S-100β CysC和NF-κB对急性缺血性脑卒中患者静脉溶栓后出血转化的预测价值

目的 探讨急性缺血性脑卒中(AIS)外周血中S-100β、CysC和NF-κB水平对静脉溶栓后出血转化的影响及预测价值。方法 收集2019-03—2022-03接受溶栓治疗的AIS患者140例,根据溶栓后24 h是否发生出血转化(HT)将患者分为非HT组(n=112)和HT组(n=28)。比较2组一般临床资料,采用多因Logistic回归分析影响HT发生的危险因素,受试者工作特征(ROC)曲线评估S-100β、CysC和NF-κB预测HT发生的价值。结果 HT组与非HT组相比,患者年龄、发病至溶栓时间、房颤、TOAST分型、C反应蛋白、凝血酶原时间、S-100β、CysC、NF-κB、白质高信号和脑微出血等均有统计学差异(P<0.05)。Logistic多因素回归分析显示,房颤、S-100β、CysC和NF-κB为影响HT发生的危险因素。S-100β、CysC和NF-κB预测AIS患者静脉溶栓后出血转化的曲线下面积分别为0.915(0.902~0.923)、0.874(0.856~0.882)和0.789(0.771~0.796),均具有一定的预测价值。结论 S-100β、CysC和NF-κB为AIS患者静脉溶栓后HT发生的危险因素,对HT发生具有一定的预测价值。

公共价值计分卡评估下的社区平台组织建设研究——以S居民区“商居联盟”为例

随着城市化进程的演进和市场化改革的推进,商业性元素逐步进入城市社区,原本依靠血缘关系或文化习俗维系的社区邻里关系变得原子化、碎片化,社区各类成员的认同感、归属感、安全感日益匮乏,不同群体之间的矛盾冲突愈发凸显。社区平台组织作为各方平等对话、交流、协作的载体,成为城市社区治理中化解矛盾的有力抓手。本文结合S居民区“商居联盟”平台组织的实践,总结平台组织的经验做法;建立公共价值计分卡模型,评估平台组织存在的问题及其原因。研究发现:联盟平台组织在价值目标设定方面行政化,解决专业性、复杂性矛盾的成效不足;在授权环境方面多元力量支持不足,主体参与积极性有待提高;在运作能力方面整合内外资源、化解矛盾的能力有待提高。对此,需要强化共同使命,增强组织向心力;加强内部建设,增强组织生命力;健全制度规范,增强组织稳定性,进一步优化联盟建设。

Intestinal complications in patients with Crohn's disease in the Brazilian public healthcare system between 2011 and 2020

BACKGROUND This is a secondary database study using the Brazilian public healthcare system database.AIM To describe intestinal complications(ICs)of patients in the Brazilian public healthcare system with Crohn’s disease(CD)who initiated and either only received conventional therapy(CVT)or also initiated anti-tumor necrosis factor(anti-TNF)therapy between 2011 and 2020.METHODS This study included patients with CD[international classification of diseases–10th revision(ICD-10):K50.0,K50.1,or K50.8](age:≥18 years)with at least one claim of CVT(sulfasalazine,azathioprine,mesalazine,or methotrexate).IC was defined as a CD-related hospitalization,pre-defined procedure codes(from rectum or intestinal surgery groups),and/or associated disease(pre-defined ICD-10 codes),and overall(one or more type of ICs).RESULTS In the 16809 patients with CD that met the inclusion criteria,the mean follow-up duration was 4.44(2.37)years.In total,14697 claims of ICs were found from 4633 patients.Over the 1-and 5-year of follow-up,8.3%and 8.2%of the patients with CD,respectively,presented at least one IC,of which fistula(31%)and fistulotomy(48%)were the most commonly reported.The overall incidence rate(95%CI)of ICs was 6.8(6.5–7.04)per 100 patient years for patients using only-CVT,and 9.2(8.8–9.6)for patients with evidence of anti-TNF therapy.CONCLUSION The outcomes highlighted an important and constant rate of ICs over time in all the CD populations assessed,especially in patients exposed to anti-TNF therapy.This outcome revealed insights into the real-world treatment and complications relevant to patients with CD and highlights that this disease remains a concern that may require additional treatment strategies in the Brazilian public healthcare system.

