Evaluation of the Effectiveness of Community Pharmacy Services on Rational Medication Use in Elderly Patients with Hypertension and Diabetes

Objective:To explore the effect of community pharmacy services on rational medication use in elderly patients with hypertension and diabetes.Methods:Between November 2022 and December 2023,80 elderly patients with hypertension and diabetes were selected and randomly divided into a control group(routine medication guidance)and an observation group(community pharmacy services),with 40 subjects each.The medication effect scores,blood pressure,blood sugar levels,and quality of life scores before and after intervention were compared between the two groups.Results:Comparison of pharmaceutical knowledge,medication compliance,and safe medication behavior scores showed that the observation group had higher scores as compared to the control group(P<0.05);blood pressure(systolic blood pressure,diastolic blood pressure,heart rate)and blood sugar(fasting blood glucose,2 h postprandial blood glucose,glycated hemoglobin)index levels were compared,and the observation group’s index levels were lower than those of the control group(P<0.05);the scores of physical health,mental health,social relationships,and environment in the observation group were higher than those in the control group(P<0.05).Conclusion:Community pharmacy services improved the rational medication effect of elderly patients with hypertension and diabetes,and improved their blood pressure,blood sugar control levels,and quality of life.

Outcomes of Integrated Community Care Interventions for Frail Elderly People: A Literature Review

This study aimed to summarize the outcome measures and the significant outcome of effective integrated community care for frail elderly people through a literature review. A literature search was conducted using the Cochrane Library and PubMed for articles published up to November 2016 with the following search terms: Integrated community care, primary care, community, frail elderly, and effectiveness. A total of 106 articles were identified, of which eight with an interventional research design the inclusion criteria. All outcome measures were classified into the three categories: Functional abilities, quality of life, and health. As the significant effect, the physical function was perceived in four references, the quality of life in one reference, and the mental health in one reference. The evidence of the effectiveness of integrated community care seemed to be lacking. More studies will need to be conducted.

Implementation Strategy of“Eight in One”of Integrated Medical and Care Service System in China:A Case Study of Anshan City of Liaoning

With the gradual intensification of aging in China,the issue of elderly care has become the primary issue that needs to be urgently solved in society.The construction of a reasonable and scientific integrated medical and care service system can not only efficiently allocate medical resources and services,but also better meet the needs of the elderly.Due to the involvement of multiple disciplines such as architecture,sociology,psychology,and behavioral science in the construction of the system,as well as the restriction of various objective factors such as medical capacity,spatial scale,and operating costs,the government and elderly care institutions have always been unable to find the best solution for how to scientifically and reasonably construct an integrated medical and care service system.This paper is based on Anshan City,Liaoning Province,which has prominent aging issues and distinct characteristics of the elderly population.Through extensive field research in elderly care institutions,and face-to-face communication with personnel from relevant government departments such as the Municipal Commission on Aging,the Civil Affairs Bureau,the Health Commission,the Medical Insurance Bureau,and the Human Resources and Social Security Bureau,it truly understands the problems that arise in the construction of the urban integrated medical and care service system.From three aspects:urban situation,institutional situation and the needs of the elderly,it is proposed to establish a clear departmental linkage mechanism with clear rights and responsibilities,a policy guarantee mechanism tailored to local conditions,a multi-measure operation mechanism,a technology first intelligent response mechanism,a warm and efficient service mechanism for the people,an overall layout mechanism,an evaluation and supervision mechanism for full process control,and a talent supply mechanism of external introduction and internal training.It aims to provide reference for the construction of an integrated medical and care service system in similar cities.

Optimization Strategies of Beijing Elderly Care Service Stations Based on Questionnaire Survey Method: A Case Study of Zhanlan Road Street of Xicheng District

4 elderly care service stations in Zhanlan Road Street,Xicheng District,Beijing are selected,and questionnaires are designed and distributed to the surrounding elderly population to understand their needs and satisfaction with the station environment.By observing elderly care service stations on site,the characteristics,obstacles,and shortcomings of the environment are recorded,and relevant data are collected and analyzed,such as the characteristics of the elderly population being interviewed,the planning and design data of the station environment,and the distribution of service facilities.The overall characteristics of the spatial environment of elderly care stations are summarized,and renovation measures and optimization suggestions are provided for the current shortcomings,thereby providing some basis for the spatial design of community elderly care service stations in the future.

