A Review on the Research of Precision Medicine for Cancer Based on the Integration of Traditional Chinese and Western Medicine

Objective:To summarize and expound the general situation of precision medicine for cancer according to the current international research hot spots and treatment frontiers of cancer.Methods:This article makes an inventory from five aspects of treatment methods,detection technology,new drug research and development,information data and traditional Chinese medicine,"from point to surface","from outside to inside"and"integration of Chinese and Western medicine",to explore the whole picture of tumor treatment and research.Results:With the rapid development of tumor precision medicine in recent years,the tumor treatment methods have been developed from single to multiple.The research technology has been changed from macro to micro.The therapeutic drugs have been changed from systemic chemotherapy to targeted therapy,immunotherapy and other technologies.The treatment concept has been changed from local to overall.And the integrated treatment methods of traditional Chinese and Western medicine have brought a new chapter to tumor treatment.Conclusion:Through a series of new technologies,new methods and new concepts,tumor precision medicine promotes the treatment of tumors with integrated Chinese and Western medicine into a new chapter.

Opinion on pharmacology research and new drug development from precision medicine

Since President Obama announced the Precision Medicine Initiative from a national strategy perspective in his State of the Union address,precision medicine has rapidly become a world-wide hotspot and drawn global attention in the medical field.Precision medicine aims at applying genetic information of individual diseases to guide his or her diagnosis and treatment.

Intellectual Property Protection and High Quality Development of Radix Glycyrrhizae for Treatment of COVID-19

Radix Glycyrrhizae is the dominant native medicinal material variety in the north and northwestern medicinal materials producing areas.It is a main Chinese medicine of effective TCM drugs and formulas for the treatment of new coronavirus disease(COVID-19).This paper introduces the medicinal value of Radix Glycyrrhizae,involving the labeling,cultural heritage,and creative intellectual property rights of the Chinese medicinal materials,analyzes the poverty-stricken areas that are ecologically suitable for Radix Glycyrrhizae cultivation,and the superior counties and production bases of the Chinese medicinal materials.Besides,mainly from the aspects of perfect intellectual property rights,the establishment of authentic medicinal material production bases,and the construction of quality control systems,etc.,it discusses the rural revitalization strategy and the development strategy of traditional Chinese medicine(TCM),agricultural intellectual property protection and high-quality development strategy of Radix Glycyrrhizae.

Challenges in Research and Development of Traditional Chinese Medicines

This review is briefly to recall the history of research and development (R&D) of Chinese materia medica (CMM) and to discuss the challenges of Chinese traditional and herbal medicines (CTHM) facing the modern science and technology. The R&D of CTHM is thought to be an important pathway for new drug discovery. Since1949, about 140 approved new drugs have been developed, among which about 80 originated directly or indirectly from medicinal plants. CTHM has gained interest from the international medical, biomedical, and pharmaceutical institutions as a valuable source of potential medicines. For the modernization of CMM and innovative research of CTHM, there are following challenges to be faced: (1) to evaluate the efficacy, pharmacological properties, action mechanism, and active chemical constituents; (2) to develop new methodologies for the quality and safety of CTHM; (3) to apply new '-omics' techniques to accelerate drug discoveries developed from CTHM; and (4) to apply international practices including good agricultural practice, good manufacturing practice, good laboratory practice, and good clinical practice in the R&D of CTHM.

New Progress in Modern Research on Chinese Herbal Medicines Published in Chinese Traditional and Herbal Drugs in 2010:Annual Highlights and Comments

As a series of and continuous publication,the papers published on Chinese Traditional and Herbal Drugs in 2010 were selectively and briefly highlighted to reflect the new progress on modern research of Chinese herbal medicines.Within 617 articles,chemical constituents(127),pharmaceutics and technology(149),pharmacological studies and clinical observation,and medicinal materials are still major categories.Some comments have also been personally provided.

Regulating effect of Chinese herbal medicine on the peritoneal lymphatic stomata in enhancing ascites absorption of experimental hepatofibrotic mice

