Application Value of Lung-Fire-Clearing,Phlegm-Resolving,and Bowels-Relaxing Decoction in Treating Lung Cancer of Phlegm-Heat Pattern

Objective:This article explores the clinical effects of lung-fire-clearing,phlegm-resolving and bowels-relaxing decoction in the treatment of lung cancer of phlegm-heat pattern.Methods:A total of 6 cases of lung cancer patients with phlegm-heat pattern were randomly selected from Inner Mongolia Baicaotang Qin's Zhong Meng Medical Hospital from March 2018 to December 2020 to conduct the study.They were divided into the reference group and the study group by using the digital table method.The patients in the reference group were treated with conventional Western medicine whereas the patients in the study group were treated with lung-fire-clearing,phlegm-resolving,and bowels-relaxing decoction to observe the curative effect.Results:There were no significant differences in the levels of the tumor markers between the two groups before treatment(P>0.05).However,after treatment,the levels of cytokeratin 19 soluble fragments(CYFRA21-1),carbohydrate antigen 125(CA125),and carcinoembryonic antigen(CEA)in the study group were lower than those in the control group(P<0.05).The effective rate and the quality of life score of the study group were higher than those of the reference group while the incidence of adverse reactions was lower than that of the reference group,P<0.05.Conclusion:Lung-fire-clearnig,phlegm-resolving,and bowels-relaxing decoction can effectively improve the symptoms of patients with lung cancer and improve their quality of life.

自拟方清肺化痰汤治疗小儿风热闭肺型肺炎喘嗽临床观察

目的观察自拟方清肺化痰汤治疗小儿肺炎喘嗽风热闭肺证的临床疗效。方法选取我院2021年8月—2022年8月收治的肺炎喘嗽风热闭肺证60例,采用随机数字表法分为观察组30例,对照组30例。对照组行常规抗感染及对症治疗,观察组在对照组基础治疗上加用自拟方清肺化痰汤,比较两组患儿治疗总有效率,症状消失时间(发热、咳喘、肺部啰音)、血常规(白细胞)、炎症因子(C反应蛋白水平、降钙素原)。结果观察组治疗有效率高于对照组(P<0.05),观察组症状消失时间均短于对照组(P<0.01),观察组治疗后红细胞、白细胞、C反应蛋白水平以及降钙素原均低于对照组(P<0.01)。结论自拟方清肺化痰汤治疗小儿肺炎喘嗽,可以快速改善患儿相关症状,改善炎症,促进患儿迅速康复。

平喘煎联合特布他林雾化吸入对婴幼儿喘息性支气管炎急性发作期的影响

目的:观察自拟平喘煎联合特布他林雾化吸入治疗婴幼儿喘息性支气管炎急性发作期(外寒内热、痰热壅肺证)的临床效果,探讨其对外周血白细胞介素17(interleukin-17,IL-17)、可溶性细胞间黏附分子1(soluble intercellular adhesion molecule-1,sICAM-1)、干扰素γ(interferonγ,IFN-γ)水平的影响。方法:将喘息性支气管炎急性发作期(外寒内热、痰热壅肺证)婴幼儿100例按照随机数字表法分为对照组和观察组各50例。对照组患儿给予特布他林雾化吸入,观察组患者在对照组基础上用自拟平喘煎治疗,连续治疗4周后观察两组患儿喘息、咳嗽、肺部喘鸣音等主要临床症状消失时间,中医证候积分及血清IL-17、sICAM-1、IFN-γ水平变化情况。结果:有效率对照组为78.0%(39/50),低于观察组的92.0%(46/50)(P<0.05);观察组治疗后喘息、咳嗽、肺部喘鸣音等主要临床症状消失时间较对照组短(P<0.05);观察组治疗后呼吸道症状中医证候积分低于对照组(P<0.05);肺功能指标最大呼气峰流速(peak expiratory flow rate,PEF)、肺活量(forced vital capacity,FVC)、第一秒用力呼气量(forced expiratory volume in one second,FEV1)、FEV1/FVC、呼吸系统静态顺应性(Crs SO)水平高于对照组,呼吸系统阻力(Rrs SO)较对照组降低(P<0.05);血清IL-17、sICAM-1水平低于对照组,IFN-γ水平高于对照组(P<0.05)。结论:自拟平喘煎联合特布他林雾化吸入有利于减轻喘息性支气管炎急性发作期患儿的临床症状,促进症状消失,改善肺功能,其机制可能在于通过调控血清IL-17、sICAM-1、IFN-γ水平减轻炎性反应所致的气道损伤,调节Thl/Th2免疫平衡。