Reacting to Epidemics:The Innovative Imperial Public Health System during the Late Northern Song Dynasty

Song China was a period in which China experienced a great increase in its population.Concurrently,the Song dynasty also experienced a rise in the frequency of epidemics and two major wars with the Western Xia and Liao dynasties during the 1000s and 1040s.The consequences of these changes were exacerbated by the increased geographical mobility of certain social groups such as traders and examinees attending civil service examinations.Thus,casualties of wars,epidemics,or disease,especially of people whose families were far away and could not care for them were left without care and“their corpses often lay bare along the roads.”This new social environment created a need for general relief.The Northern Song government(960-1127 CE),especially during the reign of Emperor Huizong,established an innovative public health system to address this issue.The public health system included poorhouses,public hospitals,and pauper’s cemeteries.The first were more of charity organizations,whereas the latter two promoted public health by providing medical services for the poor and burial for those that nobody cared for.In terms of rationale behind these institutions,on the one hand,they constituted an attempt to get the poor and homeless off the streets while providing them relief or burial.On the other hand,it seems that Huizong’s deep concern with medicine propelled him to design and implement a comprehensive public health system oriented to prevent contagion and outbreak of epidemics.This article depicts the background,the organization,and the functions of the system.The article also discusses the conditions and reasons that gave rise to such a unique undertaking by the Northern Song government.

Value Assessment of Ecosystem Service Function of Ecological Public Welfare Forests in Wenchang City

Based on the measurement parameters and index in the Specifications for Assessment of Forest Ecosystem Services in China(LY/T 1721-2008),the value of ecological service functions of ecological public welfare forest in Wenchang City was evaluated and analyzed.The results show that the total value of ecosystem service function of ecological public welfare forests in Wenchang City is about 4.020 billion yuan,of which storage capacity,water purification,soil fixation,fertilizer(N,P and K fertilizer) conservation,carbon sequestration,oxygen release,forest nutrient accumulation,providing anion,absorbing SO_2,absorbing fluoride,absorbing NxO,absorbing soil heavy metals,dust retention,noise reduction,sterilization,forest protection,forest biodiversity protection,and forest recreation(ecological tourism) are about 1.510 billion,582.256 million,3.769 million,268.220 million,116.360 million,2.747 million,22.682 million,42.817 million,1.722 million,74,600,131,900,187.918 million,77.409 million,28.721 million,43.090 million,180.696 million,571.036 million,and 108.677 million yuan

血清CysC、α-Syn联合MRI对帕金森病病情评估及诊断的意义

目的 探讨血清胱抑素C(CysC)、α-突触核蛋白(α-Syn)联合磁共振成像(MRI)对帕金森病(PD)病情评估及诊断的意义。方法 将2019年4月至2021年4月于河北省沧州中西医结合医院(下称该院)治疗的PD患者87例纳入研究作为PD组。根据Hoehn-Yahr(H-Y)分期,将PD患者分为Ⅰ~Ⅱ期13例,Ⅲ~Ⅳ期45例,Ⅴ期29例。另外,选取同期于该院进行体检的健康志愿者87例作为对照组。比较各组血清CysC、α-Syn水平,分析血清CysC、α-Syn联合MRI对PD的诊断效能。结果 PD组血清CysC、α-Syn水平均高于对照组,差异有统计学意义(P<0.05);血清CysC、α-Syn水平随着H-Y分期递增升高(P<0.05)。PD组MRI各相位值与对照组比较,黑质致密带、苍白球、壳核、尾状核相位值均降低(P<0.05);不同H-Y分期的PD患者黑质致密带、苍白球、壳核相位值比较,差异均有统计学意义(P<0.05)。CysC、α-Syn单独诊断PD的AUC分别为0.786、0.767,灵敏度分别为58.62%、64.37%,特异度分别为88.51%、87.36%;与MRI、血清CysC、α-Syn单独诊断相比,三项联合诊断PD的灵敏度和准确度较高同,差异有统计学意义(P<0.05)。结论 血清CysC、α-Syn联合MRI对PD诊断效能较高,能为评估PD病情严重程度和早期诊断提供依据。

