In low- and middle-income countries such as the Congo, access to dialysis is low and inequitable. The prevention of CKD involves raising awareness among patients at risk, such as those suffering from arterial hypertension (AH), by improving their knowledge of CKD. The objectives of our work were to determine the level of knowledge about CKD among hypertensive patients and to identify the factors associated with a low level of knowledge. Methodology: We conducted a 3-month descriptive and analytical cross-sectional study from 1 August to 30 October 2023 in 3 large public hospitals in Brazzaville (capital of the Republic of Congo). We included: hypertensive patients aged 18 and over who had freely consented to participate in our study and were able to answer the questions on the survey form. Patients with known hypertension who had been followed for less than 3 years and those with known chronic renal failure were not included. Results: The mean age was 58.4 ± 14.4 years (29 - 88 years). There were 121 men and 150 women (sex ratio = 0.8). All the patients were educated;37.2% with a higher level of education and 13.6% with primary education. 24 patients (9%) had a good level of knowledge about CKD and 153 (56%) had poor knowledge. A good level of knowledge was associated with the duration of hypertension, intellectual level and the existence of associated heart disease. Conclusion: Our study reveals a significant lack of knowledge about chronic kidney disease among hypertensive patients in Brazzaville.展开更多

Purpose and Objectives: To appraise the community that acknowledges burns, first aid, and associated factors among the community population in Jazan City, Saudi Arabia. The paper aims to identify limitations to encourage additional research and persuade legislators to develop improved burn-injury care recommendations and training programs. Materials and Methods: An observational-based sample survey was conducted among the people who live in Jazan City aging 13 years or more, during April 5 to May 5, 2023. Data collection was done by a validated online self-administrated questionnaire sent randomly to community members in different parts of Jazan City via social media platforms. Collected data were coded and cleaned by an excel program, and finally exported on SPSS 26.0 software. The variables were analyzed using descriptive statistics like frequencies and percentages. Also, the Chi-square test was used to investigate the relation between different variables, with a significance value of P Results: This study included 243 participants (about 62%) among them were mostly male participants (151) having a university degree. The majority of participants 75% did not take any form of BFA training in the past. This study shows that 69.9% of the participants have inadequate awareness, despite 72% having a constructive attitude towards burn first aid. Previous burn-related first aid training was significantly associated with participants’ knowledge of BFA at a p-value less than 0.05. Conclusion: This study indicates a high frequency of Jazan population having inadequate knowledge of burn first aid despite the high prevalence of a favorable attitude. There is a need to develop an effective nationwide burn prevention program and early burn first aid treatment in Saudi Arabia and promote a consistent guideline for burn first aid.展开更多

This study was conducted and designed to educate society about scoliosis and measure the knowledge and awareness about it. Methods: This was a cross-sectional study conducted on members of the Saudi Society who have consented to participate in this research between the 8th of January and 5th of February 2023. Results: A total of 427 people agreed to participate in the study. The highest percentage of the participants was female 69.1%. About 41% participants out of 427 were in the age group 18-25 years. Most of the participants were from the Middle Region. (71.9%) of participants were unfamiliar with scoliosis, while (72.1%) had no prior knowledge of the condition. Conclusion: This study indicates insufficient awareness and knowledge about scoliosis in Saudi Arabia. Therefore, further studies are needed to raise awareness about scoliosis and help people to learn more about this condition.展开更多

The rate of early detection of prostate cancer is low in Türkiye. Therefore, it is important to measure the level of awareness regarding prostate cancer. In our research, we investigate the knowledge levels of prostate cancer among adult individuals in four different geographic region in Türkiye. In addition, we aimed to compare the level of awareness regarding prostate cancer and the depression and anxiety levels among the individuals. The prostate cancer awareness level survey was selected as the data collection tool. In the survey, 20 questions are asked to measure the knowledge level of the participants about prostate cancer. In addition, an evaluation of anxiety and depression was conducted by using the Hospital Anxiety and Depression Scale (HADS). Between April 2022 and December 2022, 834 participants were reached.72.9% of the participants answered the questionnaire correctly. A significant difference was found in terms of correct response rates in four different provinces located in four different regions of Türkiye (79% in Eskisehir, 75.2% in Canakkale, 73% in Ankara, and 54.4% in Maras;p < 0.05). According to the HADS scale, 240 (28.8%) individuals were found to have anxiety and 129 (15.5%) of them had depression. The knowledge level of most of the participants about prostate cancer was found to be above the average. There was a significant difference between provinces in terms of knowledge levels. A significant relationship was observed between depression and knowledge level. However, no significant difference was found for anxiety.展开更多

