The results show that the annual fluctuations of the monthly mean sea levels in the Bohai Sea are the results of the coupling response of seasonal variations of the marine hydrometeorological factors. Furthermore, the regression prediction equation is obtained by using the double screening stepwise regression analysis method . We evaluated medical records of c...AIM: To identify a mean platelet volume(MPV) cutoff value which should be able to predict the presence of bacterial infection.METHODS: An observational, analytic, retrospective study. We evaluated medical records of cirrhotic patients who were hospitalized from January 2012 to January 2014 at the Gastroenterology Department of "Hospital General de México Dr. Eduardo Liceaga", we included 51 cirrhotic patients with ascites fluid infection(AFI), and 50 non-infected cirrhotic patients as control group. Receiver operator characteristic curves were used to identify the best cutoff value of several parameters from hematic cytometry, including MPV, to predict the presence of ascites fluid infection.RESULTS: Of the 51 cases with AFI, 48 patients(94.1%) had culture-negative neutrocytic ascites(CNNA), 2(3.9%) had bacterial ascites, and one(2%)had spontaneous bacterial peritonitis. Infected patients had greater count of leucocytes and polymorphonuclear cells, greater levels of MPV and cardiac frequency(P < 0.0001), and lower mean arterial pressure compared with non-infected patients(P = 0.009). Leucocytes, polymorphonuclear count, MPV and cardiac frequency resulted to be good or very good predictive variables of presence of AFI in cirrhotic patients(area under the receiving operating characteristic > 0.80). A cutoff MPV value of 8.3 fl was the best to discriminate between cirrhotic patients with AFI and those without infection. CONCLUSION: Our results support that MPV can be an useful predictor of systemic inflammatory response syndrome in cirrhotic patients with AFI, particularly CNNA.展开更多

Nevirapine and EFV have comparable clinical efficacy when administere...Background: There are two approved non-nucleoside reverse transcriptase inhibitor antiretroviral drugs;namely Nevirapine (NVP) and Efavirenz (EFV). Nevirapine and EFV have comparable clinical efficacy when administered in combination regimens. But there is a lack of recent evidence showing the effect of NVP and EFV-based ARTs on immunological responses in HIV infected individuals in Ethiopia in general and Addis Ababa in particular. Methods: Retrospective cohort study design was used to compare immunological response rate of NVP and EFV based HAART regimen in Addis Ababa. Four hundred ninety two HIV infected patients who started HAART in ten selected health facilities were included in the study. Rate of immunologic response was examined at the 6th, 12th, 18th, and 24th months of follow-up period. The time required to get immunological response was analyzed by Kaplan-Meier survival curve. Adjusted hazard ratio was calculated with a 95% confidence interval by Cox proportional hazards model to determine the rate of immunological response. To ascertain the association, bivariate and multi variable Cox proportional hazard model was used. Statistical significance was considered with two sides P-value of 0.05. Results: The mean CD4 count ranged between 132.2 cell/μl at baseline and 302.3 cell/μl at the end of the follow-up period. This change was significant at 95% of CI but did not show significant differences among the comparison group. The median time to get immunological response was 18 (75% percental 12) months. At the end of the follow-up period, 73.2% (76.6% for NVP and 69.8% for EVF P-value 0.13) of the study population had immunological response. Conclusion: As a conclusion, there was a robust and sustained CD4 response and the effect of NVP and EFV based ART on change of mean CD4 count and immunological response was comparable and effective. Initiation of ART with high baseline CD4 count, in combination of IPT and with either NVP or EFV based NNTI was recommended.展开更多

These missing observations may be for either auxiliary variable or study variable. The biases and mean square errors of the proposed estimators have been derived, up to the first order of approximation. The proposed estimators are compared theoretically with that of the existing ratio type estimators defined by [1]. It has been found that the proposed exponential ratio type estimators perform better than the mean per unit estimator even for the low positive correlation between study variable and auxiliary variable. Moreover, we obtained the conditions for which our proposed estimators are better than the corresponding ratio type estimators of [1]. To verify the theoretical results obtained, a simulation study is carried out finally.展开更多
A novel vibration isolation device called the nonlinear energy sink(NES)with NiTiNOL-steel wire ropes(NiTi-ST)is applied to a whole-spacecraft system.The NiTi-ST is used to describe the damping of the NES,which is cou...A novel vibration isolation device called the nonlinear energy sink(NES)with NiTiNOL-steel wire ropes(NiTi-ST)is applied to a whole-spacecraft system.The NiTi-ST is used to describe the damping of the NES,which is coupled with the modified Bouc-Wen model of hysteresis.The NES with NiTi-ST vibration reduction principle uses the irreversibility of targeted energy transfer(TET)to concentrate the energy locally on the nonlinear oscillator,and then dissipates it through damping in the NES with NiTi-ST.The generalized vibration transmissibility,obtained by the root mean square treatment of the harmonic response of the nonlinear output frequency response functions(NOFRFs),is first used as the evaluation index to analyze the whole-spacecraft system in the future.An optimization analysis of the impact of system responses is performed using different parameters of NES with NiTi-ST based on the transmissibility of NOFRFs.Finally,the effects of vibration suppression by varying the parameters of NiTi-ST are analyzed from the perspective of energy absorption.The results indicate that NES with NiTi-ST can reduce excessive vibration of the whole-spacecraft system,without changing its natural frequency.Moreover,the NES with NiTi-ST can be directly used in practical engineering applications.展开更多
Global warming causes an unstable response in tree radial growth at high latitudes in the Northern Hemisphere.Additionally,different climatic responses of different age groups of trees have been found due to their dif...Global warming causes an unstable response in tree radial growth at high latitudes in the Northern Hemisphere.Additionally,different climatic responses of different age groups of trees have been found due to their different physiological mechanisms.In this study,the response stability and growth trend of three age groups(young<100 a,middle 100-200 a,old≥200 a)of Picea schrenkiana(Schrenk spruce)to climate change and the causes of the different responses in different age groups were analyzed in the relatively dry climate of the eastern Tianshan Mountains.The results showed that:(1)With the abrupt increase in temperature in 1989,the annual mean minimum temperature became the dominant radial growth-limiting factor of the three age groups of Schrenk spruce.(2)The radial growth of the middle and young groups was more sensitive than that of the old group based on growth-climate correlation analysis.(3)The radial growth of the different age groups had different responses to climate factors,and all age groups were unstable on time scales.(4)The trend of the linear regression simulation of the basal area increment(BAI)indicated that the Schrenk spruce had the same growth trends in different age groups with growth first increased and then decreased;however,the decreased growth rate was higher in the middle and young age groups than in the old age group after the abrupt increase in temperature.Therefore,we should pay active attention to the impact of drought on Schrenk spruce in the eastern Tianshan Mountains and should particularly strengthen the conservation and management of the middle and young age groups.展开更多

What is needed is not only a few more government regulations or a little more international cooperation based on the unquestioned presupposition of the priority of national sovereignty and economic growth,but also a different perspective on our responsibilities for the wellbeing of the commons, recognizing that increasingly the environmental commons is global in scope and requires some transnational means of governance. It also holds that a"responsibilities"approach calls for the greening of all kinds of responsibility and not only those of national governments and international organizations.展开更多
