The Turpan-Hami(Tuha)Basin in Xinjiang Uygur Autonomous Region of China,holds significant strategic importance as a key economic artery of the ancient Silk Road and the Belt and Road Initiative,necessitating a holisti...The Turpan-Hami(Tuha)Basin in Xinjiang Uygur Autonomous Region of China,holds significant strategic importance as a key economic artery of the ancient Silk Road and the Belt and Road Initiative,necessitating a holistic understanding of the spatiotemporal evolution of land use/land cover(LULC)to foster sustainable planning that is tailored to the region's unique resource endowments.However,existing LULC classification methods demonstrate inadequate accuracy,hindering effective regional planning.In this study,we established a two-level LULC classification system(8 primary types and 22 secondary types)for the Tuha Basin.By employing Landsat 5/7/8 imagery at 5-a intervals,we developed the LULC dataset of the Tuha Basin from 1990 to 2020,conducted the accuracy assessment and spatiotemporal evolution analysis,and simulated the future LULC under various scenarios via the Markov-Future Land Use Simulation(Markov-FLUS)model.The results revealed that the average overall accuracy values of our LULC dataset were 0.917 and 0.864 for the primary types and secondary types,respectively.Compared with the seven mainstream LULC products(GlobeLand30,Global 30-meter Land Cover with Fine Classification System(GLC_FCS30),Finer Resolution Observation and Monitoring of Global Land Cover PLUS(FROM_GLC PLUS),ESA Global Land Cover(ESA_LC),Esri Land Cover(ESRI_LC),China Multi-Period Land Use Land Cover Change Remote Sensing Monitoring Dataset(CNLUCC),and China Annual Land Cover Dataset(CLCD))in 2020,our LULC data exhibited dramatically elevated overall accuracy and provided more precise delineations for land features,thereby yielding high-quality data backups for land resource analyses within the basin.In 2020,unused land(78.0%of the study area)and grassland(18.6%)were the dominant LULC types of the basin;although cropland and construction land constituted less than 1.0%of the total area,they played a vital role in arid land development and primarily situated within oases that form the urban cores of the cities of Turpan and Hami.Between 1990 and 2020,cropland and construction land exhibited a rapid expansion,and the total area of water body decreased yet resurging after 2015 due to an increase in areas of reservoir and pond.In future scenario simulations,significant increases in areas of construction land and cropland are anticipated under the business-as-usual scenario,whereas the wetland area will decrease,suggesting the need for ecological attention under this development pathway.In contrast,the economic development scenario underscores the fast-paced expansion of construction land,primarily from the conversion of unused land,highlighting the significant developmental potential of unused land with a slowing increase in cropland.Special attention should thus be directed toward ecological and cropland protection during development.This study provides data supports and policy recommendations for the sustainable development goals of Tuha Basin and other similar arid areas.展开更多
BACKGROUND Pigmented villonodular synovitis(PVNS)is a benign proliferative disorder that affects the synovial joints,bursae,and tendon sheaths.To date,few studies have reported on the treatment of postoperative pain a...BACKGROUND Pigmented villonodular synovitis(PVNS)is a benign proliferative disorder that affects the synovial joints,bursae,and tendon sheaths.To date,few studies have reported on the treatment of postoperative pain and edema in patients with PVNS.Herein,we present the case of a woman who developed pain and edema in the left lower limb 1 wk after synovectomy and arthroscopic partial meniscectomy and was unable to walk due to limited flexion and extension of the left knee.CASE SUMMARY A 32-year-old woman underwent synovectomy and arthroscopic partial meniscectomy successively and was treated with a combination of manual lymphatic drainage(MLD)and kinesio taping(KT)in our hospital to alleviate postoperative pain and edema.The following parameters were assessed at 2 wk post-treatment and 1 wk post-discharge follow up:suprapatellar circumference,infrapatellar circumference,visual analog scale score,knee range of motion,pittsburgh sleep quality index score,hamilton anxiety rating scale(HAMA)score,and hamilton depression rating scale(HAMD)score.After treatment,the postoperative pain and edema in the patient’s left knee were effectively relieved,resulting in improved sleep quality and remarkably attenuated HAMA and HAMD scores.CONCLUSION Combined MLD and KT may be an effective approach for relieving postoperative pain and edema in patients with PVNS.展开更多

BACKGROUND Rectus abdominis separation(DRA)affects pelvic stability and body image.No studies have explored the effects of manual massage on early postpartum DRA and postpartum depression.AIM To analyze the curative e...BACKGROUND Rectus abdominis separation(DRA)affects pelvic stability and body image.No studies have explored the effects of manual massage on early postpartum DRA and postpartum depression.AIM To analyze the curative effect of massage on early postpartum DRA and its impact on postpartum depression and thus its ability promote the overall psychosomatic rehabilitation of postpartum women.METHODS Data were retrospectively collected on 70 primiparous women with postpartum DRA who underwent rehabilitation at the Postpartum Rehabilitation Center of Huzhou Maternal and Child Health Hospital from October 2022 to September 2023.The patients were divided into the Group S(35 cases,biomimetic electrical