Effect of proximal femur nail anti-rotation on unstable intertrochanteric fractures: A prospective observational study 被引量:2

Objective:To assess the effect of proximal femur nail anti-rotation on the functional and radiological outcome of unstable intertrochanteric fractures.Methods:This prospective observational study was conducted in the orthopedic department of a tertiary care hospital.Altogether 86 patients with unstable intertrochanteric fractures treated with proximal femur nail anti-rotation between January 2010 and January 2015 were included.Patients were followed in the outpatient clinic at regular intervals after discharge to assess the radiological union of fractures and complications.The functional outcomes were evaluated after 2 years by Harris hip score.Results:All patients achieved a radiological union of fractures after a mean duration of 24.6 weeks.The follow up showed 23 complications(systemic and local).Eight patients developed urinary tract infections,and three patients developed chest infections,two patients had screw cut-out,one patient had knee stiffness,one patient developed superficial surgical site infection,and four patients developed varus collapse and shortening subsequently.The two year follow up showed that 69(80.2%)patients had an excellent and good functional outcome according to Harris hip score.Conclusions:With lower complication rates,proximal femur nail is a valid and reasonable option especially in treating unstable intertrochanteric fractures. 展开更多

With the aging population,intertrochanteric femur fracture in the elderly has become one of the most serious public health issues and a hot topic of research in trauma orthopedics.Due to the limitations of internal fixation techniques and the insufficient mechanical design of nails,the occurrence of complications delays patient recovery after surgical treatment.Design of a proximal femur bionic nail(PFBN)based on Zhang’s N triangle theory provides triangular supporting fixation,which dramatically decreases the occurrence of complications and has been widely used for clinical treatment of unstable intertrochanteric femur fracture worldwide.In this work,we developed an equivalent biomechanical model to analyze improvement in bone remodeling of unstable intertrochanteric femur fracture through PFBN use.The results show that compared with proximal femoral nail antirotation(PFNA)and InterTan,PFBN can dramatically decrease the maximum strain in the proximal femur.Based on Frost’s mechanostat theory,the local mechanical environment in the proximal femur can be regulated into the medium overload region by using a PFBN,which may render the proximal femur in a state of physiological overload,favoring post-operative recovery of intertrochanteric femur fracture in the elderly.This work shows that PFBN may constitute a panacea for unstable intertrochanteric femur fracture and provides insights into improving methods of internal fixation. 展开更多

Objective: To evaluate the clinical and functional outcomes of modular endoprosthetic replacement (EPR) compared to proximal femur intramedullary nailing (IMN) for the treatment of proximal femur metastases. Methods: We retrospectively studied the records of patients with proximal femur metastatic lesions treated with surgical stabilization between January 2007 and December 2014 in terms of operation time, blood loss, postoperative score, soreness, Karnofsky performance score (KPS) and survival time. Results: There were 34 patients treated with surgical stabilization. The mean follow-up period was 12.1~8.6 months (range: 10-47 months). Thirteen were treated with EPR and 21 were stabilized with IMN (20 males, 14 females; mean age: 68.7 years). The median survival time was 11.0 months for both groups (P=0.147). The operation time, blood loss and Harris score of IMN group were lower than those of EPR group (P=0.001, P=0.001, P=0.002, respectively). Conclusions: Both EPR and IMN for treating proximal femur metastasis achieved effective clinical outcomes. Therefore, the suitable surgical methods depended on the general conditions and medical requirements of patients, as well as the technical advantages of the doctor. 展开更多

