Environmental and occupational risk...Hidradenitis suppurativa (HS) is a chronic, inflammatory skin condition characterized by painful nodules, abscesses, and scarring, primarily affecting areas rich in apocrine glands. Environmental and occupational risk factors, including prolonged exposure to heat, friction, and certain chemicals, have been identified as exacerbating conditions for HS. Yet, there remains a lack of tailored recommendations for modifying work environments to alleviate these triggers. This review presents novel suggestions for work modifications to reduce HS flare-ups, such as implementing breathable, friction-reducing workwear, ergonomic adjustments to minimize pressure on affected areas, and introducing low-irritant hygiene protocols in workplaces with chemical exposure. These recommendations are grounded in emerging evidence linking specific occupational exposures to the exacerbation of HS symptoms. The novelty of this approach lies in its proactive stance, shifting from reactive management of HS symptoms to preventative environmental modifications. Future studies should explore the long-term efficacy of these modifications and investigate the potential for personalized work environment adjustments based on individual patient profiles. Targeted workplace interventions may significantly improve the quality of life for HS patients, highlighting the need for multidisciplinary collaboration between dermatologists, occupational health specialists, and employers.展开更多

文摘It is discovered that the product of the current and the electric field in a PN junction should be regarded as the rate of work(power)done by the electric field force on moving charges(hole current and electron current),which was previously misinterpreted as solely a Joule heating effect. We clarify that it is exactly the work done by the electric field force on the moving charges to stimulate the emergence of non-equilibrium carriers, which triggers the novel physical phenomena. As regards to Joule heat,we point out that it should be calculated from Ohm’s law, rather than simply from the product of the current and the electric field.Based on this understanding,we conduct thorough discussion on the role of the electric field force in the process of carrier recombination and carrier generation. The thermal effects of carrier recombination and carrier generation followed are incorporated into the thermal equation of energy. The present study shows that the exothermic effect of carrier recombination leads to a temperature rise at the PN interface, while the endothermic effect of carrier generation causes a temperature reduction at the interface. These two opposite effects cause opposite heat flow directions in the PN junction under forward and backward bias voltages, highlighting the significance of managing device heating phenomena in design considerations. Therefore, this study possesses referential significance for the design and tuning on the performance of piezotronic devices.

文摘Electronic interactions of the Group 2A elements with magnesium have been studied through the dilute solid solutions in binary Mg-Ca,Mg-Sr and Mg-Ba systems. This investigation incorporated the difference in the‘Work Function'(ΔWF)measured via Kelvin Probe Force Microscopy(KPFM),as a property directly affected by interatomic bond types,i.e.the electronic structure, nanoindentation measurements, and Stacking Fault Energy values reported in the literature. It was shown that the nano-hardness of the solid-solutionα-Mg phase changed in the order of Mg-Ca>Mg-Sr>Mg-Ba.Thus,it was shown,by also considering the nano-hardness levels, that SFE of a solid-solution is closely correlated with its‘Work Function'level.Nano-hardness measurements on the eutectics andΔWF difference between eutectic phases enabled an assessment of the relative bond strength and the pertinent electronic structures of the eutectics in the three alloys.Correlation withΔWF and at least qualitative verification of those computed SFE values with some experimental measurement techniques were considered important as those computational methods are based on zero Kelvin degree, relatively simple atomic models and a number of assumptions. As asserted by this investigation,if the results of measurement techniques can be qualitatively correlated with those of the computational methods,it can be possible to evaluate the electronic structures in alloys, starting from binary systems, going to ternary and then multi-elemental systems. Our investigation has shown that such a qualitative correlation is possible. After all,the SFE values are not treated as absolute values but rather become essential in comparative investigations when assessing the influences of alloying elements at a fundamental level, that is,free electron density distributions. Our study indicated that the principles of‘electronic metallurgy'in developing multi-elemental alloy systems can be followed via practical experimental methods,i.e.ΔWF measurements using KPFM and nanoindentation.
