Plasma chitinase 3-like 1 is persistently elevated during first month after minimally invasive colorectal cancer resection 被引量:4

AIM: To assess blood chitinase 3-like 1(CHi3L1) levels for 2 mo after minimally invasive colorectal resection(MICR) for colorectal cancer(CRC). METHODS: CRC patients in an Institutional Review Board approved data/plasma bank who underwent elective MICR for whom preoperative(PreO p), early postoperative(PostO p), and 1 or more late PostO p samples [postoperative day(POD) 7-27] available were included. Plasma CHi3L1 levels(ng/m L) were determined in duplicate by enzyme linked immunosorbent assay. RESULTS: PreOp and PostOp plasma sample were available for 80 MICR cancer patients for the study. The median PreOp CHi3L1 level was 56.8 CI: 41.9-78.6 ng/mL(n = 80). Significantly elevated(P < 0.001) median plasma levels(ng/mL) over PreOp levels were detected on POD1(667.7 CI: 495.7, 771.7; n = 79), POD 3(132.6 CI: 95.5, 173.7; n = 76), POD7-13(96.4 CI: 67.7, 136.9; n = 62), POD14-20(101.4 CI: 80.7, 287.4; n = 22), and POD 21-27(98.1 CI: 66.8, 137.4; n = 20, P = 0.001). No significant difference in plasma levels were noted on POD27-41. CONCLUSION: Plasma CHi3L1 levels were significantly elevated for one month after MICR. Persistently elevated plasma CHi3L1 may support the growth of residual tumor and metastasis. 展开更多

The clinical efficacy was compared between 3D navigation-assisted percutaneous iliosacral screw(3DPS)and minimally invasive reconstruction plate(MIRP)in treating sacroiliac complex injury and the surgical procedures of 3DPS were introduced.A retrospective analysis was performed on 49 patients with sacroiliac complex injury from March 2013 to May 2017.Twenty-one cases were treated by 3DPS,and 28 cases by MIRP.Intraoperative indexes as operative time,blood loss,incision length,length of hospital stay and postoperative complications were respectively documented.Quality of reduction was postoperatively evaluated by Matta radiological criteria,and clinical effect was assessed by Majeed scoring criteria at the last followup.Operative time and hospital stay were significantly shortened,and blood loss,and incision length were significantly reduced in 3DPS group as compared with those in MIRP group(P<0.05).No statistically significant difference was found between 3DPS group and MIRP group in the assessment of reduction and function(P>0.05).It was concluded that both 3DPS and MIRP can effectively treat the sacroiliac complex injury,and 3DPS can provide an accurate,safe and minimally invasive fixation with shorter operative time and hospital stay. 展开更多

Two new metal-organic frameworks(MOFs), namely, [Co_2(L_1)(bix)(μ_3-OH)]·2H_2O(1) and [Co_2(L_2)(bix)(μ_3-OH)]·2.5H_2O(2)(H_3L_1 = 5-oxyacetate isophthalic acid, H_3L_2 = 3,5-bis-oxyacetate-benzoic acid, bix = 1,4-bis(imidazol-1-ylmethyl)benzene), have been synthesized under hydrothermal conditions. Their structures were determined by single-crystal X-ray diffraction analysis and further characterized by elemental analysis, IR spectra, and powder X-ray diffraction(PXRD) analysis. Both complexes 1 and 2 demonstrate identical three-dimensional(3D)(3,8)-connected tfz-d nets with(4~3)_2(4~6·6^(18)·8~4) topologies, where the tetranuclear [Co_4(μ_3-OH)_2] clusters act as 8-connected nodes and aromatic multicarboxylic ligands as 3-connected nodes. The results show that the ligands with different geometrical conformations can form products with the same topological structures. Their thermal and magnetic properties were also investigated. 展开更多

