Background: Maxillofacial war injuries usually cause severe facial organ defects and deformities, handicapping the patient's daily activities, even result in a tendency to commit suicide. The application of maxillofacial prosthesis is an alternative to surgery in functional–aesthetic facial reconstruction. Computer aided design and computer aided manufacturing has opened up a new approach to the fabrication of maxillofacial prosthesis. An intelligentized rapid simulative design and manufacture system for prosthesis was developed to facilitate the prosthesis fabrication procedure. Methods: Maxillofacial prosthesis rapid simulation design and rapid fabrication system consists of three components: digital impression, intelligentized prosthesis designing, and rapid manufacturing. The patients' maxillofacial digital impressions were taken with Structured-light 3D scanner; and then the 3D model of prostheses and their negative molds could be designed in specific software; finally, with the resin molds fabricated by rapid prototyping machine, the prostheses could be produced directly and quickly. Results: Fifteen patients of maxillofacial defect caused by traumatic injuries received prosthesis rehabilitation provided by the established system. The contour of the prostheses coordinated properly with the appearance of the patients, and the uniform-thickness border sealed well to adjacent tissues. All the patients were satisfied with their prostheses. Conclusions: The rapid simulative rehabilitation system of maxillofacial defects has been approaching completion. It could provide advanced technological reservation for the Army in the issue of maxillofacial defect rehabilitation. 展开更多

In stroke rehabilitation, rehabilitation equipments can help with the training. But traditional equipments are not convenient to carry, which limits patients to use related rehabilitation techniques. To solve this kind of problem,a new embedded rehabilitation system based on brain computer interface(BCI)is proposed in this paper. The system is based on motor imagery(MI)therapy,in which electroencephalogram(EEG)is evoked by grasping motor imageries of left and right hands, then collected by a wearable device. The EEG is transmitted to a Raspberry Pie processing unit through Bluetooth and decoded as the instructions to control the equipment extension.Users experience the limb movement through the visual feedback so as to achieve active rehabilitation.A pilot study shows that the user can control the movement of the rehabilitation equipment through his mind, and the equipment is convenient to carry. The study provides a new way to stroke rehabilitation. 展开更多

Physical inactivity remains in high levels after cardiac surgery, reaching up to 50%.Patients present a significant loss of functional capacity, with prominent muscle weakness after cardiac surgery due to anesthesia, surgical incision, duration of cardiopulmonary bypass, and mechanical ventilation that affects their quality of life. These complications, along with pulmonary complications after surgery, lead to extended intensive care unit(ICU)and hospital length of stay and significant mortality rates.Despite the well-known beneficial effects of cardiac rehabilitation, this treatment strategy still remains broadly underutilized in patients after cardiac surgery.Prehabilitation and ICU early mobilization have been both showed to be valid methods to improve exercise tolerance and muscle strength.Early mobilization should be adjusted to each patient’s functional capacity with progressive exercise training, from passive mobilization to more active range of motion and resistance exercises.Cardiopulmonary exercise testing remains the gold standard for exercise capacity assessment and optimal prescription of aerobic exercise intensity.During the last decade, recent advances in healthcare technology have changed cardiac rehabilitation perspectives, leading to the future of cardiac rehabilitation.By incorporating artificial intelligence, simulation, telemedicine and virtual cardiac rehabilitation, cardiac surgery patients may improve adherence and compliance, targeting to reduced hospital readmissions and decreased healthcare costs. 展开更多

With the amalgamation of wearable systems equipped with inertial sensors, such as a gyroscope, and machine learning a therapy regimen can be objectively quantified, and then the initial phase and final phase of a one year therapy regimen can be distinguished through machine learning. In the context of rehabilitation of a hemiplegic ankle, a longitudinal therapy regimen incorporating stretching and then a series of repetitions for raising and lowering the foot of the hemiplegic ankle can be applied over the course of a year. Using a smartphone equipped with an application to function as a wearable and wireless gyroscope platform mounted to the dorsum of the foot by an armband, the initial phase and final phase of a one year longitudinally applied therapy regimen can be objectively quantified and recorded for subsequent machine learning. Considerable classification accuracy is attained to distinguish between the initial phase and final phase by a support vector machine for a one year longitudinally applied hemiplegic ankle therapy regimen based on the gyroscope signal data obtained by a smartphone functioning as a wearable and wireless inertial sensor system. . 展开更多

The current parallel ankle rehabilitation robot(ARR)suffers from the problem of difficult real-time alignment of the human-robot joint center of rotation, which may lead to secondary injuries to the patient. This study investigates type synthesis of a parallel self-alignment ankle rehabilitation robot(PSAARR)based on the kinematic characteristics of ankle joint rotation center drift from the perspective of introducing"suitable passive degrees of freedom(DOF)"with a suitable number and form.First, the self-alignment principle of parallel ARR was proposed by deriving conditions for transforming a human-robot closed chain(HRCC)formed by an ARR and human body into a kinematic suitable constrained system and introducing conditions of"decoupled"and"less limb".Second, the relationship between the self-alignment principle and actuation wrenches(twists)of PSAARR was analyzed with the velocity Jacobian matrix as a"bridge".Subsequently, the type synthesis conditions of PSAARR were proposed.Third,a PSAARR synthesis method was proposed based on the screw theory and type of PSAARR synthesis conducted. Finally,an HRCC kinematic model was established to verify the self-alignment capability of the PSAARR.In this study,93 types of PSAARR limb structures were synthesized and the self-alignment capability of a human-robot joint axis was verified through kinematic analysis, which provides a theoretical basis for the design of such an ARR. 展开更多