Objective:To compare the outcomes of dynamic hip screws(DHS)and intramedullary nailing(IMN)in the treatment of extra-capsular metastatic carcinoma of the proximal femur.Methods:A retrospective case analysis method was used to examine data of patients with proximal metastatic cancer of the femur who were treated with internal fixation in Department of Orthopaedics,Beijing Friendship Hospital,from January 2007 to December 2018.Blood loss,postoperative pain,functional score,length of stay,and survival rates were compared,and postoperative complications were assessed.Results:Complete follow-up data were available for 33 patients.The mean follow-up period was 12.2±3.6(range:9-32)months and the average age was 72.3±4.7(range:59-83)years old.There were 20 females and 13 males.Twenty-three patients had undergone IMN and 10 DHS,according to bone defects and the patient’s overall condition.The median survival time was 10 months in the IMN group and 11 months in the DHS group.Duration of surgery(t=-7.366,P<0.001)and length of hospital stay(t=-3.509,P<0.001)differed significantly between the two groups.There was one case of breakage of internal fixation in the IMN group.Conclusions:There was no significant difference between DHS and IMN in terms of surgical efficacy.IMN and DHS were different in terms of surgical time and hospital stay.However,due to the limited number of cases in this study,multi-factor analysis has not been performed and needs to be further verified in future analysis.When developing a surgical plan,it is recommended to consider the patient’s condition and the surgeon’s experience. 展开更多

Intertrochanteric fractures have become a severe public health problem in elderly patients.Proximal femoral nail anti-rotation(PFNA)is a commonly used intramedullary fixation device for unstable intertrochanteric fractures.Pelvic perforation by cephalic screw is a rare complication.We reported an 84-year-old female who fell at home and sustained an intertrochanteric fracture.The patient underwent surgery with PFNA as the intramedullary fixation device.Routine postoperative examination revealed medial migration of the helical blade that eventually caused pelvic perforation.We performed a cemented total hip arthroplasty as the savage procedure.At the latest follow-up of 12 months after total hip arthroplasty,the patient had no pain or loosening of the prosthesis in the left hip.Pelvic perforation should be considered when choosing PFNA as the intramedullary fixation device,especially in patients with severe osteoporosis wherein the helical blade can be easily inserted during the operation.The lack of devices to avoid oversliding of the helical blade in PFNA is an unreported cause of this complication and should be considered in such cases. 展开更多

BACKGROUND Proximal femur fractures,including both intracapsular(femoral neck fractures)and extracapsular fractures(intertrochanteric femoral fractures,IFFs),affect around 1.5 million people per year worldwide.Mechanical failures of intertrochanteric nailing in IFFs could be managed with revision total hip arthroplasty(THA).AIM To describe the surgical complexity and the procedure-related complication rates in patients with trochanteric nailing failure and treated with THA.METHODS Patients referred to our level I trauma center between April 2012 and July 2018 with failed cephalomedullary nailing following trochanteric fractures were retrospectively recruited.All patients underwent a salvage surgical procedure,i.e.,cephalomedullary nail removal and conversion to THA.The same surgical and anesthesiology team performed the surgical procedures under spinal anesthesia.All patients underwent clinical and radiographic follow-ups for at least 24 mo.Complications and re-operations were recorded.RESULTS Seventy-four patients met the inclusion criteria(male:29;female:45;mean age:73.8-years-old;range:65-89)and were included in the current study.The average operative time was 117 min(76-192 min).The average blood loss was 585 mL(430-1720 mL).Among the 74 patients,43(58.1%)required transfusion of three or more blood units.Two patients died within the 4th d after surgery because of pulmonary embolism,and 1 patient died 9 mo after surgery due to ischemic myocardial infarction.The complication rate in the 71 patients who completed the minimum 24-mo follow-up was 22.5%.In 3 cases out of 71(4.2%)periprosthetic acetabular fracture was observed during the followup.One of these periacetabular fractures occurred intraoperatively.An intraoperative periprosthetic femur fracture was observed in 5 patients out of 71(7.0%).Four of these patients needed a re-operation to fix the fracture with plates and cerclages;in one of these patients,femoral stem revision was also necessary.In 4 patients out of 71(5.6%),an early THA dislocation was observed,whereas in 1 case(1.4%)a late THA dislocation was observed.Three patients out of 71(4.2%)developed a periprosthetic joint infection during the study follow-up.CONCLUSION The present study demonstrated that salvage options for IFF fixation failure are complex procedures with a relevant intraoperative and postoperative complication rate. 展开更多