stimulation treatment)and Group L(35 cases,biomimetic electrical stimulation combined with manual massage treatment).Baseline data,the edinburgh postpartum depression scale(EPDS)score,and the visual analog scale(VAS)scores for rectus abdominis distance,waist circumference,and lower back pain before and after treatment were compared.RESULTS No significant differences were found in the baseline data,rectus abdominis distance,waist circumference,and VAS and EPDS scores between the two groups before treatment(P>0.05).After treatment,the distance between rectus abdominis and waist circumference in Group L were significantly smaller than those in Group S(P<0.05).Furthermore,lower back pain(VAS score)and the EPDS score in Group L were significantly lower than those in Group S(P<0.05).CONCLUSION Manual massage can significantly reduce early postpartum DRA,waist circumference,and back pain and improve the patient’s mental state and postpartum depression.展开更多
BACKGROUND Immunoglobulin G4-related disease(IgG4-RD)is a complex immune-mediated condition that causes fibrotic inflammation in several organs.A significant clinical feature of IgG4-RD is hypertrophic pachymeningitis...BACKGROUND Immunoglobulin G4-related disease(IgG4-RD)is a complex immune-mediated condition that causes fibrotic inflammation in several organs.A significant clinical feature of IgG4-RD is hypertrophic pachymeningitis,which manifests as inflammation of the dura mater in intracranial or spinal regions.Although IgG4-RD can affect multiple areas,the spine is a relatively rare site compared to the more frequent involvement of intracranial structures.CASE SUMMARY A 70-year-old male presented to our hospital with a two-day history of fever,altered mental status,and generalized weakness.The initial brain magnetic resonance imaging(MRI)revealed multiple small infarcts across various cerebral regions.On the second day after admission,a physical examination revealed motor weakness in both lower extremities and diminished sensation in the right lower extremity.Electromyographic evaluation revealed findings consistent with acute motor sensory neuropathy.Despite initial management with intravenous immunoglobulin for presumed Guillain-Barrésyndrome,the patient exhibited progressive worsening of motor deficits.On the 45th day of hospitalization,an enhanced MRI of the entire spine,focusing specifically on the thoracic 9 to lumbar 1 vertebral level,raised the suspicion of IgG4-related spinal pachymeningitis.Subsequently,the patient was administered oral prednisolone and participated in a comprehensive rehabilitation program that included gait training and lower extremity strengthening exercises.CONCLUSION IgG4-related spinal pachymeningitis,diagnosed on MRI,was treated with corticosteroids and a structured rehabilitation regimen,leading to significant improvement.展开更多
Karl Ludwig Kahlbaum(1828-1899)was the first to conceptualize and describe the main clinical features of a novel psychiatric illness,which he termed catatonia in his groundbreaking monograph published 150 years ago.Al...Karl Ludwig Kahlbaum(1828-1899)was the first to conceptualize and describe the main clinical features of a novel psychiatric illness,which he termed catatonia in his groundbreaking monograph published 150 years ago.Although Kahlbaum postulated catatonia as a separate disease entity characterized by psychomotor symptoms and a cyclical course,a close examination of his 26 cases reveals that most of them presented with motor symptom complexes or syndromes associated with various psychiatric and medical conditions.In his classification system,Kraepelin categorized catatonic motor symptoms that occur in combination with psychotic symptoms and typically have a poor prognosis within his dementia praecox(schizophrenia)disease entity.Because of the substantial influence of Kraepelin’s classification,catatonia was predominantly perceived as a component of schizophrenia for most of the 20th century.However,with the advent of the psychopharmacotherapy era starting from the early 1950s,interest in catatonia in both clinical practice and research subsided until the early 2000s.The past two decades have witnessed a resurgence of interest in catatonia.The Diagnostic and Statistical Manual of Mental Disorders Fifth Edition,marked a paradigmatic shift by acknowledging that catatonia can occur secondary to various psychiatric and medical conditions.The introduction of an independent diagnostic category termed“Catatonia Not Otherwise Specified”significantly stimulated research in this field.The authors briefly review the history and findings of recent catatonia research and highlight promising directions for future exploration.展开更多

Objective:To apply and verify the application of intelligent audit rules for urine analysis by Cui et al.Method:A total of 1139 urine samples of hospitalized patients in Tai’an Central Hospital from September 2021 to...Objective:To apply and verify the application of intelligent audit rules for urine analysis by Cui et al.Method:A total of 1139 urine samples of hospitalized patients in Tai’an Central Hospital from September 2021 to November 2021 were randomly selected,and all samples were manually microscopic examined after the detection of the UN9000 urine analysis line.The intelligent audit rules(including the microscopic review rules and manual verification rules)were validated based on the manual microscopic examination and manual audit,and the rules were adjusted to apply to our laboratory.The laboratory turnaround time(TAT)before and after the application of intelligent audit rules was compared.Result:The microscopic review rate of intelligent rules was 25.63%(292/1139),the true positive rate,false positive rate,true negative rate,and false negative rate were 27.66%(315/1139),6.49%(74/1139),62.34%(710/1139)and 3.51%(40/1139),respectively.The approval consistency rate of manual verification rules was 84.92%(727/856),the approval inconsistency rate was 0%(0/856),the interception consistency rate was 12.61%(108/856),and the interception inconsistency rate was 0%(0/856).Conclusion:The intelligence audit rules for urine analysis by Cui et al.have good clinical applicability in our laboratory.展开更多
