Complex Regional Pain Syndrome: Outpatient Pain Management in the Chronic Setting: A Case Report

CRPS can be a d... Background: Complex Regional Pain Syndrome (CRPS), previously known as reflex sympathetic dystrophy and causalgia, is a neuropathic pain condition that usually develops after an injury to an extremity. CRPS can be a debilitating condition with high levels of pain and reduced function. Aim: This case report aims to discuss the multimodal approach in the management of a patient who presented with ongoing poorly controlled pain secondary to CRPS from an injury that happened years prior. Case Presentation: A 45-year-old female was involved in a motor vehicle accident where her right leg was injured. She underwent several surgeries and developed CRPS that significantly reduced her mobility and quality of life. She presented to the pain clinic years after her initial injury and a multimodal regimen was started for her resulting in significantly improved function. Conclusion: CRPS can be a severely debilitating condition. While early diagnosis and management are important, ongoing management in the outpatient chronic pain setting is important in maintaining a good level of function. 展开更多

Tourism is not only a significant tool of prosperity in well-known tourist destinations, but it is also an important potential development element in developing rural regions. In Czech Republic, the field of regional development and managing destinations started to develop in the second half of the 1990s. Organizing tourism is in the phase of building and passes through natural development. Fierce competition between individual destinations imposes natural pressures on creating marketing managements of these destinations and such offer of products that perfectly come up to tourists' expectations. Together with increasing requirements, the demands on quality and well-prepared destinations within home tourism revive. In the final consequence, the level of destination management influences the increase in marketability and attendance in the region. The paper outlines the main theoretical basis, principles and tools of destination management, and subsequently it summarizes the results of the analysis, and conducted survey, the objective of which was to prove the functioning of these principles in practice. The practical verification of the functioning of destination management principles in Czech Republic has been conducted in the South-Moravian region (JMK) which is one of the most-often-visited regions in the country. The set targets have been reached not only by analysing available secondary data, but the techniques of depth interviews with experts in a destination management company have also been used, as well as the techniques of an orientation questionnaire survey among travel agencies and offices that should be one of the partners for a well-functioning destination management company. 展开更多

The Atlantic Ocean houses three of these: the Northwest, Southwest and Northeast RMUs. The most studied is, so far, the Northwest RMU, but the other two have focused attention of researchers. In contrast, marine turtles from the African Atlantic region (Southeast) belong to a complex but little-known region. What is their role in the Atlantic? Are these RMUs connected? To understand these questions, research was made of reports and publications concerning the Northeast Atlantic (NEA) RMU. The asymmetry in information and available knowledge about NEA is high when compared to other RMUs. This demonstrates that there is still a lack of conservation programs besides Cape Verde, and that there is difficulty in transforming data (nesting ecology, molecular, telemetry, etc.) into publications. However, this issue is evident for other marine turtle species of East African Atlantic. There is a need for better scientific support, to enable local conservation programs to deliver data in reports, and even scientific publications. There are so many aspects of loggerhead’s life cycle that will only be unravelled by more and better supported studies. This is a paper that, by analysing the available information of the NEA loggerhead RMU, reflects the following steps to address in loggerhead conservation for the African Atlantic coast. 展开更多

In this article,it discusses the di£ferences in economic development between urban and rural areas and regions in our country from the perspective of education investment and fixed asset investment.Based on the provincial data of 31 provinces from 1999 to 2017 released by National Bureau of Statistics,it expends the Cobb-Douglas model and Lucas model,and analyses the data with multiple linear regression models.From the study,it finds that compared with investment in fixed assets,investment in education has a larger role in promoting economic development,which is more obvious in the underdeveloped central and western regions and rural areas.However,at the same time it needs to note that the positive effects of education investment will be restricted by the economic structure and policy environment,and education expenditure policies should also be implemented in accordance with time and local conditions. 展开更多

As mountain tourism service research is a professional field with several disciplines involved, a multi-disciplinary management pIatform is needed and it facilitates participation in sustainable mountain development by diverse stakeholders. With the source regions of the Yangtze and the Yellow River as a case study, this paper presents a conceptual framework for an adaptation management of mountain tourism services according to technical, policy, social and economic dimensions. The framework is based on a vulnerability assessment of mountain ecosystems, and can serve as a reference for the development of tourism service in other mountain areas. 展开更多

