Existing studies have focused on the behavior of the retaining wall equipped with expanded polystyrene(EPS)geofoam inclusions under semi-infinite surcharge loading rather than limited surcharge loading.In this paper,the failure mode and the earth pressure acting on the rigid retaining wall with EPS geofoam inclusions and granular backfills(henceforth referred to as EPS-wall),under limited surcharge loading are investigated through two-and three-dimensional model tests.The testing results show that different from the sliding of almost all the backfill in the EPS-wall under semi-infinite surcharge loading,only an approximately triangular backfill slides in the wall under limited surcharge loading.The distribution of the lateral earth pressure on the EPS-wall under limited surcharge loading is non-linear,and the distribution changes from the increase of the wall depth to the decrease with the increase of the limited surcharge loading.An approach based on the force equilibrium of a differential element is developed to predict the lateral earth pressure behind the EPS-wall subjected to limited surcharge loading,and its performance was fully validated by the three-dimensional model tests. 展开更多

Introduction: Total hip arthroplasty of complex morphology is a challenge for the orthopaedic surgeon. Careful analysis of the hip’s anatomy helps to unravel the difficulties and anticipate the procedures to be performed and the implants to be planned. The aim was to identify the types of hip that make first-line THR difficult, specify the technical procedures to be used on these sites, and assess the functional results of the series. Material and Methods: This was a retrospective study that concerned patients operated on for total hip arthroplasty between January 2015 and December 2022 at the medical center “La Grâce” in Bobo-Dioulasso, Burkina Faso. Patients with coxarthrosis (on hip dysplasia, acetabular protrusio, acetabular malunion or neurological hip) and those with ankylosis of the hip, osteonecrosis secondary to neglected dislocation of the hip or hemoglobinopathy were included. Results: A total of 31 total hip replacements were performed in 30 patients. The mean age of patients at the surgery time was 36.2 years with extremes of 17 and 61 years. The male-to-female sex ratio was 1. The main indications for THA were the dysplasic hip osteoarthritis (11 cases) and the neglected hip dislocations (7 cases). In situ femoral neck osteotomy before hip dislocation was performed in seven cases. The acetabulum reconstruction techniques varied from the structural iliac bone graft (n = 3) and cancellous bone graft (n = 4) to the Kerboull plate (n = 1). After 45 months of mean follow-up, all hips were evaluated. The mean PMA score increased from 7.1 [4 - 8] before the surgery to 13.2 [13 - 17]. Conclusion: The large spectrum of challenges in complex hip management requires effective preoperative planning. Preoperative planning minimizes complications and ensures a better outcome. 展开更多

Three-dimensional analysis of age and sex differences in femoral head asphericity in asymptomatic hips in the United States

BACKGROUND The sphericity of the femoral head is a metric used to evaluate hip pathologies and is associated with the development of osteoarthritis and femoral-acetabular impingement.AIM To analyze the three-dimensional asphericity of the femoral head of asymptomatic pediatric hips.We hypothesized that femoral head asphericity will vary significantly between male and female pediatric hips and increase with age in both sexes.METHODS Computed tomography scans were obtained on 158 children and adolescents from a single institution in the United States(8-18 years;50%male)without hip pain.Proximal femoral measurements including the femoral head diameter,femoral head volume,residual volume,asphericity index,and local diameter difference were used to evaluate femoral head sphericity.RESULTS In both sexes,the residual volume increased by age(P<0.05).Despite significantly smaller femoral head size in older ages(>13 years)in females,there were no sex-differences in residual volume and aspherity index.There were no age-related changes in mean diameter difference in both sexes(P=0.07)with no significant sex-differences across different age groups(P=0.06).In contrast,there were significant increases in local aspherity(maximum diameter difference)across whole surface of the femoral head and all quadrants except the inferior regions in males(P=0.03).There were no sex-differences in maximum diameter difference at any regions and age group(P>0.05).Increased alpha angle was only correlated to increased mean diameter difference across overall surface of the femoral head(P=0.024).CONCLUSION There is a substantial localized asphericity in asymptomatic hips which increases with age in.While 2D measured alpha angle can capture overall asphericity of the femoral head,it may not be sensitive enough to represent regional asphericity patterns. 展开更多

