Effects of different intervention methods on psychological flexibility,negative emotions and sleep quality in chronic hepatitis B

BACKGROUND Patients with chronic hepatitis B(CHB)experience various problems,including low psychological flexibility,negative emotions,and poor sleep quality.Therefore,effective nursing interventions are required to reduce adverse events.Acceptance and commitment therapy(ACT)combined with enabling cognitivebehavioral education(ECBE)can improve patients'psychological and sleep.Therefore,we speculate that this may also be effective in patients with CHB.AIM To investigate the effects of different intervention methods on psychological flexibility,negative emotions,and sleep quality in patients with CHB.METHODS This retrospective study examined clinical and evaluation data of 129 patients with CHB.Intervention methods were divided into a conventional group(routine nursing,n=69)and a combination group(ACT combined with ECBE,n=60).We observed changes in psychological flexibility,negative emotions,sleep quality,and self-care ability in both groups.Observation items were evaluated using the Acceptance and Action Questionnaire-2nd Edition(AAQ-II),Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS),Pittsburgh Sleep Quality Index(PSQI),and Exercise of Self-Care Agency Scale(ESCA).RESULTS Compared with the conventional group,the AAQ-II score of the combined group was lower(F_(between-group effect)=8.548;F_(time effects)=25.020;F_(interaction effects)=52.930;all P<0.001),the SAS score(t=5.445)and SDS score(t=7.076)were lower(all P<0.001),as were the PSQI dimensions(tsleep quality=4.581,tfall sleep time=2.826,tsleep time=2.436,tsleep efficiency=5.787,tsleep disorder=5.008,thypnotic drugs=3.786,tdaytime dysfunction=4.812);all P<0.05).The ESCA scores for all dimensions were higher(thealth knowledge level=6.994,t self-concept=5.902,tself-responsibility=19.820,tself-care skills=8.470;all P<0.001).CONCLUSION ACT combined with ECBE in patients with CHB can improve psychological flexibility and sleep quality,alleviate negative emotions,and improve self-care. 展开更多

BACKGROUND Propofol and sevoflurane are commonly used anesthetic agents for maintenance anesthesia during radical resection of gastric cancer.However,there is a debate concerning their differential effects on cognitive function,anxiety,and depression in patients undergoing this procedure.AIM To compare the effects of propofol and sevoflurane anesthesia on postoperative cognitive function,anxiety,depression,and organ function in patients undergoing radical resection of gastric cancer.METHODS A total of 80 patients were involved in this research.The subjects were divided into two groups:Propofol group and sevoflurane group.The evaluation scale for cognitive function was the Loewenstein occupational therapy cognitive assessment(LOTCA),and anxiety and depression were assessed with the aid of the self-rating anxiety scale(SAS)and self-rating depression scale(SDS).Hemodynamic indicators,oxidative stress levels,and pulmonary function were also measured.RESULTS The LOTCA score at 1 d after surgery was significantly lower in the propofol group than in the sevoflurane group.Additionally,the SAS and SDS scores of the sevoflurane group were significantly lower than those of the propofol group.The sevoflurane group showed greater stability in heart rate as well as the mean arterial pressure compared to the propofol group.Moreover,the sevoflurane group displayed better pulmonary function and less lung injury than the propofol group.CONCLUSION Both propofol and sevoflurane could be utilized as maintenance anesthesia during radical resection of gastric cancer.Propofol anesthesia has a minimal effect on patients'pulmonary function,consequently enhancing their postoperative recovery.Sevoflurane anesthesia causes less impairment on patients'cognitive function and mitigates negative emotions,leading to an improved postoperative mental state.Therefore,the selection of anesthetic agents should be based on the individual patient's specific circumstances. 展开更多

