Experiments have been conducted on a self-built apparatus, testing the performance with different gas inlet strategies (bottom gas-inlet strategy and side gas-inlet strategy) and the influence of gas pipe length (0, 1, 2, 4, 6, 8, 10 m) and leakage rate (around 10, 20, 30, 40 bubbles/min) on first bubble time and bubble rate. A buffer of 110 cm3 is inserted between leakage source and gas pipe to simulate the down- stream cavum adjacent to the valve clack. Based on analyzing the experimental data, experiential parameters have also been summarized to guide leakage detection and measurement for engineering applications. A practical system has already been suc- cessfully applied in a cryogenic testing apparatus for cryogenic valves.展开更多

Firstly, the basic...For solving the difficult problem of leakage detection in city gas pipelines, a method using acoustic technique based on instantaneous energy (IE) distribution and correlation analysis was proposed. Firstly, the basic theory of leakage detection and location was introduced. Then the physical relationship between instantaneous energy and structural state variation of a system was analyzed theoretically. With HILBERT-HUANG transformation (HHT), the instantaneous energy distribution feature of an unstable acoustic signal was obtained. According to the relative contribution method of the instantaneous energy, the noise in signal was eliminated effectively. Furthermore, in order to judge the leakage, the typical characteristic of the instantaneous energy of signal in the input and output end was discussed using correlative analysis. A number of experiments were carried out to classify the leakage from normal operations, and the results show that the leakages are successfully detected and the average recognition rate reaches 93.3% among three group samples. It is shown that the method using acoustic technique with IED and correlative analysis is effective and it may be referred in other pipelines.展开更多

BACKGROUND Application of indocyanine green(ICG)fluorescence has led to new developments in gastrointestinal surgery.However,little is known about the use of ICG for the diagnosis of postoperative gut leakage(GL).In a...BACKGROUND Application of indocyanine green(ICG)fluorescence has led to new developments in gastrointestinal surgery.However,little is known about the use of ICG for the diagnosis of postoperative gut leakage(GL).In addition,there is a lack of rapid and intuitive methods to definitively diagnose postoperative GL.AIM To investigate the effect of ICG in the diagnosis of anastomotic leakage in a surgical rat GL model and evaluate its diagnostic value in colorectal surgery patients.METHODS Sixteen rats were divided into two groups:GL group(n=8)and sham group(n=8).Approximately 0.5 mL of ICG(2.5 mg/mL)was intravenously injected postoperatively.The peritoneal fluid was collected for the fluorescence test at 24 and 48 h.Six patients with rectal cancer who had undergone laparoscopic rectal cancer resection plus enterostomies were injected with 10 mL of ICG(2.5 mg/mL)on postoperative day 1.Their ostomy fluids were collected 24 h after ICG injection to identify the possibility of the ICG excreting from the peripheral veins to the enterostomy stoma.Participants who had undergone colectomy or rectal cancer resection were enrolled in the diagnostic test.The peritoneal fluids from drainage were collected 24 h after ICG injection.The ICG fluorescence test was conducted using OptoMedic endoscopy along with a near-infrared fluorescent imaging system.RESULTS The peritoneal fluids from the GL group showed ICG-dependent green fluorescence in contrast to the sham group.Six samples of ostomy fluids showed green fluorescence,indicating the possibility of ICG excreting from the peripheral veins to the enterostomy stoma in patients.The peritoneal fluid ICG test exhibited a sensitivity of 100%and a specificity of 83.3%for the diagnosis of GL.The positive predictive value was 71.4%,while the negative predictive value was 100%.The likelihood ratios were 6.0 for a positive test result and 0 for a negative result.CONCLUSION The postoperative ICG test in a drainage tube is a valuable and simple technique for the diagnosis of GL.Hence,it should be employed in clinical settings in patients with suspected GL.展开更多

