BACKGROUND The debate regarding diagnostic classification systems in psychiatry(categorial vs dimensional systems)has essential implications for the diagnosis,prevention and treatment of stress reactions.We previously...BACKGROUND The debate regarding diagnostic classification systems in psychiatry(categorial vs dimensional systems)has essential implications for the diagnosis,prevention and treatment of stress reactions.We previously found a unique pattern of stress reaction in a study executed during the coronavirus disease 2019 pandemic using large representative samples in two countries,and termed it the Complex Stress Reaction Syndrome(CSRS).AIM To investigate CSRS,Type A(psychiatric symptoms,spanning anxiety,depression,stress symptoms,and posttraumatic stress disorder(PTSD)),with or without long-coronavirus disease(COVID)residuals(CSRS,Type B,neuropsychiatric symptoms spanning cognitive deficits and fatigue,excluding systemic symptoms).Our two-tailed hypothesis was that CSRS is a condition related to an unrecognized type of stress reaction in daily life in the general population(Type A)or that it is related to the severe acute respiratory syndrome coronavirus 2 infection and its long-COVID residuals(Type B).METHODS 977 individuals in four continents(North America,Europe,Australia and the Middle East)completed the online study questionnaire in six languages using the Qualtrics platform.The study was managed by six teams in six countries that promoted the study on social media.The questionnaire assessed anxiety,depression,stress symptoms and PTSD(CSRS,Type A),cognitive deficits and fatigue(CSRS,Type B).The data were analyzed using Proportion Analyses,Multivariate Analysis of Co-Variance(MANCOVA),linear regression analyses and validated clinical cutoff points.RESULTS The results of the Proportion Analyses showed that the prevalence of 4 symptoms spanning anxiety,depression,stress symptoms,and PTSD was significantly higher than the most prevalent combinations of fewer symptoms across 4 continents,age groups,and gender.This supports the transdiagnostic argument embedded in the CSRS(Type A).The same pattern of results was found in infected/recovered individuals.The prevalence of the 4 psychiatric symptoms combination was significantly greater than that of 5 and 6 symptoms,when adding cognitive deficits and fatigue,respectively.MANCOVA showed a significant three-way interaction(age×gender×continent).Further analyses showed that the sources of this three-way interaction were threefold relating to two sub-populations at-risk:(1)Individuals that self-identified as non-binary gender scored significantly higher on all 4 psychiatric symptoms of the CSRS,Type A at young age groups(<50 years old)in North America compared to(self-identified)women and men located in the 4 continents studied,and to other ages across the adult life span;and(2)This pattern of results(CSRS,Type A)was found also in women at young ages(<40 years old)in North America who scored higher compared to men and women in other continents and other ages.Linear regression analyses confirmed the MANCOVA results.CONCLUSION These results show a combined mental health risk factor related to stress reactivity,suggesting that the CSRS is sensitive to populations at risk and may be applied to future identification of other vulnerable sub-populations.It also supports the transdiagnostic approach for more accurate prevention and treatment.Time will tell if such transdiagnostic syndromes will be part of the discussions on the next revisions of the traditional classification systems or whether the crisis in psychiatry further evolves.展开更多

A reaction-diffusion-str...Intercalation of lithium ions into the electrodes of lithium ion batteries is affected by the stress of active materials, leading to energy dissipation and stress dependent voltage hysteresis. A reaction-diffusion-stress coupling model is established to investigate the stress effects under galvanostatic and potentiostatic operations. It is found from simulations that the stress hysteresis contributes to the voltage hysteresis and leads to the energy dissipation. In addition, the stress induced voltage hysteresis is small in low rate galvanostatic operations but extraordinarily significant in high rate cases. In potentiostatic operations, the stresses and stress induced overpotentials increase to a peak value very soon after the operation commences and decays all the left time. Therefore,a combined charge-discharge operation is suggested, i.e., first the galvanostatic one and then the potentiostatic one. This combined operation can not only avoid the extreme stress during operations so as to prevent electrodes from failure but also reduce the voltage hysteresis and energy dissipation due to stress effects.展开更多