背景:股骨转子间骨折好发于老年人,手术后会出现大量的隐性失血,减少隐性失血可以降低并发症和住院时间。目的:评估延长氨甲环酸的使用时间对股骨近端防旋髓内钉治疗转子间骨折围术期隐性失血的影响。方法:选择2022年1月至2023年5月自... 背景:股骨转子间骨折好发于老年人,手术后会出现大量的隐性失血,减少隐性失血可以降低并发症和住院时间。目的:评估延长氨甲环酸的使用时间对股骨近端防旋髓内钉治疗转子间骨折围术期隐性失血的影响。方法:选择2022年1月至2023年5月自贡市第四人民医院经急诊收治入院的老年股骨转子间骨折患者共62例,均在牵引床上闭合牵引复位后置入股骨近端防旋髓内钉治疗。根据氨甲环酸的使用时间分为2组,对照组38例在切开皮肤前15-30 min给予氨甲环酸1 g静脉滴注,3 h后追加1 g;试验组24例在对照组的基础上,术后第1天再追加氨甲环酸1 g静脉滴注每12 h一次。术前、术后当天、术后第1,3,5天均复查血常规,统计血红蛋白量和红细胞压积,理论总失血量采用Gross方程计算,同时记录两组患者并发症发生情况。结果与结论:①通过统计学分析,两组患者术中显性出血量相差较小,差异无显著性意义(P>0.05);②试验组围术期血红蛋白下降量、总失血量、隐性失血量低于对照组,差异有显著性意义(P<0.05);③试验组术后第3天的血红蛋白值、术后第1,3天的红细胞压积值高于对照组,差异有显著性意义(P<0.05);④两组术后血红蛋白和血小板计数呈下降趋势,第3天的血红蛋白值为最低值,术后第1天的血小板值为最低值,然后开始回升;⑤两组患者术后并发症发生率相比差异无显著性意义(P>0.05);⑥结果表明,术后延迟氨甲环酸的使用时间,可能有助于减少股骨近端防旋髓内钉治疗股骨转子间骨折的隐性失血量,并不会增加并发症风险。 展开更多

目的 探讨髋关节囊周围神经(pericapsular nerve group, PENG)阻滞与髂筋膜间隙阻滞(fascia iliaca compartment block, FICB)对老年股骨粗隆间骨折(intertrochanteric femur fracture, IFF)患者股骨近端防旋髓内钉(proximal femoral na... 目的 探讨髋关节囊周围神经(pericapsular nerve group, PENG)阻滞与髂筋膜间隙阻滞(fascia iliaca compartment block, FICB)对老年股骨粗隆间骨折(intertrochanteric femur fracture, IFF)患者股骨近端防旋髓内钉(proximal femoral nail anti-rotation, PFNA)内固定术的镇痛效果。方法 选取2022年1—12月首都医科大学附属北京积水潭医院老年IFF患者60例,随机分为P组和F组,每组各30例。P组采用PENG阻滞,F组采用FICB。两组患者均接受椎管内麻醉进行手术,术后均给予患者静脉自控镇痛(patient controlled intravenous analgesia, PCIA)。比较两组患者不同时点(T1,神经阻滞前;T2,神经阻滞后30 min;T3,术后6 h;T4,术后24 h;T5,术后48 h)静息和运动时的视觉模拟评分(visual analogue score, VAS)、术后补救镇痛率、镇痛满意度评分、住院时间、PCIA按压次数及不良反应发生率。结果 60例患者中,男23例,女37例,年龄65~85岁,平均(70.6±6.0)岁。两组T2~T5时点静息和运动时VAS均低于T1时点,差异均有统计学意义(P <0.05);两组静息和运动时所有时点VAS、术后补救镇痛率、镇痛满意度评分、住院时间、PCIA按压次数及不良反应发生率的比较,差异均无统计学意义(P>0.05)。结论 PENG阻滞与FICB均可缓解老年IFF患者PFNA内固定术的镇痛,且镇痛效果相当。 展开更多

