BACKGROUND:The specificity in discriminating pancreatitis is limited in the positron emission tomography(PET)using Fluorine-18-fluorodeoxyglucose.Furthermore,PET is not widely available compared to the single photon emission computed tomography(SPECT).Since amino acids play a minor role in metabolism of inflammatory cells,the potential of the SPECT tracer,3-[ 123 I]iodo-L-α-methyltyrosine(123I-IMT),for detecting pancreatic cancer was examined in xenotransplantation models of human pancreatic carcinoma in mice. METHODS: 123 I-IMT was injected to eight mice inoculated with subcutaneous or orthotopic pancreatic tumors.Fused high-resolution-micro-SPECT(Hi-SPECT)and magnetic resonance imaging were performed.The gene expression level of L amino acid transport-system 1(LAT1)was analyzed and correlated with tumor uptake of 123 I-IMT. RESULTS:A high uptake of 123 I-IMT was detected in all tumor-bearing mice.The median tumor-to-background ratio (T/B)was 12.1(2.0-13.2)for orthotopic and 8.4(1.8-11.1)for subcutaneous xenotransplantation,respectively.Accordingly, the LAT1 expression in transplanted Colo357 cells was increased compared to non-malignant controls.CONCLUSIONS:Our mouse model could show a high 123 I-IMT uptake in pancreatic cancer.Fused MRI scans facilitate precise evaluation of uptake in the specific regions of interest.Further studies are required to confirm these findings in tumors derived from other human pancreatic cancer cells.Since amino acids play a minor role in the metabolism of inflammatory cells,the potential for application of 123 I-IMT to distinguish pancreatic tumor from inflammatory pancreatitis warrants further investigation. 展开更多

目的探讨123碘-间碘苄胍(^(123)I-metaiodobenzylguanidine,^(123)I-MIBG)平面显像和单光子发射型计算机断层成像(single-photon emission computed tomography,SPECT/CT)在儿童神经母细胞瘤Curie评分中的价值。方法分析211例神经母细... 目的探讨123碘-间碘苄胍(^(123)I-metaiodobenzylguanidine,^(123)I-MIBG)平面显像和单光子发射型计算机断层成像(single-photon emission computed tomography,SPECT/CT)在儿童神经母细胞瘤Curie评分中的价值。方法分析211例神经母细胞瘤患儿的^(123)I-MIBG平面显像和SPECT/CT断层显像的图像,所有患儿均属于3期或4期的高危患者。收集^(123)I-MIBG平面显像和SPECT/CT断层显像的Curie评分,以病理结果、其他相关的影像学检查及临床随访为神经母细胞瘤病灶诊断的依据,分别比较两种显像方式的差异性,以及它们与金标准的一致性和诊断准确率。结果在211例患儿中,48.82%(103/211)的患儿基于^(123)I-MIBG平面显像和SPECT/CT断层显像的Curie评分是一致的。51.18%(108/211)的患儿^(123)I-MIBG平面显像和SPECT/CT断层显像差异有统计学意义。123 I-MIBG平面显像的Curie评分为0(0,1)分,SPECT/CT断层显像的Curie评分为0(0,2)分,两种显像方式的Curie评分差异有统计学意义(Z=-3.46,P<0.001)。依据Curie评分的结果,将患者分为高分组(≥2)和低分组(<2),两种显像方式的分组差异有统计学意义(P=0.001)。^(123)I-MIBG平面显像和SPECT/CT断层显像的诊断正确率分别为51.66%和93.84%,两种显像方法的诊断准确率差异有统计学意义(P<0.001);它们与金标准的诊断一致性分别为0.44(0.34~0.54)和0.90(0.85~0.96),相较于^(123)I-MIBG平面显像,SPECT/CT断层显像具有极好的一致性。结论与^(123)I-MIBG平面显像相比,SPECT/CT断层显像具有更高的诊断准确率及诊断一致性;基于^(123)I-MIBG SPECT/CT断层显像的Curie评分,对患儿全身肿瘤情况的半定量评估具有非常重要的意义。 展开更多

