In these circumstances, more than 25% of the patients are women of reproductive age. The aim of our research was to analyze the effectiveness and safety of antiepileptic therapy in women with epilepsy during pregnancy and to analyze the pregnancies’ outcomes. We included in our research 121 pregnancies of 101 women aged at the moment of childbearing about 26.9 ± 4.57 years old. Idiopathic forms of epilepsy were predominant among all causes—47.1% (р < 0.01). Of all cases, 65.4% remained seizure-free from generalized tonic-clonic seizures (GTCS), including 69.6% of all idiopathic epilepsy cases and 68.6% among symptomatic ones. The antiepileptic drugs (AED) dosages were exceeding teratogenic level at the moment of conception in 54.7% of the cases. Worse control of epileptic seizures was associated with Benzobarbital (66.7%) and Lamotrigine (50.0%). Women with epilepsy did not receive specialized neurological therapy before conception in most cases, which leaded to the usage of AED teratogenic doses and less effectiveness of AED during pregnancy. It is necessary to plan the pregnancy and prescribe rational treatment for epilepsy starting at the stage of planning and during gestation in order to obtain a better seizures control and to decrease congenital disorders risk in fetus.展开更多

In Chinese medicine, emotional disorders and mental illnesses are classified as emotion-will (Qing Zhi) disorders. Emotion-will, i.e. seven emotions (Qi Qing: happiness, anger, anxiety, pensiveness, sorrow, fear, and fright) and five wills (Wu Zhi: happiness, anger, thinking, sorrow, and fear), play a basic role in the onset, progress and prognosis of almost all diseases, not only the mental illnesses. The emotion-will overcoming therapy (EWOT) is defined as a psychological approach that a therapist employs single or multiple emotions to overcome and eliminate patients’ abnormal morbid emotions and to heal mind-body disorders derived from the abnormality. EWOT lays a foundation for the philosophical foundation of Chinese medicine, i.e. Yin and Yang, and five elements, which is believed to be the most commonly utilized and effective modality in dealing with mental illnesses. This essay covers the origin and development, underlying mechanism, clinical application, basic researches of emotion-will system and EWOT and the comparison with conventional therapies. Thus, EWOT could draw more attention in the field of psychology and spread in clinical practice.展开更多

Accordingly, we contend that, given that emotion is a primal binding force, the bonding component of the crucially important therapeutic alliance risks being compromised in the current practice of this approach. We further argue that the prioritization of the depiction of actions that accompany the emotion expressed by the client over the actual participation in this emotion by the therapist tends to superficialize therapeutic communication. This is especially likely to happen when the therapist lapses into a formulaic, action-soliciting interrogatory mode in response to the emotion expressed by the client. In addition, we claim that the incorporation of emotion- and body-based approaches into the solution-focused canon could remedy the affective lacuna referred to above. We note, however, that such a modification would most likely face stiff resistance from some of the gatekeepers of solution-faced therapy, in particular those who subscribe to the “surface-only” descriptive approach advocated by the philosopher Ludwig Wittgenstein. And yet, the effectiveness of depth-oriented therapies, such as sensorimotor psychotherapy and somatic experiencing, is supported by neurophysiological considerations. Finally, we suggest that these therapies, and even creativogenic elements of psychoanalysis, can actually synergize solution-focused brief therapy through the generation of affect-laden images, sensations and thoughts that lend themselves to the realization of outcomes desired by the client, which is the raison d’etre of solution-focused brief therapy. This paper pleads for the incorporation of the above-noted depth approaches into solution-focused brief therapy with a view to augmenting its effectiveness through a stronger therapeutic bond owing to an increased emotional engagement on the part of the therapist.展开更多

Design: A cross-sectional study. Methods: Nursing sample (n = 435). Participants completed a self-report online survey, which included demographic information, followed by questionnaires to measure personality traits, thinking styles, and emotional intelligence. Results: Spearman’s rank correlation was computed to assess the relationship between EI and Extraversion;there was a moderate positive correlation between the two variables, r = 0.487, p r = 0.731, p r = 0.723, p r = -0.666, p r = 0.467, p Conclusion: Different studies consolidated each other, and all converge and channel into the concept of characterization of healthcare providers for better support to them and safer patient care. EI correlated with all BFI components, and both positively impacted all desirable behaviors. Therefore, it would be valuable if organizations invested in increasing EI in their providers as it might highlight areas for improvement and equip providers with appropriate and advantageous coping strategies.展开更多

文摘Epilepsy is one of the most frequent neurological disorders. It is necessary to plan the pregnancy and prescribe rational treatment for epilepsy starting at the stage of planning and during gestation in order to obtain a better seizures control and to decrease congenital disorders risk in fetus.

