Previous studies have shown that transcranial pulse current stimulation(tPCS) can increase cerebral neural plasticity and improve patients' locomotor function.However, the precise mechanisms underlying this effect remain unclear.In the present study, rat models of stroke established by occlusion of the right cerebral middle artery were subjected to tPCS, 20 minutes per day for 7 successive days.tPCS significantly reduced the Bederson score, increased the foot print area of the affected limbs, and reduced the standing time of affected limbs of rats with stroke compared with that before intervention.Immunofluorescence staining and western blot assay revealed that tPCS significantly increased the expression of microtubule-associated protein-2 and growth-associated protein-43 around the ischemic penumbra.This finding suggests that tPCS can improve the locomotor function of rats with stroke by regulating the expression of microtubule-associated protein-2 and growth-associated protein-43 around the ischemic penumbra.These findings may provide a new method for the clinical treatment of poststroke motor dysfunction and a theoretical basis for clinical application of tPCS.The study was approved by the Animal Use and Management Committee of Shanghai University of Traditional Chinese Medicine of China(approval No.PZSHUTCM190315003) on February 22, 2019. 展开更多

Most studies on peripheral nerve injury have focused on repair at the site of injury, but very few have examined the effects of repair strategies on the more proximal neuronal cell bodies. In this study, an approximately 10-mm-long nerve segment from the ischial tuberosity in the rat was transected and its proximal and distal ends were inverted and sutured. The spinal cord was subjected to pulsed electrical stimulation at T10 and L3, at a current of 6.5 m A and a stimulation frequency of 15 Hz, 15 minutes per session, twice a day for 56 days. After pulsed electrical stimulation, the number of neurons in the dorsal root ganglion and anterior horn was increased in rats with sciatic nerve injury. The number of myelinated nerve fibers was increased in the sciatic nerve. The ultrastructure of neurons in the dorsal root ganglion and spinal cord was noticeably improved. Conduction velocity of the sciatic nerve was also increased. These results show that pulsed electrical stimulation protects sensory neurons in the dorsal root ganglia as well as motor neurons in the anterior horn of the spinal cord after peripheral nerve injury, and that it promotes the regeneration of peripheral nerve fibers. 展开更多

BACKGROUND: The postoperative recovery of nerve function in patients with peripheral nerve injury is always an important problem to solve after treatment. The electric stimulation induced electromagnetic field can nourish nerve, postpone muscular atrophy, and help the postoperative neuromuscular function. OBJECTIVE: To observe the effects of low-frequency pulse percutaneous electric stimulation on the functional recovery of postoperative patients with peripheral nerve injury, and quantitatively evaluate the results of electromyogram (EMG) examination before and after treatment. DESIGN : A retrospective case analysis SETTING: The Sixth People's Hospital affiliated to Shanghai Jiaotong University PARTICIPANTS: Nineteen postoperative inpatients with peripheral nerve injury were selected from the De- partment of Orthopaedics, the Sixth People's Hospital affiliated to Shanghai Jiaotong University from June 2005 to January 2006, including 13 males and 6 females aged 24-62 years with an average of 36 years old. There were 3 cases of brachial plexus nerve injury, 3 of median nerve injury, 7 of radial nerve injury, 3 of ul- nar nerve injury and 3 of common peroneal nerve injury, and all the patients received probing nerve fiber restoration. Their main preoperative manifestations were dennervation, pain in limbs, motor and sensory disturbances. All the 19 patients were informed with the therapeutic program and items for evaluation. METHODS: ① Low-frequency pulse percutaneous electric stimulation apparatus: The patients were given electric stimulation with the TERESA cantata instrument (TERESA-0, Shanghai Teresa Health Technology, Co., Ltd.). The patients were stimulated with symmetric square waves of 1-111 Hz, and the intensity was 1.2-5.0 mA, and it was gradually adjusted according to the recovered conditions of neural regeneration following the principle that the intensity was strong enough and the patients felt no obvious upset. They were treated for 4- 24 weeks, 10-30 minutes for each time, 1-3 times a day, and 6 weeks as a course. ② EMG examination was applied to evaluate the recoveries of recruitment, motor conduction velocity (MCV) and sensory conduction velocity (SCV) before and after treatment. The patients were examined with the EMG apparatus (DIS- A2000C, Danmark) before and after the treatment of percutaneous electric stimulation. ③Standards for evaluating the effects included cured (complete recovery of motor functions, muscle strength of grade 5, no abnormality in EMG examination), obviously effective [general recovery of motor function, muscle strength of grade 4, no or a few denervation potentials, motor conduction velocity (MCV) and sensory conduction velocity (SCV)], improved (partial recovery of motor function, muscle strength of grade 3, denervation potentials and reinneration potentials, slowed MCV and SCV, invalid (no obvious changes of motor function). MAIN OUTCOME MEASURES: ① Ameliorated degree of the nerve function of the postoperative patients with peripheral nerve injury treated with percutaneous electric stimulation; ② Changes of EMG examination before and after treatment. RESULTS: All the 19 postoperative patients with peripheral nerve injury were involved in the analysis of results. ① Comparison of nerve function before and after treatment in 19 patients with peripheral nerve injury of different sites: For the patients with radial nerve injury (n=7), the nerve functions all completely recovered after 8-week treatment, and the cured and obvious rate was 100% (7/7); For the patients with brachial plexus nerve injury (n=3), 1 case had no obvious improvement, and the cured and obvious rate was 67% (2/3); For the patients with common peroneal nerve injury (n=3), the extension of foot dorsum generally recovered in 1 case of nerve contusion after 4-week treatment, and the cured and obvious rate was 67% (2/3); For the patients with median nerve injury (n=3), muscle strength was obviously recovered, and the cured and obvious rate was 100% (3/3); For the patients with ulnar nerve injury (n=3), 1 case only had recovery of partial senses, and the cured and obvious rate was 67% (2/3). Totally 9 cases were cured, 7 were obviously effective, 1 was improved, and only 2 were invalid. After 4 courses, the cured rate of damaged nerve function after four courses was 47% (9/19), and effective rate was 89% (17/19).② Comparison of EMG examination before and after treatment: Before and after percutaneous electric stimulation, he effective rates of recruitment, MCV and SCV were 89% (17/19), 58% (11/19), 47% (9/19) respectively, and there were extremely obvious differences (P〈 0.01). CONCLUSION: ①Low-frequency pulse percutaneous electric stimulation can improve the nerve function of postoperative patients with peripheral nerve injury of different sites, especially that the injuries of radial nerve and median nerve recover more obviously. ②Percutaneous electric stimulation can ameliorate the indexes of EMG examination, especially the recruitment, in postoperative patients with peripheral nerve injury. 展开更多

