This editorial explores the impact of non-steroidal anti-inflammatory drugs(NSAIDs)on postoperative recovery in hand fracture patients,amidst shifting pain management strategies away from opioids due to their adverse effects.With hand fractures being significantly common and postoperative pain management crucial for recovery,the potential of NSAIDs offers a non-addictive pain control alternative.However,the controversy over NSAIDs'effects on bone healing—stemming from their Cyclooxygenase-2 inhibition and associated risks of fracture non-union or delayed union—necessitates further investigation.Despite a comprehensive literature search,the study finds a lack of specific research on NSAIDs in postoperative hand fracture management,highlighting an urgent need for future studies to balance their benefits against possible risks.

Emergency department operations and management education in emergency medicine training

BACKGROUND:This study was undertaken to examine the current level of operations and management education within US-based Emergency Medicine Residency programs.METHODS:Residency program directors at all US-based Emergency Medicine Residency programs were anonymously surveyed via a web-based instrument.Participants indicated their levels of residency education dedicated to documentation,billing/coding,core measure/quality indicator compliance,and operations management.Data were analyzed using descriptive statistics for the ordinal data/Likert scales.RESULTS:One hundred and six(106)program directors completed the study instrument of one hundred and fifty-six(156)programs(70%).Of these,82.6%indicated emergency department(ED)operations and management education within the training curriculum.Dedicated documentation training was noted in all but 1 program(99%).Program educational offerings also included billing/coding(83%),core measure/quality indicators(78%)and operations management training(71%).In all areas,the most common means of educating came through didactic sessions and direct attending feedback or 69%-94%and 72%-98%respectively.Residency leadership was most confident with resident understanding of quality documentation(80%)and less so with core measures(72%),billing/coding/RVUs(58%),and operations management tools(23%).CONCLUSIONS:While most EM residency programs integrate basic operational education related to documentation and billing/coding,a smaller number provide focused education on the dayto-day management and operations of the ED.Residency leadership perceives graduating resident understanding of operational management tools to be limited.All respondents value further resident curriculum development of ED operations and management.

IPv6 is the foundation of the development of Next Generation Internet (NGI). An IPv6 network management and operations support system is necessary for real operable NGI. Presently there are no approved standards yet and relevant equipment interfaces are not perfect. A Network Management System (NMS) at the network layer helps implement the integrated management of a network with equipment from multiple vendors, including the network resources and topology, end-to-end network performance, network failures and customer Service Level Agreement (SLA) management. Though the NMS will finally realize pure IPv6 network management, it must be accommodated to the management of relevant IPv4 equipment. Therefore, modularized and layered structure is adopted for the NMS in order to implement its smooth transition.

Manufacturing system, with high level of complexity and with a mix of semi-repetitive and repetitive products, to become productive, should seek the standardization of products and processes to obtain the optimization of use of production resources. However, it is necessary to measure the productivity, so that the system of measurement and control of manufacturing processes are an element critical as to ensure greater visibility of the flow＇s restrictions, minimized when detected properly. In this case, the automation of factory＇s measurement process can effectively contribute to ensuring the effectiveness of the function control of a manufacturing system. It is important to consider that the automation of the system of measurement and control of manufacturing processes, of complex environment, is heavily dependent of IT tools applied directly in the interface computational between the operation systems and the corporate systems. This heavy reliance, if exploited technically properly, allows that automation of the system of measurement and control of production makes the access to time real of availability of manufacturing process＇s data, such as processing time and setup time that it can export to a specialist software in programming production, for example, feasible. In this paper, the automation of the system of measurement and control of production is approached, in order to identify the main possibilities of the design of an information system capable to integrate the flow of information in an environment internal on manufacturing organizations, with emphasis in the digital manufacturing paradigm.

