With the accelerating process of population aging in our country, the physical function of the elderly will decline, the possibility of suffering from acute and chronic diseases will increase, and the occurrence of di...With the accelerating process of population aging in our country, the physical function of the elderly will decline, the possibility of suffering from acute and chronic diseases will increase, and the occurrence of disability will become more common and normal. The potential demand for elderly care is rapidly increasing. With the growth, the care of the disabled elderly has become increasingly acute. At the same time, due to the intensification of family miniaturization and population migration, the traditional family-based long-term care has been difficult to sustain. How to incorporate long-term care into our social security system as soon as possible has become the focus of the whole society. The thesis through the combination of public policy simulation methods and the research data, the long-term care insurance system implemented at the beginning of 2018 in Shanghai is a policy blueprint. It uses statistical software SPSS, MATLAB, etc. to achieve data collation, analysis and prediction, and at the same time integrates the pilot of the long-term care insurance system. The implementation of policies in cities such as Qingdao and Nantong has been simulated. Finally, on the basis of simulation results, suggestions have been made for the future development of China's long-term care insurance system.展开更多
With decades of hard work,socialism with Chinese characteristics has crossed the threshold into a new era.The transformation of the principal contradiction in the new era has imposed new and higher requirements on the...With decades of hard work,socialism with Chinese characteristics has crossed the threshold into a new era.The transformation of the principal contradiction in the new era has imposed new and higher requirements on the rural elderly care service system.However,the current service system based on welfare-multiplex and urban community environment is often difficult to operate in the rural areas of central and western China.And the urban home care service for the aged is inappropriate to the rural reality;community service for the aged faces difficulties in practice;institution care for the aged develops slowly;the synergy among different elderly care service models is poor.Through observing the rural areas of central and western China in the field for a long time,this paper proposes to build the rural elderly care service system based on family supporting,supported by community mutual assistance care,supplemented by institution care for the aged,combined with medical care,and localized with diversified forms for targeted groups according to different realities.And the system should follow the fundamental guidance of development-oriented family policy,adhere to the governance mode of multiple subjects with one core and respect rural reality,which could provide a feasible way for the reform and reconstruction of the rural elderly care service system.展开更多

On the one hand, Indonesian demogr...A closer look to the state of affairs of the elderly in Indonesia has disclosed the fact that the Indonesian elderly facing what this paper is coined as "multiple vulnerabilities." On the one hand, Indonesian demographic is showing that the Indonesian people are getting old. The fertility rate has significantly decreased since 1970 when the total fertility rate decreases from 5.0 to 2.1 in 2025. This alone gives rise to various problems of the elderly, among whom are the dependency ratio, aging index, potential support ratio, and the feminization of aging. These problems show the level of vulnerability of the Indonesian elderly. From the perspective of the dependency ratio, for example, the number of people under the age of 15 and above 65 who depend on the labor force (15-64 years) has increased since 1970, from 7.0 to 29.0 in 2050. Again, this shows the high level of vulnerability of Indonesian elderly given that the dependency ratio itself has influenced on health care, living arrangements and family support, and so on. Aside from that, the economic burden will be very visible in the workforce. On the other hand, the policies on the elderly are still far from maximum realization. Law on the Welfare of the Elderly (13/1998) has recommended that every hospital must have geriatrics and gerontology section, yet only 8 hospitals fulfill the requirement. The situation is worsened due to the lack of adequate medical staff at the health center, which can serve elderly patients in a professional and humane way. Again, this brings forth the other side of the state of the elderly in Indonesia, which indicates the high level of their vulnerability. In this paper I will analyze the state of vulnerability of the elderly people in Indonesia from the perspective of ethics of vulnerability and personalism. I consider these two approaches as representing all ethical discussion about the ethical treatment of the elderly due to their characters of valuing the elderly as persons with dignity. Thus, this paper will show that the elderly in Indonesia is in danger of not being treated humanely.展开更多

Within the context of the national policy on the elderly,the paper presents trends in demographic ageing,health and life quality of the elderly in Russia.Social and political consequences of the XX century have result...Within the context of the national policy on the elderly,the paper presents trends in demographic ageing,health and life quality of the elderly in Russia.Social and political consequences of the XX century have resulted in significant fluctuations in the number of individual generations,forming a long-term fluctuating trend in demographic ageing in Russia.These historical disproportions have overlapped with regional variations in ageing rates due to significant inter-regional population movements,rural-urban migration,substantial gender differences in trends and scales of mortality and fertility dynamics.As a result,levels of demographic ageing in Russia vary more than three-fold,worsening in females and rural population.Ageingrelated problems are aggravated by unsatisfactory health status of the elderly,manifested in high burden of chronic diseases(especially cardiovascular diseases),leading to life restrictions up to loss of the ability to self-care.High-tech cardiac surgery contributes to life prolongation and improved physical and psychological functioning.However,during rehabilitation,most patients are reluctant to change their unhealthy lifestyle associated with cardio pathology.Polymorbidity and psychoactive substance abuse(in males)are the risk factors of suicidal behaviour,while accessibility of medications,including sleeping pills,firearms,etc.contributes to realization of this risk.Living conditions of the elderly and their access to quality social services are important components of wellbeing in terms of health.Despite satisfactory living conditions,the situation significantly differs by type(urban/rural)and size of the settlement.Maintaining microsocial contacts with children,family and friends is a powerful source of psychological comfort and social wellbeing.展开更多

文摘从国际比较的角度,回顾社区研究、社区服务及有关历史,反思中国的"社区"及其在社会政策实践中的作用。分析中西方养老模式与理念之异同以吸取有益经验,对中国式"社区养老"与西方的community care(社区照顾)以及近年来流行的aging in place(AIP,就地养老)作了回顾比较,并在更广阔的社会经济背景下探讨未来发展趋势。社区养老将成为中国养老方式的主流,但社区养老不是变相家庭养老。家庭的养老功能急剧减弱,不久的将来,当大批"空巢"(包括孤独鳏寡)老人进入高龄阶段后,尤其是需要长期照顾(long-term care)的时候,"以居家为基础"更不能简单理解为以自我或家庭照顾为主,居家社区服务必须跟上,机构养老一床难求的局面要尽早改变。正视独生子女政策以及城市化发展的后续问题,针对即将到来的巨大社会养老需求,大力发展社会服务(包括机构养老),这是非常时期应对"潮独"(婴儿潮出生、成为独生子女父母)一代特别突出的"空巢"家庭问题积极开展研究的必然思路。综合社会服务体系框架设计、分户不分居的代际支持方式、城乡老年人健康分化等,都是与社区养老密切相关的研究课题。中国总体公共政策已经由"经济国家"向"后经济国家"转型,为社区养老事业朝正确方向发展提供了历史条件,国家"十三五"规划为此做了新的战略部署。政策研究还要注意避免这种有针对性的养老举措在将来过时而造成国家负担过重。