The governing boundary layer equations and boundary conditions are cast into a dimen- sionless form by similarity transformations, and the resulting system of equations is solved by a finite difference method. The results are preSented and discussed for concentration profiles, as well as the Nusselt number and the Sherwood number for various values of the parameters, which govern the problem. The calculation results show that, when the joint is brazed at 1123 K for 300 s (low brazing parameters), the maximum shear stress value occurs on (Cu, Ni) layer near TiC cermets, which is 92.16 MPa as the temperature is 300 K. When the joint is brazed at 1273 K for g00 s (high brazing parameters), the maximum shear stress value occurs on (Cu, Ni)+(Fe, Ni) layer, which is 39.18 MPa as the temperature is 300 K. The fracture sites of the joints obtained from numerical simulation calculation accord with experimental results.展开更多
Objective:To observe the effect of dexmedetomidine on the stress reaction caused by sevoflurance anesthesia during the perianesthesia period in school-age children with different personality characteristics.Methods:A ...Objective:To observe the effect of dexmedetomidine on the stress reaction caused by sevoflurance anesthesia during the perianesthesia period in school-age children with different personality characteristics.Methods:A total of 160 children who were admitted in our hospital for transumbilical single-port laparoscopic hernia inner ring ligation were included in the study and randomized into dexmedetomidine (D) group and midazolam (M) group. The children were performed with Eysenck Personality Questionnaire before operation (children edition). Children were divided into the emotion group (group I) and stable group (group II). The operation was performed under sevoflurance inhalation anesthesia. Patients in DI and DII groups were given 1 μg/kg DEX, MI and MII groups were given 0.05 mg/kg midazolam, for 15 min. MAP and HR one day before operation (T0), before anesthesia induction (T1), 1 min after pneumoperitoneum establishment (T2), the time after pulling out the laryngeal mask (T3), and the time after shifting from the recovery room (T4) were recorded. A volume of 6 mL venous blood 1 d before operation and 4 h after operation was collected. Blood sugar concentration, cortisol and IL-6 levels were detected.Results:MAP at T1, T2, T3, and T4 in DI and MI groups were significantly elevated when compared with at T0. MAP at T2 and T3 in DII group was significantly elevated when compared with at T0. MAP at T2, T3, and T4 in MII group was significantly elevated when compared with at T0. When compared with DI group, MAP at T1, T2, and T3 in DII group was significantly reduced;MAP at T2, T3, and T4 in MI group was significantly elevated;MAP at T1 in MII group was significantly reduced. When compared with DII group, MAP at T2, T3, and T4 in MI and MII groups was significantly elevated. HR at T1, T2, T3, and T4 was significantly elevated when compared with at T0. When compared with DI group, HR at T1, T2, and T3 in DII group was significantly reduced;HR at each timing point in MI group was significantly elevated;HR at T1 in MII group was significantly reduced, while at T2, T3, and T4 was significantly elevated. When compared with DII group, HR at each timing point in MI group was significantly elevated;HR at T2, T3, and T4 in MII group was significantly elevated. Except for DII group, the blood sugar, cortisol, and IL-6 levels were significantly elevated when compared with before operation. When compared with D1 group, the blood sugar and cortisol levels at T1 in DII group were significantly reduced;the blood sugar, cortisol, and IL-6 levels at T1 in MI and MII groups were significantly elevated. When compared with DII group, the blood sugar, cortisol, and IL-6 levels at T1 in MI and MII groups were significantly elevated.Conclusions:Application of hydrochloric DEX in sevoflurance anesthesia in children can effectively inhibit the excitability of sympathetic nervous system caused by operation, alleviate the stress reaction, and maintain the stability of hemodynamics, which is benefit for the postoperative rehabilitation.展开更多

