A Retrospective Analysis of Glucagon-Like Peptide 1 Receptor Agonists in Treating Type 2 Diabetes Mellitus Complicated by Nonalcoholic Fatty Liver Disease

Background: The objective of this study was to compare and analyze the variations in clinical indices before and after treatment of type 2 mellitus (T2DM) combined with nonalcoholic fatty liver disease (NAFLD) that were treated with glucagon-like peptide 1 receptor agonists (GLP-1RAs). Methods: The electronic medical record system was utilized to search for a total of 16 patients with type 2 diabetes complicated by NAFLD who were hospitalized at the First Affiliated Hospital of Yangtze University from October 2022 to April 2023 and treated with GLP-1RA for the first time. The clinical indices were compared before and after 12 weeks of treatment with GLP-1RA. Results: The liver-spleen CT ratio (L/S), alanine aminotransferase (ALT), gamma-glutamyltransferase (GGT), total cholesterol (TC), triglyceride (TG), and low-density lipoprotein cholesterol (LDL-C) in all patients treated with GLP-1RA after 12 weeks were significantly different (P 0.05). The patients were categorized into two groups based on the types of GLP-1RAs. The changes in L/S, TC, TG, and LDL-C in the long-acting group after treatment were statistically significant (P Conclusions: GLP-1RAs can improve liver function, regulate lipid metabolism, and reduce the severity of fatty liver in patients with T2DM complicated by NAFLD, which demonstrates the importance of clinical applications. 展开更多

Glucagon-like peptide-1(GLP-1)receptor agonists result in greater improvements in glycemic control than placebo and promote weight loss with minimal hypoglycemia in patients with type 2 diabetes mellitus.A number of case reports show an association of GLP-1receptor agonists,mainly exenatide,with the development of acute kidney injury.The present review aims to present the available data regarding the effects of GLP-1 receptor agonists on renal function,their use in subjects with chronic renal failure and their possible association with acute kidney injury.Based on the current evidence,exenatide is eliminated by renal mechanisms and should not be given in patients with severe renal impairment or end stage renal disease.Liraglutide is not eliminated by renal or hepatic mechanisms,but it should be used with caution since there are only limited data in patients with renal or hepatic impairment.There is evidence from animal studies that GLP-1 receptor agonists exert protective role in diabetic nephropathy with mechanisms that seem to be independent of their glucose-lowering effect.Additionally,there is evidence that GLP-1 receptor agonists influence water and electrolyte balance.These effects may represent new ways to improve or even prevent diabetic nephropathy. 展开更多

Glucagon- like peptide-1 receptor agonists (GLP-1RAs) are a new class of incretin-based anti-diabetic drugs. They function to stimulate insulin secretion while suppressing glucagon secretion. GLP-1-based therapies are now well established in the management of type 2 diabetes mellitus (T2DM), and recent literature has suggested potential applications of these drugs in the treatment of obesity and for protection against cardiovascular and neurological diseases. As we know, along with change in lifestyles, the prevalence of non-alcoholic fatty liver disease (NAFLD) in China is rising more than that of viral hepatitis and alcoholic fatty liver disease, and NAFLD has become the most common chronic liver disease in recent years. Recent studies further suggest that GLP-1RAs can reduce transaminase levels to improve NAFLD by improving blood lipid levels, cutting down the fat content to promote fat redistribution, directly decreasing fatty degeneration of the liver, reducing the degree of liver fibrosis and improving inflammation. Cardiovascular death is the leading cause of mortality for patients with type 2 diabetes mellitus. The etiologyof cardiovascular disease in diabetes may be divided into hyperglycemia per se and factors operating through components of metabolic syndrome(Met S). Hyperglycemia causes direct injury to vascular endothelium and possibly on cardiac myocytes. Met S is a cluster of risk factors like obesity, hyperglycemia, hypertension and dyslipidemia. The incidence of this syndrome is rising globally. Glucagon-like peptide-1 receptor agonists(GLP-1RA) are a group of drugs, which address all components of this syndrome favorably. Experimental evidence suggests that they have favorable actions on myocardium as well. Several compounds belonging to GLP-1RA class are in market now and a large number awaiting their entry. Although, originally this class of drugs emerged as a treatment for type 2 diabetes mellitus, more recent data generated revealed beneficial effects on multiple metabolic parameters. We have studied literature published between 2000 and 2016 to look into effects of GLP-1RA on components of Met S. Results from recently concluded clinical trials suggest that some of the molecules in this class may have favorable effects on cardiovascular outcome. 展开更多