文摘The perception of nursing staff’s attitude influences patient fear.Understanding this dynamic is crucial for fostering a supportive environment conducive to patient well-being and effective healthcare practices. The purpose of this research is to investigate how the attitudes and behaviours of nursing staff influence the fear and anxiety levels of patients recovering from benign tumors, aiming to improve patient care and recovery outcomes.Data was collected from a sample of 100 participants, comprising 20 nursing staff and 80 patients recovering from benign tumors.Surveys were administered to gather quantitative data on attitudes and fear levels.Participants were selected randomly from hospital records and outpatient clinics. Our analysis encompassed nursing staff attitude, patient fear levels, the influence of family support, progression of tumor recovery,patient-reported satisfaction, and the quality of healthcare services provided. The quantitative aspect utilized PLS-SEM software to perform regression analysis, evaluating both direct and indirect effects.Statistical analysis assessed the relationships between nursing staff attitudes, patient fear during benign tumor recovery, and the mediating role of family support. The findings of the study demonstrate that better nurse attitudes(Hypothesis 1,β=0.45,p<0.001)and stronger family support(Hypothesis 2,β=0.32;p<0.001) are linked to lower levels of patient fear.Partially mediating the relationship between nurse attitudes and patient fear, according to Hypothesis 3(β=0.28,p<0.002),is family support.Patients’perceptions of family support are highly influenced by nursing behaviour,as demonstrated by Hypothesis 4(β=0.38;p<0.001).Our research showed a strong relationship between the attitudes of nursing personnel and patient fear levels.Family support demonstrated a strong mediating effect on patient fear.Patient-reported satisfaction is positively correlated with family support. However,no significant relationship was found between healthcare service quality and patient fear.
文摘BACKGROUND Lateral ankle sprains are the most common traumatic musculoskeletal injuries of the lower extremity, with an incidence rate of 15%-20%.The high incidence and prevalence highlights the economic impact of this injury.Ankle sprains lead to a high socioeconomic burden due to the combination of the high injury incidence and high medical expenses.Up to 40%of patients who suffer from an ankle sprain develop chronic ankle instability.Chronic instability can lead to prolonged periods of pain, immobility and injury recurrence.Identification of factors that influence return to work(RTW)and return to sports(RTS)after a lateral ankle sprain(LAS)may help seriously reduce healthcare costs.AIM To explore which factors may potentially affect RTW and RTS after sustaining an LAS.METHODS EMBASE and PubMed were systematically searched for relevant studies published until June 2023.Inclusion criteria were as follows:(1)Injury including LAS or chronic ankle instability;(2)Described any form of treatment;(3)Assessment of RTW or RTS;(4)Studies published in English;and(5)Study designs including randomized controlled clinical trials, clinical trials or cohort studies.Exclusion criteria were:(1)Studies involving children(age<16 year);or(2)Patients with concomitant ankle injury besides lateral ankle ligament damage.A quality assessment was performed for each of the included studies using established risk of bias tools.Additionally quality of evidence was assessed using the GRADEpro tool in cases where outcomes were included in the quantitative analysis.A best evidence synthesis was performed in cases of qualitative outcome analysis. For all studied outcomes suitable for quantitative analysis a forest plot was created to calculate the effect on RTW and RTS.RESULTS A total of 8904 patients were included in 21 studies,10 randomized controlled trials,7 retrospective cohort studies and 4 prospective cohort studies.Fifteen studies were eligible for meta-analysis.The overall RTS rate ranged were 80%and 83%in the all treatments pool and surgical treatments pool, respectively. The pooled mean days to RTS ranged from 23-93 d.The overall RTW rate was 89%.The pooled mean time to RTW ranged from 5.8-8.1 d.For patients with chronic ankle instability, higher preoperative motivation was the sole factor significantly and independently(P=0.001)associated with the rate of and time to RTS following ligament repair or reconstruction.Higher body mass index was identified as a significant factor(P=0.04)linked to not resuming sports or returning at a lower level(median 24,range 20-37),compared to those who resumed at the same or higher level(median 23,range 17-38).Patients with a history of psychological illness or brain injury, experienced a delay in their rehabilitation process for sprains with fractures and unspecified sprains. The extent of the delayed rehabilitation was directly proportional to the increased likelihood of experiencing a recurrence of the ankle sprain and the number of ankle-related medical visits. We also observed that 10%of athletes who did return to sport after lateral ankle sprain without fractures described non-ankle-related reasons for not returning.CONCLUSION All treatments yielded comparable results, with each treatment potentially offering unique advantages or benefits.Preoperative motivation may influence rehabilitation after LAS.Grading which factor had a greater impact was not possible due to the lack of comparability among the included patients.
文摘Hidradenitis suppurativa (HS) is a chronic, inflammatory skin condition characterized by painful nodules, abscesses, and scarring, primarily affecting areas rich in apocrine glands. Targeted workplace interventions may significantly improve the quality of life for HS patients, highlighting the need for multidisciplinary collaboration between dermatologists, occupational health specialists, and employers.
