Therefore, iPTH level assessment in the postoperative period can be used to predict the development of hypocalcaemia. The optimal time point to measure serum iPTH is important for the accurate prediction of hypocalcaemia. Aim: This paper aims to evaluate the ability of iPTH as an early predictive marker of hypocalcaemia and determine which time iPTH is more able to predict postoperative hypocalcaemia. Method: This prospective observational study was conducted in the Department of Otolaryngology-Head & Neck Surgery, BSMMU, Dhaka, from July 2020 to December 2021, with 67 patients who underwent total thyroidectomy. iPTH levels were measured on the day before the operation and at 1 hour, 4 hours, and 24 hours after the operation. S.calcium levels were measured on the day before the operation and 1<sup>st</sup> postoperative day. All the data were compiled and sorted properly and were analyzed statistically. Results: Postoperative hypocalcaemia developed in 18 cases, with an incidence of 26.9%. Pearson correlation showed a significant correlation between postoperative iPTH at 1 hr, 4 h, and 24 hr with 1st postoperative calcium value. The Receiver operating characteristic (ROC) curve was processed for the postoperative iPTH at 1 hr, 4 h, and 24 hr. The sensitivity, specificity, cut-off value, and mean AUC found 93.9%, 94.4%, ≤14.0, 0.988;95.9%, 94.4%, ≤09.5, 0.993 and 91.8%, 94.4%, ≤11.0, 0.993 respectively. Conclusion: iPTH can be used as an early predictor of post-thy-roidectomy hypocalcaemia. 4 hr iPTH showed more sensitivity and specificity for a cut-off value near the laboratory reference range.展开更多
Objectives:To access the effectiveness of our modified right-ventricular overhauling procedure on tricuspid valve(TV)growth in patients with pulmonary atresia with intact ventricular septum(PAIVS).Methods:We retrospec...Objectives:To access the effectiveness of our modified right-ventricular overhauling procedure on tricuspid valve(TV)growth in patients with pulmonary atresia with intact ventricular septum(PAIVS).Methods:We retrospectively reviewed 21 patients with PAIVS who underwent modified right ventricular overhauling(mRVoh)between 2008 and 2019 at two institutions.Our mRVoh consisted of wide resection of hypertrophied infundibular and trabecular muscle,peeling off fibrotic endocardial tissue in the right ventricle(RV)cavity,surgical pulmonary valvotomy,and Blalock-Taussig shunt or banding of ductus arteriosus under cardiopulmonary bypass.The TV annulus sizes were measured and analyzed using echocardiography before and after mRVoh.Results:No mortalities were observed during a median follow-up of 3 years(interquartile range:1.3–4.7 years)of follow-up were noted.mRVoh was performed at a median age of 163.5 days(range:21–560 days),including seven neonates and two infants(<60 days).During follow-up,the median TV annular z-score increased significantly from−2.24 to−1.15 before and after mRVoh(p=0.004).In ten patients with a prior history of percutaneous interventions for RV outflow tract(RVOT)widening at least 6 months before mRVoh,the TV annular z-score significantly changed during the period after mRVoh(−2.03 to−1.61,p=0.028)compared with the period before mRVoh(−2.51→–2.03,p=0.575)after percutaneous intervention only.Conclusions:mRVoh in PAIVS patients was positively associated with TV annular growth,and it was more effective than percutaneous RVOT widening interventions without mRVoh.展开更多

