Background:To protect neurological tissues,underdosing occurs in most cases of T4 nasopharyngeal carcinoma(NPC) with intracranial extension.In this study,we aimed to evaluate the effect of dosimetric inadequacy on local control and late neurological toxicities for patients treated with intensity-modulated radiotherapy(IMRT) plus chemotherapy.Methods:We prospectively enrolled patients who had non-metastaticT4 NPC with intracranial extension treated between January 2009 and November 2013.The prescribed dose was 66.0-70.4 Gy to the primary planning target volume(primary gross tumor volume [GTVp;i.e.,the nasopharyngeal tumor] +5.0 mm).Dose-volume histogram parameters were calculated,including minimum point dose(D_(min)) and dose to 95% of the target volume(D95).All patients received chemotherapy with the cisplatin,5-fluorouracil,and docetaxel regimen.Survivals were estimated using the Kaplan-Meier method and compared using the log-rank test.Results:In total,41 patients were enrolled.The local partial response rate was 87.8% after induction chemotherapy.With a median follow-up of 51 months,7 patients experienced failure in the nasopharynx;the 3-year local failure-free survival and overall survival rates of the 41 patients were 87.4% and 90.2%,respectively.The actual mean D_(min) to the GTVp was 55.2 Gy(range 48.3-67.3 Gy),and D95 was 61.6 Gy(range 52.6-69.0 Gy).All doses received by neurological organs remained well within their dose constraints.No patients developed temporal lobe necrosis or other neurological dysfunctions.Conclusions:With relative underdosed IMRT plus effective chemotherapy,the patients achieved satisfactory local control with few late toxicities of the central nervous system.Determining the acceptable extent of dosimetric inadequacy requires further exploration. 展开更多

We report a theoretical work on the properties of modulational instability and bright type nonlinear localized modes in one-dimensional easy-axis weak ferromagnetic spin lattices involving next-nearest-neighbor couplings.With a linear stability analysis, we calculate the growth rates of the modulational instability, and plot the instability regions.When the strength of the next-nearest-neighbor coupling is large enough, two new asymmetric modulational instability regions appear near the boundary of the first Brillouin zone.Furthermore, analytical forms of the bright nonlinear localized modes are constructed by means of a quasi-discreteness approach.The influence of the next-nearest-neighbor coupling on the Brillouin zone center mode and boundary mode are discussed.In particular, we discover a reversal phenomenon of the propagation direction of the Brillouin zone boundary mode. 展开更多

Objective: To investigate the effects of intensity modulated radiation therapy + local hyperthermia on the cancer cell apoptosis and invasion in liver cancer lesion. Methods:A total of 94 patients with middle-advanced primary liver cancer who were diagnosed and treated in this hospital between November 2015 and February 2017 were divided into control group (n=47) and experimental group (n=47) by random number table method. Control group received intensity modulated radiation therapy and experimental group received intensity modulated radiation therapy + local hyperthermia. Both groups accepted peritoneal lesion biopsy before and after treatment, and the expression of apoptosis and invasion genes in specimen tissue were detected by fluorescence quantitative PCR. Results: There was no statistically significant difference in apoptosis and invasion gene expression between the two groups of patients before treatment. After treatment, apoptosis genes Fas, caspase-3, Bax and p53 mRNA expression in lesion tissue of experimental group were higher than those of control group whereas FasL and Bcl-2 mRNA expression were lower than those of control group;invasion genes Cofilin-1, Bmi-1, STAT3 and SOX18 mRNA expression in lesion tissue of experimental group were lower than those of control group whereas Tip30 and TP53IP1 mRNA expression were higher than those of control group. Conclusion: intensity modulated radiation therapy + local hyperthermia can effectively promote cancer cell apoptosis and inhibit its invasion activity in patients with middle and advanced primary liver cancer. 展开更多

Indoor localization methods can help many sectors,such as healthcare centers,smart homes,museums,warehouses,and retail malls,improve their service areas.As a result,it is crucial to look for low-cost methods that can provide exact localization in indoor locations.In this context,imagebased localization methods can play an important role in estimating both the position and the orientation of cameras regarding an object.Image-based localization faces many issues,such as image scale and rotation variance.Also,image-based localization’s accuracy and speed(latency)are two critical factors.This paper proposes an efficient 6-DoF deep-learning model for image-based localization.This model incorporates the channel attention module and the Scale PyramidModule(SPM).It not only enhances accuracy but also ensures the model’s real-time performance.In complex scenes,a channel attention module is employed to distinguish between the textures of the foregrounds and backgrounds.Our model adapted an SPM,a feature pyramid module for dealing with image scale and rotation variance issues.Furthermore,the proposed model employs two regressions(two fully connected layers),one for position and the other for orientation,which increases outcome accuracy.Experiments on standard indoor and outdoor datasets show that the proposed model has a significantly lower Mean Squared Error(MSE)for both position and orientation.On the indoor 7-Scenes dataset,the MSE for the position is reduced to 0.19 m and 6.25°for the orientation.Furthermore,on the outdoor Cambridge landmarks dataset,the MSE for the position is reduced to 0.63 m and 2.03°for the orientation.According to the findings,the proposed approach is superior and more successful than the baseline methods. 展开更多

