Cystic lesions of the anterior mediastinum in children suggest a well-known group of benign lesions that are comparatively frequent.Thymic cysts(TCs)are mostly positioned in the anterior mediastinum and some patients in the neck.Benign TCs classified as congenital intra-thoracic mesothelial cysts are commonly asymptomatic and have slight clinical significance.Multilocular TC,which can mimic another anterior mediastinal cystic tumor and is seen in adults,is more clinically important. It is a sporadic mediastinal lesion thought to arise in the course of acquired inflammation.Congenital mediastinal cysts represent 3%-6%of all mediastinal tumors and 10%-18%of radiologically reported mediastinal masses.Mediastinal TCs are uncommon and it is hard to know their true incidence.About 60%of cases with mediastinal TCs are asymptomatic, and the remainder of patients complains of nonspecific symptoms(e.g.,chest pain, dyspnea,or cough). The literature suggests that most cysts are benign, but an indefinite percentage may have a neoplastic process and result in significant compressive symptoms over time.Clinical symptoms of TCs vary depending on the location. In addition, frequent symptoms at the appearance of enlarged benign thymic and mediastinal cysts generally contain compressive symptoms(e.g.,respiratory distress, thymic pain, and symptoms related to Horner syndrome, hoarseness, dysphonia, dyspnea, orthopnea, wheezing, and fever). Many TCs have cystic density and a neat border and are simple to diagnose with radiological imaging. However, some TCs are hard to identify before surgery and may be misidentified as thymomas depending on their site and computed tomography results.Excision by thoracotomy, median sternotomy,or video-assisted techniques is essential for conclusive diagnosis, management, and abolition of relapse of anterior mediastinal masses and TCs.Histopathologic examination may be required after surgery.Considering the extent of the mass and the preliminary inability to make a definitive diagnosis,en bloc excision of the cyst was thought to be preferred to circumvent likely complications(e.g.,perforation,spillage of the contents,or incomplete excision). 展开更多

Multiple tuned mass dampers(MTMDs)reduce dynamic response with multiple specified frequencies of building structures. Many optimization algorithms for placement design exist, though they rarely conform to code-based verification nor produce high quality solutions without high computational effort and high complexity. This study proposes an inverse element exchange method(IEEM)with multi-level programming and compares it to a single tuned mass damper(STMD)and uniform distribution of multiple tuned mass dampers in the frequency and time domains.A ten-story shear building is used for the numerical case study. The results show that the proposed method can offer improvement over the STMD,uniform distribution of multiple tuned mass dampers, and distribution optimized by genetic algorithms(GA)with regard to minimizing the interstory drift ratio(IDR)in both the frequency and time domains and the time consumption for optimization. 展开更多

In hilly regions, the existence of surface cracks in rock mass induces a potential threat to structural stability. Thus, the present research aims to explore the impact of surface cracks on the loadbearing capacity of strip footing placed on the rock mass.By taking into account the various boundary constraints across the surface of crack edges, the study investigates the presence of two categories of surface cracks,namely(1)separated crack,and(2)fine crack. The lower bound limit analysis is employed in conjunction with the finite element method(LBFELA)to conduct the numerical analysis. In order to evaluate rock mass yielding, the power conic programming(PCP)method is utilized to implement the generalized Hoek-Brown(GHB)failure criterion. The stability of the strip footing is analyzed by determining the bearing capacity factor(Nσγ),which is presented in the form of design charts by varying the strength parameters of rock, including the Geological Strength Index(GSI),Hoek-Brown material parameter(mi),Disturbance factor(D),and Normalised Uniaxial Compressive Strength(σci/γB),whereγis the unit weight of rock mass, and B is the width of strip footing. The study also investigates the impact of cracks on strip footings, considering different positions of the crack(LC)and depths of the crack(DC).The results demonstrate that the influence of the fine crack is only noticeable until the LC/B ratio reaches 6.However,for the separated crack, its impact remains significant even when the LC/B ratio exceeds 16.The appearance of fine crack at the edge of the footing results in a decrease in the magnitude Nσγof up to 45%,indicating a substantial reduction in the stability of the footing. The failure patterns are presented and discussed in detail for various cases in this study to examine the effect of surface cracks on the strip footing and to address the extent of the plastic collapse. 展开更多

