目的 探讨小切口Internal brace人工韧带联合Brostr?m改良术修复距腓前韧带损伤的手术疗效。方法 17例慢性踝关节外侧不稳患者采取小切口Internal brace人工韧带联合Brostr?m改良术修复距腓前韧带损伤,其中男11例,女6例,年龄28~56岁,平... 目的 探讨小切口Internal brace人工韧带联合Brostr?m改良术修复距腓前韧带损伤的手术疗效。方法 17例慢性踝关节外侧不稳患者采取小切口Internal brace人工韧带联合Brostr?m改良术修复距腓前韧带损伤,其中男11例,女6例,年龄28~56岁,平均(40.20±13.75)岁,体质量指数(BMI)30.12~35.10 kg/m^(2),平均(32.27±1.83)kg/m^(2)。比较手术前后足踝结局评分(FAOS)和足踝功能测试(FAAM)评分,评估手术疗效。结果 所有患者手术切口一期愈合,未出现神经、血管、肌腱损伤等并发症。17例患者获得12~24个月完全随访。FAOS评分由术前(57.10±11.32)分提高到(91.35±1.27)分(P<0.05),FAAM评分由术前(55.76±10.12)分提高到(91.10±0.69)分(P<0.05)。有2例在早期行走时感觉踝关节外侧有紧绷感,8周后症状消除。结论 小切口Internal brace人工韧带联合Brostr?m改良术修复距腓前韧带损伤可以有效减轻患者症状,提高踝关节功能。 展开更多

目的 观察自体富血小板血浆(platelet-rich plasma,PRP)联合改良Broström-Gould法修复距腓前韧带(anterior talofibular ligament,ATFL)的早期临床疗效。方法 回顾性分析我院2020年3月至2022年2月收治的37例ATFL损伤病人的临床资料... 目的 观察自体富血小板血浆(platelet-rich plasma,PRP)联合改良Broström-Gould法修复距腓前韧带(anterior talofibular ligament,ATFL)的早期临床疗效。方法 回顾性分析我院2020年3月至2022年2月收治的37例ATFL损伤病人的临床资料,采用改良Broström-Gould法治疗,根据是否接受PRP治疗分为对照组(14例)和PRP组(23例),比较两组术前和术后6周时距骨倾斜角(talar tilt angle,TTA)、美国足踝外科医师协会(American Orthopaedic Foot and Ankle Society,AOFAS)踝与后足功能评分系统评分和疼痛视觉模拟量表(visual analogue scale,VAS)评分。结果 两组病人术后6周AOFAS评分、TTA和VAS评分均优于术前,差异有统计学意义(P<0.05);组间AOFAS评分[(82.3±7.1)分vs.(85.5±9.2)分]、TTA(3.7°±0.5°vs.3.5°±0.6°)的差异无统计学意义(t=1.261,P=0.564;t=1.016,P=0.641);VAS评分[(2.8±0.5)分vs.(0.9±0.2)分]的差异有统计学意义(t=3.857,P=0.028)。结论 自体PRP联合改良BroströmGould法修复ATFL损伤,能够有效减轻术后疼痛度,有利于加速康复锻炼,值得临床进一步推广。 展开更多

Modified constraint-induced movement therapy alters synaptic plasticity of rat contralateral hippocampus following middle cerebral artery occlusion 被引量:19