目的 探讨机器人导航辅助下股骨近端防旋髓内钉(PFNA)与C型臂X射线机透视下PFNA治疗股骨粗隆间骨折(IFOF)的疗效。方法 选择我院单侧IFOF患者100例为研究对象,按照随机数字表法分为对照组(50例)和机器人辅助组(50例),对照组在C型臂X射... 目的 探讨机器人导航辅助下股骨近端防旋髓内钉(PFNA)与C型臂X射线机透视下PFNA治疗股骨粗隆间骨折(IFOF)的疗效。方法 选择我院单侧IFOF患者100例为研究对象,按照随机数字表法分为对照组(50例)和机器人辅助组(50例),对照组在C型臂X射线机透视下行PFNA内固定治疗,机器人辅助组在机器人导航辅助下行PFNA内固定治疗。比较2患者组手术指标、骨折复位及骨折愈合情况、应激反应指标、髋关节功能、生活质量评分以及手术相关并发症发生率。结果 机器人辅助组患者手术时间、总透视时间、置钉时间、骨折愈合时间短于对照组(P<0.05),术中出血量、导针调整次数、透视次数少于对照组(P<0.05)。机器人辅助组患者骨折复位情况、骨折愈合情况均优于对照组(P<0.05)。术后2 d,2组患者血清去甲肾上腺素(NE)、血管紧张素Ⅱ(Ang Ⅱ)、超氧化物歧化酶(SOD)水平均高于术前(P<0.05);机器人辅助组术后2 d血清NE、Ang Ⅱ、SOD水平均低于对照组(P<0.05)。2组患者术后7 d、术后3个月Harris评分及健康状况调查量表(SF-36)评分高于术前(P<0.05),且术后3个月上述评分高于术后7 d(P<0.05);术后7 d、术后3个月,机器人辅助组患者Harris评分、SF-36评分均高于对照组(P<0.05)。机器人辅助组患者并发症发生率低于对照组(P<0.05)。结论 与C型臂X射线机透视相比,机器人导航辅助下PFNA治疗IFOF能进一步缩短手术时间、减小手术创伤及降低并发症发生率,骨折复位、愈合效果更佳,机体应激更小,更能改善患者髋关节功能及生活质量。 展开更多

股骨近端防旋髓内钉治疗高龄股骨粗隆间骨折的临床效果

目的分析高龄股骨粗隆间骨折患者应用股骨近端防旋髓内钉治疗的价值。方法80例高龄股骨粗隆间骨折患者,按照随机数字表法分为观察组和对照组,每组40例。对照组采取常规髓内钉固定治疗,观察组采取股骨近端防旋髓内钉治疗。比较两组患者... 目的分析高龄股骨粗隆间骨折患者应用股骨近端防旋髓内钉治疗的价值。方法80例高龄股骨粗隆间骨折患者,按照随机数字表法分为观察组和对照组,每组40例。对照组采取常规髓内钉固定治疗,观察组采取股骨近端防旋髓内钉治疗。比较两组患者手术指标(切口长度、手术时间、术中出血量)、骨折愈合时间、术后开始负重时间以及不同时间点髋关节功能Harris评分、下肢Fugl-Meyer评测法(FMA)评分。结果观察组切口长度(4.24±0.36)cm短于对照组的(13.51±0.11)cm,手术时间(108.56±12.15)min、骨折愈合时间(12.26±1.11)周、术后开始负重时间(12.26±1.57)周均短于对照组的(134.16±11.51)min、(16.58±1.74)周、(16.62±1.52)周,术中出血量(152.16±14.15)ml少于对照组的(219.65±13.68)ml(P<0.05)。观察组治疗后3个月、6个月、1年的髋关节功能Harris评分分别为(66.52±2.14)、(81.56±1.11)、(86.25±1.27)分,较对照组的(61.28±3.26)、(72.26±2.46)、(79.85±2.46)分明显更高(P<0.05)。观察组治疗后3个月、6个月、1年的下肢FMA评分分别为(18.62±2.13)、(24.13±2.41)、(26.63±1.27)分,较对照组的(15.24±1.39)、(20.16±2.11)、(22.13±2.17)分明显更高(P<0.05)。结论高龄股骨粗隆间骨折采用股骨近端防旋髓内钉治疗可显著提高治疗效果,缩短术后康复时间,对提高髋关节与下肢功能有重要作用,值得临床广泛推广。 展开更多