The catalytic conversion of polystyrene (PS) was studied in the presence of the materials type HZSM-5, CeO<sub>2</sub>, 10% CeO<sub>2</sub>/HZSM-5 and 20% CeO<sub>2</sub>/HZSM-5, which were characterized by scanning electron microscopy (SEM), X-ray diffraction (XRD) and nitrogen adsorption at 77K. The catalytic tests were performed via thermogravimetric analysis (TG) at heating rates of 5, 10 and 20˚C min<sup>−1</sup> in a temperature range from 30˚C to 900˚C. For the tests, a ratio of 30% by mass of each catalytic material mixed with PS was used and the activation energy of the degradation process was determined by the Vyazovkin method. The obtained results showed that the addition of the catalyst to the PS in general reduced its degradation temperature. The 10% CeO<sub>2</sub>/HZSM-5 catalyst showed greater efficiency, as it resulted in lower activation energy for PS degradation. Thus, the combination of CeO<sub>2</sub> with HZSM-5 resulted in materials with potential for application in the catalytic degradation of polystyrene and the results indicate that the production of a composite material can be a good strategy to generate an increase in catalytic activity and a decrease in energy process activation. 展开更多

Managing agricultural waste and expanded polystyrene (EPS) poses significant environmental and economic challenges. This study aims to create composites from millet husks, rice husks, and recycled EPS, using a manufacturing method that involves dissolving the polystyrene in a solvent followed by cold pressing. Various particle sizes and two binder dosages were investigated to assess their influence on the physico-mechanical properties of the composites. The mechanical properties obtained range from 2.54 to 4.47 MPa for the Modulus of Rupture (MOR) and from 686 to 1400 MPa for the Modulus of Elasticity in Bending (MOE). The results indicate that these composites have potential for applications in the construction sector, particularly for wood structures and interior decoration. Moreover, surface treatments could enhance their durability and mechanical properties. This research contributes to the valorization of agricultural and plastic waste as eco-friendly and economical construction materials. 展开更多

Electrospinning is a simple technique used to fabricate polymeric nano-fibrous membranes.These nano-fibers have found a wide range of valuable applications in the biomedical field.However,it has not been utilized with solid high explosives yet.Herein,the electrospinning technique has been used to fabricate polystyrene(PS)/1,3,5-trinitro-1,3,5-triazinane(RDX)composite nanofibers.The governed electrospinning parameters,voltage,distance from the collector,flow rate,mandrel rotating speed,time,and solution concentration,that greatly affect the morphology of the obtained nanofibers were optimized.The fabricated PS/RDX nano-fibers were characterized using scanning electron microscopy(SEM),X-ray diffractometer(XRD),and Fourier Transform Infrared(FTIR)spectroscopy.The impact and friction sensitivities of PS/RDX were also measured.The thermal behavior of the prepared composite and the pure materials were studied by the thermal gravimetric analysis technique(TGA).SEM results proved the fabrication of PS/RDX fibers in the nano-size via electrospinning.FTIR spectroscopy confirmed the existence of the characteristic functional groups of both PS and RDX in the composite nano-fibers.XRD sharp peaks showed the conversion of amorphous PS into crystalline shape via electrospinning and also confirmed the formation of PS/RDX composite.The PS fibers absorbed the heat and increased the onset decomposition of the pure RDX from 181.5 to 200.7℃in the case of PS/RDX fibers.Interestingly,PS/RDX nano-fibers showed the relatively low impact and friction sensitivities of 100 J and 360 N respectively.These results could introduce PS/RDX nanofibrous composite in the field of explosives detection with high levels of safety. 展开更多