Sign language fills the communication gap for people with hearing and speaking ailments.It includes both visual modalities,manual gestures consisting of movements of hands,and non-manual gestures incorporating body mo...Sign language fills the communication gap for people with hearing and speaking ailments.It includes both visual modalities,manual gestures consisting of movements of hands,and non-manual gestures incorporating body movements including head,facial expressions,eyes,shoulder shrugging,etc.Previously both gestures have been detected;identifying separately may have better accuracy,butmuch communicational information is lost.Aproper sign language mechanism is needed to detect manual and non-manual gestures to convey the appropriate detailed message to others.Our novel proposed system contributes as Sign LanguageAction Transformer Network(SLATN),localizing hand,body,and facial gestures in video sequences.Here we are expending a Transformer-style structural design as a“base network”to extract features from a spatiotemporal domain.Themodel impulsively learns to track individual persons and their action context inmultiple frames.Furthermore,a“head network”emphasizes hand movement and facial expression simultaneously,which is often crucial to understanding sign language,using its attention mechanism for creating tight bounding boxes around classified gestures.The model’s work is later compared with the traditional identification methods of activity recognition.It not only works faster but achieves better accuracy as well.Themodel achieves overall 82.66%testing accuracy with a very considerable performance of computation with 94.13 Giga-Floating Point Operations per Second(G-FLOPS).Another contribution is a newly created dataset of Pakistan Sign Language forManual and Non-Manual(PkSLMNM)gestures.展开更多
Inventory counting is crucial to manufacturing industries in terms of inventory management,production,and procurement planning.Many companies currently require workers to manually count and track the status of materia...Inventory counting is crucial to manufacturing industries in terms of inventory management,production,and procurement planning.Many companies currently require workers to manually count and track the status of materials,which are repetitive and non-value-added activities but incur significant costs to the companies as well as mental fatigue to the employees.This research aims to develop a computer vision system that can automate the material counting activity without applying any marker on the material.The type of material of interest is metal sheet,whose shape is simple,a large rectangular shape,yet difficult to detect.The use of computer vision technology can reduce the costs incurred fromthe loss of high-value materials,eliminate repetitive work requirements for skilled labor,and reduce human error.A computer vision system is proposed and tested on a metal sheet picking process formultiple metal sheet stacks in the storage area by using one video camera.Our results show that the proposed computer vision system can count the metal sheet picks under a real situation with a precision of 97.83%and a recall of 100%.展开更多

Establishing trends in blood stream infections and resistance...Introduction: Antimicrobial Resistance surveillance is predicated on blood culture as a priority clinical specimen in especially resource limited settings. Establishing trends in blood stream infections and resistance patterns can inform institutional and national policy on antimicrobial stewardship, surveillance, infection prevention and control. Methodology: Blood Culture isolates in children (0 - 18 years) by conventional method from 2008-2012 and Bactec Automated culture system from 2015-2020 were retrieved. Information analyzed included age, sex, month, and year and culture growth/identity of microorganisms and their sensitivity/resistance patterns. Clinical and Laboratory Standards Institute (CLSI) guideline for antibiotic susceptibility testing was used. Results: 20,540 children were admitted: 8964 (44.6%) and 11,630 (55.4%) in the Manual and Bactec blood culture era respectively. Blood cultures were done in 5271 in the manual culture era and 1077 in the Bactec culture era;of these cultures, 514 (9.7%) and 461 (42.8%) were positive for isolates in the respective era (p = 0.01). There were no statistically significant differences in trend between positive and negative blood cultures in males and females. Newborns, followed by children 1 - 5 years had more blood culture performed on them than other age categories. In general, there is no significant relationship in blood culture outcomes between the age categories and sex of the patients. The isolation of Staph aureus, Citrobacter and Alkaligenes increased two-fold with Bactec automated system. Resistance to the quinolones and the penicillin was high. Resistance trend to Genticin, an aminoglycoside was less than 40%. Resistance to Ceftazidime was high. Conclusion: Antimicrobial resistance surveillance is critical to reduce AMR related morbidity and mortality.展开更多