Non-alcoholic fatty liver disease(NAFLD) is a common liver disease and it represents the hepatic manifestation of metabolic syndrome, which includes type 2 diabetes mellitus(T2DM), dyslipidemia, central obesity and hypertension. Glucagon-like peptide-1(GLP-1) analogues and dipeptidyl peptidase-4(DPP-4) inhibitors were widely used to treat T2 DM. These agents improve glycemic control, promote weight loss and improve lipid metabolism. Recent studies have demonstrated that the GLP-1 receptor(GLP-1R) is present and functional in human and rat hepatocytes. In this review, we present data from animal researches and human clinical studies that showed GLP-1 analogues and DPP-4 inhibitors can decrease hepatic triglyceride(TG) content and improve hepatic steatosis, although some effects could be a result of improvements in metabolic parameters. Multiple hepatocyte signal transduction pathways and m RNA from key enzymes in fatty acid metabolism appear to be activated by GLP-1 and its analogues. Thus, the data support the need for more rigorous prospective clinical trials to further investigate the potential of incretin therapies to treat patients with NAFLD. 展开更多

Both GLP-1 receptor agonists(GLP-1RA)and SGLT-2 inhibitors(SGLT-2I)are newer classes of anti-diabetic agents that lower HbA1c moderately and decrease body weight and systolic blood pressure(SBP)modestly.Combination therapy with GLP-1RA plus SGLT-2I have shown a greater reduction in HbA1c,body weight,and SBP compared to either agent alone without any significant increase in hypoglycemia or other side effects.Since several agents from each class of these drugs have shown an improvement in cardiovascular(CV)and renal outcomes in their respective cardiovascular outcome trials(CVOT),combination therapy is theoretically expected to have additional CV and renal benefits.In this comprehensive opinion review,we found HbA1c lowering with GLP-1RA plus SGLT-2I to be less than additive compared to the sum of HbA1c lowering with either agent alone,although body weight lowering was nearly additive and the SBP lowering was more than additive.Our additional meta-analysis of CV outcomes with GLP1RA plus SGLT-2I combination therapy from the pooled data of five CVOT found a similar reduction in three-point major adverse cardiovascular events compared to GLP-1RA or SGLT-2I alone,against placebo.Interestingly,a greater benefit in reduction of heart failure hospitalization with GLP-1RA plus SGLT-2I combination therapy was noted in the pooled meta-analysis of two randomized controlled trials.Future adequately powered trials can confirm whether additional CV or renal benefit is truly exerted by GLP-1RA plus SGLT-2I combination therapy. 展开更多

GLP-1 receptor agonists are approved for the treatment of type 2 diabetes, and more recently for obesity treatment. The glucagon-like-peptide-1 (GLP-1) is a glucose dependent hormone produced by intestinal cells, which is involved in insulin secretion and glucagon suppression. This hormone controls glucose plasma levels and reduces food intake. Additional effects were reported in slowing gastric emptying and in inducing satiety. In clinical practice, theGLP-1 receptor agonists are associated with significant reductions in glycosylated hemoglobin (HbA1c) and weight loss, despite showing a low risk of hypoglycemia. Beneficial effects have also been observed on blood pressure and lipid profile. The most common side effects associated with GLP-1 receptor agonists are gastro-intestinal motility disorders, such as nausea, vomiting and diarrhea, which are not associated with long-term health risks. Therefore, GLP-1 receptor agonists represent a relevant medication for type 2 diabetes, whose benefits may go far beyond glycemic control. 展开更多