Analysis of Professional Quality Cultivation Strategies for Infant and Child Care Service Talents from the Perspective of Collaboration of Medical Care,Parenting,and Education,and Integration of Industry and Education

The collaboration of medical care,parenting,and education aims to integrate nursing,midwifery,infant and child care services,and management with speech and hearing rehabilitation technology,among other elements related to the infant care industry chain.This integration targets pediatrics talent training in nine infant care positions,including nursing,infant health care and management,and child rehabilitation,to ensure that the capabilities and quality of professional talents can meet the health care needs of infants and young children.This article briefly explains the background of the“collaboration of medical care,parenting,and education,and integration of industry and education.”It analyzes the necessity of cultivating infant and child care service talents based on the perspective of“collaboration of medical care,parenting,and education,and integration of industry and education.”Based on this perspective,we conducted an in-depth study of the cultivation of professional qualities of infant and child care service talents.

Significant Differences in Elderly Health between Urban and Rural Communities: A Literature Review

This study aimed to examine measures pertaining to elderly health in urban versus rural settings, and to identify differences in the health of elderly people living in urban and rural communities through a literature review. An electronic literature search was performed using PubMed for English articles published in peer-reviewed journals up to August 2018, with the following search terms: “urban”, “rural”, “comparison of community”, “elderly health”, and “comparison of community health”. A total of 35 articles were extracted for a critical full-text review, and six articles that met the inclusion criteria were subjected to analysis. Measures related to elderly health in urban and rural communities were classified into the following three categories: functional abilities, health, and health perception. Five of the six articles described functional abilities (e.g., social function) and health (e.g., mental health, depression) as categories with significant differences in elderly health between urban and rural communities. The results suggest that elderly health measures related to social function and mental health or depression are more important outcome measures of effective person-centered integrated community care systems from the perspective of community characteristics. As there were only a few articles reporting on elderly health according to differences in environment between urban and rural communities, further investigation is globally warranted.

Because of family miniaturization, core, and currently limited social accumulation are difficult to meet the demand of the elderly in long-term care.Community provides the elderly long-term care services, and provides the elderly all kinds of household helping services. They just make up for the inadequacy of the family and society.And how to make the community elderly long-term care services become more suitable for the needs of the elderly at home, provide better service for the elderly, are our current problems to be solved.

Patient-centered medical home and integrated care in the United States: An opportunity to maximize delivery of primary care

The reciprocal relationship between mental and physical health is well established.Undiagnosed,untreated,and poorly managed mental health conditions are associated with numerous physical health complications,poor treatment adherence,and decreased quality of life.Despite growing evidence regarding the importance of effectively addressing these conditions in primary care,the rates of identification remain low and follow-up and management by primary care providers has been criticized.The objective of this review was to demonstrate the role of Patient-Centered Medical Home(PCMH)and mental health integration in addressing comprehensive health care needs in primary care patients,and to describe common barriers and facilitators to the implementation of these types of programs.

居家社区医养结合服务利用及满意度现状研究

目的 了解居家社区医养结合服务的利用情况和满意度现状,提出促进医养结合发展的对策和建议。方法 通过目的抽样法于2023年5-7月在我国东、中、西部分别选取无锡市、长沙市、成都市对2 125名60岁及以上正在接受或曾接受过服务的老年人进行问卷调查。采用描述性分析、卡方检验、单因素方差分析、多元线性回归等统计学方法对有效问卷数据进行分析。结果 不同地区的就医习惯、服务方式、常用药的获取情况差异具有统计学意义(P<0.05)。服务总体满意度均值为(4.32±0.625)分,处于“满意”与“非常满意”之间。线性回归结果显示,失能程度、慢性病患病情况、地区差异、就医习惯、服务方式、常用药获取情况、地理可及性情况是服务满意度的影响因素(P<0.05)。结论 老年人所接受的居家社区医养结合服务对于基层医疗卫生机构利用情况较好,服务总体满意度较高,但存在地区之间发展不平衡的问题。应进一步以基层医疗卫生机构为重点发展居家社区医养结合服务,完善医保支付机制,加快建立健全长护险制度,不断提高居家社区老年人的满意度和居家社区医养结合服务质量。