Objective: To assess the knowledge of medical students about pain and palliative care, as well as to identify their perception of teaching these topics during hospitalization. Methods: A cross-sectional observational study, with a descriptive and exploratory approach, data collection for which was carried out between August and November 2020. The target population was medical students, who responded to an online survey of a quantitative, anonymous and follow-up nature. The survey study variables concerned knowledge about pain management and palliative care. Results: An expressive majority of academics showed difficulty in understanding the pathophysiology of pain related to prescribing drugs for pain management purposes, and all of them believe that it is necessary to acquire more knowledge about pain treatment. In parallel, only 9.3% report having received sufficient information regarding palliative care during medical school. Conclusion: The results suggest a certain lack of knowledge and insecurity among medical school students with respect to pain management and care for patients receiving palliative care. The didactical approach to this theme is still deficient in the medical curriculum and requires immediate improvement and new proposals that address the training of these professionals in a more specific and effective way.展开更多

It often leads to disabilities and handicaps. In Africa, epilepsy is almost exclusively treated by general practitioners (GPs) because of a shortage of epilepsy specialists. It is therefore important to know the level of knowledge about epilepsy among GPs in order to improve their skills. Objectives: To assess the level of knowledge about the management of anti-epileptic drug treatment among GPs in Brazzaville;to investigate the relationship between demographic factors and GPs’ knowledge. Methods: This was a cross-sectional analytical study. It was conducted from 20 July to 1 September 2021. It focused on GPs working in public hospitals and private care centers in Brazzaville. Information on treatment aspects was collected through a standardized 11-item questionnaire. Results: Among the 137 participants, there were 84 (61.3%) men and 53 (38.7%) women. Of these participants, 36 (26.3%) were trained in Congo versus 101 (73.7%) in other countries. Only 21 (15.3%) GPs had good knowledge about the management of anti-epileptic drug treatment. The overall average knowledge score among GPs was low (31.4%). No significant associations were found between low and good levels of knowledge and gender (OR = 1.03;95% CI = 0.40 - 2.68;p = 1.000), age groups (OR 0.05), training country (OR = 0.62;95% CI = 0.19 - 1.98;p = 0.591), practice hospital (OR = 0.40;95% CI = 0.05 - 3.20;p = 0.695) and duration of professional experience (OR 0.05). Conclusion: The study population has insufficient knowledge about the management of anti-epileptic drug treatment. Demographic factors have no impact on GPs’ knowledge. Epilepsy education programs are needed to improve GPs’ knowledge and skills.展开更多
High-efficiency and low-cost knowledge sharing can improve the decision-making ability of autonomous vehicles by mining knowledge from the Internet of Vehicles(IoVs).However,it is challenging to ensure high efficiency...High-efficiency and low-cost knowledge sharing can improve the decision-making ability of autonomous vehicles by mining knowledge from the Internet of Vehicles(IoVs).However,it is challenging to ensure high efficiency of local data learning models while preventing privacy leakage in a high mobility environment.In order to protect data privacy and improve data learning efficiency in knowledge sharing,we propose an asynchronous federated broad learning(FBL)framework that integrates broad learning(BL)into federated learning(FL).In FBL,we design a broad fully connected model(BFCM)as a local model for training client data.To enhance the wireless channel quality for knowledge sharing and reduce the communication and computation cost of participating clients,we construct a joint resource allocation and reconfigurable intelligent surface(RIS)configuration optimization framework for FBL.The problem is decoupled into two convex subproblems.Aiming to improve the resource scheduling efficiency in FBL,a double Davidon–Fletcher–Powell(DDFP)algorithm is presented to solve the time slot allocation and RIS configuration problem.Based on the results of resource scheduling,we design a reward-allocation algorithm based on federated incentive learning(FIL)in FBL to compensate clients for their costs.The simulation results show that the proposed FBL framework achieves better performance than the comparison models in terms of efficiency,accuracy,and cost for knowledge sharing in the IoV.展开更多