Effects of pulsed radiofrequency on spasticity in patients with spinal cord injury:a report of two cases 被引量:3

Spasticity following spinal cord injury(SCI) results in functional deterioration and reduced quality of life. Herein, we report two SCI patients who presented with good response to pulsed radiofrequency(PRF) for the management of spasticity in the lower extremities. Patient 1(a 47-year-old man) had complete thoracic cord injury and showed a phasic spasticity on the extensor of both knees(3–4 beats clonus per every 30 seconds) and tonic spasticity(Modified Ashworth Scale: 3) on both hip adductors. Patient 2(a 64-yearold man) had incomplete cervical cord injury and showed a right ankle clonus(approximately 20 beats) when he walked. After the application of PRF to both L2 and L3 dorsal root ganglion(DRG)(patient 1) and right S1 DRG(patient 2) with 5 Hz and 5 ms pulsed width for 360 seconds at 45 V under the C-arm guide, all spasticity disappeared or was reduced. Moreover, the effects of PRF were sustained for approximately 6 months with no side effects. We believe that PRF treatment can be useful for patients with spasticity after SCI. 展开更多

Non-Invasive Treatment to Grade 1 Essential Hypertension by Percutaneous Laser and Electric Pulse to Acupoint with Music:A Randomized Controlled Trial 被引量:3

Objective: To study a non-drug therapy for hypertension disease by combining percutaneous laser and electric pulse stimulation to acupoint with music, and to test the efficiency of the combining treatment to grade 1 essential hypertension. Methods: A total of 174 patients with grade 1 essential hypertension were randomly assigned to 3 groups with a random number table after Chinese medicine(CM) syndrome differentiation: the photoelectric and musical treatment group(Group 1, with a self-developed multi-mode audio frequency pulse photoelectric therapeutic apparatus), acupuncture group(Group 2), and oral placebo group(Group 3), 58 cases per group. The curative effect of each group was evaluated by the changes of blood pressure and CM syndrome integral before and after treatment. Results: Compared with Group 3, there were significant decrease of blood pressure and CM syndrome integral in Group 1 and Group 2(P〈0.01). Compared with Group 2, Group 1 showed the highest decrease in systolic pressure(P〈0.017). The total effective rate of anti-hypertension in Group 1(91.38%, 53/58) was significantly higher than that in Group 2(74.13%, 43/58) and Group 3(18.97%, 11/58, P〈0.05 or P〈0.01); and that in Group 2 was also significantly higher than that in Group 3(P〈0.01). There were significant difference in the total effective rate of CM syndrome integral in both Group 1(93.10%, 54/58) and Group 2(84.48%, 49/58) as compared with Group 3(17.24%, 10/58, P〈0.01), while there was no significant difference between Group 1 and Group 2(P〉0.05). Conclusions: The multi-mode audio frequency pulse photoelectric therapeutic apparatus, combining music, laser and electric pulse stimulation, is clinically useful for grade 1 essential hypertension. This "three in one" therapy method is non-invasive, easy and simple to handle. It is expected to be popularized as a new alternative treatment. 展开更多

We present a method by which to determine the bulk viscosity of water from pulse duration measurements of stimulated Brillouin scattering (SBS). Beginning from a common model of Brillouin scattering, the bulk viscosity is shown to play an important role in Brillouin linewidth determination. Pulse durations of SBS back-reflected optical pulses are measured over the temperature range of 5-40℃. SBS linewidths are de- termined via Fourier transformation of the time-domain results, and the bulk viscosity of water is measured and derived from the obtained values. Our results show that the proposed method for measurement of pulse durations is an effective approach for determining bulk viscosity. The method can be easily extended to determine bulk viscosities of other Newtonian liquids. 展开更多

Thermal effects are typically considered as obstacles to high-repetition-rate stimulated Brillouin scattering(SBS)pulse compression.In this paper,a novel method is proposed for improving the SBS output characteristics by exploiting thermal effects on the liquid medium.Using HT270,the SBS output parameters with the medium purification and rotating off-centered lens methods are studied at different repetition rates.The results indicate that these two methods can alleviate thermal effects and improve the energy efficiency,but the rotating method reduces the energy stability because of the aggravated optical breakdown at the kilohertz-level repetition rate.For a 35-mJ pump energy,the energy efficiency at 2 kHz without the rotating method is 30%higher than that at 100 Hz and 70%higher than that at 500 Hz.The enhancement of the SBS output characteristics by thermal effects is demonstrated theoretically and experimentally,and 2-kHz high-power SBS pulsecompression is achieved with HT270. 展开更多
