulate a future outbreak of EVD in Sudan using our model that contains two susceptible populations,one being more informed about EVD.Our finding show how a more educated population results in fewer cases of EVD and highlights the importance of ongoing public health education. 展开更多

In this paper,we formulate a six dimensional drug transmission model to study the effect of family education and public health education.The dynamical behaviors of the model are discussed in terms of the basic reproduction number R0.By constructing Lyapunov functions,we obtain the drug-free equilibrium is globally asymptotically stable if R0≤1 and the drug addiction equilibrium is globally asymptotically stable if R0>1.Sensitivity analyses are performed to seek for effective control measures for drug spread.The analysis show that both the family and public health education can influence the spread of drug transmission.However,the combination of family and public health education is more effective to reduce the prevalence of drugs.Some numerical simulations are given to confirm the obtained theoretical results. 展开更多

目的探讨基于健康信念模式的健康教育在消化道早癌筛查中的应用效果。方法选取87例消化道早癌高危筛查患者,随机分为观察组(n=43)和对照组(n=44)。对照组接受常规的健康教育,观察组应用基于健康信念模式的健康教育,比较2组患者的筛查依... 目的探讨基于健康信念模式的健康教育在消化道早癌筛查中的应用效果。方法选取87例消化道早癌高危筛查患者,随机分为观察组(n=43)和对照组(n=44)。对照组接受常规的健康教育,观察组应用基于健康信念模式的健康教育,比较2组患者的筛查依从性,干预前后疾病知识掌握情况(包括消化道早癌相关知识、健康行为、筛查相关知识及心理疏导评分)、自我管理能力[自我管理能力测评量表(AHSMSRS)评分]、自护能力[自我护理能力量表(ESCA)评分]、健康行为[健康促进生活方式量表(HPLP-Ⅱ)评分]、负性情绪状况[焦虑自评量表(SAS)和抑郁自评量表(SDS)评分],护理满意度[纽卡斯尔护理服务满意度量表(NSNS)评分]。结果干预后,观察组疾病知识掌握情况各项评分、HPLP-Ⅱ、AHSMSRS和ESCA评分均高于对照组,SAS和SDS评分均低于对照组(P<0.05或P<0.001)。观察组总筛查依从率和总护理满意率均高于对照组(97.67%比81.82%,P<0.05;97.67%比79.55%,P<0.001)。结论将基于健康信念模式的健康教育应用于消化道早癌筛查患者中,能促进患者健康行为的养成,提高患者自护能力、筛查依从性,改善患者负性情绪状况。 展开更多

目的探讨以家庭为中心的赋能健康教育模式在剖宫产初产妇早期母乳喂养中的应用效果。方法选取2021年11月-2022年1月在我院计划行剖宫产的初产妇及其照顾者93例作为对照组,采用常规健康教育宣教方法,宣教对象主要为初产妇本人;选取2022年... 目的探讨以家庭为中心的赋能健康教育模式在剖宫产初产妇早期母乳喂养中的应用效果。方法选取2021年11月-2022年1月在我院计划行剖宫产的初产妇及其照顾者93例作为对照组,采用常规健康教育宣教方法,宣教对象主要为初产妇本人;选取2022年3-7月在我院计划行剖宫产的初产妇及其照顾者98例作为观察组,在对照组基础上实施以家庭为中心的赋能健康教育模式。比较2组初产妇及其照顾者干预前后母乳喂养知识情况;干预前后初产妇婴儿喂养态度、母乳喂养家庭支持;干预后初产妇泌乳Ⅱ期启动时间和出院时纯母乳喂养率。结果干预前2组初产妇及照顾者各指标比较,差异均无统计学意义(P>0.05);干预后,观察组初产妇及其照顾者母乳喂养知识,以及初产妇婴儿喂养态度、母乳喂养家庭支持、泌乳Ⅱ期启动时间和出院时纯母乳喂养率方面均优于对照组(P<0.05)。结论以家庭为中心的赋能健康教育模式能提高产妇及照顾者的母乳喂养知识,提升婴儿喂养态度及母乳喂养家庭支持,缩短泌乳Ⅱ期启动时间,提高产妇出院时纯母乳喂养率。 展开更多

目的 探究老年肺癌放疗病人接受多形式健康教育模式联合简易呼吸功能锻炼的效果。方法 选取本院于2021年3月至2023年9月收治的60例肺癌放疗病人进行前瞻性研究,按照随机数表法分为研究组(30例)与对照组(30例),对照组接受常规护理干预,... 目的 探究老年肺癌放疗病人接受多形式健康教育模式联合简易呼吸功能锻炼的效果。方法 选取本院于2021年3月至2023年9月收治的60例肺癌放疗病人进行前瞻性研究,按照随机数表法分为研究组(30例)与对照组(30例),对照组接受常规护理干预,研究组接受多形式健康教育模式联合简易呼吸功能锻炼干预,比较2组干预前后癌因性疲乏[Piper疲乏量表(PFS)评分]、生活质量[生活质量核心问卷(QLQ-C30)评分]、呼吸功能[呼出气一氧化氮(FeNO)、呼吸频率及氧合指数]、遵医行为、护理满意度(病人护理需求及满意程度调查表)情况。结果 干预前,2组PFS、QLQ-C30评分,FeNO、呼吸频率、氧合指数水平比较,差异无统计学意义(P>0.05);干预后,2组上述指标均有改善,差异有统计学意义(P<0.05),且研究组PFS评分、FeNO水平和呼吸频率低于对照组,QLQ-C30评分和氧合指数高于对照组,差异有统计学意义(P<0.05);护理期间,研究组的遵医率及护理干预满意度评分均高于对照组,差异有统计学意义(P<0.05)。结论 对老年肺癌放疗病人采取多形式健康教育模式联合简易呼吸功能锻炼干预,能够显著改善其癌因性疲乏,提高老年病人遵医性与生活质量,提升其对护理干预的满意度,具有较好的临床应用价值。 展开更多