It is one of the world’s most common cardiovascular diseases, present in one in five people over the age of 60, and carries an increased risk of morbidity and mortality. The aim of this study was to assess the knowledge of patients and their carers about obliterative arteriopathy of the lower limbs at Conakry University Hospital. Material and Methods: We conducted a descriptive cross-sectional study lasting 3 months, from March 12 to June 12, 2022 in the hospitals of the University Hospital of Conakry (Donka and Ignace Deen). Following free and informed consent, they were interviewed on the basis of a pre-established questionnaire. Socio-demographic data (age, gender, level of education) were collected, and knowledge of obliterative arterial disease of the lower limbs. Results: The study involved 159 people, comprising 106 patients (66.66%) and 53 accompanying persons (33.34%) in the hospitals of the University Hospital of Conakry (Donka and Ignace Deen). The 60 to 79 age group was the most represented (44.7%), with an average age of 55.4 ± 15.9 years and extremes of 20 to 84 years. Half (47.8%) had not attended school, and almost all (96.4%) of those who had attended school had a low level of education (primary and secondary). Almost all of them (91.2%) had never heard of AOMI. Only 5% identified at least one risk factor, diabetes, hypertension and smoking. Amputation was the most frequently identified complication. About 98% did not know that AOMI is associated with myocardial infarction and stroke, and 93.7% did not know of any complications. Almost all the participants (99.4%) had poor knowledge of the following complications. Conclusion: Obliterative arterial disease of the lower limbs could be prevented or delayed by screening and controlling risk factors. Our results show a huge deficit in knowledge of this pathology, including risk factors, preventive measures, clinical signs and complications among patients and their relatives. A study conducted outside the hospital environment would provide a better understanding of the extent to which the general population is unaware of this disease.展开更多

Purpose and Objectives: To appraise the community that acknowledges burns, first aid, and associated factors among the community population in Jazan City, Saudi Arabia. The paper aims to identify limitations to encourage additional research and persuade legislators to develop improved burn-injury care recommendations and training programs. Materials and Methods: An observational-based sample survey was conducted among the people who live in Jazan City aging 13 years or more, during April 5 to May 5, 2023. Data collection was done by a validated online self-administrated questionnaire sent randomly to community members in different parts of Jazan City via social media platforms. Collected data were coded and cleaned by an excel program, and finally exported on SPSS 26.0 software. The variables were analyzed using descriptive statistics like frequencies and percentages. Also, the Chi-square test was used to investigate the relation between different variables, with a significance value of P Results: This study included 243 participants (about 62%) among them were mostly male participants (151) having a university degree. The majority of participants 75% did not take any form of BFA training in the past. This study shows that 69.9% of the participants have inadequate awareness, despite 72% having a constructive attitude towards burn first aid. Previous burn-related first aid training was significantly associated with participants’ knowledge of BFA at a p-value less than 0.05. Conclusion: This study indicates a high frequency of Jazan population having inadequate knowledge of burn first aid despite the high prevalence of a favorable attitude. There is a need to develop an effective nationwide burn prevention program and early burn first aid treatment in Saudi Arabia and promote a consistent guideline for burn first aid.
文摘Objective: To understand the current situation of prehospital first aid knowledge, attitude and behavior of university students in Jingzhou City. Methods: A prehospital first aid knowledge questionnaire and the convenience sampling method were used to survey 307 university students in Jingzhou City. Results: The mean score of prehospital first aid knowledge of university students in Jingzhou City was 12.85 ± 2.643, the mean score of attitude was 50.73 ± 4.114, and the mean score of behavior was 39.05 ± 8.898;There was a statistically significant difference in the scores of prehospital first aid knowledge, attitude, and behavior of university students depending on whether or not they had received prehospital first aid training (P P Conclusion: Jingzhou University students have a positive attitude toward pre-hospital first aid, but the knowledge level and behavior are low, which suggests that the government, society and the school should create good conditions to promote the improvement of pre-hospital first aid knowledge and ability.