Three groups of patients were separately given by general anesthesia, epidural anesthesia and combined spinal and epidural anesthesia for operations;The variations of adrenocorticotrophic hormone (ACTH), Cortisol (Cor),β-endorphin (β-EP), Angiotensin-Ⅱ(Ang-Ⅱ), heart rate (HR) and blood pressure (SBP, DBP) on patients in three groups before anesthesia (T0), during skin incision (T1), after skin incision (T2) and extubation after operation (T3) were compared and analyzed.Results:During T1, T2, ACTH, Cor,β-EP and Ang-Ⅱlevels in 3 groups of patients were significantly higher than those during T0;SBP and DBP were significantly lower than that during T0;HR during T2 was significantly lower than that during T0;During T3, every index in 3 groups were recovered to levels close to that during T0;During T1, T2, ACTH, Cor,β-EP, Ang-Ⅱlevels in group B and C were significantly lower than that in group A. And levels in C was lower than that in B;SBP and DBP in group B and C were significantly higher than A. No HR statistical significance appeared between each group.Conclusions: During clinical anesthesia, we should choose suitable anesthesia method combined with actual situations of patients. We measured simple and choice reaction times of parts of the body and choice reaction times of the whole body in the participants with high and low neuroticism such as depression, cyclic tendency, inferiority feelings, and nervousness under the controlled stress scenario of being in front of people. Results indicated significant differences in reaction times of partial body, discriminative decisions and choice reaction times of participants in the high neuroticism group. Results of whole body reaction times were opposite to that of partial body reactions. However, a two-way factorial analysis of variance indicated neither a significant main effect, nor interactions between reaction times and personality traits. Whole body reactions suggest that the effects of stimulus transduction process from motor commands to motor outputs via motor neurons has a greater effect in the central nervous system than recognition and decision-making. However, it is possible that changes in the intracortical mechanisms related to cognitions and emotions could be correlated in partial body movements with nearly no muscular activity. Our findings suggested that delays indiscrimination, decision-making, and choice times could affect the performance of individuals.展开更多

Methods: A total of 90 elderly patients with gallstone who were admitted in our hospital for laparoscopic cholecystectomy (LC) were included in the study and randomized into the observation I, II, and III groups. Narcotrend (NT) was used to monitor the anesthesia depth during operation in the three groups, and the anesthesia depth was maintained at D0, D1, and D2 levels, respectively. The vital signs before anesthesia induction, before and after tracheal intubation, before and after operation, and extubation in the three groups were monitored and recorded. ACTH, COR, and TNF-α levels before anesthesia induction, before and after operation, 1 and 4 d after operation were detected. MOCA and MMSE were used to evaluate the cognition 1d before operation and 4d after operation. Results: The fluctuation of MAP and HR after tracheal intubation, before and after operation, and extubation in observation III group was the smallest. COR level before operation in observation I group was significantly higher than that in observation III group, while COR level after operation was significantly higher than that in the observation II and III groups, and COR level before and after operation in observation II group was significantly higher than that in observation III group. ACTH level before and after operation in observation I and II groups was significantly higher than that in observation III group. The comparison of TNF-α level at each timing-point among the three groups was not statistically significant. The comparison of MOCA and MMSE scores 1d before operation and 4d after operation among the three groups was not statistically significant. Conclusions: The anesthesia depth maintaining at D2 level for elderly patients during perioperative period of LC is more beneficial to maintain the stability of haemodynamics, and reduce the stress reaction.展开更多

