Objective:Staghorn calculi present a particular and challenging entity of stone morphology.Treatment is associated with lower stone-free rates and higher complication rates compared to non-staghorn stones.In this revi...Objective:Staghorn calculi present a particular and challenging entity of stone morphology.Treatment is associated with lower stone-free rates and higher complication rates compared to non-staghorn stones.In this review we looked for the most relevant data on preoperative imaging and access planning to help decision making for percutaneous surgery with this complex condition.Methods:We conducted a PubMed search of publications in the past 2 decades that include relevant information on the planning for management of staghorn stones.Non-contrast computerized tomography(NCCT)is indeed the standard imaging tool for percutaneous nephrolithotomy(PCNL);additional tools such as three-dimensional computed tomography(CT)reconstruction of the staghorn calculus may help plan access in complex cases.Ultrasound guided percutaneous access may be considered for staghorn stones when planning upper pole access in kidney malposition or complex intrarenal anatomy or with complex body habitus.Wideband doppler ultrasound and real-time virtual sonography can assist.New technologies to improve kidney access such as Uro Dyna-CT or electromagnetic sensor have been reported,but have not shown utilization in staghorn cases.Staghorn morphometry-based prediction algorithms may predict the number of tract(s)and stage(s)for PCNL monotherapy.Lower pole access can be equally effective as upper pole when planning for staghorn and complex stones,with significantly less complications rate;Stone-Tract length-Obstruction-Number of involved calyces-Essence of stone density(STONE)nephrolithometry seems to be the best system to predict outcomes of PCNL in staghorn cases.There is a growing trend of endoscopic combined intrarenal surgery(ECIRS)in concordance with PCNL to treat larger stones.Conservative management of staghorn calculi is an undesired option,but can be an alternative for a carefully selected group of high-risk patients.Conclusion:Staghorn stones may lead to deterioration of renal function and life-threatening urosepsis.This entity should be managed aggressively with planning ahead for surgery using the different tools available as the cornerstone for a successful outcome.展开更多

Methods: Eleven patients with NSCLC, who were treated with three-dimensional con-formal radiation therapy, were studied. Each patient underwent twice sequential planning CT scan, i.e., at pre-treatment, and at mid-treatment for field reduction planning. Three treatment plans were established in each patient: treatment plan A was based on the pre-treatment planning CT scans for the first course of treatment, plan B on the mid-treatment planning CT scans for the second course of treatment, and treatment plan F on the fused images for the whole treatment. The irradiation doses received by organs at risk in the whole treatment with treatment A and B plans were estimated by the plus of the parameters in treatment plan A and B, assuming that the parameters involve the different tissues (i.e. V20=AV20+BV20), or the same tissues within an organ (i.e. Dmax=ADmax+BDmax). The assessment parameters in the treatment plan F were calculated on the basis of the DVH of the whole treatment. Then the above assessment results were compared. Results: There were marked differ-ences between the assessment results derived from the plus of assessment parameters in treatment plan A and B, and the ones derived from treatment plan F. Conclusion: When a treatment plan is altered during the course of radiation treatment, image fusion technique should be performed in the establishment of a new one. When the environment model is made to adapt to local path planning, an iterative algorithm of binary conversion is used for image segmentation. Raw data of the acoustic image, which were received from serial port, are processed. By the use of "Mathematic Morphology" to filter noise, a mathematic model of environment for local path planning is established after coordinate transformation. The optimal path is searched by the distant transmission (Dt) algorithm. Simulation is conducted for the analysis of the algorithm. Experiment on the sea proves it reliable.展开更多

Prostate cancer is a major public health burden throughout the world.The high incidence of prostate cancer,combined with earlier detection and downstaging at the time of diagnosis,and the slow natural progression and ...Prostate cancer is a major public health burden throughout the world.The high incidence of prostate cancer,combined with earlier detection and downstaging at the time of diagnosis,and the slow natural progression and biological heterogeneity of the disease,has made its management a complex and controversial issue.There is growing demand for patient-specific therapies that can minimize treatment morbidity while maximizing treatment benefits.There are a number of clinical parameters and clinical nomograms to help with the choice of treatment.Magnetic resonance imaging(MRI)is a technique which makes safer,more individualized therapies possible due to high spatial resolution,superior contrast resolution,multiplanar capability,and a large field of view.Other MRI techniques such as MR spectroscopic imaging,dynamic contrast-enhanced MRI or perfusion MRI,and diffusion-weighted imaging complement MRI by reflecting tissue biochemistry,Brownian motion of water molecules,and capillary wall permeability,respectively.This editorial review highlights the incremental value of MRI in the advanced management of prostate cancer to non-invasively improve cancer staging,biologic potential,treatment planning,therapy response,local recurrence,and to guide target biopsy for clinical suspected cancer with previous negative biopsy.Finally,some future prospects for MRI in prostate cancer management are given.展开更多

Near-optimal paths in static and dynamic environment...This paper describes path re-planning techniques and underwater obstacle avoidance for unmanned surface vehicle(USV) based on multi-beam forward looking sonar(FLS). Near-optimal paths in static and dynamic environments with underwater obstacles are computed using a numerical solution procedure based on an A* algorithm. The USV is modeled with a circular shape in 2 degrees of freedom(surge and yaw). In this paper, two-dimensional(2-D) underwater obstacle avoidance and the robust real-time path re-planning technique for actual USV using multi-beam FLS are developed. Our real-time path re-planning algorithm has been tested to regenerate the optimal path for several updated frames in the field of view of the sonar with a proper update frequency of the FLS. The performance of the proposed method was verified through simulations, and sea experiments. For simulations, the USV model can avoid both a single stationary obstacle, multiple stationary obstacles and moving obstacles with the near-optimal trajectory that are performed both in the vehicle and the world reference frame. For sea experiments, the proposed method for an underwater obstacle avoidance system is implemented with a USV test platform. The actual USV is automatically controlled and succeeded in its real-time avoidance against the stationary undersea obstacle in the field of view of the FLS together with the Global Positioning System(GPS) of the USV.展开更多
