Comparison of the Effectiveness of Marked Suction Tubes vs. Plain Suction Tubes in Pediatric Mechanically Ventilated Patients

Introduction: Endotracheal suction plays a crucial role in the management of mechanically ventilated patients. This study aims to evaluate the clinical effectiveness and safety of suction tubes with markings in mechanically ventilated pediatric patients. Materials and Methods: A randomized assignment was carried out on a cohort of 52 pediatric patients who underwent mechanical ventilation in the Pediatric Intensive Care Unit at the Third Affiliated Hospital of Sun Yat-sen University, covering the period from January 2022 to December 2022. These patients were divided into two groups: an improved group (n = 26) utilizing marked suction tubes, and a regular group (n = 26) employing conventional suction tubes. The objective of our study was to evaluate the clinical effectiveness of marked suction tubes. Results: The effects of the improved group on the vital signs of children undergoing mechanical ventilation were small and statistically significant compared with the regular group (p < 0.05). Additionally, the improved group exhibited a reduced frequency of sputum suction, shorter mechanical ventilation duration, and fewer days of hospitalization in the PICU compared to the regular group during the ventilation period. Notably, the difference in the duration of PICU hospitalization was statistically significant (p < 0.05). Moreover, the incidence of adverse reactions in the improved group was notably lower, with statistically significant differences observed in airway mucous membrane damage and irritating cough when compared to the regular group (p < 0.05). Conclusion: The utilization of marked suction tubes provides clinical nurses with clear guidance for performing suctioning with ease, efficiency and safety. Consequently, advocating for the widespread implementation of marked suction tubes in clinical practice is a commendable pursuit.

Cancer Status: Mocb and tPSA Prostate Cancer Markers

On the basis of the results of magneto-optical studies and their comparison with the outcome of medical tests magneto-optical MOCB and tPSA serum markers of cancer/recovered patient are presented. Status of the cancer serum donor is recognized as presence of the laevorotatory optical active molecules of (-)p density in serum while status of the recovered donor is recognized as quantitative domination of the dextrorotatory (+)p molecules in serum. These molecular information carriers (-)p and (+)p are recorded by the magneto-optical circular birefringence MOCB effect in B2 magnetic field. The laevo/dextrorotatory carriers are enantiomers in the case of (-)p = (+)p relations fulfilled for any individual cancer and the same recovered patient. The positive predictive value PPV of MOCB results is 100%.

Serum immunoglobulin A concentration is a reliable biomarker for liver fibrosis in non-alcoholic fatty liver disease

AIM:To evaluate the diagnostic accuracy of serum Immunoglobulin A(IgA)for differentiating early stage nonalcoholic fatty liver disease(NAFLD)from nonalcoholic steatohepatitis(NASH).METHODS:All cases had fatty liver change confirmed by ultrasound and aminotransferases of at least twice the normal level.Clinical and biochemical data,including serum IgA,were obtained from 50 histologically proven NAFLD cases and 54 healthy controls.Fasting whole blood samples were obtained from the study population.Immunoturbidimetric methods were used to measure the IgA levels.All NAFLD cases were hospitalized for liver biopsy.Liver specimens were examined for steatosis,steatohepatitis and fibrosis within hepatocytes.Patients were categorized into two groups:NASH and non-NASH.Variables were compared within cases(NASH vs non-NASH)and controls.Cut-off values of serum IgA were evaluated using analysis of receiver operating characteristic(ROC curves).Associations between the variables were tested using calculations of correlation coefficients.Statistical significances were assigned to P values<0.05.RESULTS:The extent of liver fibrosis correlated positively with IgA levels.Subjects with no fibrosis in their liver biopsies had a lower IgA level(301.5±91.2 mg/dL)than subjects with any degree of fibrosis(388.8±140.8 mg/dL),(P=0.01).IgA levels were higher in NASH cases,and its value was significantly higher for higher degrees of fibrosis.Patients with perisinusoidal or pericellular fibrosis had significantly higher levels of IgA(403.5±133.9 mg/dL,418.2±129.5 mg/dL)compared to those without it(301.8±94.9 mg/dL,297.7±91.5 mg/dL),respectively.No significant correlation was found between steatosis grade and serum IgA levels.Based on ROC analysis,the best predictive IgA cutoff value for detecting liver fibrosis was 360mg/dL(61%sensitivity,81%specificity).CONCLUSION:The serum IgA level is useful to evaluate the severity of liver fibrosis and can be used serially for evaluation and follow-up of NAFLD cases.

