Multifractal Measure of Post Distribution in Post System

In order to investigate the true post distribution in the whole society, microelasticity （MIE） and macroelasticity （MAE） were defined by regarding all posts as a system. On this basis, the method for measuring post distribution was proposed. Using the Legendre dual transformation between MIE and MAE to highlight the probabilities of different levels, the post distribution were analyzed hierarchically. The two-scale Cantor model verified that the multifractal measure is applicable to the post distribution evolution process.

Advances in Research of Post Embolism Syndrome after Transarterial Chemoembolization(TACE)for Hepatocellular Carcinoma

This article reviews the concept and clinical manifestations of post embolism syndrome after transarterial chemoembolization(TACE),and the prevention or timely intervention of post embolism syndrome in advance is expected to reduce its incidence and degree in clinical treatment,and to improve the quality of treatment of Hepatocellular Carcinoma Carcinoma(HCC).

Postoperative Complications in the General Surgery Department of the Cs Ref of Commune I of the District of Bamako Mali

Postoperative complications represent important indicators for the quality of surgical care. The objectives of this work were to study post-operative complications in the general surgery department of the Cs ref of commune I in Mali, to determine the frequency of post-operative complications, to identify the factors of occurrence, to describe the clinical aspects, diagnostic and therapeutic in order to assess the additional cost linked to post-operative complications. This study was prospective, carried out in the general surgery department on 300 cases of surgical interventions. It covered all patients aged at least 15 years, operated on and hospitalized or not, and who presented complications during the 30 days postoperatively. Patients under the age of 15 were not included (our general surgery department is not a pediatric surgery department). We collected 300 patients among whom 199 (66.33%) were men and 101 (33.66%) women, i.e. a sex ratio = 1.9. The average age was 38 years with extremes of 15 and 87 years. The main initial diagnoses were: acute appendicitis, peritonitis, occlusions, wall hernias, hemorrhoids, uterine prolapse, uterine myomas, ovarian cysts and acute cholecystitis. Emergencies represented 43% (N = 129) of interventions with 6.98% post-operative complications (POC). Postoperative complications were dominated by surgical site infections, 75% of cases (N = 20), wall hemorrhage 5% (N = 1), testicular necrosis 5% (N = 1) and testicular calcification 5% (N = 1), wire rejection 5% (N = 1) and death 5% (N = 1). The management of postoperative complications was surgical in 95% and medical in 100%. Their occurrence extended the hospital stay by 3.65 days and increased the average cost of care by 60541.85 CFA francs. The mortality index lowered by efficient management of complications (IMAGE) calculated in relation to deaths was 95% of cases. Anemia, ASA score ≥ III, Alteimeir II and IV classes, duration of surgery and post-operative hospitalization were factors contributing to post-operative complications.

One solution to ensure data security is encryption, where keys are central. There is therefore a need to find robusts key generation implementation that is effective, inexpensive and non-invasive for protecting and preventing data counterfeiting. In this paper, we use the theory of electromagnetic wave propagation to generate encryption keys.

Research Progress in the Treatment of Poststroke Depression with Traditional Chinese Medicine

In recent years,the incidence rate of stroke has increased year by year.Post stroke depres‐sion is one of its main complications,which seriously affects the recovery of physiological functions and quality of life of stroke patients.Traditional Chinese medicine has signifi‐cant therapeutic effects in treating this disease.This article provides a classification and review of traditional Chinese medicine treatment methods for post-stroke depression,and looks forward to the current problems.

The Model of“Post,Course,Competition,Certificate”Integrated Education for Electronic Information Engineering Technology Major in Higher Vocational Colleges

A Computation on Velocity Field of the Post-flare Loop System of August 17, 1989

The observational line of sight velocity fields of a large post flare loop(PFL) system on August 17,1989 were derived based on “multi cloud model” (MCM) method from the Hβ spectral data obtained with the two dimensional Multi Waveband Spectra Spectro Helio Graph (MW SSHG) at Yunnan Observatory. In order to explain the fundamental features of the velocity fields observed,an assumption and approximation to be taken into account that the matter within a loop system rises up from the loop foot to the loop top along the loop leg in a helical magnetic line under the combined action of solar gravity, magnetic stress and a gradient of atmospheric pressure in the loop system, and a theoretical velocity field of the loop system was calcuated with the aid of MHD theory. From a comparison of both velocity fields, it is found that the computed velocity field is basically similar with the observed one. It indicates that the assumption and approximation mentioned above are suitable basically.