Modified constraint-induced movement therapy is an effective treatment for neurological and motor impairments in patients with stroke by increasing the use of their affected limb and limiting the contralateral limb.However,the molecular mechanism underlying its efficacy remains unclear.In this study,a middle cerebral artery occlusion(MCAO)rat model was produced by the suture method.Rats received modified constraint-induced movement therapy 1 hour a day for 14 consecutive days,starting from the 7^th day after middle cerebral artery occlusion.Day 1 of treatment lasted for 10 minutes at 2r/min,day 2 for 20 minutes at 2 r/min,and from day 3 onward for 20 minutes at 4 r/min.CatWalk gait analysis,adhesive removal test,and Y-maze test were used to investigate motor function,sensory function as well as cognitive function in rodent animals from the 1st day before MCAO to the 21^st day after MCAO.On the 21^st day after MCAO,the neurotransmitter receptor-related genes from both contralateral and ipsilateral hippocampi were tested by micro-array and then verified by western blot assay.The glutamate related receptor was shown by transmission electron microscopy and the glutamate content was determined by high-performance liquid chromatography.The results of behavior tests showed that modified constraint-induced movement therapy promoted motor and sensory functional recovery in the middle cerebral artery-occluded rats,but had no effect on cognitive function.The modified constraint-induced movement therapy upregulated the expression of glutamate ionotropic receptor AMPA type subunit 3(Gria3)in the hippocampus and downregulated the expression of the beta3-adrenergic receptor gene Adrb3 and arginine vasopressin receptor 1 A,Avprla in the middle cerebral artery-occluded rats.In the ipsilateral hippocampus,only Adra2 a was downregulated,and there was no significant change in Gria3.Transmission electron microscopy revealed a denser distribution the more distribution of postsynaptic glutamate receptor 2/3,which is an a-amino-3-hydroxy-5-methyl-4-isoxazole-propionic acid receptor,within 240 nm of the postsynaptic density in the contralateral cornu ammonis 3 region.The size and distribution of the synaptic vesicles within 100 nm of the presynaptic active zone were unchanged.Western blot analysis showed that modified constraint-induced movement therapy also increased the expression of glutamate receptor 2/3 and brain-derived neurotrophic factor in the hippocampus of rats with middle cerebral artery occlusion,but had no effect on Synapsin I levels.Besides,we also found modified constraint-induced movement therapy effectively reduced glutamate content in the contralateral hippocampus.This study demonstrated that modified constraint-induced movement therapy is an effective rehabilitation therapy in middle cerebral artery-occluded rats,and suggests that these positive effects occur via the upregulation of the postsynaptic membrane a-amino-3-hydroxy-5-methyl-4-isoxazole-propionic acid receptor expression.This study was approved by the Institutional Animal Care and Use Committee of Fudan University,China(approval No.201802173 S)on March 3,2018. 展开更多

The purpose of this work is to study the possibility of anionic dyes Reactive Red M-8B(RR) and Direct Green B(DG) adsorbed on chitosan-modified diatomite. The characteristics of adsorbent, adsorption isotherms and the influence of adsorption time, temperature and pH were researched in this work. The results show that the mo- dified diatomite had a much better adsorption capability than the natural diatomite. The adsorption capacities of chitosan-modified diatomite for RR and DG were 94.46 and 137.0 mg/g, respectively. Both adsorption time and adsorption temperature provided a positive effect on the dye adsorption. Within the experimental pH range, the adsorbance was enhanced at lower pH but reduced sharply at high pH. On the basis of the experimental results and discussion, electrostatic attraction is considered as the main mechanism of this chemisorption. 展开更多

In the meizoseismal areas hit by the China Wenchuan earthquake on May 12, 2008, the disasterprone environment has changed dramatically, making the susceptibility assessment of debris flow more complex and uncertain. After the earthquake, debris flow hazards occurred frequently and effective susceptibility assessment of debris flow has become extremely important. Shenxi gully in Du Jiangyan city, located in the meizoseismal areas, was selected as the study area. Based on the research of disaster-prone environment and the main factors controlling debris flow, the susceptibility zonations of debris flow were mapped using factor weight method(FW), certainty coefficient method(CF) and geomorphic information entropy method(GI). Through comparative analysis, the study showed that these three methods underestimated susceptible degree of debris flow when used in the meizoseismal areas of Wenchuan earthquake. In order to solve this problem, this paper developed a modified certainty coefficient method(M-CF) to reflect the impact of rich loose materials on the susceptible degree of debris flow. In the modified method, the distribution and area of loose materials were obtained by field investigations and postearthquake remote sensing image, and four data sets, namely, lithology, elevation, slop and aspect, wereused to calculate the CF values. The result of M-CF method is in agreement with field investigations and the accuracy of the method is satisfied. The method has a wide application to the susceptibility assessment of debris flow in the earthquake stricken areas. 展开更多