BACKGROUND Stroke frequently results in oropharyngeal dysfunction(OD),leading to difficulties in swallowing and eating,as well as triggering negative emotions,malnutrition,and aspiration pneumonia,which can be detrimental to patients.However,routine nursing interventions often fail to address these issues adequately.Systemic and psychological interventions can improve dysphagia symptoms,relieve negative emotions,and improve quality of life.However,there are few clinical reports of systemic interventions combined with psychological interventions for stroke patients with OD.AIM To explore the effects of combining systemic and psychological interventions in stroke patients with OD.METHODS This retrospective study included 90 stroke patients with OD,admitted to the Second Affiliated Hospital of Qiqihar Medical College(January 2022–December 2023),who were divided into two groups:regular and coalition.Swallowing function grading(using a water swallow test),swallowing function[using the standardized swallowing assessment(SSA)],negative emotions[using the selfrating anxiety scale(SAS)and self-rating depression scale(SDS)],and quality of life(SWAL-QOL)were compared between groups before and after the intervention;aspiration pneumonia incidence was recorded.RESULTS Post-intervention,the coalition group had a greater number of patients with grade 1 swallowing function compared to the regular group,while the number of patients with grade 5 swallowing function was lower than that in the regular group(P<0.05).Post-intervention,the SSA,SAS,and SDS scores of both groups decreased,with a more significant decrease observed in the coalition group(P<0.05).Additionally,the total SWAL-QOL score in both groups increased,with a more significant increase observed in the coalition group(P<0.05).During the intervention period,the total incidence of aspiration and aspiration pneumonia in the coalition group was lower than that in the control group(4.44%vs 20.00%;P<0.05).CONCLUSION Systemic intervention combined with psychological intervention can improve dysphagia symptoms,alleviate negative emotions,enhance quality of life,and reduce the incidence of aspiration pneumonia in patients with OD. 展开更多

BACKGROUND Lung cancer is a clinical disease with multiple malignant tumors.Currently,it is difficult for patients to benefit from routine clinical nursing due to the lack of a pertinent and systematic approach.AIM To investigate the effect of integrated nursing care on the negative emotions and satisfaction of lung cancer patients.METHODS From January 2018 to December 2019,92 patients with lung cancer were selected and divided into the study group and the control group;there were 46 patients in each group.The control group received routine nursing,and the study group received integrated medical care in addition to the care received by the control group.Negative emotions before and after the intervention,the self-management ability score after the intervention,family care burden after the intervention and nursing satisfaction after the intervention were measured in the two groups.RESULTS After the intervention,the self-rating anxiety scale and self-rating depression scale scores in the study group were lower than those in the control group(P<0.05);the scores for health knowledge,self-concept,self-responsibility and self-care skills in the study group were higher than those in the control group(P<0.05);the scores for individual burden and responsibility burden in the study group were lower than those before the intervention(P<0.05);and the nursing satisfaction in the study group(93.48%)was higher than that in the control group(78.26%,P<0.05).CONCLUSION An integrated nursing care approach for lung cancer patients can effectively relieve the patient’s negative feelings,improve their self-management ability,help to reduce the burden of family care and improve patient satisfaction with nursing activities. 展开更多

We hypothesized that if 'high reappraisers' (individuals who use reappraisal well in a behavioral experiment) completed two training sessions, they would exhibit more reliable patterns of neural activity related to cognitive reappraisal. In the present study, 13 high reappraisers were selected from 27 healthy volunteers through an initial behavioral experiment (first training) followed by a functional MRI experiment (second training). Emotional images selected from the International Affective Picture System were used for both the behavioral and functional MRI sessions of the experiment. The behavioral results revealed that reappraisal reduced subjective unpleasantness. The functional MRI results revealed that the cognitive reappraisal used by high reappraisers decreased the activation of emotion-responsive regions, including the amygdala, insula, and cingulate gyrus, and increased the activation of regulation-related regions, including the inferior prefrontal cortex, orbital prefrontal cortex, and dorsal medial prefrontal cortex. These findings suggest the involvement of inferior orbital and dorsal medial prefrontal cortex in constructing reappraisal strategies that modulate activity in emotion-processing systems. 展开更多

Objective:The study aims to investigate the effectiveness of yoga on negative emotions in breast cancer patients.Methods:Pubmed,Elsevier,Web of Science,Cochrane Library,CBM,CNKI,Wanfang,and VIP databases were screened throughout October 2015.Randomized control trials(RCTs)examining the effects of yoga versus a non-exercise or waitlist control group on negative moods in breast cancer patients were included.The methodological quality of included RCTs was evaluated by using the Cochrane Handbook 5.1,and data were analyzed using the Review Manager 5.3.Results:A total of 21 RCTs with 1762 participants were included.We found evidence for immediate effects on anxiety(p<0.00001),depression(p<0.00001),distress(p<0.00001),perceived stress(p<0.00001),and emotional well-being(p=0.0002).Sustained effects(3 months)were only found in depression(p=0.004)but not anxiety(p=0.43),and other outcomes were not synthesized because of heterogeneity and the limited number of studies.Conclusion:Yoga is valuable in improving negative moods in patients with breast cancer.We also concluded five key mechanisms of yoga therapy in improving negative moods.Further well-designed RCTs with large sample size and long-term follow-up are needed. 展开更多