基于模糊数学理论改进B-S模型的专利质押融资价值评估

在专利质押的法规条款不断完善的背景下,越来越多的科技型企业选择以专利作为质押物获得银行融资,所以对专利进行合理估值是高新技术企业进行专利质押融资的重要前提。而针对专利资产在价值评估过程中存在的主观性、不确定性等方面的问题,本文通过模糊数学理论对B-S模型进行改进,并通过实际案例验证该模型对专利质押价值评估具有一定的可行性,最后在对模型潜在价值关键影响因素进行敏感性分析的基础上,确定了关键因素对专利潜在价值的影响方向并提出建议。

公共价值视域下PPP项目回报机制决策研究:基于PSO-TRP模型

回报机制是PPP项目的核心属性,政府作为PPP项目中公共服务合作生产的关键决策主体,其回报机制的决策逻辑关系到PPP项目公共性与经济效率的平衡,这也是直接影响PPP项目成败的关键问题。首先,综合考虑PPP项目公共性、参照点效应和边际效用递减规律,提出研究假设。其次,引入三参照点理论构建PPP项目回报机制决策模型并分析其约束条件。最后,利用粒子群算法对政府在预算约束和底线逻辑情境中如何选择回报机制获取最大效用的决策问题进行案例模拟、情景仿真与方案甄选。研究发现:1)PPP项目中政府感知价值随着目标的升高而先升高后降低,呈倒“U”型分布,感知价值和努力水平随底线升高而升高,最优位置即为公共价值达到最大化。2)当政府底线一定、目标较高时,政府完全或部分将公共资源让渡给社会资本主体,选择使用者付费或可行性缺口补助模式。3)当政府心理权重值发生改变时,成功权重越大,在最优目标范围内主体会加大自身的投入程度,选择基于公共价值的冒险行为;当失败权重越大,决策主体会减少自身的投入程度,选择基于公共价值的保底行为。研究立足于公共价值视域并引入三参照点理论,构建了基于粒子群算法的PPP项目回报机制决策模型,为PPP项目政府决策研究提供了新思路,同时也为维持PPP项目公共性和实现公共价值提供了参考。

Prognostic Value of Semi-Quantitative 18F-FDG PET/CT Parameters in Hodgkin’s Lymphoma