文摘Background: Scoliosis is a serious condition occurring in 1% to 4% of the population. This study was conducted and designed to educate society about scoliosis and measure the knowledge and awareness about it. Methods: This was a cross-sectional study conducted on members of the Saudi Society who have consented to participate in this research between the 8th of January and 5th of February 2023. Results: A total of 427 people agreed to participate in the study. The highest percentage of the participants was female 69.1%. About 41% participants out of 427 were in the age group 18-25 years. Most of the participants were from the Middle Region. (71.9%) of participants were unfamiliar with scoliosis, while (72.1%) had no prior knowledge of the condition. Conclusion: This study indicates insufficient awareness and knowledge about scoliosis in Saudi Arabia. Therefore, further studies are needed to raise awareness about scoliosis and help people to learn more about this condition.
文摘Prostate cancer is one of the most common types of cancer in men. The rate of early detection of prostate cancer is low in Türkiye. Therefore, it is important to measure the level of awareness regarding prostate cancer. In our research, we investigate the knowledge levels of prostate cancer among adult individuals in four different geographic region in Türkiye. In addition, we aimed to compare the level of awareness regarding prostate cancer and the depression and anxiety levels among the individuals. The prostate cancer awareness level survey was selected as the data collection tool. In the survey, 20 questions are asked to measure the knowledge level of the participants about prostate cancer. In addition, an evaluation of anxiety and depression was conducted by using the Hospital Anxiety and Depression Scale (HADS). Between April 2022 and December 2022, 834 participants were reached.72.9% of the participants answered the questionnaire correctly. A significant difference was found in terms of correct response rates in four different provinces located in four different regions of Türkiye (79% in Eskisehir, 75.2% in Canakkale, 73% in Ankara, and 54.4% in Maras;p < 0.05). According to the HADS scale, 240 (28.8%) individuals were found to have anxiety and 129 (15.5%) of them had depression. The knowledge level of most of the participants about prostate cancer was found to be above the average. There was a significant difference between provinces in terms of knowledge levels. A significant relationship was observed between depression and knowledge level. However, no significant difference was found for anxiety.
文摘Background: Pain and palliative care are a reality in daily routines of medical treatment. However, the theoretical-practical curricula of traditional medical school course still unsatisfactorily contemplate pain management, as well as the palliative care approach. Objective: To assess the knowledge of medical students about pain and palliative care, as well as to identify their perception of teaching these topics during hospitalization. Methods: A cross-sectional observational study, with a descriptive and exploratory approach, data collection for which was carried out between August and November 2020. The target population was medical students, who responded to an online survey of a quantitative, anonymous and follow-up nature. The survey study variables concerned knowledge about pain management and palliative care. Results: An expressive majority of academics showed difficulty in understanding the pathophysiology of pain related to prescribing drugs for pain management purposes, and all of them believe that it is necessary to acquire more knowledge about pain treatment. In parallel, only 9.3% report having received sufficient information regarding palliative care during medical school. Conclusion: The results suggest a certain lack of knowledge and insecurity among medical school students with respect to pain management and care for patients receiving palliative care. The didactical approach to this theme is still deficient in the medical curriculum and requires immediate improvement and new proposals that address the training of these professionals in a more specific and effective way.
文摘Background: Epilepsy is a chronic brain disorder. It often leads to disabilities and handicaps. In Africa, epilepsy is almost exclusively treated by general practitioners (GPs) because of a shortage of epilepsy specialists. It is therefore important to know the level of knowledge about epilepsy among GPs in order to improve their skills. Objectives: To assess the level of knowledge about the management of anti-epileptic drug treatment among GPs in Brazzaville;to investigate the relationship between demographic factors and GPs’ knowledge. Methods: This was a cross-sectional analytical study. It was conducted from 20 July to 1 September 2021. It focused on GPs working in public hospitals and private care centers in Brazzaville. Information on treatment aspects was collected through a standardized 11-item questionnaire. Results: Among the 137 participants, there were 84 (61.3%) men and 53 (38.7%) women. Of these participants, 36 (26.3%) were trained in Congo versus 101 (73.7%) in other countries. Only 21 (15.3%) GPs had good knowledge about the management of anti-epileptic drug treatment. The overall average knowledge score among GPs was low (31.4%). No significant associations were found between low and good levels of knowledge and gender (OR = 1.03;95% CI = 0.40 - 2.68;p = 1.000), age groups (OR 0.05), training country (OR = 0.62;95% CI = 0.19 - 1.98;p = 0.591), practice hospital (OR = 0.40;95% CI = 0.05 - 3.20;p = 0.695) and duration of professional experience (OR 0.05). Conclusion: The study population has insufficient knowledge about the management of anti-epileptic drug treatment. Demographic factors have no impact on GPs’ knowledge. Epilepsy education programs are needed to improve GPs’ knowledge and skills.