BACKGROUND Sodium polystyrene sulfonate(SPS)is commonly prescribed for the management of hyperkalemia,a critical electrolyte imbalance contributing to over 800000 annual visits to emergency departments.AIM To conduct a systematic review of documented cases of SPS-induced colitis and assess its associated prognosis.METHODS Following the PRISMA-P guidelines,our study employed Medical Subject Headings and Health Sciences Descriptors,skillfully combined using Boolean operators,to conduct comprehensive searches across various electronic databases,including Scopus,Web of Science,MEDLINE(PubMed),BIREME(Biblioteca Regional de Medicina),LILACS(Latin American and Caribbean Health Sciences Literature),SciELO(Scientific Electronic Library Online),Embase,and Opengray.eu.Language criteria were confined to English,Spanish,and Portuguese,with no limitations on the publication date.Additionally,we manually scrutinized the reference lists of retrieved studies.To present our findings,we utilized simple descriptive analysis.RESULTS Our search strategy yielded a total of 442 references.After rigorous evaluation,we included 51 references,encompassing 59 documented cases of colitis.Predominant clinical presentations included abdominal pain,observed in 35(60.3%)cases,and bloating,reported in 18(31%)cases.The most frequently affected sites of inflammation were the cecum,rectum,and small intestine,accounting for 31%,25.8%,and 22.4%of cases,respectively.Colonoscopy findings were described in 28(48.2%)cases,and 29(50%)of patients required surgical intervention.Among the subset of patients for whom outcome data was available,39(67.2%)experienced favorable outcomes,while 12(20.6%)unfortunately succumbed to the condition.The mean time required for resolution was 36.7 d,with a range spanning from 1 to 120 d.CONCLUSION SPS demonstrates the capacity to effectively lower serum potassium levels within 24 h.However,this benefit is not without the risk of bowel injury.Our study highlights the absence of high-quality data pertaining to the incidence of adverse events associated with SPS usage,making it challenging to determine whether the potential risks outweigh the benefits.However,a significant mortality rate related to SPS-induced colitis was noted.Future investigations should prioritize randomized controlled trials with a sufficiently large patient cohort to ascertain the true utility and safety profile of this medication. 展开更多

目的对比小剂量艾司氯胺酮和舒芬太尼在髋关节置换术(HA)患者围术期镇痛中的应用效果。方法将我院行HA的124例患者按照随机数字表法分为对照组和观察组,每组62例。对照组患者使用舒芬太尼进行镇痛;观察组患者使用小剂量艾司氯胺酮镇痛... 目的对比小剂量艾司氯胺酮和舒芬太尼在髋关节置换术(HA)患者围术期镇痛中的应用效果。方法将我院行HA的124例患者按照随机数字表法分为对照组和观察组,每组62例。对照组患者使用舒芬太尼进行镇痛;观察组患者使用小剂量艾司氯胺酮镇痛。观察2组患者麻醉前(T1)、手术切皮时(T2)、麻醉30 min后(T3)及手术结束时(T4)的心率(HR)、呼吸频率(RR)、平均动脉压(MAP)、血氧饱和度(SpO2)等血流动力学变化;于术前(T5)、术后6 h(T6)、术后12 h(T7)及术后24 h(T8)记录患者静息状态及活动状态下疼痛视觉模拟量表(visual analogue scale,VAS)评分;统计2组患者术后镇痛泵按压次数,需补充镇痛药物情况,术中及术后24 h不良反应发生情况。结果2组患者T1时HR、MAP及RR水平比较,差异无统计学意义(P>0.05);观察组患者T2、T3、T4时HR、MAP及RR水平均高于对照组(P<0.05);2组患者各时间段SpO2水平比较差异无统计学意义(P>0.05)。2组患者T5时静息状态及活动状态VAS评分比较差异无统计学意义(P>0.05);观察组患者T6、T7、T8时静息状态及活动状态VAS评分均低于对照组(P<0.05)。观察组术后镇痛泵按压次数及需补充镇痛药物患者比例均少/低于对照组(P<0.05)。2组术中及术后不良反应发生率比较,差异无统计学意义(P>0.05)。结论与舒芬太尼相比,小剂量艾司氯胺酮应用于HA患者的镇痛效果更好,患者围术期疼痛程度更轻,术中血流动力学更稳定,安全性较好。 展开更多