In recent years, the functions of acupuncture in anesthesia, especially in regulation of circulatory properties have been developed gradually and have been applied by many physicians in controlling the stress reaction induced by tracheal intubation in general anesthesia. The inflammatory reaction, immune function, stress hormone and ovarian function levels were compared between the two groups before and after treatment (before operation and after 1 d of operation). Results: After comparison, before treatment the difference of TNF-α, CRP, CD3+, CD4+, CD8+, CD4+/CD8+, COR, NE, LH, FSH and E2 levels between the groups was not statistically significant;After treatment, the TNF-α, CRP, CD8+, COR, NE, LH and FSH levels of two groups were significantly increased, and the levels after treatment in the control group were significantly higher than the observation group, the difference was statistically significant;The levels of CD3+, CD4+, CD4+/CD8+ and E2 in the observation group were significantly higher than those in the control group, the difference was statistically significant. Methods:A total of 70 patients who ...Objective:To explore the effect of hydromorphone hydrochloride postoperative analgesia on the stress reaction and hemorheology in elderly patients after total hip replacement (THR). Methods:A total of 70 patients who were admitted in our hospital from June, 2015 to June, 2016 for THR were included in the study and randomized into the observation group and the control group with 35 cases in each group. After operation, the patients in the two groups were immediately given patient-controlled analgesia. The patients in the observation group were given hydromorphone hydrochloride, while the patients in the control group were given sufentanil. The peripheral venous blood before operation, 12 and 24 h after operation in the two groups was collected. RIA was used to detect BG and Cor. ELISA was used to detect NE. The peripheral venous blood before operation, 24 and 72 h after operation was collected. The full automatic hematology analyzer was used to detect the plasma PV, BV, and RWBV. Results:Cor and NE levels in the observation group, and BG, Cor, and NE levels in the control group 12 and 24 h after operation were significantly elevated when compared with before operation (P<0.05), but the above indicators at each timing point in the control group were significantly higher than those in the observation group (P<0.05). PV, BV (low-shear, middle-shear, and high-shear), and RWBV (low-shear, middle-shear, and high-shear) levels 24 and 72 h after operation in the two groups were significantly elevated when compared with before operation (P<0.05), but the above indicators at each timing point in the control group were significantly higher than those in the observation group (P<0.05). Conclusions:Application of hydromorphone hydrochloride in the postoperative analgesia in elderly patients with THR has a preferable analgesia effect, can effectively alleviate the postoperative stress reaction, stabilize the hemorheology, and contribute to the rehabilitation.展开更多

The results obtained show that the flow field is influenced appreciably by the presence of the chemical reaction parameter γ the order of.the chemical reaction parameter m, the Soret number St, the buoyancy ratio N, the Lewis number Le, and the dimensionless rheological parameter Ω.

文摘For the first time, considering the effect of reaction layers, numerical simulation calculation of residual stress on brazed TiC cermets/steel joint was studied by finite element method (FEIVl). The calculation results show that, when the joint is brazed at 1123 K for 300 s (low brazing parameters), the maximum shear stress value occurs on (Cu, Ni) layer near TiC cermets, which is 92.16 MPa as the temperature is 300 K. When the joint is brazed at 1273 K for g00 s (high brazing parameters), the maximum shear stress value occurs on (Cu, Ni)+(Fe, Ni) layer, which is 39.18 MPa as the temperature is 300 K. The fracture sites of the joints obtained from numerical simulation calculation accord with experimental results.
文摘Objective:To observe the effect of dexmedetomidine on the stress reaction caused by sevoflurance anesthesia during the perianesthesia period in school-age children with different personality characteristics.Methods:A total of 160 children who were admitted in our hospital for transumbilical single-port laparoscopic hernia inner ring ligation were included in the study and randomized into dexmedetomidine (D) group and midazolam (M) group. The children were performed with Eysenck Personality Questionnaire before operation (children edition). Children were divided into the emotion group (group I) and stable group (group II). The operation was performed under sevoflurance inhalation anesthesia. Patients in DI and DII groups were given 1 μg/kg DEX, MI and MII groups were given 0.05 mg/kg midazolam, for 15 min. MAP and HR one day before operation (T0), before anesthesia induction (T1), 1 min after pneumoperitoneum establishment (T2), the time after pulling out the laryngeal mask (T3), and the time after shifting from the recovery room (T4) were recorded. A volume of 6 mL venous blood 1 d before