In colon cancer surgery,ensuring the complete removal of the primary tumor and draining lymph nodes is crucial.Lymphatic drainage in the colon follows the vascular supply,typically progressing from pericolic to paraaortic lymph nodes.While NCCN guidelines recommend the removal of 10-12 lymph nodes for ade-quate oncological resection,achieving complete oncological resection involves more than just meeting these numerical targets.Various techniques have been developed and studied over time to attain optimal oncological outcomes.A key technique central to this goal is identifying the ileocolic vessels at their origin from the superior mesenteric vessels.Complete excision of the visceral and parietal mesocolon ensures the intact removal of the specimen,while D3 lymphade-nectomy targets all draining regional lymph nodes.Although these principles emphasize different aspects,they ultimately converge to achieve the same goal of complete oncological resection.This article aims to simplify the surgical steps that align with the principle of central vascular ligation and mesocolon mobilization while ensuring adequate D3 dissection. 展开更多

Prevailing tissue degeneration caused by musculoskeletal maladies poses a great demand on bioscaffolds,which are artificial,biocompatible structures implanted into human bodies with appropriate mechanical properties.Recent advances in additive manufacturing,i.e.,3D printing,facilitated the fabrication of bioscaffolds with unprecedented geometrical complexity and size flexibility and allowed for the fabrication of topologies that would not have been achieved otherwise.In our work,we explored the effect of porosity on themechanical properties of a periodic cellular structure.The structure was derived from the mathematically created triply periodic minimal surface(TPMS),namely the Sheet-Diamond topology.First,we employed a series of software including MathMod,Meshmixer,Netfabb and Cura to design the model.Then,we utilized additive manufacturing technology to fabricate the cellular structures with designated scale.Finally,we performed compressive testing to deduce the mechanical properties of each cellular structure.Results showed that,in comparison with the highporosity group,the yield strength of the low-porosity group was 3 times higher,and the modulus was 2.5 times larger.Our experiments revealed a specific relationship between porosity and Young’s modulus of PLA-made Sheet-Diamond TPMS structure.Moreover,it was observed that the high-and low-porosity structures failed through distinctive mechanisms,with the former breaking down via buckling and the latter via micro-fracturing. 展开更多

Minimally invasive surgery is an important technique used for cytopathological examination.Recently,multiple studies have been conducted on a three-dimensional(3D)puncture simulation model as it can reveal the internal deformation state of the tissue at the micro level.In this study,a viscoelastic constitutive equation suitable for muscle tissue was derived.Additionally,a method was developed to define the fracture characteristics of muscle tissue material during the simulation process.The fracture of the muscle tissue in contact with the puncture needle was simulated using the cohesive zone model and a 3D puncture finite element model was established to analyze the deformation of the muscle tissue.The stress nephogram and reaction force under different parameters were compared and analyzed to study the deformation of the biological soft tissue and guide the actual operation process and reduce pain. 展开更多

目的:探讨醒脑静辅助超早期微创手术对基底节区脑出血患者外周血干扰素(IFN-γ)、T细胞免疫球蛋白黏蛋白3(TIM-3)表达的影响。方法:收集2014年8月至2017年5月本院收治的102例基底节区脑出血患者,采用随机数字表法将患者分为观察组与对照... 目的:探讨醒脑静辅助超早期微创手术对基底节区脑出血患者外周血干扰素(IFN-γ)、T细胞免疫球蛋白黏蛋白3(TIM-3)表达的影响。方法:收集2014年8月至2017年5月本院收治的102例基底节区脑出血患者,采用随机数字表法将患者分为观察组与对照组,每组51例。对照组进行常规治疗配合超早期微创手术,观察组则在对照组治疗基础上加用醒脑静注射液辅助治疗。比较两组治疗效果(NIHSS、GCS及FMA评分)、外周血IFN-γ、TIM-3mRNA水平差异。结果:治疗后,两组NIHSS评分、外周血IFN-γ、TIM-3 mRNA水平明显低于治疗前(P<0.05),GCS评分、FMA评分明显高于治疗前(P<0.05);观察组治疗后NIHSS评分、IFN-γ水平、TIM-3mRNA水平明显低于对照组(P<0.05),GCS评分、FMA上肢及下肢评分均明显高于对照组(P<0.05)。结论:醒脑静辅助超早期微创手术具有降低基底节区脑出血患者体内IFN-γ、TIM-3mRNA水平,提高免疫功能,抑制脑部缺血再灌注损伤,恢复神经功能的效果。 展开更多

