Although immunosuppression therapy improves graft and patient's survival, it is a major risk factor for infection following kidney transplantation altering clinical manifestations of the infectious diseases and complicating both the diagnosis and management of renal transplant recipients(RTRs). Existing literature is very limited regarding osteomyelitis in RTRs. Sternoclavicular osteomyelitis is rare and has been mainly reported after contiguous spread of infection or direct traumatic seeding of the bacteria. We present an interesting case of acute, bacterial sternoclavicular osteomyelitis in a long-term RTR. Blood cultures were positive for Streptococcus mitis, while the portal entry site was not identified. Magnetic resonance imaging of the sternoclavicluar region and a three-phase bone scan were positive for sternoclavicular osteomyelitis. Eventually, the patient was successfully treated with Daptomycin as monotherapy. In the presence of immunosuppression, the transplant physician should always remain alert for opportunistic pathogens or unusual location of osteomyelitis.展开更多

However, shelf life, spoilage and hygiene levels have been postulated to vary based on the type of processing system. Live-open non-commercial processing systems are popular among majority consumers owing to their perceived “freshness”, compared to commercially dressed chickens. In between, consumers have to balance freshness and quality assurance. Ultimately, this becomes inert, remotely but an important public health issue. However, lack of empirical evidence on safety levels to guide consumer product selection leaves them to speculation. It is this need to close this gap that created an impetus for us to undertake this study. Methods: Biological samples were collected before carcass wash and after carcass wash alongside a structured questionnaire that gathered risk-associated data. Standard microbiological enumeration methods were used to isolate bacteria and enumerate contamination. Results: Broiler carcasses processed from “open” non-commercial systems were more contaminated (45.6%) than “closed-abattoir” commercially processed systems (35%). Escherichia coli were major contaminants (71.3%) and few Salmonella spices (typhi or para-typhi) in 1.3%. Risk analysis indicates washing (method) of carcasses at commercial systems was significantly more risky for contamination than non-commercial ones. Major sources of contamination were “distance from water sources”. Increased volume of slaughters per day (>15,000 birds) for commercial systems accounted for increased cross-contamination, particularly, distance from water source was a ma-jor risk factor for contamination.展开更多

Patients and Methods: A retrospective study was conducted in the Orthopedics and Trauma department of the university teaching hospital of Bobo-Dioulasso (Burkina Faso). Over a period of two years, we selected all cases of chronic osteomyelitis. We studied the epidemiological, diagnostic and therapeutic aspects of SCMO cases. Results: Eleven cases of SCMO were identified. The mean age of the patients was 11.8 years and the sex ratio was 1.75. There were 7 rural patients. The mean time to visit the hospital was 158 days. The most common reasons for consultation were pain (10 cases), swelling (9 cases), and fever (7 cases). Two were sickle cell patients. The most affected bones were the femur (10 cases), the tibia (9 cases) and the fibula (6 cases). The most frequent specific radiological lesions were sequestra (6 cases) followed by pandiaphysitis. Staphylococcus aureus was the most common aetiology. Treatment combined antibiotherapy and surgery (sequestrectomy and/or bone curettage and/or fistulectomy). Hip dislocations, pathological fractures and bone defect complicated the course of this disease. The therapeutic results was good in 3 patients and bad in 5 patients;the other 3 patients were lost to follow-up. Conclusion: Septic chronic multifocal osteomyelitis is a rare but formidable form of osteomyelitis in children. Therapeutic outcomes are often poor in hospitals with limited resources. The best strategy is prevention through early diagnosis and aggressive treatment of acute osteomyelitis.展开更多