STUDY ON THE POST-STALL BEHAVIOR OF AN AXIAL-FLOW COMPRESSION SYSTEM

Post stall behaviors of a single stage compression system are studied theoretically and experimentally in this paper. A one dimensional nonlinear model, which is able to describe the dynamically post stall behaviors of the compression system, is applied to simulate the post stall behaviors digitally. The stall types, i.e. , rotating stall and surge, are determined. The variations of annular average parameters while the compression system goes into stall are also calculated exactly. The post stall behaviors are measured on the single stage compressor test rig. The measurement shows that rotating stall and surge appear under different conditions. On the basis of experiments, it is found that the post stall behaviors are influenced remarkably by some factors, such as rotation speeds, construction type and size of the exhaust duct. Good agreement between the simulation and experiments proves that this modeling technique is valid for simulating the post stall behaviors.

Hazard analysis system of urban post-earthquake fire based on GIS

The authors study the structure, functions and data organization for the hazard analysis system of urban post-earthquake fire on the platform of GIS. A general hazard analysis model of the post-earthquake fire is presented. Taking Shanghai central district as background, a system for hazard analysis of the post-earthquake fire and auxiliary decision-against fire is developed.

Stress Distribution in Maxillary Central Incisors without Ferrules: A Finite Element Analysis of Post and Core Systems

Purpose: The purpose of this study was to identify optimal post and core materials for central incisors without ferrules using three-dimensional finite element analysis and three-point bending tests. Methods: Stress analyses were performed with six models: cast metal post and core (MP), composite resin core alone, straight fiber-reinforced post-composite resin core (FSR), tapered fiber-reinforced post-composite resin core, straight titanium post-composite resin core (TSR), and tapered titanium post-composite resin core (TTR). A 100-N load was applied to the lingual surface at a 45° angle to the long axis of the tooth. Maximum von Mises stress distributions were calculated with finite element analysis software. Five samples each of composite resin, straight fiber-reinforced post, straight titanium post, straight fiber-reinforced post and composite resin, and straight titanium post and composite resin were subjected to three-point bending tests, followed by analysis of variance and Tukey’s multiple comparison test. Results: Stress distribution was optimal on TTR. Maximum von Mises stress on the cervical side of the post was greatest in TSR (693 MPa) and TTR (556 MPa). Maximum stress on the apical side of the post was greatest in MP (110 MPa). Maximum stress in surrounding dentin was lowest in MP (203 MPa) and TTR (250 MPa). Gap distance was smallest in MP (0.09 mm) and largest in FSR (0.26 mm). Mean maximum three-point bending force was lowest in composite resin (26.9 N/mm) and highest in titanium post and composite resin (97.1 N/mm). Titanium post bending strength was consistently greater than that of the fiber-reinforced post (p < 0.01). Conclusion: These results revealed optimal stress distribution and high bending strength with the tapered titanium post and resin combination, suggesting that this combination can most effectively prevent root or post fracture in an anterior tooth without a ferrule.

New Statistical Methods for Systematizing the Nuclei Fission Fragments: Post-Scission Approach

A new statistical method for systematizing the nuclei fission fragments, investigation of their mass and charge spectra and neutron fission parameters has been suggested. In proposed method, the mass and charge yields of nucleons are determined from the equilibrium conditions of the nuclear fragments post-fission ensemble. Here we consider protons and neutrons of different fragments as statistically non-equivalent. The abilities of the proposed statistical method for explanation of the U, Pa, Th, Ac and Ra post-fission fragments ordering and neutron emission function calculation have been demonstrated.

The contribution of steel wallposts to out-of-plane behavior of non-structural masonry walls

For years,non-structural masonry walls have received little attention by code developers and professional engineers.Recently,significant efforts have been made to shed more light on out-of-plane(OOP)behavior of non-structural masonry walls.In updated provisions of the Iranian seismic code,bed joint reinforcements(BJRs)and steel wallposts have been suggested for use.BJRs are horizontal reinforcements;steel wallposts are vertical truss-like elements intended to provide additional OOP restraints for a wall.The contribution of BJRs has previously been investigated by the authors.This study is devoted to investigating the contribution of steel wallposts to the OOP behavior of non-structural masonry walls.Using pre-validated 3D finite element(FE)models,the OOP behavior of 180 non-structural masonry walls with varying configurations and details are investigated.The OOP pressure-displacement curve,ultimate strength,the response modification factor,and the cracking pattern are among the results presented in this study.It is found that steel wallposts,especially those with higher rigidity,can improve the OOP strength of the walls.The contribution of wallposts in the case of shorter length walls and walls with an opening are more pronounced.Results also indicate that masonry walls with wallpost generally have smaller modification factors compared to similar walls without wallpost.