Intensity-modulated radiation therapy (IMRT) has become the mainstay of treatment for localized prostate cancer. In IMRT, minimizing differences between the conditions used during planning CT and daily treatment is important to prevent adverse events in normal tissues. In the present study, we evaluated the impact of variation in bladder volume on the doses to various organs. A total of 35 patients underwent definitive radiotherapy at Saitama Medical Center. A Light Speed RT16 (GE Healthcare) was used for planning and to obtain examination CT images. Such images were acquired after 4 - 6 days of planning CT image acquisition. The IMRT plans were optimized using the planning CT data to satisfy the dose constraints set by our in-house protocols for the PTV and the OARs. The dose distributions were then re-calculated using the same IMRT beams, and checked on examination CT images. It was clear that bladder volume affected the doses to certain organs. We focused on the prostate, bladder, rectum, small bowel, and large bowel. Regression coefficients were calculated for variables that correlated strongly with bladder volume (p < 0.05). We found that variation in bladder volume [cm<sup>3</sup>] predicted deviations in the bladder V<sub>70Gy</sub>, V<sub>50Gy</sub>, and V<sub>30Gy</sub> [%];the maximum dose to the small bowel [cGy];and the maximum dose to the large bowel [cGy]. The regression coefficients were -0.065, -0.125, -0.180, -10.22, and -9.831, respectively. We evaluated the impacts of such variation on organ doses. These may be helpful when checking a patient’s bladder volume before daily IMRT for localized prostate cancer. 展开更多

针对复杂水下环境焊接过程对焊接电源超高动特性与精确控制需求,设计基于SiC模块的快频脉冲焊接电源功率主电路,开发全数字化控制系统,研制出局部干法水下快频脉冲MIG焊电源(local dry underwater fastfrequency pulsed MIG,LDU-FFPMIG)... 针对复杂水下环境焊接过程对焊接电源超高动特性与精确控制需求,设计基于SiC模块的快频脉冲焊接电源功率主电路,开发全数字化控制系统,研制出局部干法水下快频脉冲MIG焊电源(local dry underwater fastfrequency pulsed MIG,LDU-FFPMIG).研制的LDU-FFPMIG焊接电源额定输出电流为400 A,能精确输出多种0~30 kHz的快频脉冲电流波形,快频脉冲电流幅值高达200 A.提出LDU-FFPMIG焊接新工艺,将快频脉冲应用到304不锈钢局部干法水下MIG焊接,探究快频脉冲对传统局部干法水下脉冲MIG焊的影响.结果表明,研制的LDU-FFPMIG焊接电源能够实现稳定的局部干法水下MIG焊,可获得良好的焊缝成形;快频脉冲电流的引入可显著提高电弧能量密度和焊接稳定性,减小熔宽(B)、增大熔深(H),焊缝成形系数(B/H)减小约30%,并且可以细化晶粒. 展开更多

深度视频修复篡改的被动取证研究

深度视频修复技术就是利用深度学习技术,对视频中的缺失区域进行补全或移除特定目标对象。它也可用于合成篡改视频,其篡改后的视频很难通过肉眼辨别真假,尤其是一些恶意修复的视频在社交媒体上传播时,容易造成负面的社会舆论。目前,针... 深度视频修复技术就是利用深度学习技术,对视频中的缺失区域进行补全或移除特定目标对象。它也可用于合成篡改视频,其篡改后的视频很难通过肉眼辨别真假,尤其是一些恶意修复的视频在社交媒体上传播时,容易造成负面的社会舆论。目前,针对深度视频修复篡改的被动检测技术起步较晚,尽管它已经得到一些关注,但在研究的深度和广度上还远远不够。因此,本文提出一种基于级联Conv GRU和八方向局部注意力的被动取证技术,从时空域角度实现对深度修复篡改区域的定位检测。首先,为了提取修复区域的更多特征,RGB帧和错误级分析帧ELA平行输入编码器中,通过通道特征级融合,生成不同尺度的多模态特征。其次,在解码器部分,使用编码器生成的多尺度特征与串联的Conv GRU进行通道级融合来捕捉视频帧间的时域不连续性。最后,在编码器的最后一级RGB特征后,引入八方向局部注意力模块,该模块通过八个方向来关注像素的邻域信息,捕捉修复区域像素间的异常。实验中,本文使用了VI、OP、DSTT和FGVC四种最新的深度视频修复方法与已有的深度视频修复篡改检测方法HPF和VIDNet进行了对比,性能优于HPF且在编码器参数仅VIDNet的五分之一的情况下获得与VIDNet可比的性能。结果表明,本文所提方法利用多尺度双模态特征和引入的八方向局部注意力模块来关注像素间的相关性,使用Conv GRU捕捉时域异常,实现像素级的篡改区域定位,获得精准的定位效果。 展开更多

