In a double-blind randomized controlled trial, 112 patients with hemiplegia after stroke were randomly divided into two groups: experimental and control. In experimental group, the patients received LF r TMS and VR training treatment, and those in control group received sham r TMS and VR training treatment. Participants in both groups received therapy of 6 days per week for 4 weeks. The primary endpoint including the upper limb motor function test of Fugl-meyer assessment(U-FMA) and wolf motor function test(WMFT), and the secondary endpoint including modified Barthel index(MBI) and 36-item Short Form Health Survey Questionnaire(SF-36) were assessed before and 4 weeks after treatment. Totally, 108 subjects completed the study(55 in experimental group and 53 in control group respectively). After 4-week treatment, the U-FMA scores [mean difference of 13.2, 95% confidence interval(CI) 3.6 to 22.7, P〈0.01], WMFT scores(mean difference of 2.9, 95% CI 2.7 to 12.3, P〈0.01), and MBI scores(mean difference 16.1, 95% CI 3.8 to 9.4, P〈0.05) were significantly increased in the experimental group as compared with the control group. The results suggested the combined use of LF r TMS with VR training could effectively improve the upper limb function, the living activity, and the quality of life in patients with hemiplegia following subacute stroke, which may provide a better rehabilitation treatment for subacute stroke.展开更多

An iterative learning controller, together with a state observer and a fully-saturated learning mechanism, through Lyapunov-like synthesis, is designed to deal with time-varying parametric uncertainties. The estimations for outputs, instead of system outputs themselves, are applied to form the error equation, which helps to establish convergence of the system outputs to the desired ones. This method is then extended to repetitive learning controller design. The boundedness of all the signals in the closed-loop is guaranteed and asymptotic convergence of both the state estimation error and the tracking error is established in both cases of ILC and RLC. Numerical results are presented to verify the effectiveness of the proposed methods.展开更多

The parameters of the fuzzy rules are tuned with adaptive schemes. To attenuate chattering effectively, the discontinuous control term is approximated by an adaptive PI control structure. The bound of the discontinuous control term is assumed to be unknown and estimated by an adaptive mechanism. Based on the Lyapunov stability theory, an adaptive repetitive control law is proposed to guarantee the closed-loop stability and the tracking performance. By means of FBFNs, which avoid the nonlinear parameterization from entering into the adaptive repetitive control, the controller singularity problem is solved. Periodic disturbances are common within repetitive systems and can be represented with a time-delay model. The proposed design focuses on isolating the disturbance model and finding the overall transfer function around the delay model. The use of the small gain theorem around the delay model assures disturbance accommodation if stability conditions are achieved. This paper reintroduces the designed RC controller within the state feedback in the presence of both past error and current error structures. Robustness conditions are investigated and set to enhance system performance in the presence of modelling mismatch, which represents the novel contribution in this paper. 文摘[Objectives]To explore the clinical treatment effect of repetitive transcranial magnetic stimulation(rTMS)combined with repetitive magnetic stimulation at Taichong acupoint(LR3)therapy in the treatment of depression.[Methods]60 patients who met the inclusion standard were chosen as the research subjects. They were split into two sets using a randomized parallel control method, with 30 cases each in the experimental and control groups. The experimental group adopted the treatment with rTMS+repetitive magnetic stimulation at Taichong acupoint(LR3),and the control group adopted the treatment with rTMS alone. The course of treatment was 8 weeks. Then the clinical efficacy,Hamilton Depression Scale 17 item score(HAMD-17),Symptom Check List-90(SCL-90),Patient Health Questionnaire-15(PHQ-15)and Pittsburgh Sleep Quality Index(PSQI)were compared.[Results]After treatment, the scores of HAMD-17,SCL-90,PHQ-15 and PSQI in the two groups were lower than those before and 4 weeks after treatment(P<0.05 or P<0.01),and the scores in the experimental group were inferior to those in the control group(P<0.01),The effective rate was 96.66%in the experimental group and 76.47%in the control group(P<0.05).In the course of treatment,no serious adverse reactions happened in either group.[Conclusions]The rTMS combined with repetitive magnetic stimulation at Taichong acupoint(LR3)can enhance the control of depression in the right frontal lobe, significantly improve the symptoms of depression and somatic discomfort, and improve the sleep quality of patients. It has a wide range of application, and is safe and noninvasive,so it is worthy of clinical promotion.