Cultured meat in western media: The disproportionate coverage of vegetarian reactions, demographic realities, and implications for cultured meat marketing

This paper examines the media coverage of the 2013 London cultured meat tasting event, particularly in the United States, Canada, and the United Kingdom. Using major news outlets, prominent magazines covering food and science issues, and advocacy websites concerning meat consumption, the paper characterizes the overall emphases of the coverage, the tenor of the coverage, and compares the media portrayal of the important issues to the demographic and psychological realities of the actual consumer market into which cultured meat will compete. In particular, the paper argues that Western media gives a distorted picture of what obstacles are in the path of cultured meat acceptance, especially by overemphasizing and overrepresenting the importance of the reception of cultured meat among vegetarians. Promoters of cultured meat should recognize the skewed impression that this media coverage provides and pay attention to the demographic data that suggests strict vegetarians are a demographically negligible group. Resources for promoting cultured meat should focus on the empirical demographics of the consumer market and the empirical psychology of mainstream consumers.

Parametric Transformation of Timed Weighted Marked Graphs:Applications in Optimal Resource Allocation

Timed weighted marked graphs are a subclass of timed Petri nets that have wide applications in the control and performance analysis of flexible manufacturing systems.Due to the existence of multiplicities(i.e.,weights)on edges,the performance analysis and resource optimization of such graphs represent a challenging problem.In this paper,we develop an approach to transform a timed weighted marked graph whose initial marking is not given,into an equivalent parametric timed marked graph where the edges have unitary weights.In order to explore an optimal resource allocation policy for a system,an analytical method is developed for the resource optimization of timed weighted marked graphs by studying an equivalent net.Finally,we apply the proposed method to a flexible manufacturing system and compare the results with a previous heuristic approach.Simulation analysis shows that the developed approach is superior to the heuristic approach.

沉默MARK4通过NF-κB信号通路抑制溃疡性结肠炎细胞焦亡和炎症因子表达

目的探讨MARK4(microtubule-affinity regulating kinase 4,MARK4)在脂多糖(lipopolysaccharide,LPS)诱导的FHC细胞溃疡性结肠炎(ulcerative colitis,UC)模型中对FHC细胞焦亡、炎症因子释放和肠道屏障相关蛋白表达的影响及其可能分子机制。方法采用Western blot检测UC患者、健康人群结肠组织和LPS诱导的FHC细胞炎症模型中MARK4和细胞焦亡相关的因子Caspase-1、NLRP3和GSDMD的表达水平;通过质粒转染shRNA沉默FHC细胞中MARK4,在正常对照组、LPS组和沉默MARK4+LPS组中采用Western blot分别检测Caspase-1、NLRP3、GSDMD、肠道屏障相关蛋白和NF-κB信号通路相关蛋白的表达水平;ELISA和RT-qPCR检测炎症因子IL-1β、IL-6和TNF-α表达水平;流式细胞术检测细胞凋亡情况。结果在UC患者结肠组织和LPS诱导FHC细胞体外UC炎症模型中MARK4和焦亡相关蛋白包括NLRP3、Caspase-1和GSDMD的表达水平显著升高。沉默MARK4能够抑制LPS诱导FHC细胞中焦亡相关蛋白NLRP3、Caspase-1、GSDMD表达水平,下调炎症相关细胞因子IL-1β、IL-6和TNF-α的表达水平,抑制FHC细胞凋亡,上调肠道屏障相关蛋白ZO-1、Occludin表达和上调Claudin2的表达水平。GSEA富集分析显示,MARK4与NF-κB信号通路呈正相关。沉默MARK4能够抑制NF-κB信号通路中p-P65、p-IKKα的表达水平。结论MARK4在UC组织和细胞中显著高表达,沉默MARK4能够通过阻滞NF-κB信号通路激活,进而抑制UC细胞焦亡和炎症因子表达。MARK4可能成为UC患者治疗的一个潜在靶点。