目的探讨骨转换与老年女性髋部脆性骨折(fragile hip fracture,HF)风险及与高密度脂蛋白胆固醇(HDLc)的相关性。方法选取2021年1月至2022年12月年龄≥60岁女性住院骨质疏松症患者共661例,分为无骨折组(A组)266例和HF组(B组)395例。检测... 目的探讨骨转换与老年女性髋部脆性骨折(fragile hip fracture,HF)风险及与高密度脂蛋白胆固醇(HDLc)的相关性。方法选取2021年1月至2022年12月年龄≥60岁女性住院骨质疏松症患者共661例,分为无骨折组(A组)266例和HF组(B组)395例。检测总胆固醇(Tch)、甘油三酯(TG)、HDLc、低密度脂蛋白胆固醇(LDLc)、骨钙素N段中分子片段(N-MID OC)、Ⅰ型胶原羧基端肽β特殊序列(β-CTX)、总蛋白(TP)、白蛋白(Alb)、钙(Ca)、磷(P)、镁(Mg)、尿素(Urea)、肌酐(Cr)、胱抑素C(Cys-C)、肾小球滤过率(eGFR)、骨形成率(MoMF)、骨吸收率(MoMR)及骨转化率(MoT)。结果(1)HF组TP[118(108,128)vs 124(114,131)]、Alb[38.4±3.7 vs 40.0±4.3]、HDLc[1.51(1.24,1.71)vs 1.54(1.37,1.73)]、Ca[2.21±0.12 vs 2.29±0.12]、P[1.03±0.16 vs 1.08±0.17]含量较无骨折组降低,年龄[76(68,83)vs 73(67,78)]、β-CTX[0.57(0.40,0.77)vs 0.34(0.19,0.56)]、MoMR[1.84(1.30,2.51)vs 1.09(0.61,1.83)]及MoT[2.16(1.56,2.85)vs 1.56(1.09,2.38)]较无骨折组增加,差异有统计学意义(P<0.05)。N-MID OC、MoMF、Urea、Cr、Cys-C、eGFR、Tch、TG、LDLc及Mg差异无统计学意义(P>0.05)。(2)老年女性HF与HDLc、TP、ALB、Ca、P、Mg呈负相关(r=-0.09、-0.26、-0.16、-0.28、-0.12、-0.03,P<0.05),与年龄、β-CTX、MoMR、MoT呈正相关(r=0.17、0.36、0.36、0.26,P<0.05),与N-MID OC、MoMF、Urea、Cr、Cys-C、eGFR、Tch、TG、LDLc无相关性(P>0.05)。(3)年龄、MoT、TP、Ca、P为老年女性HF的风险因素,OR分别为1.03(95%CI:1.01~1.06)、1.75(95%CI:1.44~2.12)、0.96(95%CI:0.92~0.99)、0.06(95%CI:0.01~0.37)、0.18(95%CI:0.06~0.6),P<0.05。(4)HDLc与骨转化间存在“U”型关系,P<0.01,其切点为1.692 mmol/L。结论高骨转换是老年女性HF的危险因素,其骨转化率与HDLc存在U型相关。 展开更多

目的:探析腰硬联合麻醉在老年髋部骨折手术患者中的应用。方法:选取2020年11月—2021年11月江门市新会区人民医院麻醉科收治的80例老年髋部骨折手术患者作为研究对象。根据随机法将其分为常规组和试验组,各40例。常规组给予连续硬膜外麻... 目的:探析腰硬联合麻醉在老年髋部骨折手术患者中的应用。方法:选取2020年11月—2021年11月江门市新会区人民医院麻醉科收治的80例老年髋部骨折手术患者作为研究对象。根据随机法将其分为常规组和试验组,各40例。常规组给予连续硬膜外麻醉,试验组给予腰硬联合麻醉。比较两组入室时(T_(0))、手术开始后10 min(T_(1))、手术开始后30 min(T_(2))、手术结束时(T_(3))的心率、收缩压及舒张压。比较两组术前、术后12 h应激指标,麻醉效果,麻醉优良率,术后10 min,术后1 h、3 h、6 h、12 h疼痛情况,不良反应。结果:T_(1),试验组心率、舒张压均显著高于常规组,T_(2)、T_(3),试验组心率、舒张压及收缩压均显著高于常规组,差异有统计学意义(P<0.05)。术后12 h,试验组肾上腺素(AD)、去甲肾上腺素(NE)、C反应蛋白(CRP)水平均显著低于常规组,差异有统计学意义(P<0.05)。试验组麻醉起效时间、感觉阻滞完善时间均明显短于常规组,差异有统计学意义(P<0.05)。试验组优良率显著高于常规组,差异有统计学意义(P<0.05)。术后1 h、3 h、6 h、12 h,试验组视觉模拟评分法(VAS)评分低于常规组,差异有统计学意义(P<0.05)。试验组不良反应发生率显著低于常规组,差异有统计学意义(P<0.05)。结论:老年髋部骨折手术患者采用腰硬联合麻醉,其麻醉起效时间短,镇痛效果佳,可减轻患者的应激反应和术后疼痛,且不增加麻醉相关不良反应,可取得安全高效的麻醉效果。 展开更多

