Clinical Safety and Efficacy of Platelet-Rich Plasma in Wound Healing

Background: Platelet-rich plasma has been extensively used in several clinical settings. However, there still a lack of conclusive evidence concerning the benefits of platelet-rich plasma in the field of wound healing. We aimed to evaluate the safety and the efficacy of autologous platelet-rich plasma in acute wound healing. Methods: This prospective study enrolled forty adult patients of both sexes and aged between 18 - 50 years. All patients in need for split-thickness skin graft were included in our study. The donor sites were randomly divided into two equal halves: the platelet-rich plasma side, which was injected with recently activated platelet-rich plasma;and the control side, in which the conventional method of dressing was used. Measurement of the platelet count and transforming growth factor-B1 concentration in each platelet-rich plasma preparation and the whole blood was done for all patients. Clinical monitoring of the donor sites was done every 7 days for 3 weeks, regarding pain perception, epithelialization surface area and possible side effects of the platelet-rich plasma. Histopathological monitoring was done on the 7th postoperative day. Results: The platelet count was increased about 3.5 folds and transforming growth factor-B1 was increased 2.4 folds in the platelet-rich plasma compared to the patients’ blood. The platelet-rich plasma side had significantly lower pain scores at day 7 (4.8 ± 0.18 vs 5.9 ± 0.07) and day 14 (1.4 ± 0.11 vs 1.9 ± 0.09) postoperative (p = 0.002 and p = 0.004, respectively) and had significantly higher rate of epithelialization at day 7 (9.8 ± 0.35 cm<sup>2</sup> vs 7.5 ± 0.32 cm<sup>2</sup>) and day 14 (38.4 ± 0.36 cm<sup>2</sup> vs 36.9 ± 0.42 cm<sup>2</sup>) postoperative (p p = 0.039, respectively), while at day 21 postoperative, there was no significant difference between both sides. There was no significant difference between both sides regarding the incidence of complications. The platelet-rich plasma side showed intact epithelium, differentiation of the cells in stratum spongiosum and stratum granulosum, neovascularization and earlier collagen deposition. Conclusion: The platelet-rich plasma is safe and effective adjuvant in the management of acute wounds. However, we recommend for larger clinical trials for standardized method for PRP preparation and better understanding of the efficacy of this blood product. 展开更多

The management of diabetic wounds remains a critical therapeutic challenge. Platelet-rich plasma (PRP) gel, PRP-derived exosomes (PRP-Exos), and mesenchymal stem cell-derived exosomes (MSC-Exos) have demonstrated therapeutic potential in wound treatment. Unfortunately, their poor mechanical properties, the short half-lives of growth factors (GFs), and the burst release of GFs and exosomes have limited their clinical applications. Furthermore, proteases in diabetic wounds degrade GFs, which hampers wound repair. Silk fibroin is an enzyme-immobilization biomaterial that could protect GFs from proteases. Herein, we developed novel dual-crosslinked hydrogels based on silk protein (SP) (sericin and fibroin), including SP@PRP, SP@MSC-Exos, and SP@PRP-Exos, to promote diabetic wound healing synergistically. SP@PRP was prepared from PRP and SP using calcium gluconate/thrombin as agonist, while SP@PRP-Exos and SP@MSC-Exos were derived from exosomes and SP with genipin as crosslinker. SP provided improved mechanical properties and enabled the sustained release of GFs and exosomes, thereby overcoming the limitations of PRP and exosomes in wound healing. The dual-crosslinked hydrogels displayed shear-induced thinning, self-healing, and eradication of microbial biofilms in a bone-mimicking environment. In vivo, the dual-crosslinked hydrogels contributed to faster diabetic wound healing than PRP and SP by upregulating GFs expression, down-regulating matrix metalloproteinase-9 expression, and by promoting an anti-NETotic effect, angiogenesis, and re-epithelialization. Hence, these dual-crosslinked hydrogels have the potential to be translated into a new generation of diabetic wound dressings. 展开更多