运动对孤独症谱系障碍大鼠内侧前额叶皮层树突棘重塑的影响及MARK1/MAP1A/PSD-95通路的作用

目的:探讨游泳结合转轮跑步运动干预对丙戊酸(valproic acid,VPA)诱导的孤独症谱系障碍(autism spectrum disorder,ASD)大鼠内侧前额叶皮层树突棘结构重塑的影响,以及微管亲和力调节激酶1(microtubule affinity regulating kinase 1,MARK1)、微管相关蛋白1A(microtubule-associated protein 1A,MAP1A)和突触后致密蛋白95(postsynaptic density protein-95,PSD-95)的作用。方法:通过妊娠期VPA或生理盐水暴露的方法收集雄性子代制备ASD模型大鼠及对照大鼠,随机分为运动组和静置组,每组9~11只。采用三箱社交实验评价运动对ASD大鼠社交行为的影响,荧光示踪病毒局部感染观察内侧前额叶皮层亚区边缘前皮层(prelimbic cortex,PrL)和边缘下皮层(infralimbic cortex,IL)树突棘密度和形态,Western blot检测PSD-95和突触小泡蛋白(synaptophysin,Syn)蛋白表达,RNA原位杂交检测MARK1、MAP1A和PSD-95阳性颗粒数及共定位情况。结果:与对照组相比,ASD模型组大鼠社交行为存在缺陷,表现为在陌生大鼠箱体的活动时间、嗅探陌生大鼠的时间和频率均显著减少(P<0.05);经过运动干预后ASD大鼠的社交行为显著改善。树突棘形态观察发现ASD大鼠PrL区而不是IL区总树突棘密度显著增加,主要表现为蘑菇型和短粗型树突棘密度增加(P<0.05);运动干预能有效改善树突棘异常重塑。Western blot发现运动干预能有效逆转ASD大鼠PrL区的PSD-95蛋白表达上升的趋势;而PrL区的Syn及IL区的PSD-95和Syn蛋白表达在各组均无统计学差异。RNA原位杂交发现运动干预能有效降低ASD大鼠PrL区异常增多的MARK1、MAP1A和PSD-95的RNA阳性颗粒数,降低MARK1+MAP1A、MARK1+PSD-95和MAP1A+PSD-95的共定位。结论:运动干预可能通过PrL区MARK1/MAP1A/PSD-95分子通路改善ASD大鼠树突棘结构重塑,从而改善社交相关行为障碍。

Non-leaving-face property for marked surfaces

Abstract We consider the polytope arising from a marked surface by flips of triangulations. D. D. Sleator, R. E. Tarjan, and W. P. Thurston [J. Amer. Math. Soc., 1988, 1(3): 647 681] studied the diameter of the associahedron, which is the polytope arising from a marked disc by flips of triangulations. They showed that every shortest path between two vertices in a face does not leave that face. We give a new method, which is different from the one used by V. Disarlo and H. Parlier [arXiv: 1411.4285] to establish the same non-leaving-face property for all unpunctured marked surfaces.

Moderate Deviations for Marked Hawkes Processes

We consider a linear Hawkes process with random marks. Some limit theorems have been studied by Karabash and Zhu [Stoch. Models, 31,433-451 （2015）]. In this paper, we obtain a moderate deviation principle for marked Hawkes processes.