目的观察卡瑞利珠单抗联合调强放射治疗局部晚期食管鳞癌患者的效果。方法选择2021年1月—2023年8月本院收治的98例局部晚期食管鳞癌患者作为研究对象,通过红蓝双色球法随机分为常规组和研究组两组,每组各49例。常规组给予调强放射治疗... 目的观察卡瑞利珠单抗联合调强放射治疗局部晚期食管鳞癌患者的效果。方法选择2021年1月—2023年8月本院收治的98例局部晚期食管鳞癌患者作为研究对象,通过红蓝双色球法随机分为常规组和研究组两组,每组各49例。常规组给予调强放射治疗,研究组在常规组基础上加用卡瑞利珠单抗治疗。比较两组临床疗效、不良反应发生情况,治疗前后的肿瘤标记物水平、血清学指标、生活质量。结果研究组的客观缓解率(ORR,87.76%)、疾病控制率(DCR,93.88%)较常规组(48.98%、75.81%)明显更高,差异具有统计学意义(P<0.05)。治疗6周后两组鳞状细胞癌抗原(SCCA)、细胞角质蛋白19片段抗原21-1(CYFRA21-1)、癌胚抗原(CEA)、糖类抗原724(CA724)水平均较治疗前有所降低,研究组较常规组降低幅度更大,差异具有统计学意义(P<0.05)。治疗6周后两组可溶性程序性死亡分子-1(sPD-1)及其配体sPD-L1水平均较治疗前有所降低,研究组较常规组降低幅度更大,差异具有统计学意义(P<0.05)。两组治疗前功能、症状、总体生活领域评分比较,差异无统计学意义(P>0.05)。治疗4周后研究组较治疗前有所提升,且研究组较常规组降低幅度更大,差异具有统计学意义(P<0.05)。研究组较常规组的骨髓抑制、恶心呕吐、放射性食管炎、放射性肺炎不良反应发生率更低,差异具有统计学意义(P<0.05)。结论局部晚期食管鳞癌患者应用卡瑞利珠单抗联合调强放射治疗,临床疗效较佳,明显改善患者肿瘤标记物水平、血清学指标、生活质量,降低不良反应发生率。 展开更多

目的探讨自适应调强放疗(ART)对局部晚期宫颈癌的治疗效果及不良反应发生率。方法选取80例宫颈癌局部晚期患者,按抽签法随机分为对照组(常规放疗,40例)与干预组(ART,40例)。治疗后对比2组的疾病控制率、客观缓解率、肿瘤标志物水平、不... 目的探讨自适应调强放疗(ART)对局部晚期宫颈癌的治疗效果及不良反应发生率。方法选取80例宫颈癌局部晚期患者,按抽签法随机分为对照组(常规放疗,40例)与干预组(ART,40例)。治疗后对比2组的疾病控制率、客观缓解率、肿瘤标志物水平、不良反应率、危及器官剂量学水平与生活质量评分。结果与对照组比较,干预组的疾病控制率(95.00%)及客观缓解率(85.00%)均更高(P<0.05)。与对照组比较,干预组治疗后的血清癌胚抗原水平及血清鳞癌抗原水平均明显更低(P<0.05)。干预组的各项不良反应发生率较对照组更低(P<0.05),干预组治疗的危及器官(膀胱、小肠、直肠)剂量学水平较对照组均更低(P<0.05)。干预组经过治疗后所评估的各项生活质量评分较对照组更高(P<0.05)。结论ART应用于宫颈癌局部晚期患者治疗的效果显著,对疾病进展的控制效果佳,能有效地降低血清中的肿瘤标志物水平,缓解病情,减少不良反应,改善其生活质量,值得推广应用。 展开更多