Platelet-rich plasma(PRP)can stimulate the proliferation of stem cells and have a positive effect on tissue repair.Although many commercialized PRP preparation kits are already on the market,first-line clinical workers are still not satisfied with most of the PRP kits.The work of commercial PRP kits is based on the density of blood elements.However,the blood elements are very close in density which makes the separation challenging.Therefore,the mentioned commercialized kits are generally contaminated by leucocytes and erythrocyte.In this study,a home-designed PRP device was developed to use a separation membrane with adequate cut-off pore size of 5μm,3μm and 2μm for the groups of H5M,H3M,and H2M,respectively,to be placed in the middle of the centrifuge tube.The home-designed H2M showed a very promising results regardless of the final volume(1.82±0.09 ml),platelet yield(8.39±0.44%),Red Blood Cells(0%),White Blood Cells(0%),and Relative Concentration of Platelet Increment value(225.09%).Further,it showed a good result in cell viability and cytotoxicity and confirmed a good multilineage potentials.The concentration in PRP prepared by group H2M was relatively stable and far above average.All the fibrin fibers were linked together as bridging strands or strings and turned into an inter-connected porous structure for nutrients transportation and regenerative cell migration.We believe that the home-designed group H2M should have a great potential to develop into the final product to meet the requirements of first-line clinical workers. 展开更多

目的 回顾性分析256例经血细胞分离机采集富血小板血浆(platelet-rich plasma, PRP)的安全性以及产品质量的实用性,为PRP的安全采集和产品的质量控制提供一定参考。方法 统计分析2021年6月—2022年6月在本院行PRP治疗的256名患者资料,... 目的 回顾性分析256例经血细胞分离机采集富血小板血浆(platelet-rich plasma, PRP)的安全性以及产品质量的实用性,为PRP的安全采集和产品的质量控制提供一定参考。方法 统计分析2021年6月—2022年6月在本院行PRP治疗的256名患者资料,分析不同性别、不同年龄段以及不同血小板计数值的患者使用血细胞分离机采集自体富血小板血浆时,在采集时间、循环血量以及献血不良反应发生上的差异,以及PRP产品中血小板含量、红细胞和白细胞残存量上存在的差异。结果 1)不同性别、不同年龄段和不同血小板计数的患者在采集时间、循环血量、采集量上均差异不明显(P>0.05);2)男性和女性患者采集后的产品白细胞、红细胞残存量和血小板含量亦差异不明显(P>0.05);3)采集产品中白细胞含量随着年龄段的增加而增高,其中年龄较高组(组3,56~78岁)白细胞残存量(0.64±0.41)×10^(9)/L明显高于年龄较低组(组1,18~40岁)(0.50±0.35)×10^(9)/L,差异具有统计学意义(P<0.05);4)血小板较低组(组1,100~150)×10^(9)/L采集产品中白细胞、红细胞残存量均高于其它血小板计数组,且差异具有统计学意义(P<0.05),同时产品中的血小板计数也显著低于其它血小板计数组(P<0.05)。结论 在不同性别、年龄段以及血小板计数在(100~450)×10^(9)/L的患者中使用血细胞分离机采集的PRP产品能满足临床治疗需求。 展开更多