Measurement of externalizing disorders such as antisocial disorders,attentiondeficit/hyperactivity disorder or borderline disorder have relevant implications for the daily lives of people with these disorders.While th...Measurement of externalizing disorders such as antisocial disorders,attentiondeficit/hyperactivity disorder or borderline disorder have relevant implications for the daily lives of people with these disorders.While the Diagnostic and Statistical Manual of Mental Disorders(DSM)and the International Classification of Diseases(ICD)have provided the diagnostic framework for decades,recent dimensional frameworks question the categorical approach of psychopathology,inherent in traditional nosotaxies.Tests and instruments develop under the DSM or ICD framework preferentially adopt this categorical approach,providing diagnostic labels.In contrast,dimensional measurement instruments provide an individualized profile for the domains that comprise the externalizing spectrum,but are less widely used in practice.Current paper aims to review the operational definitions of externalizing disorders defined under these different frameworks,revise the different measurement alternatives existing,and provide an integrative operational definition.First,an analysis of the operational definition of externalizing disorders among the DSM/ICD diagnostic systems and the recent Hierarchical Taxonomy of Psychopathology(HiTOP)model is carried out.Then,in order to analyze the coverage of operational definitions found,a description of measurement instruments among each conceptualization is provided.Three phases in the development of the ICD and DSM diagnosis systems can be observed with direct implications for measurement.ICD and DSM versions have progressively introduced systematicity,providing more detailed descriptions of diagnostic criteria and categories that ease the measurement instrument development.However,it is questioned whether the DSM/ICD systems adequately modelize externalizing disorders,and therefore their measurement.More recent theoretical approaches,such as the HiTOP model seek to overcome some of the criticism raised towards the classification systems.Nevertheless,several issues concerning this model raise mesasurement challenges.A revision of the instruments underneath each approach shows incomplete coverage of externalizing disorders among the existing instruments.Efforts to bring nosotaxies together with other theoretical models of psychopathology and personality are still needed.The integrative operational definition of externalizing disorders provided may help to gather clinical practice and research.展开更多

In the European Union, some countries produce cause-of-death statistics by a manual coding of death certificates, while other countries use an automated coding system. The outcome of these two different methods in terms of the selected underlying cause of death for statistics may vary considerably. Therefore, this study explores the effect of coding method on the ranking of countries by major causes of death. Method: Age and sex standardized rates were extracted for 33 European (related) countries from the cause-of-death registry of the European Statistical Office (Eurostat). Wilcoxon’s rank sum test was applied to the ranking of countries by major causes of death. Results: Statistically significant differences due to coding method were identified for dementia, stroke and pneumonia. These differences could be explained by a different selection of dementia or pneumonia as underlying cause of death and by a different certification practice for stroke. Conclusion: Coding method should be taken into account when constructing or interpreting rankings of countries by cause of death.展开更多

Acute kidney injury (AKI) has been widely reported (13% - 50%) as a complication of Rhabdomyolysis in which the main pathophysiological mechanisms are intra-renal vasoconstriction, intraluminal cast formation (Tamm-Horsefall) and direct myoglobin toxicity. In this report we are going to describe the management of Acute Kidney Injury due to Rhabdomyolysis that is not responding to vigorous rehydration. Objective: Reporting about acute renal failure induced by Rhabdomyolysis due to Excessive Exercise and dehydration in young patient. Case Report: A 20 years male came to the outpatient clinic complaining of sever lower limb pain, back pain and vomiting. He was anuric and hypertensive (BP = 150/90 mmHg) with serum creatinine and urea levels of 15.72 mg/dl and 235 mg/dl, respectively. The diagnosis was based on the laboratory finding of creatine kinase = 3127 IU/l. The patient, then, has been referred to the emergency department. The Management plan was based on two arms: Emergency Management with Urgent Hemodialysis for AKI and high fluid replacement therapy. Patient started to recover after three hemodialysis sessions but the peak of recovery was noted after starting manual fluid replacement therapy with a target urine output of greater than 2 ml/kg, a urine pH of greater than 6. Manual fluid replacement therapy consisted of loop diuretics, intravenous fluids and intravenous sodium bicarbonate 1.26%. Full recovery was noted after one month of hospital admission with inpatient care and regular follow-up. A follow-up after one month has been set to assess the patient progression and monitor his kidney functions. Relevance and Impact: Home messages and lessons are;Firstly, young adults are vulnerable to Rhabdomyolysis, second, the diagnosis of Rhabdomyolysis can be made on the clinical bases but a confirmatory laboratory test of Creatine Kinase is mandatory, and lastly’ acute kidney injury needs to be treated urgently. Also, reducing the risk of infection is one of the management objectives to achieve recovery.展开更多