我国养老产业发展“拐点”研判——基于多案例研究的典型事实

随着人口老龄化进程加速,我国的养老需求结构正在从生存型向发展型转变,这将为养老产业的发展带来广阔的市场空间,但当前我国养老产业发展相对滞后,仍面临严重的发展困境。我国养老产业发展的“拐点”何时到来?为回答好这个问题,本文以案例研究和实证分析相结合的研究框架,首先,以法、德、日、美四国为例,系统梳理其养老产业发展历程,总结归纳出养老产业发展“拐点”出现的共性特征;其次,聚焦日本养老产业发展轨迹,重点关注其介护险实施前后养老产业相关重要指标变化;最后,选取九个养老产业相关指标,基于指数平滑、灰色模型等方法对中国养老产业未来情况进行预测,并与日本可比时期指标情况进行比较,判断中国养老产业“拐点”出现的可能时间。研究发现:养老产业发展“拐点”的出现具有四个共性特征,分别是三支柱养老金体系改革、长护险制度全面实施、多元化服务助推医养结合、代际转换背后的产业增量机会,而中国养老产业发展“拐点”可能出现的窗口期将在2026—2030年间。

健康老龄化视域下“医养结合嵌入式”养老服务的质量评价研究

健康老龄化视角下的嵌入式医养结合养老模式是未来老年人养老质量的保障,也体现了老年人对未来老年幸福美好生活的需求。但对该模式的研究多集中在理论研究层面,对实证的研究较为缺乏。本研究基于医养结合的嵌入式养老服务的评价方法和评价原则,对构建嵌入式医养结合养老模式的评价指标体系进行了较为详尽的阐释。发现目前嵌入式养老机构重在养老轻在医疗,医疗机构与养老机构之间存在着衔接不足的问题,具体表现在医护人员的服务主体不够专业、服务项目不够落实、医疗机构与养老机构的资源整合碎片化以及机构管理的多头监管等问题。研究针对相关问题提出了建议,对我国医养结合的嵌入式养老模式的进一步实施具有一定借鉴意义。

农村医养结合型养老服务政策的演进逻辑

伴随着健康中国战略的稳步推进,我国农村医养结合型养老服务体系经历了从供给不足到供需相对均衡的发展过程,逐步形成了与经济社会持续健康发展相协调的高质量社会养老服务体系,在满足公众需求、缓解社会矛盾、促进社会公平、增进老年福祉等方面发挥了重要作用。深入挖掘其在政策目标、政策内容、价值取向等方面的阶段性特征和变化趋势,通过社会网络分析发现,我国农村医疗卫生与养老服务相结合由点到面逐步拓展,二者由彼此独立走向有机融合,其结合、融合与整合的体制机制逐渐成熟,二者相结合由逐步成熟向深度融合阶段迈进。为了促进医养结合政策的有效衔接,我国应加快推进农村医养结合型养老服务体系建设,以县域医共体为核心发展农村医养结合型养老服务,探究不同类型政策工具之间的合理匹配,全方位提高农村医养结合型养老服务质量,不断提升老年人健康保障水平。

人口老龄化背景下农村医养结合服务的现状与对策——以唐山市乐亭县为例

以唐山市乐亭县农村医养结合服务模式和现状为例,针对目前农村地区医养结合服务存在的供需失衡、“医”与“养”资源融合不足、设施落后、缺乏专业服务人员等问题,提出完善政策体系和保障制度,强化医养衔接,开展多层次医养结合服务,加强人才队伍建设,构建医养结合信息化系统的对策建议,从而推动农村医养结合的发展。

基于米特-霍恩模型的中医药政策在医养结合执行中的分析研究

目的分析我国中医药政策在医养结合执行过程中存在的问题,为完善中医药政策提供借鉴,以此更好地推动医养结合服务的发展和进步。方法采用米特-霍恩模型:政策标准与目标、政策资源、执行方式、执行机构特性、执行人员价值取向、系统环境六大因素为框架,分析中医药政策在医养结合服务执行中存在的问题。结果中医药政策越来越受重视,但在执行中仍受到政策目标不清晰、资源不足、执行方式单一、执行机构特性缺乏规划、执行人员价值取向低和复杂的系统环境等因素的影响。结论为解决中医药政策在医养结合执行中存在的问题,提出建议:完善顶层设计,细化政策实施目标;强化资源保障,提升政策执行效率;丰富执行方式,确保执行效果;多部门联动,加强沟通交流;优化政策执行环境。