文摘Background:Recently, cognitive behavioral therapy interventions have been extensively applied in the field of nursing education. However,no consensus has been reached on whether these interventions can reduce negative emotions in nursing students. Objective:This meta-analysis was designed to systematically discuss the effect of cognitive behavioral therapy interventions on levels of anxiety, fear of denial, social avoidance and social distress for nursing students. Methods:The following Chinese and English databases were searched for relevant articles:PubMed,Embase,Cochrane library,Web of Science,China National Knowledge Infrastructure,Wangfang Database,VIP Database and Chinese Biomedical Literature Database. The search encompassed the establishment of these databases up until May 2021.Two reviewers separately entered the data into Review Manager Software 5.3.Results:A total of 6 studies were reviewed. It was found that cognitive behavioral therapy interventions significantly lowered levels of anxiety(SMD=−0.98,95%CI:−1.46 to−0.50,P<0.001),fear of denial(MD=−3.22,95%CI:−5.60 to−0.84,P<0.01),social avoidance(MD=−1.74,95%CI:−2.32 to−1.16,P<0.001),and social distress(MD=−2.15,95%CI:−2.81 to−1.49,P<0.001)in nursing students. Conclusion:Cognitive behavioral therapy could effectively decrease anxiety, fear of negation, and social negative emotions of nursing students as well as enhance their mental health level. However, more studies are needed to verify whether internet-based cognitive behavioral therapy has an impact on nursing students’negative emotions.
文摘The latest global cancer burden data released by the International Agency for Research on Cancer of the World Health Organization in 2020 shows that there were 19.29 million new cancer cases worldwide, with 4.57 million in China, ranking first. The number of cancer survivors is increasing, with a 5-year survival rate exceeding 85%,but there are emotional disorders.Cognitive behavioral therapy(CBT)can improve negative emotions and has significant effects on patients. However, there is a limited number of physicians and high costs,so internet interventions have become a solution. The feasibility of web-based interventions for breast cancer patients has been proven.Research on internet-delivered CBT is also increasing. The purpose of this study was to review the concept of web-based CBT and its application status in cancer survivors,in order to provide relevant intervention for scholars and provide reference and supplement for patients to provide psychological therapy.
文摘Background: This study explored nursing personality traits (Big Five Inventory BFI), emotional intelligence (EI), and thinking styles (Rational, RS, and Experiential, ES) together with demographic data to see how they could relate and the implication of this on nurses and patient safety. Therefore, it would be valuable if organizations invested in increasing EI in their providers as it might highlight areas for improvement and equip providers with appropriate and advantageous coping strategies.
文摘BACKGROUND Patients with chronic hepatitis B(CHB)experience various problems, including low psychological flexibility, negative emotions, and poor sleep quality. Therefore, effective nursing interventions are required to reduce adverse events.Acceptance and commitment therapy(ACT)combined with enabling cognitivebehavioral education(ECBE)can improve patients'psychological and sleep. Therefore,we speculate that this may also be effective in patients with CHB.AIM To investigate the effects of different intervention methods on psychological flexibility, negative emotions, and sleep quality in patients with CHB.METHODS This retrospective study examined clinical and evaluation data of 129 patients with CHB.Intervention methods were divided into a conventional group(routine nursing,n=69)and a combination group(ACT combined with ECBE,n=60).We observed changes in psychological flexibility, negative emotions, sleep quality, and self-care ability in both groups.Observation items were evaluated using the Acceptance and Action Questionnaire-2nd Edition(AAQ-II),Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS),Pittsburgh Sleep Quality Index(PSQI),and Exercise of Self-Care Agency Scale(ESCA).RESULTS Compared with the conventional group, the AAQ-II score of the combined group was lower(F_(between-group effect)=8.548;F_(time effects)=25.020;F_(interaction effects)=52.930;all P<0.001),the SAS score(t=5.445)and SDS score(t=7.076)were lower(all P<0.001),as were the PSQI dimensions(tsleep quality=4.581,tfall sleep time=2.826,tsleep time=2.436,tsleep efficiency=5.787,tsleep disorder=5.008,thypnotic drugs=3.786,tdaytime dysfunction=4.812);all P<0.05).The ESCA scores for all dimensions were higher(thealth knowledge level=6.994,t self-concept=5.902,tself-responsibility=19.820,tself-care skills=8.470;all P<0.001).CONCLUSION ACT combined with ECBE in patients with CHB can improve psychological flexibility and sleep quality, alleviate negative emotions, and improve self-care.