A wide open bottom story of a frame building is often expected by owners for use as a garage or shops. However, this leads to weak stories due to abrupt changes in lateral stiffness and often results in unexpected story collapse as observed in many previous earthquakes. To retrofit frame buildings that have experienced weak story damage,a tendon system is proposed in this study, which consists of a set of swaying columns and tendons. The swaying columns are used to uniformly redistribute the lateral deformation along the height, while the tendons provide extra lateral stiffness and renders the entire structural system a re-centering capability. To avoid unnecessary forces to swaying columns,pin-connections are used at the bottom.Tendons are placed over the entire story to gain large elastic displacements.Parametric analysis reveals that the swaying column, with a stiffness of about 0.9 times that of the weak story, and the tendons attached at the roof, with a stiffness of 0.04 times that of the weak story, can provide the optimal performance with a maximum residual story drift angle of less than 0.5%.Online hybrid tests were carried out, which demonstrated that uniformly distributed story drifts and acceptable residual deformation could be achieved by the proposed tendon system. 展开更多

The kinematic equivalent model of an existing ankle-rehabilitation robot is inconsistent with the anatomical structure of the human ankle, which influences the rehabilitation effect. Therefore, this study equates the human ankle to the UR model and proposes a novel three degrees of freedom(3-DOF)generalized spherical parallel mechanism for ankle rehabilitation. The parallel mechanism has two spherical centers corresponding to the rotation centers of tibiotalar and subtalar joints.Using screw theory, the mobility of the parallel mechanism, which meets the requirements of the human ankle,is analyzed. The inverse kinematics are presented, and singularities are identified based on the Jacobian matrix. The workspaces of the parallel mechanism are obtained through the search method and compared with the motion range of the human ankle, which shows that the parallel mechanism can meet the motion demand of ankle rehabilitation.Additionally, based on the motion-force transmissibility, the performance atlases are plotted in the parameter optimal design space, and the optimum parameter is obtained according to the demands of practical applications. The results show that the parallel mechanism can meet the motion requirements of ankle rehabilitation and has excellent kinematic performance in its rehabilitation range, which provides a theoretical basis for the prototype design and experimental verification. 展开更多

In this editorial,we comment on the article by Stafie et al.Inflammatory bowel disease(IBD)constitutes a cluster of chronic and progressive inflammatory disorders affecting the digestive system.IBD can impede an individual’s capacity to perform daily activities, hinder work productivity, limit physical capabilities, and negatively impact medical outcomes.Although physical activity and structured exercise programs are becoming increasingly important in many chronic inflammatory diseases, they are not being sufficiently implemented in IBD patients.Effective prevention of future disability and drug dependence in IBD patients requires timely diagnosis and treatment of musculoskeletal problems, including sarcopenia,as well as decreased muscle strength, aerobic capacity, and bone mineral density. To improve treatment outcomes for IBD patients,it is crucial to develop individualized rehabilitation programs tailored to their unique needs.Equally critical is the active participation of pertinent departments in this process. It is imperative to highlight the significance of creating a personalized rehabilitation program with a multidisciplinary approach in IBD management. 展开更多

This paper proposes an explicit scheme to analyze the failure of a subsea polyhedral tunnel-liner system with an inverted arch under mechanical loading and fire fields. The thin-walled liner is made of Functionally Graded Materials(FGMs),which may improve the stability behavior of the tunnel-liner system.Hydrostatic pressure is inevitable in the liner since underground water may penetrate the cracks of the tunnel, and reach the outer surface of the liner. In addition,an elevated temperature loading is taken into account, considering that fire may occur in the tunnel-liner system.Under the combination of mechanical loading and thermal loading, the liner deforms into a single-lobe shape, which is depicted by a trigonometric function. The total potential energy is expressed quantitatively after the energy approach and thin-walled shell theory are used. The minimum potential energy is obtained when the critical buckling occurs. The critical buckling pressure is calculated, which considers the effect of the thermal field. The present analytical prediction is subsequently compared precisely with other closed-form solutions. Finally, the effects of several parameters, such as the geometric shapes, temperature variations, and volume fraction indices, are discussed to further survey the buckling performance of the nonlinear buckling of an FGM polyhedral liner with an inverted arch. One may address a polyhedral liner with fewer polyhedral sides, and a lower volume fraction index is recommended to rehabilitate cracked tunnels in engineering applications. 展开更多