BACKGROUND The triglyceride glucose-body mass index(TyG-BMI)is a novel indicator of insulin resistance(IR).Obstructive sleep apnea(OSA)is a prevalent disorder characterized by recurrent complete or partial collapse of the pharyngeal airway during sleep;however, the relationship between these two conditions remains unexplored. We hypothesized that a higher TyG-BMI is associated with the occurrence of OSA.AIM To assess the association between TyG-BMI and OSA in adults in the United States.METHODS A cross-sectional study was conducted utilizing data from the National Health and Nutrition Examination Surveys spanning from 2005-2008 to 2015-2018.TyGBMI was calculated as Ln[triglyceride(mg/dL)×fasting blood glucose(mg/dL)/2]×BMI.Restricted cubic splines were used to analyze the risk of TyG-BMI and OSA occurrence. To identify potential nonlinear relationships,we combined Cox proportional hazard regression with smooth curve fitting. We also conducted sensitivity and subgroup analyses to verify the robustness of our findings.RESULTS We included 16794 participants in the final analysis.Multivariate regression analysis showed that participants with a higher TyG-BMI had a higher OSA incidence. After adjusting for all covariates,TyG-BMI was positively correlated with the prevalence of OSA(odds ratio:1.28;95%confidence interval:1.17,1.40;P<0.001);no significant nonlinear relationship was observed.Subgroup analysis showed no strong correlation between TyG-BMI and OSA in patients with diabetes. The correlation between TyG-BMI and OSA was influenced by age, sex, smoking status, marital status, hypertensive stratification, and obesity;these subgroups played a moderating role between TyGBMI and OSA.Even after adjusting for all covariates, there was a positive association between TYG-BMI and OSA prevalence.CONCLUSION A higher TyG-BMI index is linked to higher chances of developing OSA.As TyG-BMI is an indicator of IR,managing IR may help reduce the risk of OSA. 展开更多

BACKGROUND The association between body mass index(BMI)and bone mineral density(BMD)has shown inconsistent results, varying by sex and skeletal site.Despite normal or elevated bone mass, individuals with type 2 diabetes have an increased risk of hip and vertebral fractures.AIM To assess lumbar spine trabecular volumetric BMD(vBMD)across different BMI categories in individuals with and without diabetes.METHODS This cross-sectional study included 966 men over 50 years old and 1001 postmenopausal women from the Pinggu Metabolic Disease Study. The vBMD of lumbar vertebrae 2 through 4 was measured using quantitative computed tomography.Total adipose tissue, subcutaneous adipose tissue(SAT),visceral adipose tissue(VAT),and lumbar skeletal muscle area were also quantified.RESULTS In men with obesity(P=0.038)and overweight(P=0.032),vBMD was significantly higher in the diabetes group compared to non-diabetic men. After adjusting for age and sex,no significant saturation effect between BMI and BMD was found in participants with diabetes or in women without diabetes. However,a BMI threshold of 22.33 kg/m²indicated a saturation point for vBMD in nondiabetic men.Independent predictors of vBMD in men included age(r=-0.387,P<0.001),BMI(r=0.130,P=0.004),and VAT(r=-0.145,P=0.001).For women, significant predictors were age(r=-0.594,P<0.001),BMI(r=0.157,P=0.004),VAT(r=-0.112,P=0.001),and SAT(r=-0.068,P=0.035).CONCLUSION The relationship between BMI and trabecular vBMD differs in individuals with and without diabetes.Overweight and obese men with diabetes exhibit higher vBMD. 展开更多

This paper discusses the clayrocks widespread at the Permian-Triassic boundary, which are mostly of volcanic origin. Volcanogenetic textures, structures and minerals such as high-temperature quartz are found in clayrocks at the Permian-Triassic boundary in many places. Thousands of microspherules have been collected from the Boundary clayrocks, many of which exhibit the typical features of the process from melting to cooling and solidification. indicating that they were formed by volcanic eruption or extraterrestrial impact. Volcanic effects on the Permian-Triassic mass extinction may be reflected in conodonts, algae and ammonoids. The Boundary clayrocks are found in many Permian-Triassic sections along the coast of Tethys. Their orighin remains to be studied. 展开更多

With the method of differential optical absorption spectroscopy (DOAS), average concentrations of aerosol particles along light path were measured with a flashlight source in Chiba area during the period of one month. The optical thickness at 550 nm is compared with the concentration of ground-measured suspended particulate matter (SPM). Good correlations are found between the DOAS and SPM data, leading to the determination of the aerosol mass extinction efficiency (MEE) to be possible in the lower troposphere. The average MEE value is about 7.6m^2.g^-1 , and the parameter exhibits a good correlation with the particle size as determined from the wavelength dependence of the DOAS signal intensity. 展开更多