目的:探讨全髋关节置换术的CroweⅢ-Ⅳ型发育性髋关节发育不良(developmental dysplasia of the hip,DDH)患者的满意度及造成不满意的相关因素。方法:回顾性分析2013年3月至2018年3月行全髋关节置换术的169例CroweⅢ-Ⅳ型DDH患者,通过... 目的:探讨全髋关节置换术的CroweⅢ-Ⅳ型发育性髋关节发育不良(developmental dysplasia of the hip,DDH)患者的满意度及造成不满意的相关因素。方法:回顾性分析2013年3月至2018年3月行全髋关节置换术的169例CroweⅢ-Ⅳ型DDH患者,通过微信进行调查问卷,调查患者对手术总体满意度、10项日常功能满意度和患者认为对自己日常生活影响比较大的前5个问题。手术前后采用髋关节Harris评分进行功能评价。结果:收到完整调查问卷145份,所有患者获随访,时间1~5(3.23±1.22)年。145例患者分成两组,其中对手术疗效满意的118例,不满意的27例,手术总体满意率81.38%(118/145)。患者认为对生活影响比较大的前5个问题分别是术后髋部疼痛,肢体明显不等长、行走、上下楼梯、蹲起。两组术前Harris评分比较,差异无统计学意义(P>0.05),不满意组术后Harris评分较低。术后髋关节疼痛、肢体不等长是影响手术不满意的直接因素。结论:采用全髋关节置换术治疗CroweⅢ-Ⅳ型DDH患者手术难度大;术后髋关节疼痛(轻度以上),肢体不等长(>2 cm)是术后不满意的独立危险因素。 展开更多

背景:人工股骨头置换是治疗高龄不稳定性股骨转子间骨折的有效方法,但是对于Evans-Ⅲ型股骨转子间骨折,人工股骨头置换术中同期行小转子重建与否对治疗效果的影响未见报道。目的:分析小转子重建对高龄Evans-Ⅲ型股骨转子间骨折患者长柄... 背景:人工股骨头置换是治疗高龄不稳定性股骨转子间骨折的有效方法,但是对于Evans-Ⅲ型股骨转子间骨折,人工股骨头置换术中同期行小转子重建与否对治疗效果的影响未见报道。目的:分析小转子重建对高龄Evans-Ⅲ型股骨转子间骨折患者长柄人工股骨头置换治疗效果的影响。方法:回顾性分析西安交通大学第二附属医院骨关节外科2017年6月至2021年5月因Evans-Ⅲ型股骨转子间骨折行双极长柄人工股骨头置换的45例高龄患者的病历资料,根据术中小转子是否重建(复位并固定)分为2组,小转子重建组25例,小转子非重建组20例。比较两组患者的手术时间、出血量、下床时间、住院时间、术后3,6个月的Harris评分以及随访期内的并发症发生情况。结果与结论:①小转子重建组需要的手术时间(99.72±13.41)min较非重建组(88.90±16.53)min更长(t=2.369,P=0.023),二者在出血量、下床时间和住院时间方面差异无显著性意义(P>0.05);②术后3个月时,重建组的Harris评分(69.06±5.64)分高于非重建组(63.35±5.93)分(t=2.982,P=0.005);术后6个月时,重建组的Harris评分(86.67±4.49)分也高于非重建组(82.34±5.68)分(t=2.782,P=0.009);③此外重建组和非重建组在随访期内的并发症发生率未见显著差异(χ2=0.008,P=0.927);④提示对于高龄Evans-Ⅲ型股骨转子间骨折患者,在人工股骨头置换过程中行小转子重建虽然延长了手术时间,但是明显改善了患者术后早期的髋关节功能,初步说明小转子重建在高龄Evans-Ⅲ型转子间骨折人工股骨头置换过程中的重要性。 展开更多