It is observed that thetracking is more severe under the DC voltages. The tracking time is less under negative DC comparedto the positive DC voltage. The tracking mechanism is explained in detail. The leakage currentduring the tracking studies was as measured and the moving average technique was adopted tounderstand the trend in current flow. The leakage current magnitude is high with thermally agedspecimens compared to the virgin specimen, irrespective of the type of applied voltage. However, little is kno...BACKGROUND Anastomotic leakage(AL) is a severe complication in rectal cancer surgery.Various methods, including intracorporeal reinforcing suturing, have been used to reduce the incidence of AL. However, little is known about the efficacy of staple-line reinforcement by barbed suture for preventing AL.AIM To evaluate the efficacy of staple-line reinforcement using barbed suture for preventing AL in laparoscopic surgery for rectal cancer.METHODS We retrospectively reviewed the clinical datum of 319 patients undergoing laparoscopic low anterior resection combined with double stapling technique between May 1, 2017 and January 31, 2021. All surgeries were performed by the same surgical team specializing in colorectal surgery. Patients were divided into two groups depending on whether they received reinforcing sutures. Patients’ baseline characteristics did not show any significant difference between the two groups. We analyzed patient-, tumor-, as well as surgery-related variables using univariate and multivariate logistic analyses.RESULTS There were 168 patients in the reinforcing suture group and 151 patients in the non-reinforcing suture group. AL occurred in 25 cases(7.8%). Its incidence was significantly higher in the non-reinforcing suture group than in the reinforcing suture group(4.8% vs 11.3%, P = 0.031). The multivariate analyses demonstrated that the tumor site, tumor size and presence of staple-line reinforcement were independent risk factors for AL. We divided these patients into two risk groups based on the combination of tumor site and tumor size. Patients without any risk factor were assigned to the low-risk group(n = 177), whereas those having one or two risk factors were assigned to the high-risk group(n = 142). In the high-risk group, the AL incidence considerably decreased in the reinforcing suture group compared with that in the non-reinforcing suture group(P = 0.038). Nonetheless, no significant difference was found in the low-risk group between the two groups.CONCLUSION Staple-line reinforcement by barbed suture may decrease the incidence of AL. A large-scale prospective randomized controlled trial is needed for evaluating the efficacy of staple-line reinforcement for preventing AL.展开更多
Objective:To review the current literature on the practice of intramuscular injections(IMIs),focusing on immunizations.Methods:The present study comprises 2 reviews,characterized by high-quality evidence,per taining t...Objective:To review the current literature on the practice of intramuscular injections(IMIs),focusing on immunizations.Methods:The present study comprises 2 reviews,characterized by high-quality evidence,per taining to the deployment of the slow injection technique without aspiration(referred to in this paper as the without-aspiration slow injection technique[WASi T])and combined with the Zeta-track technique(ZTT).The literature review is oriented toward the analysis of 2 features associated with IMIs techniques:safety and pain,integrating new evidence on vaccinee positioning for each muscle site and general relaxation techniques in the multi-parametric analysis.Results:The rigorous and in-depth reviews in the current study reveal the usefulness of including,among the international guidelines for via intramuscular immunizations,the adoption of WASi T only for compliant vaccinees,and in combination with all validated techniques for IMIs,and the use of ZTT limitedly if specific well-developed muscles are indicated.All the technique's limitations are exposed.Conclusions:Future research directions are presented by including the author's study designs to provide indirect evidence for the validity of the rationale of the slow injection technique using scientific methods,and for the conduction of future randomized controlled trials(RCTs)focused on revisiting the adoption of ZTT in a dynamic and integrated immunizations protocol in anterolateral thigh(ALT),ventrogluteal(VG),and also in the deltoid muscle,in the specific cases analyzed.展开更多

Various techniques were proposed to reduce the leakage power at nano-scale; one of these techniques is MTV (multi-threshold voltage) In this paper, the exact and optimal value of threshold voltage (Vth) for each transistor in any sequential circuit in the design is found, so that the value of the total leakage current in the design is at the minimum. This could be achieved by applying AI (artificial intelligence) search algorithm. The proposed algorithm is called LOAIS (leakage optimization using AI search). LOAIS exploits the total slack time of each transistor's location and their contributions in the leakage current. It is introduced by AI heuristic search algorithms under 22 nm BSIM4 predictive technology model. The proposed approach saves around 80% of the sub-threshold leakage current without degrading the performance of the circuit.展开更多