目的比较internal brace(IB)与带线锚钉通过改良Broström术治疗慢性踝关节不稳的临床疗效。方法回顾性分析2019年5月至2022年2月在桂林市人民医院本院关节骨科行手术治疗的42例慢性踝关节外侧不稳患者资料,根据距腓前韧带修补所用... 目的比较internal brace(IB)与带线锚钉通过改良Broström术治疗慢性踝关节不稳的临床疗效。方法回顾性分析2019年5月至2022年2月在桂林市人民医院本院关节骨科行手术治疗的42例慢性踝关节外侧不稳患者资料,根据距腓前韧带修补所用材料的不同将患者分为IB组(19例)和带线锚钉组(23例)。比较两组患者一般资料、手术时间、并发症发生率、术后完全负重行走时间、术后恢复跑步的时间、美国足踝外科协会(American Orthopedic Foot and Ankle Society,AOFAS)踝-后足功能评分、视觉模拟评分法(visual analog scale,VAS)评分。结果所有患者术后均获得随访,随访时间12~18个月,平均(13.8±5.3)个月。两组患者基线资料差异无统计学意义(P>0.05);两组各有1例术口拆线后再出现渗液,换药后愈合;两组各有2例术口区域感觉障碍,除IB组有1例术后半年仍未完全恢复外,其余3例术后2~3个月恢复;IB组患者术后6周随访时AOFAS评分优于带线锚钉组,差异有统计学意义(t=2.239,P=0.025),但术后6周时VAS评分比较差异无统计学意义(t=0.308,P=0.760);末次随访时AOFAS评分和VAS评分比较,两组之间差异无统计学意义(t=0.045,P=0.965;t=0.203,P=0.840);IB组术后完全负重行走时间、术后恢复跑步的时间显著早于带线锚钉组,差异有统计学意义(t=26.566,P<0.01;t=4.838,P<0.01)。结论IB与带线锚钉通过改良Broström术开放治疗慢性踝关节不稳的临床疗效满意,且使用IB在早期康复和重返运动方面优于带线锚钉。 展开更多

In orthogonal frequency division multiplexing(OFDM) based muitihop communications,the conventional decodeand-forward(DF) relay scheme severely suffers from the error propagation problem.This drawback is serious in muitihop networks as errors made by any relay node may fail the decoder at the destination in great chance.In this paper,we propose a bit error rate(BER)modified DF protocol(BMDF) which can be applied to systems where error correction channel coding and M-ary modulation are used.By modeling all links except the last one as a binary symmetric channel(BSC),we derive a log likelihood ratio(LLR)modification function relying only on the accumulated BER of all previous links to be applied to the output of the soft demapper.Furthermore,to reduce the computational complexity and signaling overhead,the modification function is simplified from its original exponential expression and less BERs are delivered between nodes by making successive subcarriers share the same BER.In addition,for situations where the channel state information(CSI) of forward link is available,the proposed BMDF can be further enhanced by combining with subcarrier pairing(SP) and power allocation(PA),where a sorted-channel gain SP scheme and a greedy PA algorithm are proposed.The simulation results verify the significant performance improvement to the conventional DF. 展开更多

Stability analysis of intermittently jointed rock slopes based on the stepped failure mode

In practical engineering,due to the noncontinuity characteristics of joints in rock slopes,in addition to plane failure,stepped sliding failure may occur for intermittently jointed rock slopes.Especially for intermittently bedding jointed rock slopes,the correlation and difference in strength parameters between joints and rock bridges,along with the various failure modes and intermittency of rock bridges,contribute to the complexity of stepped failure modes and the unpredictability of failure regions.Based on the upper-bound limit analysis method and multi-sliders step-path failure mode,considering the shear and tensile failure of rock bridges and the weakened relationship between the strength parameters of rock bridges and jointed surfaces,by introducing the modified M-C failure criterion and the formula for calculating the energy consumption of tensile failure of rock bridges,two failure mechanisms are constructed to obtain the safety factor(F_(s))of intermittently jointed rock slopes.The sequential quadratic programming method is used to obtain the optimal upper-bound solution for F_(s).The influence of multiple key parameters(slope height H,horizontal distance L,Slope angleβ,shear strength parameters of the rock bridgeφr and cr,Dimensionless parameter u,weakening coefficients of the internal friction angle and cohesion between the rock bridges and joint surfaces Kφand Kc)on the stability analysis of intermittently jointed rock slopes under the shear failure mode of rock bridges as well as under the tensile failure mode is also explored.The reliability of the failure mechanisms is verified by comparative analysis with theoretical results,numerical results,and landslide cases,and the variation rules of F_(s)with each key parameter are obtained.The results show that F_(s) varies linearly withφr and cr of the rock bridge and with K_(φ)and K_(c),whereas F_(s)changes nonlinearly with H and L.In particular,with the increase in Kφand Kc,Fs increases by approximately 52.78%and 171.02%on average,respectively.For rock bridge tensile failure,F_(s) shows a nonlinearly positive correlation withφr,cr,Kφand Kc.In particular,with the increase in Kφand Kc,Fs increases by approximately 13%and 61.69%on average,respectively.Fs decreases rapidly with increasing slope gradientβand decreasing dimensionless parameterμ.When Kφand Kc are both less than 1.0,the stepped sliding surface occurs more easily than the plane failure surface,especially in the case of tensile failure of the rock bridge.In addition,rock slopes with higher strength parameters,taller heights,and greater weakening coefficients are prone to rock bridge tension failure with lower Fs,and more attention should be given to the occurrence of such accidents in actual engineering. 展开更多

