Objective: To review year 1956-2007 literature on acupuncture and moxibustion and explore the regularity of point selection and methods for acupuncture-moxibustion treatment of lumbar intervertebral disk herniation. ...Objective: To review year 1956-2007 literature on acupuncture and moxibustion and explore the regularity of point selection and methods for acupuncture-moxibustion treatment of lumbar intervertebral disk herniation. Methods: A comprehensive analysis was made by a method of metrological analysis according to the data from "China modem acupuncture-moxibustion information databank". Results: Points Huantiao (GB 30), Weizhong (BL 40), Yanglingquan (GB 34), and Jiaji (Ex-B 2) were most frequently used. Acupuncture was the most frequently used therapy and electroacupuncture, the next one. Conclusion: Electroacupuncture is a common and effective way to treat lumbar intervertebral disk herniation. Points Huantiao (GB 30), Weizhong (BL 40), Yanglingquan (GB 34), and Jiaji (Ex-B 2) can be used as a basic prescription for acupuncture-moxibustion treatment of lumbar intervertebral disk herniation. Method: All 200 cases were randomly allocated into an observation group (100 cases) and a control group (100 cases). Cases in the observation group were treated with electric acupuncture. Cases in the control group were treated with oral administration of Mobic. Result: The total effective rate in the observation group was 86.5%, and the total effective rate of the control group was 75.0%. There was a significant difference between the two groups (P〈0.01). Several studies been conducted to investigate the potential asso- ciation between IDD and F...Intervertebral disk degeneration (IDD) is strongly associated with genetic predisposition and environmental susceptibility. Several studies been conducted to investigate the potential asso- ciation between IDD and FokI polymorphism located in the gene encoding the vitamin D receptor (VDR), and inconsistent conclusions had been reached among different ethnic populations. In order to assess the association between the FokI polymorphism and the risk of IDD, we performed a comprehensive and systematic meta-analysis. Candidate articles were retrieved from PubMed, EMBASE, China National Knowledge Infrastructure (CNKI), and China Biology Medical (CBM) with strict inclusion criteria in January 2015. Among the 54 articles that were retrieved, only eight studies met the inclusion criteria. The pooled data analysis based on allele contrast, homozy- gore, heterozygote, dominant, and recessive models revealed no significant correlation between the FokI polymorphism and the risk of IDD. However, when stratified by ethnicity, significant associ- ations were detected for Hispanics based on allele contrast (OR = 1.395, 95% CI = 1.059-1.836, P = 0.018), homozygote (OR = 1.849, 95% CI = 1.001-3.416, P = 0.049), heterozygote (OR = 1.254, 95% CI = 1.049-1.498, P = 0.013), and dominant (OR = 1.742, 95% CI = 1.174-2.583, P = 0.006) models, and for Asians using the dominant model (OR = 1.293, 95% CI = 1.025-1.632, P --- 0.030), whereas there is no significant association detected for Caucasians. In conclusion, FokI polymorpbism is not generally associated with IDD, but there is increased risk for IDD in Hispanics and Asians carrying FokI allele T.展开更多

The author has treated 66 cases by massotherapy, who were confirmatively diagnosed as having protrusion of the lumbar intervertebral disc by X-ray film and computer-aided tomography, with satisfactory results as reported in the following.Clinical DataAmong the 66 cases in this series, 49 were male and 17 female, ranging in age from 26 to 59 years, averaging 36.2 years. All the patients had got lumbago and unilateral ischialgia, with the left side affected in 37 cases and the right side in 29 cases. The leg pain radiating to lateral malleolus was found in 28 cases, to dorsum of the foot in 24 cases, and to the toes in 14 cases. Intermittent claudication was present in 22 cases, numbness of the affected foot in 16 cases, pain exacerbated when coughing in 35 cases, scoliosis in 21 cases, and positive Lasegue's sign in 48 cases.展开更多