AIM: To observe the regulatory effect of Chinese herbal medicine on peritoneal lymphatic stomata and its significance in treating ascites in liver fibrosis model mice. METHODS: Two Chinese herbal composite prescriptions were used separately to treat the carbon tetrachloride-induced mouse model of liver fibrosis. The histo-pathologic changes of the liver sections (HE and VG stainings) were observed. The peritoneal lymphatic stomata was detected by scanning electron microscopy and computer image processing. The changes of urinary volume and sodium ion concentration were measured. RESULTS: In the model group, lots of fibrous tissue formed in liver and extended into the hepatic lobules to separate them incompletely. In the treated and prevention groups, the histo-pathologic changes of liver was rather milder, only showed much less fibrous tissue proliferation in the hepatic lobules. The peritoneal lymphatic stomata enlarged with increased density in the experimental groups (diameter: PA, 3.07 +/- 0.69 microm; PB, 2.82 +/- 0.37 microm; TA, 3.25 +/- 0.82 microm and TB, 2.82 +/- 0.56 microm; density: PA, 7.11 +/- 1.90 stomata.1000 microm(-2); PB, 8.76 +/- 1.45 stomata.1000 microm(-2); TA, 6.55 +/- 1.44 stomata.1000 microm(-2)and TB, 8.76+/-1.79 stomata.1000 microm(-2)), as compared with the model group (diameter: 2.00+/-0.52 microm density: 4.45+/-1.05 stomata.1000 microm(-2)). After treatment, the urinary volume and sodium ion excretion increased in the experimental groups (PA, 231.28+/-41.09 mmol.L(-1); PB, 171.69 +/- 27.48 mmol.L(-1) and TA, 231.44 +/- 34.12 mmol.L(-1)), which were significantly different with those in the model group (129.33 +/- 36.75 mmol.L(-1)). CONCLUSION: Chinese herbal medicine has marked effects in alleviating liver fibrosis, regulating peritoneal lymphatic stomata, improving the drainage of ascites from peritoneal cavity and causing increase of urinary volume and sodium ion excretion to reduce the water and sodium retention, and thus have favorable therapeutic effect in treating ascites.

Research and development of Chinese anti-COVID-19 drugs

The outbreak and spread of coronavirus disease 2019(COVID-19)highlighted the importance and urgency of the research and development of therapeutic drugs.Very early into the COVID-19 pandemic,China has begun developing drugs,with some notable progress.Herein,we summarizes the anti-COVID-19 drugs and promising drug candidates originally developed and researched in China.Furthermore,we discussed the developmental prospects,mechanisms of action,and advantages and disadvantages of the anti-COVID-19 drugs in development,with the aim to contribute to the rational use of drugs in COVID-19 treatment and more effective development of new drugs against severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)and the variants.Neutralizing antibody is an effective approach to overcome COVID-19.However,drug resistance induced by rapid virus mutation will likely to challenge neutralizing antibodies.Taking into account current epidemic trends,small molecule drugs have a crucial role in fighting COVID-19 due to their significant advantage of convenient administration and affordable and broad-spectrum.Traditional Chinese medicines,including natural products and traditional Chinese medicine prescriptions,contribute to the treatment of COVID-19 due to their unique mechanism of action.Currently,the research and development of Chinese anti-COVID-19 drugs have led to some promising achievements,thus prompting us to expect even more rapidly available solutions.

Application of immune checkpoint targets in the anti-tumor novel drugs and traditional Chinese medicine development

Immune checkpoints are the crucial regulators of immune system and play essential roles in maintaining self-tolerance,preventing autoimmune responses,and minimizing tissue damage by regulating the duration and intensity of the immune response.Furthermore,immune checkpoints are usually overexpressed in cancer cells or noninvasive cells in tumor tissues and are capable of suppressing the antitumor response.Based on substantial physiological analyses as well as preclinical and clinical studies,checkpoint molecules have been evaluated as potential therapeutic targets for the treatment of multiple types of cancers.In the last few years,extensive evidence has supported the immunoregulatory effects of traditional Chinese medicines(TCMs).The main advantage of TCMs and natural medicine is that they usually contain multiple active components,which can act on multiple targets at the same time,resulting in additive or synergistic effects.The strong immune regulation function of traditional Chinese medicine on immune checkpoints has also been of great interest.For example,Astragalus membranaceus polysaccharides can induce anti-PD-1 antibody responses in animals,and these antibodies can overcome the exhaustion of immune cells under tumor immune evasion.Furthermore,many other TCM molecules could also be novel and effective drug candidates for the treatment of cancers.Therefore,it is essential to assess the application of immune checkpoints in the development of new drugs and TCMs.In this review,we focus on research progress in the field of immune checkpoints based on three topics:(1)immune checkpoint targets and pathways,(2)development of novel immune checkpoint-based drugs,and(3)application of immune checkpoints in the development of TCMs.