To clarify the mechanism of blocking-stressors,psychological contracts,and negative emotions on post-90s employees’occupational mobility,based on the literature study,a hypothetical model of the relationship was established.Using the blocking-stressor,psychological contract,negative emotion,and occupational mobility scales,317 post-90s employees were selected to investigate their status.It used correlation analysis and intermedi-ary effect tests to verify the hypothesis model.The results showed that:first,there were significant positive cor-relations between blocking-stressors,negative emotions,and occupational mobility,while indicating a significant negative correlation with the psychological contract;second,blocking-stressors can directly and significantly pre-dict occupational mobility;and third,blocking-stressors can indirectly affect occupational mobility through the mediating role of negative emotions and the chain-mediating role of psychological contracts and negative emo-tions.The effective intervention of blocking-stressors,psychological contracts,and negative emotions can reduce the rate of occupational mobility and provide some guidance for enterprises in making rational use of human resources. 展开更多

BACKGROUND Acute pancreatitis(AP)is a common inflammatory disease of the pancreas with high mortality rates.It is of great significance to take scientific intervention measures for patients with AP in time.AIM To explore the effect of standardized nursing combined with mindfulness stress reduction training on the curative effect,negative emotion,and quality of life in patients with acute pancreatitis.METHODS A total of 80 patients with acute pancreatitis admitted to The First People's Hospital of Jiangxia District Hospital from May 2021 to May 2023 were randomly divided into control group and observation group(n=40).Patients in control group were given the standardized nursing intervention,and the observation group were given standardized nursing plus mindfulness stress reduction training intervention.The time of clinical symptom disappeared or improved,complication occurrence rate,emotional state,and quality of life score of the two groups were observed and compared.RESULTS In comparison with the control group,the bowel sound recovery time,ventosity and abdominal pain improvement time,and venting and cacation time in observation group were shorter,and the total incidence rate of complications was reduced,showing statistically significant difference(P<0.05).The scores of anxiety and depression in observation group were lower than those in control group(P<0.05).Serum levels of tumour necrosis factor alpha,interleukin(IL)-6,IL-1βand IL-8 in observation group were lower than those in control group(P<0.05).The scores of life quality in physiology,psychology,environment and social relations in observation group were higher than those in control group,and the differences were statistically significant(P<0.05).CONCLUSION The application of standardized nursing intervention combined with mindfulness stress reduction training in patients with acute pancreatitis has a definite effect,which can help to ameliorate the clinical symptoms,anxiety and depression of patients,reduce the incidence rate of complications,and improve the prognosis of patients. 展开更多

The positive and negative emotion functions related to loneliness:a systematic review of behavioural and neuroimaging studies

Loneliness is associated with high prevalences of major psychiatric illnesses such as major depression.However,the underlying emotional mechanisms of loneliness remained unclear.We hypothesized that loneliness originates from both decreases in positive emotional processing and increases in negative emotion processing.To test this,we conducted a systematic review of 29 previous studies(total participants n=19560,mean age=37.16 years,female proportion=59.7%),including 18 studies that included ques-tionnaire measures of emotions only,and 11 studies that examined the brain correlates of emotions.The main findings were that loneliness was negatively correlated with general positive emotions and positively correlated with general negative emotions.Fur-thermore,limited evidence indicates loneliness exhibited negative and positive correlations with the brain positive(e.g.the striatum)and negative(e.g.insula)emotion systems,respectively,but the sign of correlation was not entirely consistent.Additionally,loneli-ness was associated with the structure and function of the brain emotion regulation systems,particularly the prefrontal cortex,but the direction of this relationship remained ambiguous.We concluded that the existing evidence supported a bivalence model of lone-liness,but several critical gaps existed that could be addressed by future studies that include adolescent and middle-aged samples,use both questionnaire and task measures of emotions,distinguish between general emotion and social emotion as well as between positive and negative emotion regulation,and adopt a longitudinal design that allows us to ascertain the causal relationships between loneliness and emotion dysfunction.Our findings provide new insights into the underlying emotion mechanisms of loneliness that can inform interventions for lonely individuals. 展开更多