心肌^(123)I标记苄基胍闪烁摄影与脑钠肽在评估慢性心力衰竭长期预后中的价值

目的探讨心肌123I标记苄基胍(123I-MIBG)闪烁摄影及血浆脑钠肽(BNP)浓度在评价慢性心力衰竭(CHF)远期预后中的价值。方法64位CHF患者接受123I-MIBG闪烁摄影,血浆BNP浓度检测,静息左室射血分数(rest-LVEF)测定和NYHA心脏功能分级的评估... 目的探讨心肌123I标记苄基胍(123I-MIBG)闪烁摄影及血浆脑钠肽(BNP)浓度在评价慢性心力衰竭(CHF)远期预后中的价值。方法64位CHF患者接受123I-MIBG闪烁摄影,血浆BNP浓度检测,静息左室射血分数(rest-LVEF)测定和NYHA心脏功能分级的评估。根据各参数中位数分组,连续随访54个月,观察终点为心源性死亡。结果随访期末发现123I-MIBG闪烁摄影照片的心脏/纵膈(H/M)比值和血浆BNP浓度与CHF远期死亡有着显著的相关性(r分别为-0.592和0.421;P值分别为0.000和0.001);rest-LVEF和NYHA心脏功能分级与死亡没有相关性(r分别为-0.057和0.110;P值分别为0.652和0.384);123I-MIBG闪烁摄影照片的H/M值低于中位数组(Ⅰ组)和高于及等于中位数组(Ⅱ组)之间死亡例数和血浆BNP值之间有明显的统计学差异(P值分别为0.001和0.003),两组的rest-LVEF和NYHA心脏功能分级之间没有统计学意义(P值分别为0.195和0.781);H/M和BNP值高于等于中位数组与低于中位数组死亡相对危险比分别为4.66和2.56(95%CI),差异有统计学意义(P分别为0.001和0.020),但是高于等于和低于中位数的rest-LVEF和NYHA心脏功能分级组之间死亡相对危险比没有统计学意义(P分别为1.000和0.738)。结论心肌123I-MIBG闪烁摄影及血浆BNP浓度与CHF远期死亡密切相关,可以用来评价CHF的远期预后。 展开更多

The relation between ADHD and ANS-CNS integration

Background: Attention deficit hyperactivity disorder (ADHD) was long considered to be limited to children and adolescents, but it is now known that ADHD symptoms may persist into adulthood. It is plausible that the etiology of ADHD is not one-dimensional, but instead involves various neuroanatomical and neurochemical systems, with the causes of the main abnormalities believed to be catecholaminergic. Iodine-123-labeled meta-iodobenzylguanidine (123I-MIBG) is a physiological analogue of norepinephrine (NE). To the best of our knowledge, there are no reports about the abnormalities of MIBG scintigraphy in patients with ADHD. The cases presented are male adult patients with different comorbid psychiatric disorders. Case presentation: The cases presented are male adult patients with different psychiatric disorders. Case 1 was a 52-year-old male residential construction foreman, who had been diagnosed with acute schizophrenia-like psychotic disorder. 123I-MIBG planar and SPECTs of the studies were performed at 20 min (early phase) and 3 h (late phase) after intravenous injection of 111 MBq. Planar images were processed to determine the heart (H) to mediastinum (M) ratio (H/M). The early and late H/M ratios were 1.22 and 1.07, respectively. Case 2 was a 52-year-old male civil servant who was diagnosed with adjustment disorder. The early and late H/M ratios were 1.98 and 2.08, respectively. Conclusion: Clinical studies make it clear that symptoms of ADHD are more heterogenous and subtle in adults than children. Adult ADHD is significantly commorbid with wide range of other 12-month disorders. MIBG imaging can be useful to diagnose patients with preexisting psychiatric disorders and ADHD. It might be possible to diagnose objectively though the psychiatric statuses resemble or coexist with other psychiatric disorders. Needless to say, further research is important. 展开更多