This results in th...A large part of our daily lives is spent with audio information. Massive obstacles are frequently presented by the colossal amounts of acoustic information and the incredibly quick processing times. This results in the need for applications and methodologies that are capable of automatically analyzing these contents. These technologies can be applied in automatic contentanalysis and emergency response systems. Breaks in manual communication usually occur in emergencies leading to accidents and equipment damage. The audio signal does a good job by sending a signal underground, which warrants action from an emergency management team at the surface. This paper, therefore, seeks to design and simulate an audio signal alerting and automatic control system using Unity Pro XL to substitute manual communication of emergencies and manual control of equipment. Sound data were trained using the neural network technique of machine learning. The metrics used are Fast Fourier transform magnitude, zero crossing rate, root mean square, and percentage error. Sounds were detected with an error of approximately 17%;thus, the system can detect sounds with an accuracy of 83%. With more data training, the system can detect sounds with minimal or no error. The paper, therefore, has critical policy implications about communication, safety, and health for underground mine.展开更多

文摘The Turpan-Hami(Tuha)Basin in Xinjiang Uygur Autonomous Region of China,holds significant strategic importance as a key economic artery of the ancient Silk Road and the Belt and Road Initiative,necessitating a holistic understanding of the spatiotemporal evolution of land use/land cover(LULC)to foster sustainable planning that is tailored to the region's unique resource endowments.However,existing LULC classification methods demonstrate inadequate accuracy,hindering effective regional planning.In this study,we established a two-level LULC classification system(8 primary types and 22 secondary types)for the Tuha Basin.By employing Landsat 5/7/8 imagery at 5-a intervals,we developed the LULC dataset of the Tuha Basin from 1990 to 2020,conducted the accuracy assessment and spatiotemporal evolution analysis,and simulated the future LULC under various scenarios via the Markov-Future Land Use Simulation(Markov-FLUS)model.The results revealed that the average overall accuracy values of our LULC dataset were 0.917 and 0.864 for the primary types and secondary types,respectively.Compared with the seven mainstream LULC products(GlobeLand30,Global 30-meter Land Cover with Fine Classification System(GLC_FCS30),Finer Resolution Observation and Monitoring of Global Land Cover PLUS(FROM_GLC PLUS),ESA Global Land Cover(ESA_LC),Esri Land Cover(ESRI_LC),China Multi-Period Land Use Land Cover Change Remote Sensing Monitoring Dataset(CNLUCC),and China Annual Land Cover Dataset(CLCD))in 2020,our LULC data exhibited dramatically elevated overall accuracy and provided more precise delineations for land features,thereby yielding high-quality data backups for land resource analyses within the basin.In 2020,unused land(78.0%of the study area)and grassland(18.6%)were the dominant LULC types of the basin;although cropland and construction land constituted less than 1.0%of the total area,they played a vital role in arid land development and primarily situated within oases that form the urban cores of the cities of Turpan and Hami.Between 1990 and 2020,cropland and construction land exhibited a rapid expansion,and the total area of water body decreased yet resurging after 2015 due to an increase in areas of reservoir and pond.In future scenario simulations,significant increases in areas of construction land and cropland are anticipated under the business-as-usual scenario,whereas the wetland area will decrease,suggesting the need for ecological attention under this development pathway.In contrast,the economic development scenario underscores the fast-paced expansion of construction land,primarily from the conversion of unused land,highlighting the significant developmental potential of unused land with a slowing increase in cropland.Special attention should thus be directed toward ecological and cropland protection during development.This study provides data supports and policy recommendations for the sustainable development goals of Tuha Basin and other similar arid areas.

文摘BACKGROUND Pigmented villonodular synovitis(PVNS)is a benign proliferative disorder that affects the synovial joints,bursae,and tendon sheaths.To date,few studies have reported on the treatment of postoperative pain and edema in patients with PVNS.Herein,we present the case of a woman who developed pain and edema in the left lower limb 1 wk after synovectomy and arthroscopic partial meniscectomy and was unable to walk due to limited flexion and extension of the left knee.CASE SUMMARY A 32-year-old woman underwent synovectomy and arthroscopic partial meniscectomy successively and was treated with a combination of manual lymphatic drainage(MLD)and kinesio taping(KT)in our hospital to alleviate postoperative pain and edema.The following parameters were assessed at 2 wk post-treatment and 1 wk post-discharge follow up:suprapatellar circumference,infrapatellar circumference,visual analog scale score,knee range of motion,pittsburgh sleep quality index score,hamilton anxiety rating scale(HAMA)score,and hamilton depression rating scale(HAMD)score.After treatment,the postoperative pain and edema in the patient’s left knee were effectively relieved,resulting in improved sleep quality and remarkably attenuated HAMA and HAMD scores.CONCLUSION Combined MLD and KT may be an effective approach for relieving postoperative pain and edema in patients with PVNS.