文摘High-efficiency and low-cost knowledge sharing can improve the decision-making ability of autonomous vehicles by mining knowledge from the Internet of Vehicles(IoVs).However,it is challenging to ensure high efficiency of local data learning models while preventing privacy leakage in a high mobility environment.In order to protect data privacy and improve data learning efficiency in knowledge sharing,we propose an asynchronous federated broad learning(FBL)framework that integrates broad learning(BL)into federated learning(FL).In FBL,we design a broad fully connected model(BFCM)as a local model for training client data.To enhance the wireless channel quality for knowledge sharing and reduce the communication and computation cost of participating clients,we construct a joint resource allocation and reconfigurable intelligent surface(RIS)configuration optimization framework for FBL.The problem is decoupled into two convex subproblems.Aiming to improve the resource scheduling efficiency in FBL,a double Davidon–Fletcher–Powell(DDFP)algorithm is presented to solve the time slot allocation and RIS configuration problem.Based on the results of resource scheduling,we design a reward-allocation algorithm based on federated incentive learning(FIL)in FBL to compensate clients for their costs.The simulation results show that the proposed FBL framework achieves better performance than the comparison models in terms of efficiency,accuracy,and cost for knowledge sharing in the IoV.
文摘Introduction: Obliterative arterial disease of the lower limbs is a marker of advanced atherosclerosis. It is one of the world’s most common cardiovascular diseases, present in one in five people over the age of 60, and carries an increased risk of morbidity and mortality. The aim of this study was to assess the knowledge of patients and their carers about obliterative arteriopathy of the lower limbs at Conakry University Hospital. Material and Methods: We conducted a descriptive cross-sectional study lasting 3 months, from March 12 to June 12, 2022 in the hospitals of the University Hospital of Conakry (Donka and Ignace Deen). Following free and informed consent, they were interviewed on the basis of a pre-established questionnaire. Socio-demographic data (age, gender, level of education) were collected, and knowledge of obliterative arterial disease of the lower limbs. Results: The study involved 159 people, comprising 106 patients (66.66%) and 53 accompanying persons (33.34%) in the hospitals of the University Hospital of Conakry (Donka and Ignace Deen). The 60 to 79 age group was the most represented (44.7%), with an average age of 55.4 ± 15.9 years and extremes of 20 to 84 years. Half (47.8%) had not attended school, and almost all (96.4%) of those who had attended school had a low level of education (primary and secondary). Almost all of them (91.2%) had never heard of AOMI. Only 5% identified at least one risk factor, diabetes, hypertension and smoking. Amputation was the most frequently identified complication. About 98% did not know that AOMI is associated with myocardial infarction and stroke, and 93.7% did not know of any complications. Almost all the participants (99.4%) had poor knowledge of the following complications. Conclusion: Obliterative arterial disease of the lower limbs could be prevented or delayed by screening and controlling risk factors. Our results show a huge deficit in knowledge of this pathology, including risk factors, preventive measures, clinical signs and complications among patients and their relatives. A study conducted outside the hospital environment would provide a better understanding of the extent to which the general population is unaware of this disease.

