The Preventive Effect of Comprehensive Nursing Intervention on Arteriovenous Fistula Failure in Patients in the Hemodialysis Room

Objective:To explore the preventive effect of comprehensive nursing intervention on arteriovenous fistula failure in patients in the hemodialysis room.Methods:82 patients with arteriovenous fistula in the hemodialysis chamber treated in our hospital from July 2022 to October 2023 were selected as the research subjects.The random number table method divided them into a control group and an experimental group of 41 cases each.The control group received general nursing intervention measures,while the experimental group underwent comprehensive nursing intervention.The incidence of complications(internal fistula failure,bleeding,thrombosis,infection),psychological emotions(SAS scale,SDS scale),quality of life(physical pain,physiological function,emotional function,social function),and nursing satisfaction(very satisfied,basically satisfied,satisfied,dissatisfied)were compared between the two groups of patients.Results:The incidence of complications in the experimental group(4,9.76%)was significantly lower than that in the control group(29,70.73%);the SAS scores and SDS scores of the patients in the experimental group after intervention were both lower than those in the control group;the quality of life score(physical pain,physiological function,emotional function,and social function)was all higher than those of the control group;the post-intervention nursing satisfaction of the experimental group(40,97.57%)was also significantly higher than that of the control group(29,73.17%);and the listed differences were statistically significant(P<0.01).Conclusion:For patients with hemodialysis ventricular arteriovenous fistula failure,comprehensive nursing intervention can reduce the incidence of complications,relieve anxiety and stress,improve quality of life and nursing satisfaction,and achieve better preventive effects.

Exploring the spatial arrangement of patient rooms for minimum nurse travel m hospital nursing units in Korea

With in creasing dema nds on medical care services,one of the trends is the mixed patient room arrangement of single/double-bed and multi-bed rooms in a nursing unit on the same floor.This influences nurse-to-patient assignment and often causes an unbalanced workload and longer travel distances for nurses.The objective of this study was to investigate how floor configuration and room density influence nurse travel in the hospitafs medical surgical urdts in Korea.This study presented a novel approach to measure nurse travel distances in eight existingn ursing units.The agent-based simulation was con ducted to model nurses'walking trails,and the distanee of one nurse travel to assigned patient rooms was measured for each nurse.With revisions in the spatial arrangement of patient rooms,locating multibed rooms near the nurse station,symmetric room layout centering the nurse station,and planning both single/double-bed and multi-bed rooms on one side of corridors,nurse travel distanee decreased more than 15%.This study contributed to the knowledge of agent-based simulation as an evaluation framework for spatial analysis.Apart from application to Korea,these results are particularly of interest in countries where private patient rooms are not commonly economically feasible.

Assessment of the early effectiveness of a stroke unit in comparison to the general ward

Background Stroke unit is the most effective treatment method to benefit stroke patients. Our study is to evaluate the early effectiveness of a hospital stroke unit (SU). Methods Three hundred and ninety-two patients who had suffered from acute strokes and who were admitted to our hospital between December 2001 and January 2003 were recruited for this controlled study. All patients were sent at random to either the SU or the general ward (GW) for treatment. The following indices were measured by: Barthel Index (BI),National Institute of Health Stroke Scale (NIHSS),Oxford Handicap Scale (OHS). Results The mean change in BI score between the day of admission and the day of discharge was 20.00±24.36 for the SU group and 10.63±23.59 for the GW group. A difference that is statistically significant ( P =0.000). The mean change in NIHSS score was -2.01±6.61 for the SU group and 0.55±7.44 for the GW group. A difference that is also statistically significant ( P =0.000). Finally,the mean change in OHS score was -0.74±1.04 for the SU group and -0.28±0.98 for the GW group,also a statistically significant difference ( P =0.000). Among SU patients,patient satisfaction was higher ( P =0.000),the rehabilitation success rate was higher ( P =0.000),and there were fewer complications ( P =0.000).Conclusion Compared to GW patients,stroke patients treated in a special SU were able to return to normal daily activities earlier,with better social abilities,and have reduced neurological defects,without increasing the overall economic burden.