Shaw's method used to correlate 40 sections across the Permo-Triassic boundary in South China is applied in the paper. Two steps are adopted to get an Integral Composite Section (ICS) by synthesizing these data : First , South China is divided into five areas and composite section developed for each area . Then the second step . the Changxing composite section is regarded as a composite standard (CSRS) while the ICS is produced by matching the CSRS with composite sections of the other areas. Three biozones in the Changxingian and two biozones in the Griesbachian can be discerned on the basis of computing Z values in the ICS. These biozones are marked by the Z values which quantitatively represent their time ranges ; therefore , they may increase accuracy of stratigraphic time correlation . The mass extinction at the end of the Permian is an abrupt event that is supported by the relative rate of extinction near the P/T boundary . About 90% of invertebrate species died out by the end of the Permian . The duration of the mass extinction is rather short ,approximately 0.018Ma . 展开更多

The rugosan fauna from the Guanyinqiao Bed (latest Ordovician, Hirnantian) of northern Guizhou, China is known to belong to the cold or cool-water type corals. The components of the fauna are solitary corals only, and corallite septa are generally strongly dilated, especially the streptelasmatid corals are dominant comprising 98% of the whole fauna. The Guanyinqiao Bed is rich in rugosans of 18 genera, which are streptelasnmtid Streptelasma (=Helicelasma), Brachyelasma, Amplexobrachyelasma, Salvadorea, Grewingkia, Borelasma, CrassUasma, Leolasma, KenophyUum, UUernelasma, Paramplexoides, Siphonolasma, Pycnactoides, Dalmanophyllum, Bodophyllum, Axiphoria, Lambeophyllum and cystiphyllid Sinkiangolasma. Although this fauna was fairly abundant in a confined area (northern-northeastern Guizhou, southern Sichuan) during the Hirnantian age, the rugosan mass extinction (generic extinction rate 81%) happened at the end of the Hirnantian Stage. It is conduded that the mass extinction is related to the ending of maximum glaciation and ice cap melting in Gondwana in the southern hemisphere in the latest Hirnantian, resulting in rapid global sea-level rise in the earliest Silurian. In the Upper Yangtze Basin, the sea bottom environments were replaced by anoxic and warmer water during that time, so that the cool-water type rugosan became extinct. The present paper attempts to revise some already described rugose coral genera and species (He, 1978, 1985) and to supplement a few new forms from the Guanyinqiao Bed. Fourteen species of 12 genera are re-described and illustrated, of which one species- Grewingkia latifossulata is new. As a whole, the rugosan fauna of the Guanyinqiao Bed may be correlated with those contemporaneous of North Europe, Estonia and North America, indicating a dose biogeographic affinity to North Europe. 展开更多

In this study, the reasons for mass extinction in Jurassic were investigated. It was shown that galactic compression led to the activation of terrestrial nuclear reactors, which in turn led to the changes in tectonic activity, volcano eruptions, LIPs, MORBs, paleoclimate change, drift of continents, narrowing of the Earth, worldwide floods, tsunami, changes in mantle and core structures, in magnetic fields and in sedimentary isotopes. It was shown that the mass extinctions occurred during worldwide floods, caused by the narrowing of the Earth at the time of galactic gravitational compression. It was shown that the average statistical altitude distribution of dinosaurs has a bimodal distribution and corresponds to permanent migrations between the plains and the hills. It has been suggested that the skeletons of dinosaurs are well preserved as a result of covering the bodies of dinosaurs with mud flows of coastal sediments and the soil layers at worldwide tsunami. It was formulated the requirement to paleontology, consisting in the obligatory registration of altitudes of the actual place of the fossils found. The simple explanation of the presence of boundaries in the structure of the Earth is given: the 40K nuclear layer corresponds to the boundary between upper and lower mantle;the 137Cs layer located on the boundary between the lower mantle and the outer core;the Th-U nuclear layer is a border between outer and inner core. The previously abstract theories of subduction and continents drift have a clear and obvious physical sense. It was shown that the standard geological table is a registration book of galactic events during Paleozoic. It is proposed to restore the structure of the galactic arms by the geological deposits on the Earth. It was suggested to create the stations on elevated hills for rescue and regeneration of biological forms in the future. 展开更多

Extreme gravitational collapse is explored by utilizing two fundamental properties and one reasonable assumption, which together lead logically to an end-state gravitating structure. This structure, called a Terminal state neutron star, manifests nature’s ultimate density of mass and possesses the ultimate electromagnetic barrier. It is then shown how this structure is central to the remarkable mechanism whereby the density is prevented from going higher. A simple process assures that such density is not exceeded—regardless of the quantity of additional mass. As an example, the discourse focuses on the expected progression and outcome when a compact star of —far more mass than can be accommodated by the basic Terminal state structure—undergoes total gravitational collapse. An examination of what happens to the considerable excess mass leads the discussion to the principle of mass extinction by the process of aether deprivation and its profound implications for black-hole physics and the current revolution in cosmology. 展开更多