文摘This article explores China’s state-level knowledge production on medicinal ingredients in northwestern Sichuan in the 1950s.Drawing on county-level archives,published materials and interviews,this historical article traces how different levels of governments and state-owned trading companies produced knowledge about medicinal ingredients and its production.It argues that on one hand the state’s procurement standards codified the knowledge about medicinal ingredients from the marketplace and the local producers.On the other hand,direct extraction of knowledge from the medicine gatherers and cultivators verbalized and collected the previously tacit local knowledge about the production of medicinal ingredients,which would help to alleviate the shortage in the supply of traditional Chinese medicines throughout the 1950s.

文摘Purpose:This paper aims to address the limitations in existing research on the evolution of knowledge flow networks by proposing a meso-level institutional field knowledge flow network evolution model(IKM).The purpose is to simulate the construction process of a knowledge flow network using knowledge organizations as units and to investigate its effectiveness in replicating institutional field knowledge flow networks.Design/Methodology/Approach:The IKM model enhances the preferential attachment and growth observed in scale-free BA networks,while incorporating three adjustment parameters to simulate the selection of connection targets and the types of nodes involved in the network evolution process Using the PageRank algorithm to calculate the significance of nodes within the knowledge flow network.To compare its performance,the BA and DMS models are also employed for simulating the network.Pearson coefficient analysis is conducted on the simulated networks generated by the IKM,BA and DMS models,as well as on the actual network.Findings:The research findings demonstrate that the IKM model outperforms the BA and DMS models in replicating the institutional field knowledge flow network.It provides comprehensive insights into the evolution mechanism of knowledge flow networks in the scientific research realm.The model also exhibits potential applicability to other knowledge networks that involve knowledge organizations as node units.Research Limitations:This study has some limitations.Firstly,it primarily focuses on the evolution of knowledge flow networks within the field of physics,neglecting other fields.Additionally,the analysis is based on a specific set of data,which may limit the generalizability of the findings.Future research could address these limitations by exploring knowledge flow networks in diverse fields and utilizing broader datasets.Practical Implications:The proposed IKM model offers practical implications for the construction and analysis of knowledge flow networks within institutions.It provides a valuable tool for understanding and managing knowledge exchange between knowledge organizations.The model can aid in optimizing knowledge flow and enhancing collaboration within organizations.Originality/value:This research highlights the significance of meso-level studies in understanding knowledge organization and its impact on knowledge flow networks.The IKM model demonstrates its effectiveness in replicating institutional field knowledge flow networks and offers practical implications for knowledge management in institutions.Moreover,the model has the potential to be applied to other knowledge networks,which are formed by knowledge organizations as node units.

文摘In Quebec,Canada,the public healthcare system offers free medical services.However,patients with spinal pain often encounter long waiting times for specialist appointments and limited physiotherapy coverage.In contrast,private clinics provide expedited care but are relatively scarce and entail out-of-pocket expenses.Once a patient with pain caused by a spinal disorder meets a pain medicine specialist,spinal intervention is quickly performed when indicated,and patients are provided lifestyle advice.Transforaminal epidural steroid injections are frequently administered to patients with radicular pain,and steroid injections are administered on a facet joint to control low back or neck pain.Additionally,medial branch blocks are performed prior to thermocoagulation.France’s universal healthcare system ensures accessibility at controlled costs.It emphasizes physical activity and provides free physical therapy services.However,certain interventions,such as transforaminal and interlaminar epidural injections,are not routinely used in France owing to limited therapeutic efficacy and safety concerns.This underutilization may be a potential cause of chronic pain for many patients.By examining the differences,strengths,and weaknesses of these two systems,valuable insights can be gained for the enhancement of global spinal pain management strategies,ultimately leading to improved patient outcomes and satisfaction.

