AIM:To study the indications for sentinel lymph node biopsy(SLNB) in clinically-detected ductal carcinoma in situ(CD-DCIS).METHODS:A retrospective analysis of 20 patients with an initial diagnosis of pure DCIS by an i...AIM:To study the indications for sentinel lymph node biopsy(SLNB) in clinically-detected ductal carcinoma in situ(CD-DCIS).METHODS:A retrospective analysis of 20 patients with an initial diagnosis of pure DCIS by an image-guided core needle biopsy(CNB) between June 2006 and June 2012 was conducted at King Faisal Specialist Hospital.The accuracy of performing SLNB in CD-DCIS,the rate of sentinel and non-sentinel nodal metastasis,and the histologic underestimation rate of invasive cancer at initial diagnosis were analyzed.The inclusion criteria were a preoperative diagnosis of pure DCIS with no evidence of invasion.We excluded any patient with evidence of microinvasion or invasion.There were two cases of mammographically detected DCIS and 18 cases of CDDCIS.All our patients were diagnosed by an imageguided CNB except two patients who were diagnosed by fine needle aspiration(FNA).All patients underwent breast surgery,SLNB,and axillary lymph node dissection(ALND) if the SLN was positive.RESULTS:Twenty patients with an initial diagnosis of pure DCIS underwent SLNB,2 of whom had an ALND.The mean age of the patients was 49.7 years(range,35-70).Twelve patients(60%) were premenopausal and 8(40%) were postmenopausal.CNB was the diagnostic procedure for 18 patients,and 2 who were diagnosed by FNA were excluded from the calculation of the underestimation rate.Two out of 20 had a positive SLNB and underwent an ALND and neither had additional non sentinel lymph node metastasis.Both the sentinel visualization rate and the intraoperative sentinel identification rate were 100%.The false negative rate was 0%.Only 2 patients had a positive SLNB(10%) and neither had additional metastasis following an ALND.After definitive surgery,3 patients were upstaged to invasive ductal carcinoma(3/18 = 16.6%) and 3 other patients were upstaged to DCIS with microinvasion(3/18 = 16.6%).Therefore the histologic underestimation rate of invasive disease was 33%.CONCLUSION:SLNB in CD-DCIS is technically feasible and highly accurate.We recommend limiting SLNB to patients undergoing a mastectomy.展开更多

The characteristics and predictive factors for non-sentinel lymph node (NSLN) metastasis of patients with single positive SLN were also analyzed. Methods Patients with no and only one SLN metastasis (OIn and 1In group, n ≥2) were selected from 1228 cases of invasive breast carcinoma, who underwent axillary dissection in Shandong Cancer Hospital between November 1999 and December 2011, to compare the characteristics of NSLN metastasis between them. For the 1In group, the factors that influenced the NSLN metastasis were analyzed by univariate and multivariate analysis. Results Differences of the NSLN metastasis between the OIn and the 1In groups were significant (P 〈0.001). There was no significant difference between the axillary lymph node metastasis on level III in 1In group and OIn group (P=0.570). When the total SLN number was 〉4 and with one positive case, the NSLN metastasis was not significantly different from that in the OIn group (P=-0.118). In the 1In group, clinical tumor size (P = 0.012), over-expression of Her-2 (P=0.003), tumor grade (P=0.018) and the total number of SLN (P=0.047) significantly correlated with non-SLN metastasis. Clinical tumor size (P=0.015) and the expression of Her-2 (P=0.01) were independent predictive factors for non-SLN metastasis by the Logistic regression model. Conclusion Under certain conditions, breast cancer patients with single SLN metastasis could avoid ALND.展开更多

Axillary lymph node dissection (ALND) is the standard procedure for obtaining this information. However, postoperative complications are common. Sentinel lymph node (SLN) biopsy has been proposed as a potential alternative to ALND for staging breast carcinoma. This technique has been shown to be sensitive and specific for predicting the status of the axillary lymph nodes. 文摘目的:验证纪念斯隆-凯特琳癌症中心(Memorial Sloan-Kettering Cancer Center,MSKCC)模型和斯坦福大学模型(Stan-ford Online Calculator,SOC)预测中国前哨淋巴结(sentinel lymph node,SLN)阳性乳腺癌患者非前哨淋巴结(non-sentinel lymphnode,NSLN)转移的能力并进行比较。方法:收集120例SLN阳性的乳腺癌病例验证MSKCC和SOC模型,通过受试者工作特征曲线(Receiver Operating Characteristic Curve,ROC曲线)下面积(Area Under the Curve,AUC)、不同截断值的预测能力来比较两个模型在中国乳腺癌患者中的应用价值。结果:用MSKCC和SOC模型验证120例中国乳腺癌患者,AUC分别为0.688和0.734。取10%为截断值,MSKCC和SOC模型的假阴性率均为4.4%,阴性预测值分别为75.0%和90.0%。取90.0%为截断值,MSKCC和SOC模型的假阳性率分别为0.0%和6.7%,阳性预测值分别为100.0%和68.8%。结论:用MSKCC和SOC模型预测中国乳腺癌NSLN转移,结果皆劣于原始研究,SOC模型的预测能力略优于MSKCC模型。