High resolution sampling across the Cretaceous/Paleogene boundary (K/Pg) at the Galanderud section in northern Iran provides the most expanded and continuous section for us to consider biostratigraphy and the mass extinction pattern of Cretaceous planktic foraminifera. Based on planktic foraminifera, four biozones and five sub-biozones have been identified across the K/Pg boundary. These biozones include the Abathomphalus mayaroensis Biozone (Plummerita hantkeninoides subbiozone), the Guembelitria cretacea Biozone (including two sub-biozones: the Hedbergella holmdelensis and the Parvularugoglobigerina longiapertura), the Parvularugoglobigerina eugubina Biozone (including two subbiozones: the Parvularugoglobigerina Sabina and the Eoglobigerina simplicissima) and finely the Parasubbotina pseudobulloides Biozone. Planktic foraminiferal extinction occurred over a brief period, with 3% of the species disappearing in the late Maastrichtian, and 72% of the species becaming extinct at the K/Pg boundary. About 25% of the species survived into the early Danian. Extinction of 73% of the species at the K/Pg boundary is very compatible with the effect of a large asteroid impact. 展开更多

BACKGROUND Li-Fraumeni syndrome(LFS)is a rare autosomal dominant cancer-predisposing syndrome, which can manifest as a polymorphic spectrum of malignancies.LFS is associated with an early onset in life, with the majority of cases occurring prior to the age of 46.Notwithstanding the infrequency of primary cardiac tumors,it behooves clinicians to remain vigilant in considering the differential diagnosis of such tumors in LFS patients who present with a cardiac mass. This is due to the markedly elevated risk for malignancy in this particular population, far surpassing that of the general populace.CASE SUMMARY Herein,we present a case of a 30-year-old female with LFS who was found to have a tricuspid valve leaflet mass.CONCLUSION This case exemplifies valuable learning points in the diagnostic approach for this exceptionally rare patient population. 展开更多

The network of Himalayan roadways and highways connects some remote regions of valleys or hill slopes, which is vital for India’s socio-economic growth.Due to natural and artificial factors, frequency of slope instabilities along the networks has been increasing over last few decades.Assessment of stability of natural and artificial slopes due to construction of these connecting road networks is significant in safely executing these roads throughout the year.Several rock mass classification methods are generally used to assess the strength and deformability of rock mass. This study assesses slope stability along the NH-1A of Ramban district of North Western Himalayas.Various structurally and non-structurally controlled rock mass classification systems have been applied to assess the stability conditions of 14 slopes. For evaluating the stability of these slopes, kinematic analysis was performed along with geological strength index(GSI),rock mass rating(RMR),continuous slope mass rating(CoSMR),slope mass rating(SMR),and Q-slope in the present study. The SMR gives three slopes as completely unstable while CoSMR suggests four slopes as completely unstable. The stability of all slopes was also analyzed using a design chart under dynamic and static conditions by slope stability rating(SSR)for the factor of safety(FoS)of 1.2 and 1 respectively.Q-slope with probability of failure(PoF)1%gives two slopes as stable slopes.Stable slope angle has been determined based on the Q-slope safe angle equation and SSR design chart based on the FoS.The value ranges given by different empirical classifications were RMR(37-74),GSI(27.3-58.5),SMR(11-59),and CoSMR(3.39-74.56).Good relationship was found among RMR&SSR and RMR&GSI with correlation coefficient(R 2)value of 0.815 and 0.6866,respectively.Lastly,a comparative stability of all these slopes based on the above classification has been performed to identify the most critical slope along this road. 展开更多

Face stability is an essential issue in tunnel design and construction.Layered rock masses are typical and ubiquitous;uncertainties in rock properties always exist. In view of this,a comprehensive method, which combines the Upper bound Limit analysis of Tunnel face stability, the Polynomial Chaos Kriging, the Monte-Carlo Simulation and Analysis of Covariance method(ULT-PCK-MA),is proposed to investigate the seismic stability of tunnel faces.A two-dimensional analytical model of ULT is developed to evaluate the virtual support force based on the upper bound limit analysis. An efficient probabilistic analysis method PCK-MA based on the adaptive Polynomial Chaos Kriging metamodel is then implemented to investigate the parameter uncertainty effects.Ten input parameters, including geological strength indices, uniaxial compressive strengths and constants for three rock formations, and the horizontal seismic coefficients, are treated as random variables. The effects of these parameter uncertainties on the failure probability and sensitivity indices are discussed. In addition, the effects of weak layer position, the middle layer thickness and quality, the tunnel diameter, the parameters correlation, and the seismic loadings are investigated, respectively. The results show that the layer distributions significantly influence the tunnel face probabilistic stability, particularly when the weak rock is present in the bottom layer. The efficiency of the proposed ULT-PCK-MA is validated, which is expected to facilitate the engineering design and construction. 展开更多