目的探讨关节镜下Brostr?m法结合Gould技术治疗踝关节距腓前韧带损伤的临床疗效。方法 2009年3月~2015年10月,我科对17例距腓前韧带损伤采用关节镜下Brostr?m法结合Gould技术,利用带线锚钉加强缝合修复距腓前韧带,同时微创紧缩缝合伸肌... 目的探讨关节镜下Brostr?m法结合Gould技术治疗踝关节距腓前韧带损伤的临床疗效。方法 2009年3月~2015年10月,我科对17例距腓前韧带损伤采用关节镜下Brostr?m法结合Gould技术,利用带线锚钉加强缝合修复距腓前韧带,同时微创紧缩缝合伸肌支持带,加强踝关节外侧稳定性,通过疼痛及不稳症状改善情况,距骨倾斜角变化,美国足踝关节协会(American Orthopaedic Foot&Ankle Society,AOFAS)评分,行走时疼痛视觉模拟评分(Visual Analogue score,VAS)评分评价手术效果。结果 17例随访10~18个月(平均12个月)。术前AOFAS总评分(44.2±3.7)分,术后1、3、6、10个月AOFAS总评分分别为(49.4±4.1)、(70.7±5.4)、(82.1±4.1)、(92.0±5.0)分,与术前比较均显著升高(P=0.015,P=0.000,P=0.000,P=0.000)。术前VAS评分为(7.1±1.4)分,术后1、3、6、10个月分别为(7.2±1.5)、(4.8±1.1)、(2.1±0.9)、(1.3±0.7)分,除术后1个月以外,其余与术前比较均显著降低(P=0.001,P=0.000,P=0.000)。术前距骨倾斜角为8.8°±0.6°,术后1、3、6、10个月分别为4.1°±0.4°、3.5°±0.8°、3.2°±0.6°、3.2°±0.7°,与术前比较均显著变小(P=0.000,P=0.000,P=0.000,P=0.000)。结论关节镜下Brostr?m法结合Gould技术治疗踝关节距腓前韧带损伤疗效满意,值得推广。 展开更多

目的比较全关节镜下和改良开放式Broström修复距腓前韧带(anterior talofibular ligament,ATFL)治疗慢性踝关节外侧不稳的早期临床疗效。方法回顾分析我科2016年1月至2017年8月行ATFL修复的30例单侧慢性踝关节外侧不稳患者临床资... 目的比较全关节镜下和改良开放式Broström修复距腓前韧带(anterior talofibular ligament,ATFL)治疗慢性踝关节外侧不稳的早期临床疗效。方法回顾分析我科2016年1月至2017年8月行ATFL修复的30例单侧慢性踝关节外侧不稳患者临床资料。其中全关节镜下锚钉修复15例(关节镜组),男8例,女7例;年龄20~50岁,平均(35.1±10.8)岁。改良开放式Broström锚钉修复15例(开放组),男9例,女6例;年龄18~52岁,平均(30.5±11.7)岁。记录两组术中出血量、手术时间、并发症发生情况、术后恢复正常生活时间以及手术前后卡尔森踝关节功能(Karlsson ankle functional,KAF)评分、美国足踝外科协会(American orthopaedic foot and ankle society,AOFAS)评分、疼痛视觉模拟评分(visual analogue scale,VAS)。结果患者术后均获随访,随访时间12~24个月,平均(16.4±5.8)个月。两组术后均无踝关节不稳。关节镜组手术时间、术中出血量及术后恢复正常生活时间均明显优于开放组(P<0.05)。术后1个月关节镜组AOFAS评分、KAF评分及VAS均优于开放组(P<0.05);术后3个月及末次随访时两组AOFAS评分、KAF评分及VAS差异无统计学意义(P>0.05)。结论在术后至少1年,关节镜下和改良开放式Broström修复ATFL治疗慢性踝关节外侧不稳都有良好的疗效。较开放式Broström手术,关节镜下修复ATFL术中出血量少,术后疼痛较轻,恢复更快。全关节镜下修复可能是一种安全可行的替代手术。 展开更多