文摘BACKGROUND Rectus abdominis separation(DRA)affects pelvic stability and body image.No studies have explored the effects of manual massage on early postpartum DRA and postpartum depression.AIM To analyze the curative effect of massage on early postpartum DRA and its impact on postpartum depression and thus its ability promote the overall psychosomatic rehabilitation of postpartum women.METHODS Data were retrospectively collected on 70 primiparous women with postpartum DRA who underwent rehabilitation at the Postpartum Rehabilitation Center of Huzhou Maternal and Child Health Hospital from October 2022 to September 2023.The patients were divided into the Group S(35 cases,biomimetic electrical stimulation treatment)and Group L(35 cases,biomimetic electrical stimulation combined with manual massage treatment).Baseline data,the edinburgh postpartum depression scale(EPDS)score,and the visual analog scale(VAS)scores for rectus abdominis distance,waist circumference,and lower back pain before and after treatment were compared.RESULTS No significant differences were found in the baseline data,rectus abdominis distance,waist circumference,and VAS and EPDS scores between the two groups before treatment(P>0.05).After treatment,the distance between rectus abdominis and waist circumference in Group L were significantly smaller than those in Group S(P<0.05).Furthermore,lower back pain(VAS score)and the EPDS score in Group L were significantly lower than those in Group S(P<0.05).CONCLUSION Manual massage can significantly reduce early postpartum DRA,waist circumference,and back pain and improve the patient’s mental state and postpartum depression.
文摘BACKGROUND Immunoglobulin G4-related disease(IgG4-RD)is a complex immune-mediated condition that causes fibrotic inflammation in several organs.A significant clinical feature of IgG4-RD is hypertrophic pachymeningitis,which manifests as inflammation of the dura mater in intracranial or spinal regions.Although IgG4-RD can affect multiple areas,the spine is a relatively rare site compared to the more frequent involvement of intracranial structures.CASE SUMMARY A 70-year-old male presented to our hospital with a two-day history of fever,altered mental status,and generalized weakness.The initial brain magnetic resonance imaging(MRI)revealed multiple small infarcts across various cerebral regions.On the second day after admission,a physical examination revealed motor weakness in both lower extremities and diminished sensation in the right lower extremity.Electromyographic evaluation revealed findings consistent with acute motor sensory neuropathy.Despite initial management with intravenous immunoglobulin for presumed Guillain-Barrésyndrome,the patient exhibited progressive worsening of motor deficits.On the 45th day of hospitalization,an enhanced MRI of the entire spine,focusing specifically on the thoracic 9 to lumbar 1 vertebral level,raised the suspicion of IgG4-related spinal pachymeningitis.Subsequently,the patient was administered oral prednisolone and participated in a comprehensive rehabilitation program that included gait training and lower extremity strengthening exercises.CONCLUSION IgG4-related spinal pachymeningitis,diagnosed on MRI,was treated with corticosteroids and a structured rehabilitation regimen,leading to significant improvement.
文摘Karl Ludwig Kahlbaum(1828-1899)was the first to conceptualize and describe the main clinical features of a novel psychiatric illness,which he termed catatonia in his groundbreaking monograph published 150 years ago.Although Kahlbaum postulated catatonia as a separate disease entity characterized by psychomotor symptoms and a cyclical course,a close examination of his 26 cases reveals that most of them presented with motor symptom complexes or syndromes associated with various psychiatric and medical conditions.In his classification system,Kraepelin categorized catatonic motor symptoms that occur in combination with psychotic symptoms and typically have a poor prognosis within his dementia praecox(schizophrenia)disease entity.Because of the substantial influence of Kraepelin’s classification,catatonia was predominantly perceived as a component of schizophrenia for most of the 20th century.However,with the advent of the psychopharmacotherapy era starting from the early 1950s,interest in catatonia in both clinical practice and research subsided until the early 2000s.The past two decades have witnessed a resurgence of interest in catatonia.The Diagnostic and Statistical Manual of Mental Disorders Fifth Edition,marked a paradigmatic shift by acknowledging that catatonia can occur secondary to various psychiatric and medical conditions.The introduction of an independent diagnostic category termed“Catatonia Not Otherwise Specified”significantly stimulated research in this field.The authors briefly review the history and findings of recent catatonia research and highlight promising directions for future exploration.

文摘Objective:To apply and verify the application of intelligent audit rules for urine analysis by Cui et al.Method:A total of 1139 urine samples of hospitalized patients in Tai’an Central Hospital from September 2021 to November 2021 were randomly selected,and all samples were manually microscopic examined after the detection of the UN9000 urine analysis line.The intelligent audit rules(including the microscopic review rules and manual verification rules)were validated based on the manual microscopic examination and manual audit,and the rules were adjusted to apply to our laboratory.The laboratory turnaround time(TAT)before and after the application of intelligent audit rules was compared.Result:The microscopic review rate of intelligent rules was 25.63%(292/1139),the true positive rate,false positive rate,true negative rate,and false negative rate were 27.66%(315/1139),6.49%(74/1139),62.34%(710/1139)and 3.51%(40/1139),respectively.The approval consistency rate of manual verification rules was 84.92%(727/856),the approval inconsistency rate was 0%(0/856),the interception consistency rate was 12.61%(108/856),and the interception inconsistency rate was 0%(0/856).Conclusion:The intelligence audit rules for urine analysis by Cui et al.have good clinical applicability in our laboratory.