文摘The acquisition of valuable design knowledge from massive fragmentary data is challenging for designers in conceptual product design.This study proposes a novel method for acquiring design knowledge by combining deep learning with knowledge graph.Specifically,the design knowledge acquisition method utilises the knowledge extraction model to extract design-related entities and relations from fragmentary data,and further constructs the knowledge graph to support design knowledge acquisition for conceptual product design.Moreover,the knowledge extraction model introduces ALBERT to solve memory limitation and communication overhead in the entity extraction module,and uses multi-granularity information to overcome segmentation errors and polysemy ambiguity in the relation extraction module.Experimental comparison verified the effectiveness and accuracy of the proposed knowledge extraction model.The case study demonstrated the feasibility of the knowledge graph construction with real fragmentary porcelain data and showed the capability to provide designers with interconnected and visualised design knowledge.

文摘Knowledge graph(KG)serves as a specialized semantic network that encapsulates intricate relationships among real-world entities within a structured framework.This framework facilitates a transformation in information retrieval,transitioning it from mere string matching to far more sophisticated entity matching.In this transformative process,the advancement of artificial intelligence and intelligent information services is invigorated.Meanwhile,the role ofmachine learningmethod in the construction of KG is important,and these techniques have already achieved initial success.This article embarks on a comprehensive journey through the last strides in the field of KG via machine learning.With a profound amalgamation of cutting-edge research in machine learning,this article undertakes a systematical exploration of KG construction methods in three distinct phases:entity learning,ontology learning,and knowledge reasoning.Especially,a meticulous dissection of machine learningdriven algorithms is conducted,spotlighting their contributions to critical facets such as entity extraction,relation extraction,entity linking,and link prediction.Moreover,this article also provides an analysis of the unresolved challenges and emerging trajectories that beckon within the expansive application of machine learning-fueled,large-scale KG construction.
文摘Over the past two decades,the USA has witnessed a steep rise in the incidence of acute cholecystitis(AC),[1]even among children,[2]with over 200,000 cases annually.[3]Patients with AC often present to the emergency department(ED)experiencing severe distress,with most describing their pain level as“intolerable.”[4]Furthermore,there is usually a considerable delay before surgery,with a median time from diagnosis of AC to surgery of 28.5 h,[5]and a substantial percentage of patients wait as long as 10 d.[6]Prolonged delays before surgery lead to increased opioid use,which is associated with extended hospital stays and higher rates of readmission after cholecystectomy.[7]Furthermore,opioids can lead to vomiting,respiratory depression,delirium,and ultimately addiction.[8]We propose a novel regional anesthesia technique for managing AC:right phrenic nerve blockade.This method could offer improved pain control and a more favorable risk profile in selected patients when compared to current ED practices.

文摘In this editorial we comment on the article published in a recent issue of the World Journal of Clinical Cases.This article described a novel ultrasound-guided lateral recess block approach in treating a patient with lateral recess stenosis.The impact of spinal pain-related disability extends significantly,causing substantial human suffering and medical costs.Each county has its preferred treatment strategies for spinal pain.Here,we explore the lower back pain(LBP)treatment algorithm recommended in France.The treatment algorithm for LBP recommended by the French National Authority for Health emphasizes early patient activity and minimal medication use.It encourages the continuation of daily activities,limits excessive medication and spinal injections,and incorporates psychological assessments and non-pharmacological therapies for chronic cases.However,the algorithm may not aggressively address acute pain in the early stages,potentially delaying relief and increasing the risk of chronicity.Additionally,the recommended infiltrations primarily involve caudal epidural steroid injections,with limited consideration for other injection procedures,such as transforaminal or interlaminar epidural steroid injections.The fixed follow-up timeline may not accommodate patients who do not respond to initial treatment or experience intense pain,potentially delaying the exploration of alternative therapies.Despite these limitations,understanding the strengths and weaknesses of the French approach could inform adaptations in LBP treatment strategies globally,potentially enhancing patient outcomes and satisfaction across diverse healthcare systems.
文摘In this article, we present our previous research, which highlighted adenosine triphosphate(ATP) as the cause of neuropathic pain during the acute phase of neuromyelitis optica spectrum disorder(NMOSD). In NMOSD pathology, damageassociated molecular patterns(DAMPs), including ATP, are released from damaged astrocytes, triggering the activation of innate immune cells. ATP is a central mediator of acute pain in NMOSD.