目的 分析慢性踝关节不稳(CAI)患者改良Brostr?m-Gould术后焦虑和抑郁情况的改善及预后。方法 对行改良Brostr?m-Gould手术的140例CAI患者进行回顾性问卷调查,问卷内容包括一般情况调查表、焦虑自评量表(SAS)、抑郁自评量表(SDS)、视觉... 目的 分析慢性踝关节不稳(CAI)患者改良Brostr?m-Gould术后焦虑和抑郁情况的改善及预后。方法 对行改良Brostr?m-Gould手术的140例CAI患者进行回顾性问卷调查,问卷内容包括一般情况调查表、焦虑自评量表(SAS)、抑郁自评量表(SDS)、视觉模拟量表(VAS)、匹茨堡睡眠质量指数量表(PSQI)和美国足踝外科协会踝-后足量表(AOFAS)。术前以及术后3和6个月时分别使用相同问卷对患者进行调查,对调查结果进行统计分析。结果 术前CAI患者焦虑的发生率为29.2%,抑郁的发生率为37.5%。改良Brostr?m-Gould术后3个月,CAI患者焦虑和抑郁情绪、疼痛程度、睡眠障碍以及踝关节功能状态均较术前明显改善(均P <0.001)。CAI患者术后焦虑、抑郁情绪的改善与疼痛程度、睡眠质量和踝关节功能状态呈双向正相关关系(P <0.05)。结论 CAI患者术前存在明显的焦虑和抑郁情绪,改良Brostr?m-Gould手术可明显改善患者焦虑、抑郁情况及预后,应尽早对CAI患者行手术治疗。 展开更多

目的:探讨关节镜下改良Brostrom-Gould术式治疗青少年踝关节外侧副韧带损伤锚钉安全性和有效性。方法:回顾性分析我院2017年1月至2018年1月接受关节镜下改良Brostr?m-Gould术式治疗的10例(9~15岁)青少年复发性踝关节外侧副韧带损伤患者... 目的:探讨关节镜下改良Brostrom-Gould术式治疗青少年踝关节外侧副韧带损伤锚钉安全性和有效性。方法:回顾性分析我院2017年1月至2018年1月接受关节镜下改良Brostr?m-Gould术式治疗的10例(9~15岁)青少年复发性踝关节外侧副韧带损伤患者(实验组)。使用美国足踝外科协会(AOFAS)评分于术前,术后6周、6月、12月及末次随访时对患者进行疗效评价。使用疼痛视觉模拟评分(VAS)于术前,术后1周、1月、6月及末次随访时对患者进行疼痛评分。同时从我院筛选出50例(10~15岁)踝关节正常的青少年(对照组)正侧位X线片。使用影像归档与通信系统(PACS)工具测量两组患者角度α(模拟钻头与踝关节水平所形成的夹角)、距离β(腓骨尖至骨骺线的垂直距离)和距离γ(腓骨上1cm水平腓骨的宽度),并进行统计学分析。结果:实验组所有患者均获得临床一期愈合,未出现相关临床并发症,术后AOFAS评分及VAS评分较术前均有明显改善,差异具有统计学意义。实验组角度α为27.96°、距离β为23.49 mm、距离γ为20.04 mm,对照组角度α为27.15°、距离β为22.63 mm、距离γ为19.69 mm,实验组与对照组α、β、γ值差异均无统计学意义(均P>0.05)。结论:采用关节镜下改良Brostrom-Gould术式修复骨骺未闭的青少年患者外侧副韧带损伤时,术前进行影像学测量及使用改良的体表标志绘制方案,术中对腓骨远端钻孔时准确定位,置钉角度控制在0°~27°内可以有效避免损伤骨骺。 展开更多

热力学性质在高熵合金设计中起着重要作用,利用可靠的热力学模型进行理论计算来获取合金关键热力学数据已成为一种重要而有效的途径。利用现有二元热力学数据和改进的分子相互作用模型(M-MIVM)对CoCrFeNiMn系高熵合金固溶体的混合吉布... 热力学性质在高熵合金设计中起着重要作用,利用可靠的热力学模型进行理论计算来获取合金关键热力学数据已成为一种重要而有效的途径。利用现有二元热力学数据和改进的分子相互作用模型(M-MIVM)对CoCrFeNiMn系高熵合金固溶体的混合吉布斯自由能、混合焓、混合熵、过量吉布斯自由能、过量熵进行预测,同时考虑原子尺寸差异,从热力学角度评估该高熵合金形成的可能性、相稳定性和结构稳定成分。研究结果表明:CoCrFeNiMn体系满足高熵合金的热力学判据,易于形成五元高熵合金;在低Fe、高Ni和高Mn的条件下,该合金体系的混合自由能更负,结构越稳定,Co_(0.15)Cr_(0.15)Fe_(0.1)Ni_(0.3)Mn_(0.3)是一个较优的合金成分设计方案;在高温条件下,该合金体系的混合自由能显著下降,合金稳定性增强。研究可为CoCrFeNiMn系高熵合金设计提供较为可靠的热力学数据和理论指导。 展开更多