operation and 4 h after operation was collected. Blood sugar concentration, cortisol and IL-6 levels were detected.Results:MAP at T1, T2, T3, and T4 in DI and MI groups were significantly elevated when compared with at T0. MAP at T2 and T3 in DII group was significantly elevated when compared with at T0. MAP at T2, T3, and T4 in MII group was significantly elevated when compared with at T0. When compared with DI group, MAP at T1, T2, and T3 in DII group was significantly reduced;MAP at T2, T3, and T4 in MI group was significantly elevated;MAP at T1 in MII group was significantly reduced. When compared with DII group, MAP at T2, T3, and T4 in MI and MII groups was significantly elevated. HR at T1, T2, T3, and T4 was significantly elevated when compared with at T0. When compared with DI group, HR at T1, T2, and T3 in DII group was significantly reduced;HR at each timing point in MI group was significantly elevated;HR at T1 in MII group was significantly reduced, while at T2, T3, and T4 was significantly elevated. When compared with DII group, HR at each timing point in MI group was significantly elevated;HR at T2, T3, and T4 in MII group was significantly elevated. Except for DII group, the blood sugar, cortisol, and IL-6 levels were significantly elevated when compared with before operation. When compared with D1 group, the blood sugar and cortisol levels at T1 in DII group were significantly reduced;the blood sugar, cortisol, and IL-6 levels at T1 in MI and MII groups were significantly elevated. When compared with DII group, the blood sugar, cortisol, and IL-6 levels at T1 in MI and MII groups were significantly elevated.Conclusions:Application of hydrochloric DEX in sevoflurance anesthesia in children can effectively inhibit the excitability of sympathetic nervous system caused by operation, alleviate the stress reaction, and maintain the stability of hemodynamics, which is benefit for the postoperative rehabilitation.
文摘Objective:To study the influence of general anesthesia, epidural anesthesia and combined spinal and epidural anesthesia method on stress reaction and hemodynamics for elderly orthopedics patients during operations.Methods: A total of 90 cases of elder patients who received orthopedic operations were randomly divided to group A, B and C, with 30 cases per group. Three groups of patients were separately given by general anesthesia, epidural anesthesia and combined spinal and epidural anesthesia for operations;The variations of adrenocorticotrophic hormone (ACTH), Cortisol (Cor),β-endorphin (β-EP), Angiotensin-Ⅱ(Ang-Ⅱ), heart rate (HR) and blood pressure (SBP, DBP) on patients in three groups before anesthesia (T0), during skin incision (T1), after skin incision (T2) and extubation after operation (T3) were compared and analyzed.Results:During T1, T2, ACTH, Cor,β-EP and Ang-Ⅱlevels in 3 groups of patients were significantly higher than those during T0;SBP and DBP were significantly lower than that during T0;HR during T2 was significantly lower than that during T0;During T3, every index in 3 groups were recovered to levels close to that during T0;During T1, T2, ACTH, Cor,β-EP, Ang-Ⅱlevels in group B and C were significantly lower than that in group A. And levels in C was lower than that in B;SBP and DBP in group B and C were significantly higher than A. No HR statistical significance appeared between each group.Conclusions: During clinical anesthesia, we should choose suitable anesthesia method combined with actual situations of patients. Combined spinal and epidural anesthesia had a slight influence on hemodynamics of elder orthopedics patients during operation, and it could effectively alleviate stress reaction during operation.

文摘The effect of pan-milling on morphological structure,processability and properties of PVC was studied throughSEM,FTIR,granulometer,GPC and mechanical properties test in the hope of gaining ease in operation,needless ofplasticizers,a clean and efficient route for improving the processability of PVC through stress-induced reactions,fulfilling the idea of“plasticizing PVC by itself”.The experimental results show that during pan-milling at ambienttemperature,within 2-3 min,the microcrystalline structure of PVC becomes indistinct,the grain size of PVC is reducedfrom 130-160 μm to 1-50 μm the molecular weight of PVC is slightly decreased,the variation of molecular weightdistribution is indistinct,the plasticizing time and torque at balance drop a great deal from 71-132 s to 31-33 s and from18.2-22.1 Nm to 14.7-18.4 Nm,respectively,the processability of PVC is markedly improved,and the mechanicalproperties get enhanced too.