目的探讨高血压脑出血微创穿刺术后颅内感染的临床特点及血清NOD样受体热蛋白结构域相关蛋白3(NLRP3)、人S-100b蛋白(S-100b)联合检测对穿刺术后颅内感染的预测价值。方法选取上海中医药大学附属曙光医院2019年1月至2020年10月收治的32... 目的探讨高血压脑出血微创穿刺术后颅内感染的临床特点及血清NOD样受体热蛋白结构域相关蛋白3(NLRP3)、人S-100b蛋白(S-100b)联合检测对穿刺术后颅内感染的预测价值。方法选取上海中医药大学附属曙光医院2019年1月至2020年10月收治的325例高血压脑出血行微创穿刺术治疗的患者,依照患者术后30 d内颅内感染情况分为感染组(n=29)和未感染组(n=296),分析颅内感染的临床特点,比较两组患者术前1 d的血清NLRP3、S-100b水平,再采用受试者工作曲线(ROC)分析上述两个血清指标对穿刺术后颅内感染的预测价值。结果29例感染患者共分离出36株阳性菌株,其中革兰氏阳性菌23株(63.89%);术前格拉斯哥昏迷(GCS)评分≤8分、引流管留置时间≥3 d、切口皮下积液、未预防性使用抗生素患者术后颅内感染发生率明显高于术前GCS评分>8分、引流管留置时间<3 d、未发生切口皮下积液、预防性使用抗生素的患者,差异均有统计学意义(P<0.05);感染组患者术前1 d的血清NLRP3、S-100b水平分别为(113.83±10.02)pg/mL、(1.05±0.14)ng/mL,明显高于未感染组的(97.08±9.71)pg/mL、(0.77±0.10)ng/mL,差异均有统计学意义(P<0.05);血清NLRP3、S-100b联合预测高血压脑出血微创穿刺术后颅内感染发生ROC曲线下面积为0.848,明显高于NLRP3、S-100b指标单独检测曲线下面积0.738、0.717,差异均有统计学意义(P<0.05)。结论高血压脑出血微创穿刺术后颅内感染病原菌以革兰氏阳性菌为主,感染组患者血清NLRP3、S-100b表达水平高于未感染组,两项指标联合检测对术后颅内感染的发生具有较高的预测效能。 展开更多

目的通过检测急性B淋巴细胞白血病(B-ALL)患儿骨髓中叉状头/翅膀状螺旋转录因子(Foxp3)mRNA的表达及CD4+细胞中调节性T细胞(Treg)比例,探讨Treg与儿童B-ALL发生及预后的关系。方法用实时定量聚合酶链反应(PCR)检测10名正常儿童、35例初... 目的通过检测急性B淋巴细胞白血病(B-ALL)患儿骨髓中叉状头/翅膀状螺旋转录因子(Foxp3)mRNA的表达及CD4+细胞中调节性T细胞(Treg)比例,探讨Treg与儿童B-ALL发生及预后的关系。方法用实时定量聚合酶链反应(PCR)检测10名正常儿童、35例初发B-ALL患儿及68例临床缓解患儿骨髓中Foxp3mRNA水平;用流式细胞术检测上述骨髓标本中CD4+CD25highFoxp3+Treg的比例;利用酶联免疫吸附试验(ELISA)检测骨髓中细胞因子白细胞介素10(IL-10)的表达情况。结果骨髓中Foxp3 mRNA表达水平在B-ALL初发患儿组是对照组的4.44倍,微小残留病(MRD)阳性组是阴性组的2.39倍;B-ALL初发患儿骨髓中Treg占CD4+T淋巴细胞的比例[中位数(M)=12.77%]较对照组(M=8.09%)增高,差异具有统计学意义(P<0.01);MRD阳性组Treg比例(M=14.74%)与MRD持续阴性组(M=11.30%)比较,差异具有统计学意义(P<0.05)。治疗后MRD阳性组骨髓中IL-10水平[(24.69±11.73)pg/mL]显著高于MRD持续阴性组[(9.19±5.82)pg/mL,P<0.05]。结论 B-ALL初发和MRD阳性患儿骨髓中Foxp3 mRNA水平和Treg比例显著增高,且骨髓中IL-10水平与Treg的比例变化趋势相一致。Treg可作为评价B-ALL患儿预后和复发的指标,Treg可能是通过分泌抑制性细胞因子IL-10而发挥其抑制作用。 展开更多