Objective:To investigate the inhibition effect of siRNA interference on NGF induced by inflammatory factor IL-6,and JUL—1 so as to provide novel targets for clinical treatment of discogenic low back pain.Methods:The ...Objective:To investigate the inhibition effect of siRNA interference on NGF induced by inflammatory factor IL-6,and JUL—1 so as to provide novel targets for clinical treatment of discogenic low back pain.Methods:The intervertebral disc nucleus and annulus fibrosus cells of rats were separated-The cells were co-cultured with different concentrations(10 nmol/L,20nmol/L,50 nmol/L,100 nmol/L)of IL-6 and IL-1β.The NGF-siRNA was leaded into the cocultured cells with its import ability assessed by flow cytometry instrument tests,hefore and after which the NCF mRNA expression was detected by real-time Q-PCR and the NGF content was detected by ELISA.Results:Flow cytometry instrument test results showed that the NGFsiRNA cell conversion rate was 99.8%.Real-time Q-PCR detection results showed that compared with negative control group,the NGF mRNA expression of co-cultured cells treated by 10 nmol/L,20 nmol/L,50 nmol/L,100 nmol/L IL-6 and IL-1βwere respectively raised 3.4,3.7,4.7,3.7 times which were all significantly down-regulated after the import of NGF-siRNA.EILSA detection results showed that compared with negative control group,the NGF content of cocultured medium treated by 10 nmol/L,20 nmol/L,50 nmol/L,100 nmol/L I-L6 and IL-1βwere respectively raised 2.9,3.3,4.5,7.4 times which were all significantly decreased after the import of NGF-siRNA.Conclusions:These molecular biological results suggest that inflammatory factor IL-6 and IL-1βcould stimulate NCF on intervertebral disc cells in vitro culture model and its efficiency is concentration dependent,while siRNA interference can inhibit the stimulation effect of IL-6 and IL-1βon intervertebral disc cell,which provides a new targets for the clinical treatment of discogenic low back pain.展开更多

Thoracolumbar interverbal disk herniation(TL-IVDH)is a common neurological disorder in dogs,which often affects the life quality of patients and their caregivers.Acupuncture is increasingly accepted by veterinarians a...Thoracolumbar interverbal disk herniation(TL-IVDH)is a common neurological disorder in dogs,which often affects the life quality of patients and their caregivers.Acupuncture is increasingly accepted by veterinarians and animal owners,and has been used in the treatment of neurologic disorders.This study aims to evaluate the effects of acupuncture treatment for canine TL-IVDH.Clinical cases from 2016 to 2021 were collected.According to the inclusion and exclusion criteria,94 TL-IVDH cases that received acupuncture treatment were selected.Clinical classification and efficacy criteria were formulated based on the Frankel Score.For each case,clinical presentation,improvement,recovery and recurrence were recorded.Male dogs with TL-IVDH accounted for 64%of all cases.The average age was 7.49 years old,the predominant breed was poodle,and the incidence site was concentrated from T11 to L1.The overall recovery rate after receiving acupuncture treatment was 79.78%,which was comparable when considering only cases with grade 2,3,or 4 spinal cord lesions(83.33%~85.19%).In conclusion,canine with grade 2,3,and 4 spinal cord lesions due to TL-IVDH receiving acupuncture had a high recovery rate,suggesting acupuncture could be a viable treatment options for canine with TL-IVDH.展开更多

Methods: Bac-to-Bac system was used for the construction of baculovirus Ac-C...Objective: To construct the recombinant baculovirus Ac-cytomegalovirus (CMV)-hSox9 for gene therapy of intervertebral disc degeneration. Methods: Bac-to-Bac system was used for the construction of baculovirus Ac-CMV-hSox9. The cDNA of hSox9 was first cloned into a plasmid vector under the control of CMV promotor to generate the donor plasmid pFastBacDul-green fluorescene protein ( GFP )-CMV (pgGC)-hSox9. The resultant plasmid was transformed into DH10Bac cells and then the transformation mixture was spread on Luria-Bertani (LB) agarose culture medium containing isopropyl-β-D-thiogalactoside (IPTG), X-gal, gentamicin, kanamycin and tetracycline. The white colonies were selected and cultured for amplification, and the hSox9 Bacmid DNA was extracted. After verification, recombinant baculovirns Ac-CMV-hSox9 was obtained through transfecting Sf 21 cells. The expression of hSox9 gene in the intervertebral disc cells in rabbits was determined by Western blotting and immunohistochemical staining. Results: Polymerase chain reaction ( PCR ) confirmed the presence of hSox9 gene in the recombinant baculovirus and the Sf 21 cells transfected by the baculovirus showed the expression of fluorescence protein. Western blotting and indicated that exogenous hSox9 disc cells. staining analysis Conclusions: The successful construction of the recombinant baculovirus Ac-CMV-hSox9 and the confirmation of the target gene expression provides a novel expression vector system for basic research and clinical treatment of intervertebral degenerative disc disease.展开更多
Objective: To compare the influence of normal and degenerative discs on stress distribution of the thoracolumbar vertebrae under destructive load, explore the biomechanical background and clinical meaning and provide ...Objective: To compare the influence of normal and degenerative discs on stress distribution of the thoracolumbar vertebrae under destructive load, explore the biomechanical background and clinical meaning and provide theoretical basis for clinical diagnosis and treatment. Methods: A mechanical model of thoracolumbar motion segment of normal and degenerative discs was built with a three dimensional finite element method and three stresses of vertical compression, compressive flexion and distractive flexion were comparatively analyzed. Results: With vertical compression and compressive flexion loads, the thoracolumbar motion segment of the normal disc showed that the central part of the upper and lower end plates of the vertebrae and the central part of the trabecular bone adjacent to the end plate were loaded with the most intensive stresses, meanwhile, the postero lateral part of the annulus fibrosus was concentrated with stresses. Degenerative disc showed that the stress distribution of the trabecular bone was relatively averaged, the stresses of the central part adjacent to the end plate were low, while at the same time, the stresses of the peripheral part were elevated relatively. With distraction flexion load, the stresses of the cortex bone, trabecular bone, end plate and annulus fibrosus of the thoracolumbar vertebrae of degenerative discs were low, meanwhile, the stresses of the posterior structure of the vertebral body were relatively elevated compared with that of normal discs. Conclusions: There is difference in influence between normal and degenerative discs on the stress distribution of the thoracolumbar vertebrae with destructive load. The transferring way of load is changed after disc degeneration.展开更多

