Hemodynamical evaluation of a coronary artery lesion is an important diagnostic step to assess its functional impact.Fractional flow reserve(FFR)received a class IA recommendation from the European Society of Cardiology for the assessment of angiographically moderate stenosis.FFR evaluation of coronary artery disease offers improvement of the therapeutic strategy,deferring unnecessary procedures for lesions with a FFR>0.8,improving patients'management and clinical outcome.Post intervention,an optimal FFR>0.9 post stenting should be reached and>0.8 post drug eluting balloons.Non-hyperemic pressure ratio measurements have been validated in previous studies with a common threshold of 0.89.They might overestimate the hemodynamic significance of some lesions but remain useful whenever hyperemic agents are contraindicated.FFR remains the gold standard reference for invasive assessment of ischemia.We illustrate this review with two cases introducing the possibility to estimate also non-invasively FFR from reconstructed 3-D angiograms by quantitative flow ratio.We conclude introducing a hybrid approach to intermediate lesions(DFR 0.85-0.95)potentially maximizing clinical decision from all measurements. 展开更多

If the government spends all the seigniorage revenue on the provision of a public input which has positive externality on the private sector’s production, some results to be utterly different from Bronx’s have been obtained: the economy has a unique saddle-balanced growth path, but it has nothing to do with reserve ratio. As we all know, cyclic gas injection is one of the most effective development methods to improve condensate oil recovery. When dealing with the calculation of the reserves, the injection-production differences and water influx create great influence on the accuracy. Based on the existing research, we proposed a new material balance equation which considered the differences of composition between produced and injected fluids and the effect of water influx, and a solution was provided in this paper. The results of the method are closer to the actual situation because they are built on the law of conservation of mass, and the using of curve fitting method can not only avoid the use of water influx coefficient but also obtain the water influx rate and reserves at the same time. The YH-23 gas condensate reservoir is taking as a typical subject to do the research, which has been exploited by cycle gas injection for 14 years. Three different methods are used to calculate the reserves, and the results show that the method proposed in this paper has minimum error of 2.96%. 展开更多

Impact of the Political Risk on Food Reserve Ratio:Evidence Across Countries

Using an unbalanced panel data covering 75 countries from 1991 to 2019,the authors explore how the political risk impacts on food reserve ratio.The empirical findings show that an increasing political risk negatively affects food reserve ratio,and the same effects hold for both internal risk and external risk.Moreover,the authors find that the increasing external or internal risks both negatively affect food production and food exports,but external risk does not significantly impact food imports and it positively impacts food consumption,while internal risk negatively impacts food imports and food consumption.The results suggest that most governments have difficulty raising subsequent food reserve ratios in face of an increasing political risk,no matter if it is an internal risk or an external risk although the mechanisms behind the impacts are different. 展开更多

Background: Since the greater part of coronary blood flow takes place during the diastolic phase of each cardiac cycle, a time indicator of myocardial perfusion reserve, the ratio of diastolic to systolic duration (D/S ratio), was presented. The objective of this study was to evaluate the accuracy and precision, the biological implication, and the applications of D/S ratio. Methods: Multi-center pragmatic studies evaluating the time indicator of cardiac perfusion reserve were performed. Related experiments, clinical trials, and surveys were conducted at 5 centers. Results: The results showed that the measurement of D/S ratio is both accurate and precise;the mean values of D/S of all of the 3 species studied (human, rabbit, and rat) were greater than 1. These application studies on D/S ratio showed that a close negative correlation existed between D/S ratio and New York Heart Association Functional Classification (NYHA FC) (r = –0.659, p pregnant women with pre-eclampsia and either normal pregnant women or non-pregnant women were significant (p Conclusions: D/S ratio has important biological implication, which is a safe, easy, reliable, and effective indicator, can be used to evaluate fitness levels, served as a pathophysiological marker for screening of cardiovascular disease (CVD), for predicting risk of cardiac events, and for evaluating the severity and prognosis of CVD. 展开更多

In this paper, a novel signal-to-clipping noise ratio and least squares approximation tone reservation scheme(SCR-LSA TR) is proposed to reduce the peak-to-average power ratio for orthogonal frequency division multiplexing systems. During the SCR procedure, only the element with the maximal amplitude is picked for processing, which not only decreases the algorithm complexity, but also helps to overcome the BER deterioration. With the LSA method, the amplitude of the peak-cancelling signals can approximate to that of the original clipping noise as much as possible. Through the combination of the optimization factor in the LSA method, the classic SCR method can achieve better PAPR reduction with faster convergence. Simulation results show that the proposed SCR-LSA TR scheme has less in-band distortion and smaller out-of-band spectral radiation. The BER of the proposed scheme shows a better performance especially under the 16-QAM over the additive white Gaussian noise channel. 展开更多