Cervical spine fractures in osteopetrosis:a case report and review of the literature

While management of appendicular fractures has been well described in the setting of osteopetrosis, there is limited information on managing fractures of the axial spine. Here we present an osteopetrotic patient with multiple traumatic multiple, comminuted, unstable cervical spinal fractures managed with non-operative stabilization, and provide a review of the pathophysiology, genetic characteristics, and special considerations that must be explored when determining operative versus non-operative management of spinal injury in osteopetrosis. A PubMed query was performed for English articles in the literature published up to June 2016, and used the following search terms alone and in combination： ＂osteopetrosis＂, ＂spine＂, ＂fractures＂, ＂osteoclasts＂, and ＂operative management＂. Within four months after initial injury, treatment with halo vest allowed for adequate healing. The patient was asymptomatic with cervical spine dynamic radiographs confirming stability at four months. On four-year follow up examination, the patient remained without neck pain, and CT scan demonstrated partially sclerotic fracture lines with appropriate anatomical alignment. In conclusion, external halo stabilization may be an effective option for treatment of multiple unstable acute traumatic cervical spine fractures in patients with osteopetrosis. Given the challenge of surgical stabilization in osteopetrosis, further research is necessary to elucidate the optimal form of treatment in this select patient population.

Diagnosis and Treatment and Infection Protection Strategy of Osteoporotic Vertebral Compression Fractures Minimally Invasive Percutaneous Kyphoplasty Surgery during the Prevention and Control of COVID-19

Background: As the global novel coronavirus pneumonia (NCP) remains severe, elderly people are at high risk for NCP and osteoporotic vertebral compression fractures, with high complications and mortality. How to treat patients and protect medical staff from infection, and at the same time strictly prevent the occurrence of clustered transmission events in the hospital, the establishment of perfect pre-hospital emergency measures and infection prevention and control strategy is the first element to ensure success. Objective: To establish the diagnosis and treatment and infection protection strategy for Osteoporotic vertebral compression fractures (OVCF) patients undergoing minimally invasive percutaneous kyphoplasty (PKP) surgery during the prevention and control of COVID-19, so as to ensure the stable, orderly and safe medical treatment. Methods: A total of 583 OVCF patients were admitted to the First Affiliated Hospital of Hebei North University during the epidemic prevention and control period from January 2020 to July 2022. After urgent and outpatient strict standardized screening, 382 patients met the inclusion criteria, including 112 males and 270 females, aged (70.50 ± 5.49) years. The preoperative visual analogue scale (VAS) score was 6.92 ± 1.86. Preoperative Oswestry disability index (ODI) was 74.67 ± 4.84. The satisfaction rate was (45.89 ± 3.67) %. According to the clinical diagnostic criteria and classification, 367 patients were diagnosed as ordinary OVCF, including 156 cases of mild compression and 226 cases of moderate compression. The clinical classification of 15 patients with OVCF diagnosed as COVID-19 was type I, including 10 cases of mild COVID-19 and 5 cases of common COVID-19. All patients were treated with PKP. Results: All patients were followed up at 1 day, 1 month and 3 months after operation, VAS (2.01 ± 0.56, 0.45 ± 0.11, 0 ± 0), ODI (45.41 ± 4.15, 10.22 ± 2.73, 4.03 ± 1.57) and satisfaction (90.12%, 95.57%, 99.23%) were significantly improved compared with those before operation (p < 0.05), and the original medical diseases were not aggravated. In this group, 15 cases of OVCF diagnosed with COVID-19 were given priority to treat COVID-19 under strict three-level protection in the designated isolation ward. PKP was carried out after the condition was stable, and the areas, items and personnel in contact with patients during the perioperative period must be strictly and thoroughly disinfected. The patient had a good prognosis, no complications, no cross-infection in the hospital, and no infection rate among medical staff. Conclusions: The implementation of the diagnosis and treatment and infection protection strategy for OVCF patients undergoing minimally invasive PKP surgery during the prevention and control of COVID-19 has a guiding role in preventing the spread of infection, improving the cure rate, promoting rapid recovery, reducing complications and reducing mortality.

Performance of Wuxi WWTP in China

In Wuxi Wastewater Treatment Plant, the Anaerobic, anoxic and oxic (A2/O) process was employed to remove the nitrogen and phosphorus, which exhibited the positive results of the high removal efficiency for phosphorus with a range of 67.7% to 89.9% and an average value of 78.0. The effluent of phosphorus met the national discharge standard. The removal of TN was effected by both BOD variation of influent and wastewater temperature. TN removal was in the range of 28.5% to 55.8% with an average value of 39.4%. The energy cost was 0.15 kWh(m3d)-1 or 1.35 kWh(kgBOD@d)-1. The annual average sludge production was 46.3 m3d-1, the annual average dosage for the dewatering was 40 kg d-1 .