家庭养老床位政策的府际差异与模式研究——基于26个市级政策文本的比较分析

目的:家庭养老床位作为链接基层医疗服务资源与养老服务的中介,可有效解决社区、机构养老照护资源失配、错配与家庭照护负担过重的问题,对该政策进行量化分析,对完善家庭养老床位政策内容,优化资源配置,推动家庭养老床位建设具有重要意义。方法:收集我国省会及试点成效较好城市出台的共26份家庭养老床位政策文本,运用文本挖掘和PMC指数模型,构建量化指标评价已有政策,并以上海、杭州、长春、苏州作为典型案例,梳理政策特征差异。结果:政策总体评价为合格等级,质量提升潜力较大;主要呈现学习-试验型、回应-赋能型、模仿-指令型三类政策模式;政策PMC指数在空间维度上存在差异,政策一致性呈现出东高西低的阶梯状,东、中、西、东北地区政策在不同层面各具优势。结论:我国家庭养老床位政策应根据各地区政策模式进行针对性改良,强化基层医养结合制度建设,通过科技赋能优化照护资源整合与配置,并大力发展养老产业,培育照护人才。

“互联网+医养”综合管理系统设计与应用

目的:巩固宁夏回族自治区(以下简称宁夏)“互联网+医疗健康”示范区建设成果,提升医养结合服务能力。方法:结合宁夏本地医养服务现状,应用大数据和互联网技术,构建基于互联网医疗的医养综合服务信息化体系,搭建适用于不同层级医养结合机构的医养协同综合服务信息化平台。结果:面向医养结合机构、社会养老机构、社区居家养老人群提供多方位联合体机制,形成线上线下一体化的全流程医养服务及管理模式,可为老年人提供全方位多层次连续性的医疗、养老等服务。结论:“互联网+医养”综合管理系统实现了医养融合互联互通全流程管理,是“互联网+医养”融合服务模式的积极探索。

基于PEST模型的我国农村医养结合养老服务发展策略研究

我国老龄人口比例逐年攀升,尤其是农村地区,老龄化现象更为严重。文章对我国农村医养结合养老服务的发展状况进行梳理,运用PEST分析模型对其在政治、经济、社会和技术4个方面所面临的困境进行分析研究。在借鉴国外医养结合养老服务实践经验的基础上,针对性地提出加强政策支持、拓宽资金来源、增强宣传和社会参与,以及完善信息建设与人才培养等方面的发展策略,以期对我国农村医养结合养老服务的后续研究与实践提供参考。

医养结合的中期照护服务——理念、现存问题及专业化发展路径

中期照护作为世界卫生组织推出的整合式连续性服务模式的重要环节,旨在恢复健康和自立生活,具备医养结合的典型特征,也是未来养老服务发展的一个重点领域。基于康复护理视角探索我国中期照护的专业化发展路径,认为中期照护可以填补老年人从医院回到社区和家庭期间出现的照护“真空”,能够起到预防老年人失能、残疾的作用,是实施积极应对人口老龄化国家战略的一项重要举措。从我国发展中期照护制度的现实需要和问题分析出发,总结发达国家和地区中期照护模式,将其归纳为机构型、社区型和居家型。其中机构型包含护士主导病房(nurse-led units)、老年日间病房(geriatric day-hospital)等机构嵌入服务,以及康复护理院或医护疗养院(nursing home)等独立机构。并借鉴其在组织体系、筹资及支付机制、服务内容、服务人员、质量监控等方面的专业经验,探讨我国中期照护的专业化发展路径。未来应加强养老保障中照护制度的顶层设计,完善多部门协同管理体系,建立健全老年综合评估体系,建设康复护理专业机构,制定服务标准和规范,跨学科培养组建专业团队,整合医保支付等多元筹资方式。

社区居家医养结合养老服务的发展策略研究——基于生态系统理论

目的:分析我国社区居家医养结合养老服务在推进过程中存在的问题,提出发展建议。方法:基于生态系统理论,从“宏观-中观-微观”三个层面进行探讨分析。结果:我国社区居家医养结合养老服务存在政策“碎片化”且部门权责重叠、筹资机制不健全、养老观念陈旧、基层医疗卫生资源供给不充足、养老服务人才资源匮乏、服务设施落后、宣传力度不足等问题。结论:完善法规体系,优化合作机制,加强财政支持,鼓励社会参与,完善人才培养、激励和保障机制,搭建智慧养老信息服务平台,创新养老产品和服务模式,营造良好风气,促进社区居家医养结合养老服务的健康发展。