Function and application of Traditional Chinese Medicine Inheritance Calculate System

Traditional Chinese Medicine Inheritance Calculate System(TCMICS)is a computational tool designed to facilitate the analysis of diseases,symptoms,syndromes,prescriptions,and acupoints based on databases of existing prescriptions,medical cases,or compound formulas.The software,currently updated to version 3.5,comprehensively presents the relationships between diseases,symptoms,syndromes,prescriptions,herbalmedicines,and acupoints.TCMICS integrates the functions of importing,retrieving,and analyzing diseases,syndromes,symptoms,tonguemanifestations,pulsemanifestations,herbal medicines,prescriptions,medical cases,and related information.It includes 7 functional modules:statistical analysis,prescription analysis,symptom analysis,tongue manifestation analysis,pulse manifestation analysis,acupoint analysis,and comprehensive analysis.TCMICS has been recognized by academicians,national TCM veterans,and renowned TCM clinics in Beijing,Shanghai,Guizhou,Shandong,Tianjin,and so on.It is regarded as an important tool for inheriting the academic thoughts and clinical experience of renowned TCM veterans and has been applied to the inheritance of Chinese medical expertise and analysis of TCM literature,as well as the development of TCM products.

中医药与脑科学交叉融合研究的学术思路和发展路径

学科交叉与融合是学科创新发展的不二途径,中医药与脑科学的深度交叉融合必将带来整个中医药学科的长足进步与发展。这一背景下,本文试图厘清中医药与脑科学交叉融合研究的学术思路和可能的发展路径。学术思路为:以中医药理论为指导进行基于临床循证证据下的机制探索及应用转化。可能的发展路径为:中医藏象及病因病机理论的现代诠释、确有疗效复方或组分的分子及环路机制解析、防治脑病的中药新药及创新疗法转化开发。希望对中医药学科和其他现代医学生命学科的交叉融合起到重要的提示和启发意义。

Traditional Chinese medicine:research and development,globalization,and regulation

In this issue of Chinese Journal of Natural Medicines （CJNM）, we are pleased to publish two comprehensive and timely review articles [1-2] addressing the registration regulation of traditional Chinese medicine （TCM）, a critical issue in the development, regulation, and internationalization of TCM.

以现实为导向:抗战时期新四军对中医药的运用

医药短缺是影响新四军生存、发展和壮大的重要难题。抗战初期,新四军积极筹建西医人才队伍,拓宽西药获取渠道,使医药困难得以暂时缓解。皖南事变后,由于日伪实行医药管制,新四军西医西药获取渠道严重受阻,致使医药困境再度加剧。基于此,新四军遵循中共中央方针,结合自身实践经验,破除中西医门户之见,形成独具特色的中医药政策。中医药政策的制定,是新四军在极端困境下采取的应急之策。新四军通过筛选与培养中医人才扩充医疗队伍,甄别与利用中草药缓解西药短缺,使处于弱势地位的中医药为抗战事业提供了不可或缺的医疗保障。对中医药的改造与运用,是新四军华中抗日根据地在医药卫生领域践行“自力更生”策略的重要体现。新四军的中医药实践展现了军部与其所属部队在医疗卫生工作中的多维互动,体现了各部队医疗卫生实践的同一性与特殊性。

Remarkable research achievements in traditional Chinese medicine during 2012–2022

The period since the 18th National Congress of the Communist Party of China(CPC)has witnessed the emerging of innovative achievements in traditional Chinese medicine(TCM).To fulfill the requirements proposed at the 20th National Congress of the CPC and in the Opinions of the CPC Central Committee and the State Council on Promoting the Preservation,Innovation,and Development of Traditional ChineseMedicine and give play to the demonstrative role ofmajor research achievements,the China Association of ChineseMedicine has organized TCM researchers and practitioners to comprehensively review the major research achievements in TCM since the 18th National Congress of the CPC.After collection and organization of achievements,report writing,expert evaluation,and public opinion soliciting,we have compiled the Remarkable Research Achievements of Traditional Chinese Medicine during 2012–2022.

Developing new drugs from annals of Chinese medicine

Developing new pharmaceuticals requires massive amounts of time,money and efforts.The key step is how to find a safe and effective entity for a disease condition and how to develop it as new drug effectively.Unfortunately,the FDA’s rate of approving new entities has declined dramatically in the last three decades.There is a strong need to review the current strategy and to optimize process in developing new drugs,both to shorten the process and increase the success rate.Chinese medicine has used natural products to treat patients for thousands of years,and Chinese medicine practitioners have chronicled the patients and treatment methods for thousands of years.There is much information that has not yet been used.The success stories of artimisinin and arsentic trioxide are wonderful examples of how the annals of Chinese medicine can provide leads for discovering new drugs.This paper argues that the annals of Chinese medicine are valuable and describes how they can be used in modern drug discovery.The major topics addressed are:(i)why Chinese medicine is a rich resource for finding new drugs;(ii)how to identify a potential valuable record from Chinese medicine annals;(iii)when a potential valuable record is identified from annals,how to proceed;and(iv)both why and how the approach used for chemical drugs should be revised for drugs based on the historical documents related to herbal medicine.In conclusion,we argue here that the annals of Chinese medicine offer not only a rich resource for new drugs,but also several centuries of patient data with regard to safety and efficacy,that in effect represent pilot studies.Acknowledging and using these data can shorten new drug discovery time and improve efficiency of the drug development process,bringing more effective,safe drugs to market much more quickly and cheaply.