人工股骨头置换术与PFNA固定对老年股骨转子间骨折患者髋关节功能恢复的影响

目的:探讨人工股骨头置换术与近端防旋髓内钉(Proximal Femoral Nail Antirotation,PFNA)固定在老年股骨转子间骨折(Intertrochanteric fracture,IFF)中的应用效果。方法:选取2018年1月至2022年12月我院收治的36例老年IFF患者作为研究对... 目的:探讨人工股骨头置换术与近端防旋髓内钉(Proximal Femoral Nail Antirotation,PFNA)固定在老年股骨转子间骨折(Intertrochanteric fracture,IFF)中的应用效果。方法:选取2018年1月至2022年12月我院收治的36例老年IFF患者作为研究对象,按随机数字表法分为对照组和观察组,各18例。对照组进行PFNA固定术治疗,观察组进行人工股骨头置换术治疗。分析比较两组的手术情况、髋关节功能、日常生活能力和平衡功能及并发症。结果:观察组手术时间较对照组长,术中出血量较对照组多,下床活动时间、术后住院时间较对照组短,有统计学差异(P<0.05)。观察组术后Harris评分中关节功能、畸形、疼痛程度、关节活动度及总分均明显高于对照组(P<0.05)。观察组术后Barthel量表(Barthel index,BI)评分、Berg平衡量表(Berg balance scale,BBS)评分均明显高于对照组(P<0.05)。两组并发症相比无明显差异(P>0.05)。结论:人工股骨头置换术治疗老年IFF效果更佳,可缩短术后下床活动时间,加快患者髋关节功能恢复,提高BI评分及BBS评分,安全可靠。 展开更多

目的 探讨近端滑动加压技术在股骨转子间骨折(IFF)并外侧壁骨折患者股骨近端抗旋髓内钉(PFNA)术中的应用价值。方法 选取安徽省亳州市人民医院骨科2019年8月至2021年8月收治的IFF并外侧壁骨折患者50例,按随机数字表法将其分成试验组、... 目的 探讨近端滑动加压技术在股骨转子间骨折(IFF)并外侧壁骨折患者股骨近端抗旋髓内钉(PFNA)术中的应用价值。方法 选取安徽省亳州市人民医院骨科2019年8月至2021年8月收治的IFF并外侧壁骨折患者50例,按随机数字表法将其分成试验组、对照组,各25例。两组均行PFNA术,试验组在PFNA术中使用近端滑动加压技术,对照组行常规PFNA术。比较两组围手术期恢复情况与骨折复位效果,分别在术前及术后3、6个月采血,检测两组血清骨形态生成蛋白-2(BMP-2)、骨碱性磷酸酶(BALP)水平。分别在术后当日及术后3、6、12个月对两组进行髋关节功能Harris评分,并比较两组股骨颈干角的变化,记录并发症发生情况。另选取30份人工股骨标本,编号后采用随机数字表法分为PFNA术组和近端滑动加压技术联合PFNA术组,每组15份。进行相应干预后进行扭转荷载试验与压缩荷载试验,比较两组扭转1.5°时的扭转刚度、扭矩,以及荷载800 N时的压缩刚度、位移情况。结果 试验组骨折愈合时间短于对照组(P<0.05)。两组术后3、6个月血清BMP-2、BALP水平均高于术前,术后6个月高于术后3个月,试验组术后3、6个月的血清BMP-2、BALP水平均高于同期对照组(P<0.05)。两组术后3、6、12个月Harris评分均高于术后当日,术后6、12个月高于术后3个月,试验组术后3、6个月Harris评分均高于对照组(P<0.05)。两组术后3、6、12个月股骨颈干角均低于术后当日(P<0.05)。两组复位效果、并发症总发生率比较,差异无统计学意义(P>0.05)。近端滑动加压技术联合PFNA术组扭转1.5°时扭转刚度、扭矩高于PFNA术组,荷载800 N时的压缩刚度高于PFNA术组,压缩位移低于PFNA术组(P<0.05)。结论 在PFNA术中近端采用滑动加压技术,能促进IFF并外侧壁骨折患者的骨折愈合及功能恢复,生物力学性能更好。 展开更多