In order to provide the structure information for designing new exendin-4 analogues, a phage display peptide library was screened by targeting the N-terminal extracellular domain of GLP-1R(nGLP-1R). After four rounds of selection, nine sequences were obtained, four of them have higher affinity for nGLP-1R than the others. We chose two of them named X and Y peptides. Islet β-cell proliferation assay suggested that X and Y peptides didn't have any activity to increase islet β-cell proliferation. In other words, X and Y peptides were not agonists to GLP-1R. However, the conservative motifs of X and Y peptides provided us useful information to design new exendin-4 analogues. 展开更多

Efficacy and Safety of Basal-Supported Prandial GLP-1 Receptor Agonist Therapy

Aim: To assess the safety and efficacy of basal-supported prandial GLP-1 receptor agonist therapy (BPT)* in type 2 diabetes mellitus (T2DM). Methods: Patients with T2DM, who had previously received insulin injection therapy and who had had their treatment switched to BPT (liraglutide), were retrospectively recruited. The efficacy of BPT was assessed by determining changes in HbA1c, body weight and total daily insulin dose from baseline to 4 months after BPT initiation. Safety was assessed by comparing the frequency of hypoglycemic episodes at baseline and after 4 months. The Wilcoxon test was used to analyze changes in parameters throughout the study period. Results: Twenty-nine patients, previously treated with basal-supported oral therapy (BOT), basal-bolus insulin, or pre-mixed insulin, were recruited. When analyzed together, there was no change in HbA1c throughout the study period, but body weight decreased (baseline 68.8 ± 13.2 kg vs. month 4 67.3 ± 13.1 kg;p < 0.001). Total daily insulin dose decreased after 4 months (baseline 24.4 ± 15.5 U/day vs. month 4 14.7 ± 9.2 U/day;p < 0.001), and there was no change in the frequency of hypoglycemic episodes. Analysis was conducted within sub-groups based on previous treatment modality. In the BOT group, HbA1c decreased from baseline after 2 months and body weight did not change throughout the study period. In both the basal-bolus insulin group and the pre-mixed insulin group, HbA1c remained steady throughout and there was a decrease in body weight. No change in the frequency of hypoglycemia was observed in any of the sub-groups. Conclusion: BPT in T2DM was associated with weight loss without changes in glycemic control over 4 months, suggesting that it may be an effective and safe therapy. 展开更多

目的探讨胰高血糖素样肽-1受体激动剂(GLP-1RA)对糖尿病肾损伤保护的相关机制。方法选用小鼠肾小球系膜细胞(MCs)为体外研究对象,并分为对照组(C组)、高糖高脂组(GP组)和GLP-1RA exendin-4处理组(EX-GP组)。采用CCK-8检测细胞存活率;采... 目的探讨胰高血糖素样肽-1受体激动剂(GLP-1RA)对糖尿病肾损伤保护的相关机制。方法选用小鼠肾小球系膜细胞(MCs)为体外研究对象,并分为对照组(C组)、高糖高脂组(GP组)和GLP-1RA exendin-4处理组(EX-GP组)。采用CCK-8检测细胞存活率;采用转录组测序(RNA-seq)对细胞进行转录组测序;采用qRT-PCR测定磷酸果糖激酶(Pfk)、柠檬酸合酶(Cs)、α-酮戊二酸脱氢酶(Ogdh)、微管相关蛋白轻链3(LC3)、重组人自噬效应蛋白(Beclin-1)、线粒体DNA(mtDNA)、沉默信息调节因子1(Sirt-1)和过氧化物酶体增殖物激活受体辅助活化因子1α(Pgc-1α)的基因表达。结果在高糖高脂诱导的肾小球MCs损伤模型中,GP组MCs存活率降低,EX-GP组存活率恢复。RNA-seq结果显示,3组间的基因表达有明显差异。通过京都基因与基因组百科全书(KEGG)富集分析,基因差异表达通路涉及细胞脂肪酸代谢过程、mTOR信号通路、自噬等。GP组抑制了Pfk表达,EX-GP组可部分恢复其表达(P<0.05)。exendin-4处理后,mtDNA、LC3和Beclin-1基因表达均增加(P<0.05)。与GP组比较,EX-GP组的Sirt-1和Pgc-1α基因表达升高(P<0.05)。结论RNA-seq及能量代谢相关基因研究提示,GLP-1RA exendin-4改善高糖高脂诱导的MCs损伤机制可能涉及能量代谢相关通路以及Sirt-1、Pgc-1α和自噬、糖酵解基因的表达。 展开更多