Objective:To assess the prognostic value of maximum standardized uptake value(SUVmax),metabolic tumor volume(MTV),and total lesion glycolysis(TLG)determined by 18F-fluorodeoxyglucose positron emission tomography-computed tomography(18F-FDG PET/CT)imaging in Hodgkin’s lymphoma patients.Methods:A total of 148 Hodgkin’s lymphoma patients diagnosed with lymph node biopsy from October 2014 to October 2015 were retrospectively analyzed followed by categorizing into good(125 cases)and poor(23 cases)prognosis groups.The chi-squared test was used to analyze the clinicopathological characteristics of Hodgkin’s lymphoma patients with the semi-quantitative 18F-FDG PET/CT parameters;the Spearman method was used to analyze the correlation between the semi-quantitative parameters and clinicopathological features of Hodgkin’s lymphoma;receiver operating characteristic curve was used to analyze the predictive value of the semi-quantitative parameters for poor prognosis of Hodgkin’s lymphoma patients.Results:Mean SUVmax,MTV,and TLG of the 148 cases of Hodgkin’s lymphoma were 7.26±2.38,12.46±3.14 cm3,and 76.83±18.56 g,respectively.Significant variations in the Ann Arbor stage and clinical classification were observed with different levels of semi-quantitative parameters(P<0.05).The semi-quantitative parameters were not correlated with age and gender(P>0.05)but positively correlated with Ann Arbor stage and clinical classification(P<0.05).These parameters in the poor prognosis group were higher than those in the good prognosis group(P<0.05).The area under the curve(AUC)of SUVmax,MTV,and TLG in predicting the poor prognosis group was 0.881,0.875,and 0.838,with cut-off values of 7.264,12.898 cm3,and 74.580g,as well as specificity of 88.8%,84.0%,and 78.4%,and sensitivity of 87.0%,87.0%,and 78.3%,respectively;the AUC of the combined prediction was 0.986,with a specificity of 97.6%and sensitivity of 86.3%.Conclusion:The semi-quantitative 18F-FDG PET/CT parameters provide valuable insights for Hodgkin’s lymphoma prognosis assessment.

Optimal Allocation of Public Transport Hub Based on Load Loss Value and the Economy of Distribution Network

The rapid development of electric buses has brought a surge in the number of bus hubs and their charging and discharging capacities.Therefore,the location and construction scale of bus hubs will greatly affect the operation costs and benefits of an urban distribution network in the future.Through the scientific and reasonable planning of public transport hubs on the premise of meeting the needs of basic public transport services,it can reduce the negative impact of electric bus charging loads upon the power grids.Furthermore,it can use its flexible operation characteristics to provide flexible support for the distribution network.In this paper,taking the impact of public transport hub on the reliability of distribution network as the starting point,a three-level programming optimization model based on the value and economy of distribution network load loss is proposed.Through the upper model,several planning schemes can be generated,which provides boundary conditions for the expansion of middle-level optimization.The normal operation dispatching scheme of public transport hub obtained from the middle-level optimization results provides boundary conditions for the development of lower level optimization.Through the lower level optimization,the expected load loss of the whole distribution system including bus hub under the planning scheme given by the upper level can be obtained.The effectiveness of the model is verified by an IEEE-33 bus example.

Creating Inclusive Global Value Chains under the BRI from the Perspective of Public Goods Supply and Demand

Supply and demand of public goods provide a brand-new theoretical framework for research on the creation of inclusive global value chains(GVCs)under the Belt and Road Initiative(BRI).The supply of international public goods is an important positive variable for creating global value chains under the BRI,in which China and other developing countries involved,as well as their small and medium-sized enterprises(SMEs),are microscopic entities.When the BRI’s supply of instrumental,institutional,and conceptual international public goods is in equilibrium with the demand of GVC entities for public goods,the BRI creates necessary conditions for the extension of GVCs.GVCs created under the BRI are of great value in promoting inclusiveness,fair income distribution,balanced regional development,and the restructuring of international economic governance.Under the current GVC system,some developed countries have been wary about the BRI and even developed misgivings and biases.Nevertheless,developed countries may play an active role in bridging gaps in the BRI’s development and complement their respective strengths with developing countries for win-win cooperation.

长期冷冻对ELISA法检测血浆标本中HBsAg结果的影响

目的评价献血者HBsAg阳性血液标本在-20°C冻存8年后ELISA法HBsAg检测的结果,评估血站目前留样保存方式的有效性。方法收集本站2014年5月—2015年3月100份经HBsAg ELISA检测阳性的献血者血浆标本,冻存在-20°C冰箱,于2023年解冻标本并通过同种方法再次检测。结果100份血浆标本的HBsAg再检定性结果均为阳性,再检符合率100%,冻存后S/CO值降低明显(27.52 vs 19.03,P<0.05)。结论长期冻存会使HBsAg ELISA检测S/CO值下降,但不影响阳性定性结果。