Rapid growth of Chineseeconomy comes mainly from investment. There are two main ways for obtaining high growth ofinvestment. One is government expenditure which usually invests in regional facility and amenityblock, which is regarded as stimulus for attracting investment. The other is the creation ofinvesting center and corresponding capital source areas, both of which need the central city withthe highest growth rate of investment among regions. Taking an example of Majiayu Catchment Area (14.15 ha) in Taoyuan County of HunanProvince, the soil and water resources dynamics, fertility evolution characteristics andland productivity changing situation were studied. Fixed observation results from 1993to 2002 showed that pools covering about 15% of total area could store up 10% of surfacerunoff, keep 78.1% of eroded soil and 65.4% of lost nutrients. The yearly ratio ofinterception and evapotranspiration in land, storage in pools and drainage was 7:2:1,which ensured the resources and nutrients equilibrium and a benign recycle in thecatchment area system, and benefited the aquatic culture and helped to resist seasonaldrought. Moreover, the results showed that soil erosion modulus decreased significantly,equal to or lower than soil loss tolerance (≤500 tkm-2) in reddish yellow soil regions.Soil organic matter, total and available N content in sloping land, dryland and paddyfield increased steadily (>10%); water storage enhanced by more than 20% in sloping landand dryland in drought season; crop production increased by more than 20%; and productionof trees, fruits, tea and fish as well as land productivity increased yearly. 展开更多

Development is an ever growing process and its impact is often detrimental to environment which in turn may lead to adverse consequences for human life. Environment Impact Assessment (EIA) is an anticipatory and participatory environment management tool which helps in the decision-making process for different upcoming development projects by analyzing the environmental consequences of the action. The large scale projects like setting up of Special Investment Regions (SIR), Special Economic Zones (SEZ) and Industrial Estates (IE) have cumulative impacts on the environmental condition of the region. This paper intends to describe different practices of impact assessment and seeks to direct this debate towards the potential benefits of applying Regional EIA over the project specific EIA to capture the cumulative impact of large development projects like SIRs and IE on the environmental condition of the region and underline the challenges often faced during the study. Regional EIA is a new concept in India and the process of development of an SIR is different from the other industrial projects such as SEZ or IE. Unlike SEZ which comprises of only industrial activity, SIR also includes residential commercial and other amenities over an area of more than 100 square kilometers. Further, the implementation of Regional EIA in Indian context is also constrained by problem in baseline data collection and lack of exhaustive ecological and socioeconomic indicators for impact assessment and restricted use of state of art technologies such as Geographical Information System due to many factors such as cost involved and lack of awareness among consultants. A study on community soil fertility management concepts and practices was conducted through rapid and participatory method of research in 15 villages of the Gourma region in Eastern part of Burkina Faso. These investigations aimed at determining farmers' knowledge on soil resources: local soil taxonomy and indicators of soil degradation, soil fertility management practices, and capacity for adoption of new technologies in soil fertility management. The results of the study showed that the main parameter for soil classification for all the investigated villages was soil texture composition; soil degradation was evaluated according to crops yield decrease and the development Striga sp. in 100% of the investigated villages; the use of organic manure and long term fellow were the main practices for soil fertility management; concerning the villages where modem technologies of soil fertility management were introduced, the lack of tools and capacity building were the main limiting factors of the adoption of these improved practices at small scale farmers' level. Even if discordance between scientific and local soil taxonomy were revealed by our investigations; significant similarities between indigenous and scientific indices of soil degradation were noticed. With regard to the actual magnitude of soil degradation; the local techniques of soil fertility management need to be improved and accessible to a big number of farmers. 展开更多

