It is unclear whether and to what extent purportedly empowering practices in mental health care, like rehabilitation programs, recovery-supporting mental health care environments and peer-run services, contribute to the process of empowerment. Several American empowerment questionnaires have been developed in recent years, facilitating the measurement of empowerment outcomes. Given likely major transatlantic cultural differences in a value-sensitive concept such as empowerment, this article describes the development of the Netherlands Empowerment List (NEL) and its psychometric properties. Methods. Patients in Dutch mental health services provided meaning to the empowerment concept from which the NEL was derived. Based on 531 completed questionnaires, analyses in agreement with COSMIN criteria examined aspects of internal consistency, content validity, structural validity, convergent validity, discriminant validity, reproducibility and responsiveness of the NEL. Results. The NEL is a 40-item self-report questionnaire with six subscales: Social support, Professional help, Connectedness, Confidence and purpose, Self-management and Caring community. Internal consistency (Cronbach’s alpha = 0.94), aspects of validity, reproducibility (intraclass correlation = 0.79) and responsiveness were good. Correlation with existing scales was the highest for the Mental Health Confidence Scale (r = 0.78) and the lowest for the Boston Empowerment Scale (r = 0.61). Conclusion. The NEL appears to be a suitable instrument to capture the dimension of empowerment in European mental health settings.展开更多

The physical and mental burden placed on families raising severely disabled children, particularly the primary caregivers, is great in home settings. For families to effectively utilize services and over-come child rearing problems, the families themselves need the “strength” to cooperate with others for the purpose of raising a severely disabled child. The ultimate goal of family support is to enable such families to achieve satisfaction and self-growth in child rearing. Methods: We used a questionnaire to survey 75 primary caregivers to empirically elucidate the empowerment and positive feelings towards child rearing of families raising children with severe motor and intellectual disabilities and the related factors. The t-test and Spearman’s rank correlation coefficient were used to examine the association with bivariates. A multiple regression analysis was conducted for empowerment and positive feelings. Results: Results revealed that life events, livelihood, awareness of social support and the child’s sleep problems were factors related to empowerment. Of these, awareness of social support from outside of the family was found to contribute the most to empowerment. Furthermore, improvement and maintenance of positive feelings towards child rearing reaffirmed the existence of empowerment in addition to reducing negative feelings towards child rearing and ensuring social support. Conclusions: Raising children with severe motor and intellectual disabilities requires specialist knowledge and skills. Support from professionals to empower the entire family is therefore important in order to strengthen positive feelings towards child rearing.展开更多

文摘Objectives: The families of these children experience distress both at the time of diagnosis and afterward.A top priority is to understand family empowerment, family function, and family members' quality of life (QoL) and to effectively support these families in Japan. The objective of this study was to assess the actual conditions of families living with children having DDs and to explore the factors associated with family empowerment and parents' QoL.Methods: We surveyed ninety-three parents (78 mothers,15 fathers) from 78 families which lived with children with DDs in the capital region of Japan. We assessed two main outcomes using the Japanese versions of the following instruments: Family Empowerment Scale (FES),World Health Organization Quality of Life 26 (WHOQOL26),and other six outcomes.Correlation and multiple regression analyses were conducted. Results: No medication, cooperation with child rearing, assistance from a developmental support center, solved problems related to child rearing, and higher scores in Problem Solving contributed to higher FES scores.Higher WHOQOL26 scores were related to being a full-time housewife, higher self-esteem,no developmental support,a broad emotional support network, higher scores in Problem Solving and Role Function, and lower scores in Affective Reaction and General Function. Conclusions: We revealed that family empowerment and QoL of parents rearing children with DDs in Japan were affected by various subscales of family function and other family attributes.Effective interventions for improving family empowerment and QoL should be researched in the future.
文摘Objectives:This study aimed to develop an empowerment model for burnout syndrome and quality of nursing work life(QNWL).Methods:This study adopted a mixed-method cross-sectional approach. The variables included structural empowerment, psychological empowerment, burnout syndrome and QNWL.The population consisted of nurses who have civil servant status in one of the regional hospitals in Indonesia. The participants were recruited using multi-stage sampling measures with 134 respondents.Data were collected using questionnaires, which were then analysed using partial least squares.A focus group discussion was conducted with nurses, chief nurses and the hospital management to identify strategic issues and compile recommendations. Results:Structural empowerment influenced psychological empowerment(path coefficient=0.440;t=6.222)and QNWL(path coefficient=0.345;t=4.789).Psychological empowerment influenced burnout syndrome(path coefficient=-0.371;t=4.303),and burnout syndrome influenced QNWL(path coefficient=-0.320;t=5.102).Structural empowerment increased QNWL by 39.7%.Conclusion:The development of a structural empowerment model by using the indicators of resources, support and information directly influenced the psychological empowerment of the sample of nurses. As an indicator of meaning, psychological empowerment decreased burnout syndrome. In turn, burnout syndrome,as the indicator of personal achievement, could affect the QNWL.Structural empowerment directly influenced the QNWL,particularly within the workplace context.Further studies must be conducted to analyse the effects of empowerment, leadership styles and customer satisfaction.
文摘Objective:Post-stroke disability and psychosocial disorders cause burdens for the families of stroke patients, including physical and financial burdens. The physical and psychological health of family caregivers determines the quality of care they provide to patients. The purpose of this study was to identify the effectiveness of the Caregiver Empowerment Program Based on the Adaptation Model(CEP-BAM)in increasing the family caregiver’s coping ability against various problems and reducing their burden while caring for the stroke patient. Methods:This research was a quasi-experimental study with pre-and post-test control group design. The intervention group received CEP-BAM,while the control group received a conventional intervention in the form of a discharge planning program for family caregivers in the hospital. The samples were caregivers who care for and facilitate the recovery of stroke patients during their convalescence at home.We selected the samples from the population using the stratified random sampling method. The number of samples completed in the study was 40 in the intervention group and 40 in the control group.Measurement of outcome variables(coping and caregiver burden)was carried out 4 times including pre-test before the intervention,post-test 1 at 4 months after the intervention,post-test 2 at 5 months after the intervention, and post-test 3 at 6 months after the intervention. Results:There were significant differences in caregiver’s coping(P=0.016)and caregiver’s burden(P=0.009)in measurements between the two groups. Conclusions:The CEP-BAM interventions were effective in increasing adaptive coping strategies and reducing the burden of caregivers 4 months and continuing 6 months after the intervention.