文摘Effects of neuroticism on partial and whole body reaction times were investigated under stress, or choking conditions in student participants (N = 127). We measured simple and choice reaction times of parts of the body and choice reaction times of the whole body in the participants with high and low neuroticism such as depression, cyclic tendency, inferiority feelings, and nervousness under the controlled stress scenario of being in front of people. Results indicated significant differences in reaction times of partial body, discriminative decisions and choice reaction times of participants in the high neuroticism group. Results of whole body reaction times were opposite to that of partial body reactions. However, a two-way factorial analysis of variance indicated neither a significant main effect, nor interactions between reaction times and personality traits. Whole body reactions suggest that the effects of stimulus transduction process from motor commands to motor outputs via motor neurons has a greater effect in the central nervous system than recognition and decision-making. However, it is possible that changes in the intracortical mechanisms related to cognitions and emotions could be correlated in partial body movements with nearly no muscular activity. Our findings suggested that delays indiscrimination, decision-making, and choice times could affect the performance of individuals.
文摘Objective: To observe the effect of anesthesia depth on the stress reaction and cognitive level in elderly patients underwent minimally invasive cholecystectomy in order to provide a basis for the selection of anesthesia depth. Methods: A total of 90 elderly patients with gallstone who were admitted in our hospital for laparoscopic cholecystectomy (LC) were included in the study and randomized into the observation I, II, and III groups. Narcotrend (NT) was used to monitor the anesthesia depth during operation in the three groups, and the anesthesia depth was maintained at D0, D1, and D2 levels, respectively. The vital signs before anesthesia induction, before and after tracheal intubation, before and after operation, and extubation in the three groups were monitored and recorded. ACTH, COR, and TNF-α levels before anesthesia induction, before and after operation, 1 and 4 d after operation were detected. MOCA and MMSE were used to evaluate the cognition 1d before operation and 4d after operation. Results: The fluctuation of MAP and HR after tracheal intubation, before and after operation, and extubation in observation III group was the smallest. COR level before operation in observation I group was significantly higher than that in observation III group, while COR level after operation was significantly higher than that in the observation II and III groups, and COR level before and after operation in observation II group was significantly higher than that in observation III group. ACTH level before and after operation in observation I and II groups was significantly higher than that in observation III group. The comparison of TNF-α level at each timing-point among the three groups was not statistically significant. The comparison of MOCA and MMSE scores 1d before operation and 4d after operation among the three groups was not statistically significant. Conclusions: The anesthesia depth maintaining at D2 level for elderly patients during perioperative period of LC is more beneficial to maintain the stability of haemodynamics, and reduce the stress reaction.
文摘By summarizing the formation of stress reaction induced by tracheal intubation and method of medicinal control, the recognition of Chinese medicine in treatment of cardiac arrhythmia and hypertension with acupuncture and the clinical application of acupuncture on stress reaction induced by tracheal intubation, it is concluded that the side effects of tracheal intubation are inevitable, even though there are many methods presented for the prevention and treatment for it. In recent years, the functions of acupuncture in anesthesia, especially in regulation of circulatory properties have been developed gradually and have been applied by many physicians in controlling the stress reaction induced by tracheal intubation in general anesthesia. Being a kind of dual-directional and positive regulation and stimulation, acupuncture provides definite and safe effects on controlling the stress reaction induced by tracheal intubation.
文摘Objective: To investigate the effect of laparoscopic myomectomy on inflammatory reaction, immune function, stress hormones and ovarian function. Methods: Selected a total of 82 patients with hysteromyoma accorded with the screening criteria as the objects of study, according to the random data table, the patients were divided into the control group (n=41) and the observation group (n=41);The patients in the control group received traditional laparotomy myomectomy, and the observation group patients received laparoscopic myomectomy. The inflammatory reaction, immune function, stress hormone and ovarian function levels were compared between the two groups before and after treatment (before operation and after 1 d of operation). Results: After comparison, before treatment the difference of TNF-α, CRP, CD3+, CD4+, CD8+, CD4+/CD8+, COR, NE, LH, FSH and E2 levels between the groups was not statistically significant;After treatment, the TNF-α, CRP, CD8+, COR, NE, LH and FSH levels of two groups were significantly increased, and the levels after treatment in the control group were significantly higher than the observation group, the difference was statistically significant;The levels of CD3+, CD4+, CD4+/CD8+ and E2 in the observation group were significantly higher than those in the control group, the difference was statistically significant. Conclusion: Laparoscopic myomectomy can effectively reduce the inflammatory reaction and stress reaction, and have a less influence on the immune function and ovarian function, which is more conducive to postoperative rehabilitation.