An explosion event of cavitation, which can only occur in intact conduit at water...Cavitation in plant conduits only involves two processes of air bubbles: the gradual expansion and elongation, and the explosion event. An explosion event of cavitation, which can only occur in intact conduit at water tension, trigs acoustic (or ultrasound) emission and induces air to diffuse with high speed, simultaneously. Synchrotron X-ray phase contrast microscopy (XPCM) was used to capture cavitation event in intact conduits of leaves of corn and rice. Cavitation events occur in certain areas of leaves and have a certain time frame. Before XPCM experiment, several preliminary experiments were done as follows: 1) Paraffin sections of leaves of different species were observed to select samples and to determine the occurrence area of cavitation event of leaves. 2) The time frame of cavitation occurrence was determined by ultrasonic emission. 3) The water potentials of leaves were determined, to know the water state of the leaves during cavitation. Locked the area and time frame of cavitation event in the leaves, consecutive XPCM images of cavitation process were more easily acquired. The images show that the phenomenon of gas bubble fully filling conduits for an instant took place in intact conduits of detached leaves of corn and rice more easily. It is that the gas diffusing in a moment was caused by the explosion of the air seeds which had entered in the intact conduits of the leaves. For living plants, it is suggested that the explosion event of cavitation is the most important for embolism formation.展开更多
目的分析西安市某三甲医院药字号肠内营养制剂的使用情况,为临床合理使用药字号肠内营养制剂提供参考。方法从该院信息管理系统(hospital information system,HIS)中提取2019年至2021年药字号肠内营养制剂的用药数据,对全院药字号营养...目的分析西安市某三甲医院药字号肠内营养制剂的使用情况,为临床合理使用药字号肠内营养制剂提供参考。方法从该院信息管理系统(hospital information system,HIS)中提取2019年至2021年药字号肠内营养制剂的用药数据,对全院药字号营养制剂的销售情况及构成比、用药频度(defined daily doses,DDDs)、限定日费用(defined daily cost,DDC)等药物经济学指标以及各科室使用量进行回顾性统计分析。结果该院肠内营养制剂使用量2019年至2020年呈增长趋势,但2021年度有所下降。DDDs排名前3的制剂分别是百普力、能全素和瑞素。DDC排序前3的制剂是瑞先、百普素和百普力。肝胆外科3年间短肽类制剂使用量位居第1,整蛋白制剂使用量前3的科室分别是神经内科、重症医学科和康复科。结论该院肠内营养制剂平均年使用量偏低,肠内营养与肠外营养治疗比例不均衡。整蛋白制剂瑞先价格较高,患者经济负担重,肠内营养制剂应保质保效,同时应降低价格。临床肠内营养治疗率远低于实际营养不良反应发生率,营养科需进一步加强与临床科室的沟通协作,提高住院患者肠内营养治疗比例。展开更多

Geomechanical parameters of intact metamorphic rocks determined from laboratory testing remain highly uncertain because of the great intrinsic variability associated with the degrees of metamorphism.The aim of this pa...Geomechanical parameters of intact metamorphic rocks determined from laboratory testing remain highly uncertain because of the great intrinsic variability associated with the degrees of metamorphism.The aim of this paper is to develop a proper methodology to analyze the uncertainties of geomechanical characteristics by focusing on three domains,i.e.data treatment process,schistosity angle,and mineralogy.First,the variabilities of the geomechanical laboratory data of Westwood Mine(Quebec,Canada)were examined statistically by applying different data treatment techniques,through which the most suitable outlier methods were selected for each parameter using multiple decision-making criteria and engineering judgment.Results indicated that some methods exhibited better performance in identifying the possible outliers,although several others were unsuccessful because of their limitation in large sample size.The well-known boxplot method might not be the best outlier method for most geomechanical parameters because its calculated confidence range was not acceptable according to engineering judgment.However,several approaches,including adjusted boxplot,2MADe,and 2SD,worked very well in the detection of true outliers.Also,the statistical tests indicate that the best-fitting probability distribution function for geomechanical intact parameters might not be the normal distribution,unlike what is assumed in most geomechanical studies.Moreover,the negative effects of schistosity angle on the uniaxial compressive strength(UCS)variabilities were reduced by excluding the samples within a specific angle range where the UCS data present the highest variation.Finally,a petrographic analysis was conducted to assess the associated uncertainties such that a logical link was found between the dispersion and the variabilities of hard and soft minerals.展开更多