Changes in both soil organic C storage and soil respiration in farmland ecosystems may affect atmospheric CO2 concentration and global C cycle. The objective of this field experiment was to study the effects of three crop field management practices on soil CO2 emission and C balance in a cotton field in an arid region of Northwest China. The three management practices were irrigation methods(drip and flood), stubble managements(stubble-incorporated and stubble-removed) and fertilizer amendments(no fertilizer(CK), chicken manure(OM), inorganic N, P and K fertilizer(NPK), and inorganic fertilizer plus chicken manure(NPK+OM)). The results showed that within the C pool range, soil CO2 emission during the whole growing season was higher in the drip irrigation treatment than in the corresponding flood irrigation treatment, while soil organic C concentration was larger in the flood irrigation treatment than in the corresponding drip irrigation treatment. Furthermore, soil CO2 emission and organic C concentration were all higher in the stubble-incorporated treatment than in the corresponding stubble-removed treatment, and larger in the NPK+OM treatment than in the other three fertilizer amendments within the C pool range. The combination of flood irrigation, stubble incorporation and application of either NPK+OM or OM increased soil organic C concentration in the 0-60 cm soil depth. Calculation of net ecosystem productivity(NEP) under different management practices indicated that the combination of drip irrigation, stubble incorporation and NPK+OM increased the size of the C pool most, followed by the combination of drip irrigation, stubble incorporation and NPK. In conclusion, management practices have significant impacts on soil CO2 emission, organic C concentration and C balance in cotton fields. Consequently, appropriate management practices, such as the combination of drip irrigation, stubble incorporation, and either NPK+OM or NPK could increase soil C storage in cotton fields of Northwest China. 展开更多

Recent economic crises like the 2008 financial tsunami has demonstrated a critical need for better understanding of the topologies and various economic,social,and technical mechanisms of the increasingly interconnected global financial system.Such a system largely relies on the interconnectedness of various financial entities such as banks,firms,and investors through complex financial relationships such as interbank payment networks,investment relations,or supply chains.A network-based perspective or approach is needed to study various financial networks in order to improve or extend financial theories,as well as develop business applications.Moreover,with the advance of big data related technologies,and the availability of huge amounts of financial and economic network data,advanced computing technologies and data analytics that can comprehend such big data are also needed.We referred this approach as financial network analytics.We suggest that it will enable stakeholders better understand the network dynamics within the interconnected global financial system and help designing financial policies such as managing and monitoring banking systemic risk,as well as developing intelligent business applications like banking advisory systems.In this paper,we review the existing research about financial network analytics and then discuss its main research challenges from the economic,social,and technological perspectives. 展开更多

In 1995, the amount of foreign direct investment (FDI) in China was US$ 37.81 billion that was from more than 170 countries and regions. The regional structure of capital market of FDI in China has three characteristics: (1) The most of FDI was from the near countries or regions, Hong Kong Macao, Taiwan, Japan, South Korea, Thailand and Malaysia provided 80.3% of the amount of FDI in 1995. (2) Only several countries and regions provided most of FDI in China. (3) According to the contributory rate, the main capital market of FDI in China could be divided into four classes, including the guiding market, subsidiary market, new and development market, opportunity market. Most of FDI in China was from the developing countries and regions, which had a negative effect on the size and technical level of foreign capital enterprises. The author puts forward the countermeasures to improve the regional structure of capital market: (1) to keep the guiding market stable, (2) to open up chiefly the subsidiary market, (3) to guide actively the new and developing market, (4) to try hard to start the opportunity market. 展开更多

A comparison of the management models of protected areas between China and the African south region allows reading and evaluating the similarities and differences in the use of management model as a management tool for protected areas and specifi- cally some positive and negative features of the management approaches in these two regions. Previous to this study it verified the designation of protected areas as increasing at a faster rate than ever before, comparatively much faster now in China than southern Africa regions. With the aim of evaluating similarities and differences in the use of management model as a management tool for pro- tected areas in China and southern Africa Region, both qualitative and quantitative data were collected. Qualitative data were mainly from policy documents, scientific articles and magazine reports, whereas quantitative (secondary data) statistical data from Interna- tional Union for Conservation of Nature Resources (IUCN) and World Database on Protected Areas (WDPA). In view of the data sources above, the study found that in China all the protected areas are state owned whilst in southern Africa regions there are some protected areas that are privately owned; also that the models or typology of governance applied are successful for the fact that they both combine co-management or collaborative management, community-conserved areas and private protected areas that are subject to greater success and can help design planning and management than those who use exclusively government management. To this, the study concluded that the use of management model is influenced by the type of governance a country applies to its reserved areas. 展开更多