文摘Psychological trauma refers to the catastrophic or traumatic events harmful to individuals' inner world. Strong emotional and psychological reaction caused by traumatic factors will form psychological shadows, and the improper coping of these events will lead to a post-traumatic stress disorder. In this paper, the author makes an exploration on the roots of psychological trauma and of post-traumatic stress reaction or disorders, which were caused by natural traumatic event, such as floods, snowstorms, earthquakes and other man-made factors such as wars, accidents from a psychological point of view. Finally, some suggestions of intervention are provided for the relevant departments to make relevant rebuilding policies.
文摘Objective:To study the effect of remifentanil combined with etomidate on systemic stress reaction during painless fiberoptic bronchoscopy.Methods:A total of 84 patients who received painless fiberoptic bronchoscopy in outpatient and ward of our hospital between May 2014 and October 2016 were selected as the research subjects and randomly divided into two groups, observation group of patients accepted remifentanil combined with etomidate anesthesia, control group of patients received surface anesthesia, and in various time periods of inspection (before, during and after inspection), serum was collected respectively to test stress indexes.Results:Before inspection, serum NE, E, ACTH, Cor, PRA, AT-II, ALD, CRP, TNF-α, IL-6 and MCP-1 contents were not significantly different between two groups of patients;during and after inspection, serum NE, E, ACTH, Cor, PRA, AT-II, ALD, CRP, TNF-α, IL-6 and MCP-1 contents of both groups of patients were significantly higher than those before inspection, and serum NE, E, ACTH, Cor, PRA, AT-II, ALD, CRP, TNF-α, IL-6 and MCP-1 contents of observation group were significantly lower than those of control group. Conclusion:Remifentanil combined with etomidate can significantly inhibit the activation of systemic stress reaction and the release of stress hormones during painless fiberoptic bronchoscopy.
文摘Objective:To explore the effect of hydromorphone hydrochloride postoperative analgesia on the stress reaction and hemorheology in elderly patients after total hip replacement (THR). Methods:A total of 70 patients who were admitted in our hospital from June, 2015 to June, 2016 for THR were included in the study and randomized into the observation group and the control group with 35 cases in each group. After operation, the patients in the two groups were immediately given patient-controlled analgesia. The patients in the observation group were given hydromorphone hydrochloride, while the patients in the control group were given sufentanil. The peripheral venous blood before operation, 12 and 24 h after operation in the two groups was collected. RIA was used to detect BG and Cor. ELISA was used to detect NE. The peripheral venous blood before operation, 24 and 72 h after operation was collected. The full automatic hematology analyzer was used to detect the plasma PV, BV, and RWBV. Results:Cor and NE levels in the observation group, and BG, Cor, and NE levels in the control group 12 and 24 h after operation were significantly elevated when compared with before operation (P<0.05), but the above indicators at each timing point in the control group were significantly higher than those in the observation group (P<0.05). PV, BV (low-shear, middle-shear, and high-shear), and RWBV (low-shear, middle-shear, and high-shear) levels 24 and 72 h after operation in the two groups were significantly elevated when compared with before operation (P<0.05), but the above indicators at each timing point in the control group were significantly higher than those in the observation group (P<0.05). Conclusions:Application of hydromorphone hydrochloride in the postoperative analgesia in elderly patients with THR has a preferable analgesia effect, can effectively alleviate the postoperative stress reaction, stabilize the hemorheology, and contribute to the rehabilitation.