医疗机构基于“三结合”中药新药研发及管理体系的构建

目的:在中药审评审批制度改革的政策背景下,围绕医疗机构,探索中医药理论、人用经验和临床试验相结合(简称“三结合”)的审评证据体系落地举措,为医疗机构中药制剂及中药新药研发提供参考。方法:根据“三结合”审评证据体系下中药新药研发的相关要求,从机构层面加强人用经验研究相关配套建设,从管理层面搭建中药研发、成果转化服务平台,从临床层面规范开展人用经验研究,构建医疗机构中药新药研发及管理体系。结果:医疗机构中药新药研发及管理体系的构建,有助于将临床诊疗数据转变为支持中药新药注册的人用经验证据,通过提高中药研发水平促进中药新药转化。结论:在医疗机构构建基于“三结合”的中药新药研发及管理体系,是“三结合”审评证据体系落地、政策引导研发实践的路径之一。

国家重点研发计划中医药现代化研究文献计量分析

目的:分析国家重点研发计划中医药现代化研究重点专项(以下简称专项)的研究进展。方法:基于中国知网(CNKI)、中国学术期刊数据库(万方数据)、中文科技期刊数据库(VIP)、中国生物医学文献数据库(CBM)和Web of Science核心合集数据库(WoSCC),检索2017—2023年专项资助发表的论文,利用文献计量学和CiteSpace软件对论文作者、机构、研究热点等进行分析。结果:共检索到中文文献2701篇,涉及核心作者108位、形成2个较大的研究团队,中国中医科学院发文量最多,研究热点包括用药规律、中医证候等中医药临床评价和机制研究,主要研究疾病包括心血管病、恶性肿瘤等。英文文献732篇,其中中国科学院发文量最多,药物效用机制、药物代谢动力学等药理学研究是研究热点。结论:在专项支持下,中医药基础研究和基于循证医学评价的临床研究不断深入,在中医药防治重大疾病研究方面取得了重要进展。未来应加强科技成果转化,深化人工智能技术在中医药现代化研究中的应用。

中医药产业高质量发展的内涵、评价体系与基本路径探析

实现中医药产业高质量发展是构建现代中医药产业体系、推动中医药整体事业高质量发展的基本命题和重要支撑。文章通过梳理中医药产业高质量发展的内涵与评价体系,认为中医药产业高质量发展仍面临着技术升级和科技创新、产业集群打造、产业融合发展、龙头企业带动辐射、国际市场占领、产业服务平台完善等压力。为此,实现中医药产业高质量发展的政策导向和基本路径在于:建设产业基地、产业集群和产业园,完善产业链和产业体系;开发道地品牌中药材优势,打造富民兴业支柱产业;发挥特色脱贫致富模式效应,推进经济社会可持续发展;实施科技创新驱动战略,推动创造性转化创新性发展;构建“中医药+”融合发展新格局,推进现代化产业化;建设服务出口基地,促进对外贸易高速发展;聚焦发展瓶颈,探索产学研医政企协同共建新模式。

高质量发展下山西省三级公立中医医院“1+N+X”专兼职运营管理模式的建设研究

近年来,我国医药卫生体制改革蹄疾而步稳,西医类综合医院已整体迈入高质量发展的新征程,中医医院正面临着高质量发展的关键转折点。山西作为传统中医药文化大省,三级公立中医医院运营管理水平较为落后,未建立真正意义上的运营管理模式,目前还处在一个不断探索的实践过程中。本文通过分析总结四川大学华西医院和上海交通大学医学院附属新华医院两家公立医院运营管理模式的成功实践经验,结合山西省三级公立中医医院运营管理困局,探讨高质量发展下“1+N+X”专兼职运营管理模式在医院生存和发展中的重要作用,从而进一步加强医院运营管理制度建设,提升医院精细化管理水平,促进医院高质量发展,为加快健康山西建设提供有力支撑。