Three hundred cases...Objective: To observe the clinical efficacy of electroacupuncture and waist-building exercise in treating lumbar disk herniation. Methods: A multi-center, randomized controlled trial was adopted. Three hundred cases of lumbar disk herniations were divided into two groups, an observation group in which 149 cases were treated by electroacupuncture and waist-building exercise, and a control group in which 151 cases were treated by electroacupuncture alone. The clinical efficacy and long-term relapse rate were observed and compared between the two groups. Results: Among 149 cases in the observation group, 80 cases were cured, 57 cases improved, 12 cases failed and 4 cases relapsed; the cure rate was 53.7%, the total effective rate was 91.9% and the relapse rate was 5.0%. Among 151 cases in the control group, 74 cases were cured, 51 cases improved, 26 cases failed and 12 cases relapsed; the cure rate was 49.0%, the total effective rate was 82.8% and the relapse rate was 16.2%. There were significant differences in the cure rate and the total effective rate between the two groups (P〈0.05). Conclusion: Combined electroacupuncture and waist-building exercise had better effects than electroacupuncture in treating lumbar disk herniation.展开更多

文摘Objective:To investigate the inhibition effect of siRNA interference on NGF induced by inflammatory factor IL-6,and JUL—1 so as to provide novel targets for clinical treatment of discogenic low back pain.Methods:The intervertebral disc nucleus and annulus fibrosus cells of rats were separated-The cells were co-cultured with different concentrations(10 nmol/L,20nmol/L,50 nmol/L,100 nmol/L)of IL-6 and IL-1β.The NGF-siRNA was leaded into the cocultured cells with its import ability assessed by flow cytometry instrument tests,hefore and after which the NCF mRNA expression was detected by real-time Q-PCR and the NGF content was detected by ELISA.Results:Flow cytometry instrument test results showed that the NGFsiRNA cell conversion rate was 99.8%.Real-time Q-PCR detection results showed that compared with negative control group,the NGF mRNA expression of co-cultured cells treated by 10 nmol/L,20 nmol/L,50 nmol/L,100 nmol/L IL-6 and IL-1βwere respectively raised 3.4,3.7,4.7,3.7 times which were all significantly down-regulated after the import of NGF-siRNA.EILSA detection results showed that compared with negative control group,the NGF content of cocultured medium treated by 10 nmol/L,20 nmol/L,50 nmol/L,100 nmol/L I-L6 and IL-1βwere respectively raised 2.9,3.3,4.5,7.4 times which were all significantly decreased after the import of NGF-siRNA.Conclusions:These molecular biological results suggest that inflammatory factor IL-6 and IL-1βcould stimulate NCF on intervertebral disc cells in vitro culture model and its efficiency is concentration dependent,while siRNA interference can inhibit the stimulation effect of IL-6 and IL-1βon intervertebral disc cell,which provides a new targets for the clinical treatment of discogenic low back pain.