文摘Sign language fills the communication gap for people with hearing and speaking ailments.It includes both visual modalities,manual gestures consisting of movements of hands,and non-manual gestures incorporating body movements including head,facial expressions,eyes,shoulder shrugging,etc.Previously both gestures have been detected;identifying separately may have better accuracy,butmuch communicational information is lost.Aproper sign language mechanism is needed to detect manual and non-manual gestures to convey the appropriate detailed message to others.Our novel proposed system contributes as Sign LanguageAction Transformer Network(SLATN),localizing hand,body,and facial gestures in video sequences.Here we are expending a Transformer-style structural design as a“base network”to extract features from a spatiotemporal domain.Themodel impulsively learns to track individual persons and their action context inmultiple frames.Furthermore,a“head network”emphasizes hand movement and facial expression simultaneously,which is often crucial to understanding sign language,using its attention mechanism for creating tight bounding boxes around classified gestures.The model’s work is later compared with the traditional identification methods of activity recognition.It not only works faster but achieves better accuracy as well.Themodel achieves overall 82.66%testing accuracy with a very considerable performance of computation with 94.13 Giga-Floating Point Operations per Second(G-FLOPS).Another contribution is a newly created dataset of Pakistan Sign Language forManual and Non-Manual(PkSLMNM)gestures.

文摘Inventory counting is crucial to manufacturing industries in terms of inventory management,production,and procurement planning.Many companies currently require workers to manually count and track the status of materials,which are repetitive and non-value-added activities but incur significant costs to the companies as well as mental fatigue to the employees.This research aims to develop a computer vision system that can automate the material counting activity without applying any marker on the material.The type of material of interest is metal sheet,whose shape is simple,a large rectangular shape,yet difficult to detect.The use of computer vision technology can reduce the costs incurred fromthe loss of high-value materials,eliminate repetitive work requirements for skilled labor,and reduce human error.A computer vision system is proposed and tested on a metal sheet picking process formultiple metal sheet stacks in the storage area by using one video camera.Our results show that the proposed computer vision system can count the metal sheet picks under a real situation with a precision of 97.83%and a recall of 100%.

文摘Introduction: Antimicrobial Resistance surveillance is predicated on blood culture as a priority clinical specimen in especially resource limited settings. Establishing trends in blood stream infections and resistance patterns can inform institutional and national policy on antimicrobial stewardship, surveillance, infection prevention and control. Methodology: Blood Culture isolates in children (0 - 18 years) by conventional method from 2008-2012 and Bactec Automated culture system from 2015-2020 were retrieved. Information analyzed included age, sex, month, and year and culture growth/identity of microorganisms and their sensitivity/resistance patterns. Clinical and Laboratory Standards Institute (CLSI) guideline for antibiotic susceptibility testing was used. Results: 20,540 children were admitted: 8964 (44.6%) and 11,630 (55.4%) in the Manual and Bactec blood culture era respectively. Blood cultures were done in 5271 in the manual culture era and 1077 in the Bactec culture era;of these cultures, 514 (9.7%) and 461 (42.8%) were positive for isolates in the respective era (p = 0.01). There were no statistically significant differences in trend between positive and negative blood cultures in males and females. Newborns, followed by children 1 - 5 years had more blood culture performed on them than other age categories. In general, there is no significant relationship in blood culture outcomes between the age categories and sex of the patients. The isolation of Staph aureus, Citrobacter and Alkaligenes increased two-fold with Bactec automated system. Resistance to the quinolones and the penicillin was high. Resistance trend to Genticin, an aminoglycoside was less than 40%. Resistance to Ceftazidime was high. Conclusion: Antimicrobial resistance surveillance is critical to reduce AMR related morbidity and mortality.

