The finger was held...We describe the presentation of a patient with sudden, sharp pain associated with a snapping sensation, swelling and pain over the metacarpophalangeal joint (MCPJ) with no history of direct trauma. The finger was held in 30 degrees of flexion and significantly deviated to the ulnar side with loss of extension. A diagnosis of traumatic rupture of the radial sagittal band of the extensor mechanism was made but the cause at exploration was found to be impingement of an osteochondral fracture fragment. This is a rare cause of irreducible loose body ‘locking’ of the metacarpophalangeal joint.展开更多

Among ...In this manuscript, the authors have studied the Disc Displacement (DD) status of patients with acute Closed Lock (CL) to determine differences between DD with Reduction (DDwR) and DD without Reduction (DDwoR). Among the acute CL patients who visited our hospital within 2 weeks of the onset of CL, we studied 10 patients whose CL was released (DDwR) and 13 patients whose CL was not released (DDwoR). The DDwoR group was significantly older than the DDwR group. Although the mouth opening distance was significantly greater in the DDwoR group than in the DDwR group, the two groups were identical in the duration of CL. Sagittal MRI images showed no significant differences between the two groups in disc length and disc thickness (anterior band, intermediate zone, and posterior band). Multisection sagittal and coronal images identified lateral DD in 7 of the 10 patients in the DDwR group, although no specific direction of the DD was observed in the DDwoR group. Furthermore, deformation of the medial disc was common in the DDwoR group but uncommon in the DDwR group.展开更多

In general, these solutions pr...CDMA Timing and phase offsets tracking remain as one of considerable factors that influence the performances of communication systems. Many algorithms are proposed to solve this problem. In general, these solutions process separately the chip sampling offset and phase rotation. In addition, most of proposed solutions can not assure a compromise between robustness criteria and low complexity for implementation in real time applications. In this paper we present an efficient algorithm for chip sampling and phase synchronization. This algorithm allows estimating and correcting jointly in real time, sampling instant and phase errors. The robustness and the low complexity of this algorithm are evaluated, firstly by simulation and then tested by real experimentation for UMTS standard. Simulation results show that the proposed algorithm provides very efficient compensation for sampling clock offset and phase rotation. A real time implementation is achieved, based on TigerSharc DSP, while using a complete UMTS transmission-reception chain. Experimental results show robustness in real conditions.展开更多