文摘Objectives:Family caregivers raising children with severe motor and intellectual disabilities(SMID)experience the enormous burden of care. The concept of family empowerment is one of the important assessment indexes of family nursing from the perspective of providing comprehensive support for these families. The objective of this study was to identify the factors associated with the empowerment of families raising a child with SMID in Japan. Methods:We conducted a nationwide questionnaire survey involving 1659 primary caregivers raising a child with SMID through 89 special schools. We assessed the main outcomes using the Family Empowerment Scale(FES).We then conducted a multiple linear regression analysis to reveal the factors associated with family empowerment. Results:In total,1362 primary caregivers were included in our study. Our results show that factors contributing to high FES scores are higher age of the primary caregiver, higher education, greater recognition of regional support, lower childcare burden, higher utilization of home visit services, higher usage of a childcare institution, higher household income, and stronger family bonding. Conclusion:Healthcare professionals should carefully assess the state of family empowerment of the primary caregivers who are younger and those who have low education, low household income, high childcare burden, and fragile bonding with the family.Second, they should encourage such families to use regional support resources for childcare. That is,policy makers should consider ways to promote home visits and institutional services for the care of children with SMID,aiming especially for the provision of well-coordinated care and services.
文摘Reasons for the initiation of farming systems/participatory approaches in non-Green Revolution(i.e., in more heterogeneous and less favourable production environments) areas in low-income countries is followed by a discussion of their evolution.Four phases are described along, with a brief description of some of the significant methods in which farmer participation has been sought, accessed and evaluated. They enabled/facilitated the evolutionary process.Although, over the last 40 years there has been a progress in making small-scale farmers much more visible in driving the agricultural research/development agenda through using the four basic stages embodied in the farming systems approach(i.e., descriptive, diagnosis, testing/evaluation and finally dissemination), much still needs to be done. This leads on to a discussion as to some of the barriers still inhibiting true farmer empowerment and why further, such empowerment is imperative, if the agricultural challenges of this century are to be addressed successfully, namely substantially increasing agricultural productivity in an ecologically sustainable manner.Given the heterogeneity of the production environments and that many identified improvements are likely to be incremental rather than revolutionary in nature, this will require farmers' intimate involvement in their identification, evaluation and dissemination.Discussion in the paper recognizes that there is increasing globalization/commercialization of agriculture and is predicated on the need to consider the whole farmer-research-development continuum involving multiple stakeholders(i.e., farmers, scientists, extension workers, input/output service providers and policymakers).Such a continuum has been explicitly recognized in the operational plans for the recently reformed international agricultural research(i.e., CGIAR) system. The paper concludes with a short discussion on the potential role of formal modelling.
文摘Objective:The objective of this study was to evaluate the effect of empowerment education on depression level and laboratory indicators of patients treated with hemodialysis. Methods:We searched databases including Cochrane Library,Web of Science,Embase,PubMed,Chinese Biomedical Literature Database(CBM),Chinese Academic Journals Full-text Database(CNKI),VIP,and Wanfang. We included all the researches about the application of empowerment education in patients treated with hemodialysis. We used RevMan 5.3 to analyze the data after assessing the quality of researches. Results:We included 10 studies eventually. The results of meta-analysis showed that the self-efficacy[MD=1.10,95%confidence interval(CI;0.71,1.48),P<0.01],depression[MD=-6.94,95%CI(-8.53,-5.34),P<0.01],serum creatinine[Scr;MD=-116.46,95%CI(-171.90,-61.03),P<0.01],albumin[Alb;MD=2.93,95%CI(2.00,3.86),P<0.01],and blood urea nitrogen[BUN;MD=-0.95,95%CI(-1.89,-0.01),P=0.05]in the empowerment education group had significantly statistical difference compared with those in the usual education group. However, there was no significant difference in hemoglobin[Hb;MD=3.03,95%CI(-0.64,6.70),P=0.11]between two groups. Conclusions:Empowerment education can improve the self-efficacy in patients treated with hemodialysis, relieve depression, and improve Scr,Alb,and BUN.However, there is no significant improvement in Hb;it needs further exploration in clinical practice.
文摘Objectives. Conclusion. The NEL appears to be a suitable instrument to capture the dimension of empowerment in European mental health settings.
文摘Background: “Children with severe motor and intellectual disabilities” refers to children with markedly limited activity due to severe overlapping of physical and intellectual disabilities. Support from professionals to empower the entire family is therefore important in order to strengthen positive feelings towards child rearing.