The Moon provides a unique environment for investigating nearby astrophysical events such as supernovae.Lunar samples retain valuable information from these events, via detectable long-lived“fingerprint”radionuclides such as^(60)Fe.In this work,we stepped up the development of an accelerator mass spectrometry(AMS)method for detecting^(60)Fe using the HI-13tandem accelerator at the China Institute of Atomic Energy(CIAE).Since interferences could not be sufficiently removed solely with the existing magnetic systems of the tandem accelerator and the following Q3D magnetic spectrograph,a Wien filter with a maximum voltage of±60 kV and a maximum magnetic field of 0.3 T was installed after the accelerator magnetic systems to lower the detection background for the low abundance nuclide^(60)Fe.A 1μm thick Si_(3)N_(4) foil was installed in front of the Q3D as an energy degrader. For particle detection,a multi-anode gas ionization chamber was mounted at the center of the focal plane of the spectrograph.Finally,an^(60)Fe sample with an abundance of 1.125×10^(-10)was used to test the new AMS system. These results indicate that^(60)Fe can be clearly distinguished from the isobar^(60)Ni.The sensitivity was assessed to be better than 4.3×10^(-14)based on blank sample measurements lasting 5.8 h,and the sensitivity could,in principle,be expected to be approximately 2.5×10^(-15)when the data were accumulated for 100 h,which is feasible for future lunar sample measurements because the main contaminants were sufficiently separated. 展开更多

Rock mass quality serves as a vital index for predicting the stability and safety status of rock tunnel faces. In tunneling practice, the rock mass quality is often assessed via a combination of qualitative and quantitative parameters. However, due to the harsh on-site construction conditions,it is rather difficult to obtain some of the evaluation parameters which are essential for the rock mass quality prediction. In this study,a novel improved Swin Transformer is proposed to detect, segment, and quantify rock mass characteristic parameters such as water leakage, fractures, weak interlayers. The site experiment results demonstrate that the improved Swin Transformer achieves optimal segmentation results and achieving accuracies of 92%,81%,and 86%for water leakage, fractures, and weak interlayers, respectively.A multisource rock tunnel face characteristic(RTFC)dataset includes 11 parameters for predicting rock mass quality is established.Considering the limitations in predictive performance of incomplete evaluation parameters exist in this dataset,a novel tree-augmented naive Bayesian network(BN)is proposed to address the challenge of the incomplete dataset and achieved a prediction accuracy of 88%.In comparison with other commonly used Machine Learning models the proposed BN-based approach proved an improved performance on predicting the rock mass quality with the incomplete dataset.By utilizing the established BN,a further sensitivity analysis is conducted to quantitatively evaluate the importance of the various parameters, results indicate that the rock strength and fractures parameter exert the most significant influence on rock mass quality. 展开更多

This paper deals with the bearing capacity determination of strip footing on a rock mass in hilly area by considering the influence of inclined and eccentric loading. Applying the generalized HoekBrown failure criterion, the failure behavior of the rock mass is modeled with the help of the power cone programming in the lower bound finite element limit analysis framework. Using bearing capacity factor(Ns), the change in bearing capacity of the strip footing due to the occurrence of eccentrically inclined loading is presented. The variations of the magnitude of Ns are obtained by examining the effects of the Hoek-Brown rock mass strength parameters(uniaxial compressive strength(sci), disturbance factor(D), rock parameter(mi), and Geological Strength Index(GSI)) in the presence of different magnitudes of eccentricity(e) and inclination angle(λ) with respect to the vertical plane, and presented as design charts. Both the inclined loading modes, i.e., inclination towards the center of strip footing(+λ) and inclination away from the center of strip footing(-λ), are adopted to perform the investigation. In addition, the correlation between the input parameters and the corresponding output is developed by utilizing the artificial neural network(ANN). Additionally, from sensitivity analysis, it is observed that inclination angle(λ) is the most sensitive parameter. For practicing engineers, the obtained design equation and design charts can be beneficial to understand the bearing capacity variation in the existence of eccentrically inclined loading in mountain areas. 展开更多