Negative emotion classification refers to the automatic classification of negative emotion of texts in social networks.Most existing methods are based on deep learning models,facing challenges such as complex structures and too many hyperparameters.To meet these challenges,in this paper,we propose a method for negative emotion classification utilizing a Robustly Optimized BERT Pretraining Approach(RoBERTa)and p-norm Broad Learning(p-BL).Specifically,there are mainly three contributions in this paper.Firstly,we fine-tune the RoBERTa to adapt it to the task of negative emotion classification.Then,we employ the fine-tuned RoBERTa to extract features of original texts and generate sentence vectors.Secondly,we adopt p-BL to construct a classifier and then predict negative emotions of texts using the classifier.Compared with deep learning models,p-BL has advantages such as a simple structure that is only 3-layer and fewer parameters to be trained.Moreover,it can suppress the adverse effects of more outliers and noise in data by flexibly changing the value of p.Thirdly,we conduct extensive experiments on the public datasets,and the experimental results show that our proposed method outperforms the baseline methods on the tested datasets. 展开更多

Objective:The purpose of the current study was to discuss the interrelation among alexithy-mia and negative psychology of nursing personnel and their coping style,so as to provide a theoret-ical basis for better mental health education and psychological intervention for nursing personnel.Methods:A questionnaire survey was administered to 503 nursing personnel sampled from a hospital in Henan Province between May and June 2010 by means of survey instruments of the Toronto Alexithymia Scale-20 Items(TAS-20),the Trait Coping Style Questionnaire(TCSQ),and the General Health Questionnaire(GHQ-20).Results:(1)Alexithymia and a negative coping style had a positive correlation with negative psychology(P<0.01),while a positive coping style had a negative correlation with negative psy-chology(P<0.01).(2)The model fitting indices of coping style as intermediate variables between alexithymia and negative psychology were χ^(2)/df=1.459,RMSEA=0.030,CFI=0.995,IFI=0.995,RFI=0.966,TCI=0.989,and NEI=0.985,indicating a good model fitting.Ninety-nine percent of the total variation of negative psychology could be explained by alexithymia and coping style,of which the indirect effect affecting negative psychology by coping style accounted for 94.59%of the total.Conclusion:Alexithymia and coping style were very important factors for improvement of current mental status of the nursing personnel,especially for a change in negative coping style and selection of good coping style. 展开更多

Chronic pain is a common clinical condition that is frequently linked to negative emotions such as anxiety and depression.Electroacupuncture(EA) has been shown to have beneficial therapeutic effects in analgesia and the reduction of pain-induced negative emotions,and promising results have been obtained in the study of its neural circuit mechanism.Optogenetics,chemogenetics,neurocircuit tracing,functional magnetic resonance imaging(fMRI),and conditional gene knockdown experiments have shown that EA activates parvalbumin(PV) interneurons in the anterior cingulate cortex(ACC),inhibits Protein kinase Mzeta-glutamate receptor(PKMzeta-GluR1) signaling pathway in ACC,and upregulates the expression of the neuropeptide S/neuropeptide S receptor(NPS/NPSR) system in ACC to alleviate pain and pain-induced anxiety.EA activates endogenous cannabinoid receptors 1(CB1Rs),inhibits y-aminobutyric acid(^(GABA))-ergic neurons and activates glutamatergic neurons in the ventrolateral periaqueductal gray(vlPAG) to exert an analgesic effect,while EA exerts an anxiolytic effect by downregulating CB1R in ^(GABA)ergic neurons in the ventral hippocampus(vHPC).EA relieves only pain-induced anxiety but not pain through inhibiting the rostral anterior cingulate cortex(rACC)Glu-thalamus circuit.By inhibiting the medial prefrontal cortex(mPFC)Glu-vlPAC^(GABA)circuit,EA relieves only pain but not pain-induced anxiety.EA activated dopamine receptor D1(DRD1) or inhibited dopamine receptor D2(DRD2) in the basolateral amygdala(BLA) to alleviate anxiety-like behaviors.Taken together,these findings would offer a novel theoretical framework for a thorough investigation of theoretical studies and clinical promotion of EA analgesic mechanisms. 展开更多