目的:探讨新伤续断汤对股骨转子间骨折股骨近端防旋髓内钉(PFNA)术后骨折愈合的影响。方法:选取符合纳入标准的PFNA术后患者64例,随机分为对照组、观察组,每组32例。术后常规治疗为对照组,术后常规治疗基础上口服新伤续断汤为观察组。... 目的:探讨新伤续断汤对股骨转子间骨折股骨近端防旋髓内钉(PFNA)术后骨折愈合的影响。方法:选取符合纳入标准的PFNA术后患者64例,随机分为对照组、观察组,每组32例。术后常规治疗为对照组,术后常规治疗基础上口服新伤续断汤为观察组。记录两组患者在治疗6周及术后3个月时髋关节Harris评分、影像学变化。记录两组患者首次下床锻炼时间,及术前、术后第1天、治疗2周、治疗6周血红蛋白(Hb)值。检测两组患者术前、术后3个月的骨代谢标志物。结果:收集64例患者因术后并发症等原因脱落5例,对照组29例、观察组30例,完成率92%。治疗6周及术后3个月时,观察组Harris评分优于对照组,差异有统计学意义(P<0.05)。在首次下床锻炼时间方面,观察组优于对照组,差异有统计学意义(P<0.05)。观察组治疗2、6周时Hb水平低于对照组,差异有统计学意义(P<0.05)。骨代谢方面,在术后3个月时观察组人N中段骨钙素、总Ⅰ型胶原氨基端延长肽、25-羟基维生素D水平较对照组升高,而β-胶原特殊序列较对照组下降,差异有统计学意义(均P<0.05)。影像学方面,治疗6周、术后3个月时观察组骨痂生长情况优于对照组,术后3个月观察组的骨折愈合优于对照组,差异有统计学意义(均P<0.05)。结论:新伤续断汤能有效改善髋关节功能,减少卧床时间,改善贫血症状,调节骨代谢,促进成骨细胞,抑制破骨细胞,刺激骨痂生长,促进骨折愈合,提升患者生活质量。 展开更多

目的探讨Gamma3髓内钉应用于股骨粗隆间骨折手术治疗中的效果。方法选择2020年1月—2022年12月丹阳市中医院收治的82例股骨粗隆间骨折患者为研究对象,以随机数表法分为对照组(n=41,给予抗旋型股骨近端髓内钉手术治疗)与观察组(n=41,给予... 目的探讨Gamma3髓内钉应用于股骨粗隆间骨折手术治疗中的效果。方法选择2020年1月—2022年12月丹阳市中医院收治的82例股骨粗隆间骨折患者为研究对象,以随机数表法分为对照组(n=41,给予抗旋型股骨近端髓内钉手术治疗)与观察组(n=41,给予Gamma3髓内钉手术治疗),比较两组患者的手术治疗相关指标、髋关节功能与并发症。结果两组患者的手术用时、手术总失血量对比,差异无统计学意义(P均>0.05)。观察组患者的住院时间(12.14±1.16)d、术后负重时间(5.63±1.07)周、骨折愈合时间(7.32±1.18)周均短于对照组,差异有统计学意义(t=12.535、9.884、6.992,P均<0.05)。与术前比较,两组患者术后1、3、6个月时的Harris评分均升高,差异有统计学意义(P均<0.05);但各时间段组间比较,差异无统计学意义(P均>0.05)。两组髋关节功能优良率、并发症发生率对比,差异无统计学意义(P均>0.05)。结论Gamma3髓内钉与抗旋型股骨近端髓内钉治疗股骨粗隆间骨折的效果、安全性均相当,但Gamma3髓内钉能加快患者术后功能恢复。 展开更多
