Method: The Medical Outcomes Study Short Form-36 (SF-36) health status questionnaire was used to measure the HRQoL and compare the mean scores of the questionnaire subscales and physical and mental component summaries (PCS and MCS) of 33 patients with MetS, 18 patients with MSDs, and 30 apparently healthy (AH) subjects. Regression analysis was used to measure the prediction power of the study group, age and gender of the participants in estimating the HRQoL. Results: Results showed that the mean scores of the physical subscales, the PCS, the mental subscales and MCS were arranged in descending order from AH subjects, patients with MetS, to patients with MSDs. The mean scores difference among the 3 study groups were statistically significant with the only exception for the general mental health (GMH) subscale (P = 0.404). The study group and age accounted for 41.8% of the variability of PCS while the study group accounted for 19.6% of the variability in the MCS. The resulted equation to estimate the PCS score was as follows: PCS = 113.18 - 12.85 (Group: 0 for AH, 1 for MetS, and 2 for MSDs) - 0.67 age. On the other hand the resulted equation to estimate the MCS score was as follows: MCS = 76.203 - 10.426 (Group: 0 for AH, 1 for MetS, and 2 for MSDs). Conclusion: Patients with MetS and patients with MSDs had lower HRQoL than AH subjects. All the physical and mental dimensions of HRQoL are negatively influenced with MetS and with MSDs with the only exception for the GMH subscale. The physical and mental burden of MSDs is more dominant. The study group and age can be used to predict the PCS while the study group can be used to predict the MCS.展开更多

Non-adherence means not taking medications, missing medications, taking too much, not taking enough, wrongtiming, wrong dose and/or wrong pill, but may also refer to missing appointments, not booking appointments, not doing blood work, not returning calls and/or refusal to follow the treatment regimen. In renal transplantation, adherence to immunosuppressive medication is a fundamental requisite in order to preserve graft function, since non-adherence is one of the main causes for late acute rejection, incomplete recovery after rejection treatment, chronic graft dysfunction, graft loss, and death. Transplantation failure due to treatment nonadherence is economically, socially, ethically and morally unjustifiable. This is a very prevalent issue: in some studies, its incidence is as high as 70% of patients. The self-reported nonadherence levels found in certain studies, including those performed immediately after transplantation show the need for early and continued intervention after kidney transplantation in order to maximise adherence and consequently clinical outcomes. There is not a single method to assess non adherence, thus combining several measures increases diagnostic accuracy. Electronic monitoring with a microdevice that records each time a pill bottle is opened is considered the "gold standard" for measuring adherence, but selfreport at a confidential interview was the best measure of adherence. Thus non-adherence risk can be effectively assessed using clinically available assessment tools. Medication Adherence Scale, Brief Medical Questionnaire, Immunosuppressant Therapy Adherence Scale, Immunosuppressant Therapy Barrier Scale, Long-Term Medication Behavior Self-Efficacy Scale and Simplified Medication Adherence Questionnaire are some of the self-reported questionnaires. There are multiple factors associated with non-adherence in immunosuppressant therapy: Younger patients(adolescent, especially), poor health coverage, poor social support, unmarried, no family, non-Caucasian, immigrant, lower income, lower socioeconomic class, greater parental distress and lower family cohesion; complex medical regimens, higher number of drugs, longer time after transplant, toxicity, side effects, poor tolerance to medication, higher number of physicians involved, poor provider-patient rapport; psychological(dependency, high levels of anxiety and hostility, poorer behavioral functioning and greater distress in children) and psychiatric(depression) illnesses, low self-efficacy with medicine intake, perception of immunosuppressive therapy as not been necessary to preserve kidney function, forgetfulness, rebelliousness, poor perception of health, poor satisfaction, low Health-related quality of life, addictions, lack of coping strategies and avoidance behavior; patient morbidity: comorbidity, receiving a transplant from a live donor, retransplantation, and noninsulin-dependent diabetes. The most frequent strategies to promote medication-taking must focus on modifiable risk factors. Reasons for non-adherence are complex and diverse and any successful intervention aimed at improving adherence must be multidimensional. Although effective intervention strategies are needed to improve immunosuppressant therapy adherence, few intervention studies have been conducted in the adult renal transplant population. In this study, we perform an exhaustive review of the different strategies reported in the literature. A number of key reasons for non-adherence are also provided.展开更多

The study group and age can be used to predict the PCS while the study group can be used to predict the MCS.

文摘Non-adherence is a priority public health concern. A number of key reasons for non-adherence are also provided.

文摘Objective: Excavate the medication rule of traditional Chinese medicine in the treatment of prostate cancer, and predicting the biomolecular level mechanism of high-frequency drug compatibility. Methods: Relevant documents in CNKI, Wanfang Medical Network and VIP Chinese Biomedical Periodical Database Pubmed, EMbase were collected and collated systematically. Frequency statistics, association rule analysis and new party mining were carried out using TCMISSV2.5. BATMAN-TCM was used to analyze the interaction relationship and related pathways between high-frequency drug targets. Results: Huangqi (Astragalus membranaceus) was the single drug most used of the 102prescriptions included in the standard. There are 6 pairs of combinations with high confidence in association rule analysis. System entropy cluster analysis resulted in 20 core drug combinations and 9 new prescriptions. Through KEGG pathway analysis of Huangqi, Fuling (Poria cocos), Gancao (Glycyrrhiza uralensis) and Dihuang (Rehmannia glutinosa), it was found that the number of potential targets of the neural active ligand receptor rented pathway and purine metabolism pathway was the largest. Conclusions: Prostate cancer is mainly treated with deficiency-tonifying drugs, which are combined with drugs for promoting blood circulation, removing blood stasis, clearing heat, promoting diuresis, detoxifying and resolving hard mass. The mechanism of action of high-frequency traditional Chinese medicine may be realized by interfering with the neuroactive ligand receptor interaction pathway and purine metabolism pathway.
文摘Turkey has always been one of the leading countries in the field of health and fundamental rights.Though Turkey is a member of many international organizations and a candidate state for the European Union, had some regulations on fundamental rights, patients rights and right to health, even before the international papers, such as the Universal Declaration of Human Rights, did not come into force.Turkey always follows closely to the new developments in health care technologies, that is why Turkey continues to be one of the most chosen countries in international health tourism. These improvements in health care drive Turkey to adjust its regulations related to patients'fundamental rights and right to access to health. In the 2000s,health law postgraduate programs were founded in some universities in Turkey. With these programs, research in health law has been accelerated.Turkey will be one of the leading countries in health law too in the next few years. In this study,we started with the fundamental sources of the right to health in Turkey;then we continued with current objects at issue in Turkish health law;then we gave place to the current problems of Turkish health law such as reproductive rights, problems related to organ and tissue transplantations, increasing numbers of legal cases against health care professionals, their possible solutions and the future expectations.