目的探讨胰高糖素样肽1受体激动剂(GLP-1RA)与二肽基肽酶4抑制剂(DPP-4i)治疗2型糖尿病(T2MD)的疗效及对患者并发症的影响。方法选取2020年6月至2022年6月邯郸市第一医院收治的110例T2MD随机分为GLP-1RA组和DPP-4i组,每组55例,GLP-1RA... 目的探讨胰高糖素样肽1受体激动剂(GLP-1RA)与二肽基肽酶4抑制剂(DPP-4i)治疗2型糖尿病(T2MD)的疗效及对患者并发症的影响。方法选取2020年6月至2022年6月邯郸市第一医院收治的110例T2MD随机分为GLP-1RA组和DPP-4i组,每组55例,GLP-1RA组采用利拉鲁肽或艾塞那肽治疗,DPP-4i组采用西格列汀或利格列汀治疗。对比2组临床疗效,治疗前后糖脂代谢指标[空腹血糖(FPG)、糖化血红蛋白(HbA1c)、总胆固醇、三酰甘油]、炎症指标[白介素-6(IL-6)、C反应蛋白(CRP)、中性粒细胞/淋巴细胞(NLR)]、肾功能[尿素氮、肌酐、胱抑素C];观察并统计2组并发症及不良反应。结果2组总有效率、并发症总发生率比较差异均无统计学意义(P>0.05)。治疗18周后,2组FPG、HbA1c、三酰甘油、总胆固醇水平低于治疗前,且GLP-1RA组低于DPP-4i组(P<0.05)。治疗18周后,2组IL-6、CRP、NLR水平低于治疗前(P<0.05),但2组间差异无统计学意义(P>0.05)。2组治疗前和治疗18周后尿素氮、肌酐、胱抑素C水平比较差异均无统计学意义(P>0.05)。GLP-1RA组不良反应总发生率高于DPP-4i组(P<0.05)。结论GLP-1RA与DPP-4i均能改善T2MD患者糖脂水平,减轻炎性反应,保护肾功能,预防并发症发生,但GLP-1RA在控制血糖、调脂方面优于DPP-4i,而DPP-4i耐受性更好。 展开更多

目的评估5种胰高血糖素样肽-1受体激动剂(GLP-1RAs)治疗二甲双胍控制血糖不佳的2型糖尿病(T2DM)的长期经济性。方法提取既往发表的荟萃分析及其纳入的随机对照研究(RCT)中患者的基线数据,使用英国前瞻性糖尿病研究结果模型2.1预测各组... 目的评估5种胰高血糖素样肽-1受体激动剂(GLP-1RAs)治疗二甲双胍控制血糖不佳的2型糖尿病(T2DM)的长期经济性。方法提取既往发表的荟萃分析及其纳入的随机对照研究(RCT)中患者的基线数据,使用英国前瞻性糖尿病研究结果模型2.1预测各组患者的生存情况、长期疗效和成本,采用成本-效用分析法比较5种GLP-1RAs(利拉鲁肽、利司那肽、艾塞那肽、度拉糖肽和司美格鲁肽)的经济性;采用敏感性分析和情境分析验证基础分析结果的稳定性。结果共纳入21项RCT,6796名患者。生存曲线表明,司美格鲁肽在降低因心血管疾病死亡风险上、度拉糖肽在降低全因死亡风险上较其他GLP-1RAs具有优势。成本-效用分析结果显示,5种方案的经济性从优到劣排序依次为利司那肽、司美格鲁肽、艾塞那肽、度拉糖肽和利拉鲁肽。单因素敏感性分析和概率敏感性分析表明基础分析结果稳健。情境分析结果显示,司美格鲁肽的价格至少降低54.64%,降至369.21元,其对比利司那肽才具有经济性。结论对于使用二甲双胍治疗后血糖控制不佳的我国T2DM患者,临床可考虑优先选择利司那肽和司美格鲁肽。 展开更多