Community Perspectives on the Use of Recycled Plastic Posts in Mitigating Livestock Predation in Amboseli Ecosystem, Kenya

In the last few decades, the impacts of human activities on ecosystem have rapidly increased resulting to detrimental ecosystem changes. Human-wildlife conflict (HWC) is one of the greatest consequences of human impacts on the environment. Although HWC has been there for decades, its intensity seems to be growing with the spread of human settlements, changes in land use patterns and diminishing suitable natural habitats for wildlife. To mitigate HWC, various strategies have been devised and implemented. This study focused on predator-proof bomas (PPBs) approach that is ongoing in Amboseli ecosystem. The aim was to establish reasons behind the slow adoptions of the recycled plastics poles by owners of PPBs that were constructed using wooden posts between the years 2010 to 2013. Randomly selected 36 PPB homestead owners were interviewed and the physical structures of their PPBs assessed. Results revealed a positive relationship between PPB size and number of poles damaged;and a significant association between the damaged posts and the number of indigenous posts replaced in the three group ranches (X2 = 34.9331, df = 2, p < 0.05). Most of the respondents (89%) cited posts and doors (72%) as the main parts that needed repairs. About 80% of the respondents would recommend the plastic posts for use in constructing new PPBs by other people. Most of the PPB beneficiaries whose PPBs were constructed in 2010-2013 have opted for an “easy” alternative source of posts from the sparsely distrusted trees around their homesteads. It is recommend that an open window period should be tried to entice the wooden post PPBs owners to adopt the recycled plastic posts. This may necessitate upgrading the existing old wooden bomas to a “smart PPBs”. It is suggested that the “smart PPBs” should not only have recycled plastic posts but also “free” energy saving stoves in each household to reduce the level of deforestation.

Emerging role of the gut microbiome in post-infectious irritable bowel syndrome:A literature review

Post-infectious irritable bowel syndrome(PI-IBS)is a particular type of IBS,with symptom onset after an acute episode of infectious gastroenteritis.Despite infectious disease resolution and clearance of the inciting pathogen agent,10%of patients will develop PI-IBS.In susceptible individuals,the exposure to pathogenic organisms leads to a marked shift in the gut microbiota with prolonged changes in host-microbiota interactions.These changes can affect the gut-brain axis and the visceral sensitivity,disrupting the intestinal barrier,altering neuromuscular function,triggering persistent low inflammation,and sustaining the onset of IBS symptoms.There is no specific treatment strategy for PI-IBS.Different drug classes can be used to treat PI-IBS similar to patients with IBS in general,guided by their clinical symptoms.This review summarizes the current evidence for microbial dysbiosis in PI-IBS and analyzes the available data regarding the role of the microbiome in mediating the central and peripheral dysfunctions that lead to IBS symptoms.It also discusses the current state of evidence on therapies targeting the microbiome in the management of PI-IBS.The results of microbial modulation strategies used in relieving IBS symptomatology are encouraging.Several studies on PI-IBS animal models reported promising results.However,published data that describe the efficacy and safety of microbial targeted therapy in PI-IBS patients are scarce.Future research is required.

Emerging peril of post-dengue mucormycosis:A case report

Rationale:Dengue fever is a leading cause of death in tropical and subtropical countries.Although most patients have a self-limited febrile illness,the viral infection can induce virus-mediated host changes,making immunocompetent persons susceptible to deadly fungal infections.However,there are only a few reports of such an association.Here we present a case of this deadly co-infection.Patient’s Concern:A 17-year-old male patient was diagnosed with dengue fever.He presented to us with facial swelling,periorbital edema,and black discoloration over the palate during the second week of his illness.Diagnosis:Diagnostic tests confirmed the presence of fungal hyphae.A diagnosis of post-dengue mucormycosis was made.No other comorbidity or underlying immune deficit was detected.Interventions:The patient underwent surgical debridement and antifungal treatment.Outcomes:The patient recovered and showed signs of palatal healing with an advancing mucosal edge.Lessons:Dengue virus and mucor co-infection has brought to light a new pathogenic paradigm.Clinicians need to be aware of this emerging medical condition and maintain a high index of suspicion for mucor co-infections while treating dengue patients.