文摘Thoracolumbar interverbal disk herniation(TL-IVDH)is a common neurological disorder in dogs,which often affects the life quality of patients and their caregivers.Acupuncture is increasingly accepted by veterinarians and animal owners,and has been used in the treatment of neurologic disorders.This study aims to evaluate the effects of acupuncture treatment for canine TL-IVDH.Clinical cases from 2016 to 2021 were collected.According to the inclusion and exclusion criteria,94 TL-IVDH cases that received acupuncture treatment were selected.Clinical classification and efficacy criteria were formulated based on the Frankel Score.For each case,clinical presentation,improvement,recovery and recurrence were recorded.Male dogs with TL-IVDH accounted for 64%of all cases.The average age was 7.49 years old,the predominant breed was poodle,and the incidence site was concentrated from T11 to L1.The overall recovery rate after receiving acupuncture treatment was 79.78%,which was comparable when considering only cases with grade 2,3,or 4 spinal cord lesions(83.33%~85.19%).In conclusion,canine with grade 2,3,and 4 spinal cord lesions due to TL-IVDH receiving acupuncture had a high recovery rate,suggesting acupuncture could be a viable treatment options for canine with TL-IVDH.

文摘Objective: To construct the recombinant baculovirus Ac-cytomegalovirus (CMV)-hSox9 for gene therapy of intervertebral disc degeneration. Methods: Bac-to-Bac system was used for the construction of baculovirus Ac-CMV-hSox9. The cDNA of hSox9 was first cloned into a plasmid vector under the control of CMV promotor to generate the donor plasmid pFastBacDul-green fluorescene protein ( GFP )-CMV (pgGC)-hSox9. The resultant plasmid was transformed into DH10Bac cells and then the transformation mixture was spread on Luria-Bertani (LB) agarose culture medium containing isopropyl-β-D-thiogalactoside (IPTG), X-gal, gentamicin, kanamycin and tetracycline. The white colonies were selected and cultured for amplification, and the hSox9 Bacmid DNA was extracted. After verification, recombinant baculovirns Ac-CMV-hSox9 was obtained through transfecting Sf 21 cells. The expression of hSox9 gene in the intervertebral disc cells in rabbits was determined by Western blotting and immunohistochemical staining. Results: Polymerase chain reaction ( PCR ) confirmed the presence of hSox9 gene in the recombinant baculovirus and the Sf 21 cells transfected by the baculovirus showed the expression of fluorescence protein. Western blotting and indicated that exogenous hSox9 disc cells. staining analysis Conclusions: The successful construction of the recombinant baculovirus Ac-CMV-hSox9 and the confirmation of the target gene expression provides a novel expression vector system for basic research and clinical treatment of intervertebral degenerative disc disease.
文摘Objective: To compare the influence of normal and degenerative discs on stress distribution of the thoracolumbar vertebrae under destructive load, explore the biomechanical background and clinical meaning and provide theoretical basis for clinical diagnosis and treatment. Methods: A mechanical model of thoracolumbar motion segment of normal and degenerative discs was built with a three dimensional finite element method and three stresses of vertical compression, compressive flexion and distractive flexion were comparatively analyzed. Results: With vertical compression and compressive flexion loads, the thoracolumbar motion segment of the normal disc showed that the central part of the upper and lower end plates of the vertebrae and the central part of the trabecular bone adjacent to the end plate were loaded with the most intensive stresses, meanwhile, the postero lateral part of the annulus fibrosus was concentrated with stresses. Degenerative disc showed that the stress distribution of the trabecular bone was relatively averaged, the stresses of the central part adjacent to the end plate were low, while at the same time, the stresses of the peripheral part were elevated relatively. With distraction flexion load, the stresses of the cortex bone, trabecular bone, end plate and annulus fibrosus of the thoracolumbar vertebrae of degenerative discs were low, meanwhile, the stresses of the posterior structure of the vertebral body were relatively elevated compared with that of normal discs. Conclusions: There is difference in influence between normal and degenerative discs on the stress distribution of the thoracolumbar vertebrae with destructive load. The transferring way of load is changed after disc degeneration.
文摘Objective: To observe the clinical efficacy of electroacupuncture and waist-building exercise in treating lumbar disk herniation. Methods: A multi-center, randomized controlled trial was adopted. Three hundred cases of lumbar disk herniations were divided into two groups, an observation group in which 149 cases were treated by electroacupuncture and waist-building exercise, and a control group in which 151 cases were treated by electroacupuncture alone. The clinical efficacy and long-term relapse rate were observed and compared between the two groups. Results: Among 149 cases in the observation group, 80 cases were cured, 57 cases improved, 12 cases failed and 4 cases relapsed; the cure rate was 53.7%, the total effective rate was 91.9% and the relapse rate was 5.0%. Among 151 cases in the control group, 74 cases were cured, 51 cases improved, 26 cases failed and 12 cases relapsed; the cure rate was 49.0%, the total effective rate was 82.8% and the relapse rate was 16.2%. There were significant differences in the cure rate and the total effective rate between the two groups (P〈0.05). Conclusion: Combined electroacupuncture and waist-building exercise had better effects than electroacupuncture in treating lumbar disk herniation.