In this paper, we prove that if any set of |E(G)|- |V(G)| + 1 facial cycles of a 3-connected planar graph G embedded in the plane doesn't form a minimum cycle base of G, then any minimum cycle base of G contains a separating cycle, and G has a minor isomorphic to T6, where T6 is the graph obtained from the complete graph K6 by deleting a path with four edges. 展开更多

目的对微创接骨板在四肢骨折手术中的应用进行了分析,并且对其疗效与其他的技术进行了对比,以分析其疗效和运用。方法选择在2009年2月~2011年2月收治的90例四肢骨折患者为研究对象,对照组采用了传统内固定术进行治疗,观察组采用了微创... 目的对微创接骨板在四肢骨折手术中的应用进行了分析,并且对其疗效与其他的技术进行了对比,以分析其疗效和运用。方法选择在2009年2月~2011年2月收治的90例四肢骨折患者为研究对象,对照组采用了传统内固定术进行治疗,观察组采用了微创接骨板技术对四肢骨折进行治疗,治疗后对其疗效进行比较。结果观察组的术中平均出血量、手术时间、住院时间、术后功能锻炼时间分别为(61.3±5.2)ml,(59.8±4.3)min,(9.6±1.1)d。对照组分别为(202.1±4.3)ml,(90.5±2.2)min,(14.2±1.0)d。观察组指标显著优于对照组,P<0.05。观察组术后出现肘关节功能障碍比率为5.13%,对照组术后出现肩关节功能障碍比率为8.0%,观察组术后并发症显著少于对照组,P<0.05。观察组和对照组的优良率分别为93.02%、72.3%,有显著性差异,P<0.05。结论微创接骨板技术治疗四肢骨折并发症少,功能恢复优良率较高,具有较大的临床推广价值。 展开更多

目的探讨3-硝基酪氨酸(3-NT)对轻微型肝性脑病(MHE)的诊断价值。方法采用肝性脑病心理测试评分(PHES)量表对90例无显性肝性脑病的乙肝肝硬化患者(肝硬化组)及90名健康体检者(正常对照组)进行评测。检测入组者外周血中3-NT、蛋氨酸、瓜... 目的探讨3-硝基酪氨酸(3-NT)对轻微型肝性脑病(MHE)的诊断价值。方法采用肝性脑病心理测试评分(PHES)量表对90例无显性肝性脑病的乙肝肝硬化患者(肝硬化组)及90名健康体检者(正常对照组)进行评测。检测入组者外周血中3-NT、蛋氨酸、瓜氨酸、IL-6、IL-18水平。以PHES为金标准,评估各项指标诊断MHE的敏感性和特异性。结果根据PHES量表,39例患者被诊断为MHE。与非MHE亚组比较,MHE亚组3-NT、IL-6、IL-18、蛋氨酸、瓜氨酸水平显著升高(均P <0. 01)。Logistics回归分析显示,3-NT(P=0. 005)、IL-6(P=0. 029)、IL-18(P=0. 018)是MHE的相关危险因素,而蛋氨酸(P=0. 401)、瓜氨酸(P=0. 087)无预测价值。受试者工作特征曲线提示3-NT具有良好的有效性及可靠性,曲线下面积0. 967,截断点为13. 230 nmol,敏感度为92. 5%,特异度为94. 0%。结论 3-NT对MHE具有良好的诊断价值,且简便易行,敏感度和特异度较高。 展开更多