Motor cortex transcranial magnetic stimulation to reduce intractable postherpetic neuralgia with poor response to other threapies:Report of two cases

BACKGROUND Postherpetic neuralgia(PHN)is a typical neuropathic pain condition that appears in the lesioned skin regions following the healing of shingles.The pain condition tends to persist,which is often accompanied by negative emotions(e.g.,anxiety and depression)and substantially reduces the quality of life.In addition to analgesia(e.g.,pregabalin and gabapentin),nerve radiofrequency technology is an effective treatment for intractable PHN.However,there is still a significant portion of patients who do not benefit from this treatment.As a non-invasive form of brain stimulation,repetitive transcranial magnetic stimulation(rTMS)targeting the motor cortex is able to reduce neuropathic pain with grade A evidence.CASE SUMMARY Here we report two cases in which motor cortex rTMS was used to treat intractable PHN that did not respond to initial drug and radiofrequency therapies.Moreover,we specifically investigated rTMS efficacy at 3 mo following treatment.CONCLUSION Motor cortex rTMS can treat intractable PHN that did not respond to initial drug and radiofrequency therapies.

Cardiovascular sequelae in post-COVID-19 patients with moderate to severe CT severity score: A follow-up study

Objective:To study cardiovascular sequelae of post-COVID-19 patients with moderate to severe computed tomography(CT)severity score.Methods:A prospective,non-randomized,observational study was conducted on 100 post-COVID-19 patients with moderate to severe CT severity scores from January 2021 to December 2021.Fifty-nine were male[mean age(54.1±12.2)years]and 41 were female[mean age(46.9±15.1)years].Patients with previous cardiovascular disease,previous chronic lung disease,and pre-existing primary or secondary pulmonary hypertension were excluded.Patients were examined,and serial electrocardiogram and 2D echocardiography were performed to detect any cardiovascular abnormality.Results:Post-COVID-19 patients had persistent symptoms,the most common being fatigue(59%).Most of these symptoms were relieved on follow-up.A rise in systolic,diastolic blood pressure,and pulse rate was observed.The electrocardiographic evaluation revealed ST-T segment changes,sinus tachycardia,ventricular hypertrophy,and arrhythmias among a considerable number of patients.On echocardiography,left ventricular diastolic dysfunction was most common(43%).Pulmonary hypertension,as evidenced by elevated pulmonary artery systolic pressure,was seen in 15%of patients.Conclusions:The present findings reveal an increased incidence of cardiovascular complications after recovery from COVID-19 infection in those without pre-existing cardiovascular or chronic lung disease.

Intraoperative pancreas stump perfusion assessment during pancreaticoduodenectomy:A systematic scoping review

BACKGROUND Post-operative pancreatic fistula(POPF)is the primary cause of morbidity following pancreaticoduodenectomy.Rates of POPF have remained high despite well known risk factors.The theory that hypoperfusion of the pancreatic stump leads to anastomotic failure has recently gained interest.AIM To define the published literature with regards to intraoperative pancreas perfusion assessment and its correlation with POPF.METHODS A systematic search of available literature was performed in November 2022.Data extracted included study characteristics,method of assessment of pancreas stump perfusion,POPF and other post-pancreatic surgery specific complications.RESULTS Five eligible studies comprised two prospective non-randomised studies and three case reports,total 156 patients.Four studies used indocyanine green fluorescence angiography to assess the pancreatic stump,with the remaining study assessing pancreas perfusion by visual inspection of arterial bleeding of the pancreatic stump.There was significant heterogeneity in the definition of POPF.Studies had a combined POPF rate of 12%;intraoperative perfusion assessment revealed hypoperfusion was present in 39%of patients who developed POPF.The rate of POPF was 11%in patients with no evidence of hypoperfusion and 13%in those with evidence of hypoperfusion,suggesting that not all hypoperfusion gives rise to POPF and further analysis is required to analyse if there is a clinically relevant cut off.Significant variance in practice was seen in the pancreatic stump management once hypoperfusion was identified.CONCLUSION The current published evidence around pancreas perfusion during pancreaticoduodenectomy is of poor quality.It does not support a causative link between hypoperfusion and POPF.Further well-designed prospective studies are required to investigate this.

Advances in post intensive care unit care: A narrative review

As the treatment options,modalities and technology has grown,mortality in intensive care unit(ICU)has been on the decline.More and more patients are being discharged to wards and in the care of their loved ones after at times prolonged treatment,sometimes in isolation.These survivors have a lower life expectancy and a poorer quality of life.They can have substantial familial financial implications and an economic impact on the healthcare system in terms of increased and continued utilisation of services,the so-called post intensive care syndrome(PICS).But it is not only the patient who is the sufferer.The mental health of the loved ones and family members may also be affected,which is termed as PICS-family.In this review,we shall be reviewing the definition,epidemiology,clinical features,diagnosis and evaluation,treatment and follow up of PICS.We shall also focus on measures to prevent,rehabilitate and understand the ICU stay from patients’perspective on how to redesign the ICU,post ICU care needs for a better patient outcome.