文摘Measurement of externalizing disorders such as antisocial disorders,attentiondeficit/hyperactivity disorder or borderline disorder have relevant implications for the daily lives of people with these disorders.While the Diagnostic and Statistical Manual of Mental Disorders(DSM)and the International Classification of Diseases(ICD)have provided the diagnostic framework for decades,recent dimensional frameworks question the categorical approach of psychopathology,inherent in traditional nosotaxies.Tests and instruments develop under the DSM or ICD framework preferentially adopt this categorical approach,providing diagnostic labels.In contrast,dimensional measurement instruments provide an individualized profile for the domains that comprise the externalizing spectrum,but are less widely used in practice.Current paper aims to review the operational definitions of externalizing disorders defined under these different frameworks,revise the different measurement alternatives existing,and provide an integrative operational definition.First,an analysis of the operational definition of externalizing disorders among the DSM/ICD diagnostic systems and the recent Hierarchical Taxonomy of Psychopathology(HiTOP)model is carried out.Then,in order to analyze the coverage of operational definitions found,a description of measurement instruments among each conceptualization is provided.Three phases in the development of the ICD and DSM diagnosis systems can be observed with direct implications for measurement.ICD and DSM versions have progressively introduced systematicity,providing more detailed descriptions of diagnostic criteria and categories that ease the measurement instrument development.However,it is questioned whether the DSM/ICD systems adequately modelize externalizing disorders,and therefore their measurement.More recent theoretical approaches,such as the HiTOP model seek to overcome some of the criticism raised towards the classification systems.Nevertheless,several issues concerning this model raise mesasurement challenges.A revision of the instruments underneath each approach shows incomplete coverage of externalizing disorders among the existing instruments.Efforts to bring nosotaxies together with other theoretical models of psychopathology and personality are still needed.The integrative operational definition of externalizing disorders provided may help to gather clinical practice and research.
文摘Background: Cause-of-death rankings are often used for planning or evaluating health policy measures. In the European Union, some countries produce cause-of-death statistics by a manual coding of death certificates, while other countries use an automated coding system. The outcome of these two different methods in terms of the selected underlying cause of death for statistics may vary considerably. Therefore, this study explores the effect of coding method on the ranking of countries by major causes of death. Method: Age and sex standardized rates were extracted for 33 European (related) countries from the cause-of-death registry of the European Statistical Office (Eurostat). Wilcoxon’s rank sum test was applied to the ranking of countries by major causes of death. Results: Statistically significant differences due to coding method were identified for dementia, stroke and pneumonia. These differences could be explained by a different selection of dementia or pneumonia as underlying cause of death and by a different certification practice for stroke. Conclusion: Coding method should be taken into account when constructing or interpreting rankings of countries by cause of death.
文摘Introduction: Rhabdomyolysis is severe and life threating condition in which skeletal muscles are damaged by dissolution of striped muscles. Acute kidney injury (AKI) has been widely reported (13% - 50%) as a complication of Rhabdomyolysis in which the main pathophysiological mechanisms are intra-renal vasoconstriction, intraluminal cast formation (Tamm-Horsefall) and direct myoglobin toxicity. In this report we are going to describe the management of Acute Kidney Injury due to Rhabdomyolysis that is not responding to vigorous rehydration. Objective: Reporting about acute renal failure induced by Rhabdomyolysis due to Excessive Exercise and dehydration in young patient. Case Report: A 20 years male came to the outpatient clinic complaining of sever lower limb pain, back pain and vomiting. He was anuric and hypertensive (BP = 150/90 mmHg) with serum creatinine and urea levels of 15.72 mg/dl and 235 mg/dl, respectively. The diagnosis was based on the laboratory finding of creatine kinase = 3127 IU/l. The patient, then, has been referred to the emergency department. The Management plan was based on two arms: Emergency Management with Urgent Hemodialysis for AKI and high fluid replacement therapy. Patient started to recover after three hemodialysis sessions but the peak of recovery was noted after starting manual fluid replacement therapy with a target urine output of greater than 2 ml/kg, a urine pH of greater than 6. Manual fluid replacement therapy consisted of loop diuretics, intravenous fluids and intravenous sodium bicarbonate 1.26%. Full recovery was noted after one month of hospital admission with inpatient care and regular follow-up. A follow-up after one month has been set to assess the patient progression and monitor his kidney functions. Relevance and Impact: Home messages and lessons are;Firstly, young adults are vulnerable to Rhabdomyolysis, second, the diagnosis of Rhabdomyolysis can be made on the clinical bases but a confirmatory laboratory test of Creatine Kinase is mandatory, and lastly’ acute kidney injury needs to be treated urgently. Also, reducing the risk of infection is one of the management objectives to achieve recovery.
文摘A large part of our daily lives is spent with audio information. Massive obstacles are frequently presented by the colossal amounts of acoustic information and the incredibly quick processing times. This results in the need for applications and methodologies that are capable of automatically analyzing these contents. These technologies can be applied in automatic contentanalysis and emergency response systems. Breaks in manual communication usually occur in emergencies leading to accidents and equipment damage. The audio signal does a good job by sending a signal underground, which warrants action from an emergency management team at the surface. This paper, therefore, seeks to design and simulate an audio signal alerting and automatic control system using Unity Pro XL to substitute manual communication of emergencies and manual control of equipment. Sound data were trained using the neural network technique of machine learning. The metrics used are Fast Fourier transform magnitude, zero crossing rate, root mean square, and percentage error. Sounds were detected with an error of approximately 17%;thus, the system can detect sounds with an accuracy of 83%. With more data training, the system can detect sounds with minimal or no error. The paper, therefore, has critical policy implications about communication, safety, and health for underground mine.