目的 探讨局部应用富血小板血浆(PRP)对皮瓣成活的影响.方法 取新西兰大白兔15只,将每只兔子背部两侧对称各取一块随意皮瓣,随机选择一侧为观察组,给予皮瓣基底部均匀涂抹PRP,另一侧为空白对照组,皮瓣基底均匀涂抹等量的0.9%氯化钠... 目的 探讨局部应用富血小板血浆(PRP)对皮瓣成活的影响.方法 取新西兰大白兔15只,将每只兔子背部两侧对称各取一块随意皮瓣,随机选择一侧为观察组,给予皮瓣基底部均匀涂抹PRP,另一侧为空白对照组,皮瓣基底均匀涂抹等量的0.9%氯化钠注射液.于术后3,7,14d计算皮瓣成活率,取皮瓣标本行大体观察及苏木精-伊红(HE)染色观察、免疫组织化学检测CD34计算血管密度.结果 皮瓣成活率:3d观察组(74.4±4.7)%,空白对照组(65.8±6.8)%;7 d观察组(72.4±7.5)%,空白对照组(58.5±7.0)%;14 d观察组(74.5±5.0)%,空白对照组(65.0±5.4)%.HE染色,可见皮瓣远端和蒂部组织随着时间的延长,炎症反应越来越轻,血管密度增加,14 d炎症反应最轻,血管密度最大.观察组炎症反应均较空白对照组轻.CD34阳性计数,3d观察组血管密度(13.9±2.0)个/高倍视野,空白对照组(11.1±1.3)个/高倍视野;7d观察组血管密度(15.7±1.5)个/高倍视野,空白对照组(12.1±1.2)个/高倍视野;14 d观察组血管密度(19.6±1.2)个/高倍视野,空白对照组(12.7±0.8)个/高倍视野,观察组与空白对照组差异均有统计学意义.结论 PRP能够促进兔随意皮瓣成活. 展开更多

背景:富血小板血浆(platelet-rich plasma.PRP)中的生物调节因子以一种细胞特有的方式调节细胞的增殖和分化,继而增强组织的修复和再生能力。已知PRP中浓缩的生长因子可上调细胞活动,进而促进体内牙周组织的再生。本研究旨在评价... 背景:富血小板血浆(platelet-rich plasma.PRP)中的生物调节因子以一种细胞特有的方式调节细胞的增殖和分化,继而增强组织的修复和再生能力。已知PRP中浓缩的生长因子可上调细胞活动,进而促进体内牙周组织的再生。本研究旨在评价和比较激活的PRP加入结缔组织移植术(connective tissue graft,CTG)中治疗牙龈退缩的临床效果。方法:基本手术方法是CTG和冠向复位瓣术所采用的方法。对15例MilletⅠ度或Ⅱ度牙龈退缩病损用CTG和PRP进行联合治疗(CTG+PRP组).在翻开龈瓣后,用激活的PRP凝胶涂抹牙槽骨及根面.将采集到的结缔组织亦先用PRP凝胶冲洗.再移植放置于裸露的根面上.缝合固定移植的结缔组织后.将龈瓣冠向复位,覆盖结缔组织。对相同患者的另外15例牙龈退缩病损.单纯采用CTG和冠向复位瓣术治疗(CTG组)。在膜龈手术前及术后6个月分别记录退缩垂直高度(VRD)、探诊深度(PD)、临床附着丧失(CAL)以及角化组织宽度(KTW):并在术后第1、2、3周分别记录愈合指数(HI).以评价临床愈合状况。结果:在CTG+PRP组,VRD平均值由(361±0.70)mm显著降低至(0.30±0.45)mm(P〈0.01)(平均根面覆盖达91.68%);在CTG组,VRD平均值由(345±0.84)mm显著降低至(0.38±0.48)mm(P〈0.01)(平均根面覆盖88.96%).但两组之间的差异无统计学意义。KTW测量结果显示.在CTG+PRP组.KTW由术前(1.32±0.66)mm显著增加至术后6个月的(3.20±0.54)mm(P〈0.01).CTG组由(1.41±0.58)mm增至(255±0.45)mm(P〈001).两组间的差异有统计学显著性(P〈0.05)。两组间PD及CAL降低值的差异无统计学显著性。术后第1、2周记录的HI值.CTG+PRP纠(311±0.32)mm、(4.20±0.27)mm]显著高于对照纠(225±0.54)mm、(3.05±0.38)mm]。结论:本研究结果进一步证实了这两种方法在解决牙龈退缩问题中均是有效且具有高度可预测性的手术方法,PRP的加入可增加角化组织宽度.并可促进组织愈合。 展开更多