目的 探讨血清正五聚蛋白 3(pentraxin 3, PTX3),乙酰肝素酶 (heparinase, HPA) 及 Adropin 蛋白的表达水平与脑室出血患者微创锥颅软通道引流术后预后的关系。方法 选择 2019 年 10 月~ 2020 年 5 月邢台市第三医院收治的 122 例脑室... 目的 探讨血清正五聚蛋白 3(pentraxin 3, PTX3),乙酰肝素酶 (heparinase, HPA) 及 Adropin 蛋白的表达水平与脑室出血患者微创锥颅软通道引流术后预后的关系。方法 选择 2019 年 10 月~ 2020 年 5 月邢台市第三医院收治的 122 例脑室出血患者作为观察组,同时选取同期 60 例健康志愿者为对照组,检测两组受试者血清 PTX3,HPA和 Adropin 蛋白表达。所有患者均行脑室出血微创软通道引流术,收集患者年龄、性别、脑出血量、发病至手术时间及术前血液学指标等基线资料。随访 3 个月,应用改良版 Rankin 量表评估患者预后情况,分成预后良好组和预后不良组。单因素和多因素 Logistic 回归分析确定血清 PTX3,HPA 和 Adropin 蛋白确定脑室出血患者微创锥颅软通道引流术后预后不良的关系。结果 观察组患者血清 PTX3(14.78±2.02ng/ml vs 7.89±1.87ng/ml) 和 HPA(4.08±0.98ng/ml vs 2.01±0.50ng/ml) 水平明显高于对照组,差异具有统计学意义 (t=22.157,18.866, 均 P<0.001)。观察组患者血清 Adropin 蛋白明显低于对照组 (1.87±0.22μg/ml vs 3.70±0.35μg/ml),差异具有统计学意义 (t=37.060, P<0.001)。预 后 良 好 组 患 者 血 清 PTX3(13.64±3.36ng/ml vs 16.98±3.72ng/ml) 和 HPA(3.36±0.87ng/ml vs 4.97±1.01ng/ml) 明显低于预后不良组,差异具有统计学意义 (t=5.010, 9.144, 均 P<0.001)。血清 Adropin 蛋白明显高于预后不良组(2.39±0.42μg/ml vs 1.30±0.28μg/ml),差异具有统计学意义 (t=16.932, P<0.001)。根据 ROC 曲线可得,PTX3,HPA和 Adropin 蛋白诊断的最佳临界值分别为 15.01ng/ml,3.82ng/ml 和 1.85μg/ml,对应的敏感度、特异度和 AUC 分别为 67.44%,69.74% 和 0.798(95%CI:0.748~0.851);65.12%,71.05% 和 0.760(95%CI:0.707~0.812);60.46%,60.53% 和0.631(95%CI:0.567~0.0.698)。预后良好组和预后不良组的患者在年龄、高血压、高血糖、脑出血量、病情严重程度、发病至手术时间、二次手术及 PTX3,HPA,Adropin 等差异均有统计学意义 (χ^(2)=16.937, 8.848, 8.567, 10.301, 15.899, 7.461,4.015, 13.223, 10.976, 16.158, P<0.05)。Logistic 回归分析结果显示年龄 (OR=1.655, 95%CI:1.355~2.022)、病情严重程度 (OR=1.565, 95%CI:1.056~2.321)、高血压 (OR=1.835, 95%CI:1.015~3.317)、出血量 (OR=1.650, 95%CI:1.098~2.481)、PTX3(OR=2.435, 95%CI: 2.045~2.899)、HPA(OR=2.465, 95%CI: 2.018~3.010) 及 Adropin 蛋白 (OR=2.418, 95%CI: 1.972 ~2.965) 均是患者预后不良的独立影响因素 (P<0.05)。结论 脑室出血患者血清 PTX3,HPA 和 Adropin 蛋白呈现异常表达,且是微创锥颅软通道引流术后预后不良的独立影响因素。 展开更多