目的研究胰高血糖素样肽-1受体激动剂(glucagon-like peptide-1 receptor agonists,GLP-1 RA)对2型糖尿病(type 2 diabetes mellitus,T2DM)患者急性缺血性脑卒中后认知功能的影响。方法采用病例对照研究,将249例急性缺血性脑卒中合并T2D... 目的研究胰高血糖素样肽-1受体激动剂(glucagon-like peptide-1 receptor agonists,GLP-1 RA)对2型糖尿病(type 2 diabetes mellitus,T2DM)患者急性缺血性脑卒中后认知功能的影响。方法采用病例对照研究,将249例急性缺血性脑卒中合并T2DM患者根据既往是否规律使用GLP-1 RA(选取司美格鲁肽为代表)分为GLP-1 RA组和非GLP-1 RA组(胰岛素降糖治疗),记录基线状态下的人口学特征、生化检验结果以及在院期间血糖监测水平等,并使用蒙特利尔认知评估量表(Montreal Cognitive Assessment,MoCA)评估认知功能,排除得分<26分的患者。两组患者的基线水平资料差异无统计学意义。筛选合格者219例,分别于卒中后3个月随访,再次使用MoCA评分评估认知功能,以得分<22分判定为发生卒中后认知障碍(post-stroke cognitive impairment,PSCI)。采用二元Logistic回归分析比较两组患者的评分情况,比较不同得分患者组间踝肱指数及衍生葡萄糖目标范围内时间(derived time in range,dTIR)的差异。结果研究结束时,GLP-1 RA组110例患者中有32例发生PSCI,PSCI发生率为29.09%;非GLP-1 RA组109例患者中有45例发生PSCI,PSCI发生率为41.28%。对两组患者进行认知功能评分,结果显示司美格鲁肽干预显著降低PSCI发生率。二元Logistic回归分析提示,司美格鲁肽干预的优势比为2.285,dTIR参数的优势比为0.985。结论GLP-1 RA降糖治疗能够减轻缺血性脑卒中合并T2DM患者的PSCI程度。 展开更多

目的探究胰高血糖素样肽-1受体激动剂(glucagon-like peptide-1 receptor agonist,GLP-1RA)联合钠-葡萄糖协同转运蛋白2抑制剂(sodium-glucose transporter 2 inhibitors,SGLT2i)治疗超重/肥胖2型糖尿病(type2 diabetes mellitus,T2DM)... 目的探究胰高血糖素样肽-1受体激动剂(glucagon-like peptide-1 receptor agonist,GLP-1RA)联合钠-葡萄糖协同转运蛋白2抑制剂(sodium-glucose transporter 2 inhibitors,SGLT2i)治疗超重/肥胖2型糖尿病(type2 diabetes mellitus,T2DM)患者的效果及安全性。方法非随机选取2020年3月—2022年5月肇庆市第二人民医院收治的62例超重/肥胖T2DM患者作为研究对象,按照治疗方法的不同分成两组,各31例。对照组接受SGLT2i治疗,研究组接受GLP-1RA联合SGLT2i治疗,对比两组血糖指标、体重指数、治疗效果、不良反应发生情况。结果研究组的血糖水平和体重指数均低于对照组,差异有统计学意义(P均<0.05)。研究组治疗有效率为96.8%(30/31),优于对照组的77.4%(24/31),差异有统计学意义(χ^(2)=4.897,P<0.05)。研究组的不良反应总发生率与对照组对比,差异无统计学意义(P>0.05)。结论超重/肥胖T2DM患者应用GLP-1RA联合SGLT2i治疗,可以大幅度降低机体的血糖水平与体重指数,且不增加不良反应,治疗效果显著。 展开更多