Background and Objectives:The aims of this study were(1)to examine the diagnostic accuracy of resting timeaveraged distal coronary pressure(Pd)to mean aortic pressure(Pa)ratio to predict hyperemic fractional flow reserve(FFR)and(2)to identify a resting Pd/Pa value that can preclude the need for hyperemic FFR assessed with use of a monorail pressure catheter.Methods:A total of 191 stenoses were assessed.After exclusions,157 FFR data sets from 103 patients were analyzed.Results:Resting Pd/Pa showed poor agreement with hyperemic FFR(r=0.619,P<0.001).The receiver operating characteristic curve for resting Pd/Pa with reference to hyperemic FFR of 0.80 or less showed an area under the curve of 0.800(95%confi dence interval 0.732– 0.868,P<0.001),with the greatest diagnostic accuracy of 74.5%for resting Pd/Pa of less than 0.85.Resting Pd/Pa of 0.96 or greater had a sensitivity of 100%and a negative predictive value of 100%,and resting Pd/Pa of 0.82 or less had a specifi city of 98.9%and a positive predictive value of 94.1%to predict abnormal FFR of 0.80 or less.These results were consistent regardless of the vessels studied,the location of lesions,and the severity of stenosis.Conclusions:Resting Pd/Pa showed poor agreement with hyperemic FFR assessed with use of a monorail pressure microcatheter.However,resting Pd/Pa of 0.96 or greater had excellent sensitivity and negative predictive value to predict normal hyperemic FFR,and resting Pd/Pa of 0.82 or less had excellent specifi city and positive predictive value to predict abnormal hyperemic FFR. 展开更多

Background: It is difficult to observe the phenomena of cardiac fatigue under a low or moderate workload test, and little was reported about cardiac fatigue under a heavy workload test because of the potential risk (sudden death) of prolonged strenuous exercise. Animal experiments would be helpful to resolve this problem. The objective of this study was to obtain baseline data of two new indicators of cardiac reserve in rats, rabbits, and dogs. Methods: Ten New Zealand White rabbits (1. 5 to 2 kg, 4 females), 10 male Wistar rats (2 months old), and 10 Golden Retriever dogs (40 days old, 5 females) were included in this study. Phonocardiogram of each animal was recorded. The basic points concerning heart sound quantitative analysis were: 1) measuring the duration and the amplitude of relevant heart sound components;2) calculating and analyzing relevant indicators based on the data obtained from the above measurements, including the ratio of diastolic to systolic duration (D/S) and the ratio of the amplitude of the first heart sound to the amplitude of the second heart sound (S1/S2). Results: The baseline data of D/S ratio and S1/S2 ratio in rats, rabbits, and dogs were obtained. The swimming time to exhaustion for rabbits was several to a dozen minutes, and for rats, several hours. Conclusion: D/S ratio has an important biological implication, which is a safe and easy indicator for evaluating the cardiac health status of both animals and humans. 展开更多

血管内超声测定定量血流分数对临界病变冠状动脉介入手术策略的价值

目的结合血管内超声结果分析定量血流分数在评估冠状动脉中重度狭窄及介入手术策略的指导价值。方法回顾性收集2022年2—10月在北京安贞医院住院行冠状动脉造影(CA)和/或经皮冠状动脉介入治疗(PCI)并接受靶血管血管内超声检查(IVUS)的患... 目的结合血管内超声结果分析定量血流分数在评估冠状动脉中重度狭窄及介入手术策略的指导价值。方法回顾性收集2022年2—10月在北京安贞医院住院行冠状动脉造影(CA)和/或经皮冠状动脉介入治疗(PCI)并接受靶血管血管内超声检查(IVUS)的患者110例,基于IVUS结果定量测定靶血管最小管腔面积(MLA)和斑块负荷(PB)等参数。利用患者的冠状动脉造影图像数据,对接受治疗的靶病变血管计算定量血流分数(QFR)。以IVUS结果为依据,评价QFR对血管病变程度的诊断及对行介入手术策略的价值。结果筛选临界病变并行IVUS检查110例,其中IVUS图像伪影严重13例,血管严重迂曲18例,造影显示全程病变或图像质量不佳24例,最终共计入组患者55例、靶血管55支。以IVUS管腔面积≤3 mm^(2)或IVUS管腔面积≤4 mm^(2)并且IVUS斑块负荷≥70%,作为血管显著狭窄引起心肌缺血的诊断标准。患者QFR值为0.75±0.12,MLA为(3.32±0.74)mm^(2),PB为(70±9)%。QFR值≤0.800时与IVUS评价冠状动脉狭窄缺血的一致性良好(Kappa=0.656,P<0.01,95%CI 0.452~0.860)。QFR的敏感度为0.853,特异度为0.810,阳性预测值为87.9%,阴性预测值为77.3%;MLA和QFR值之间存在中度正相关关系(r=0.566,P<0.01)。PB和QFR值之间存在轻度负相关关系(r=-0.371,P<0.01)。结论QFR对冠脉临界病变狭窄治疗决策的指导具有良好的诊断意义。 展开更多

目的探讨冠状动脉病变特征对冠状动脉功能性狭窄的预测价值。方法回顾性分析2021年10月至2023年3月广东省人民医院收治的192例患者共计192条血管的相关数据。在接受干预措施之前,这些患者均已进行了血流储备分数(fractional flow reserv... 目的探讨冠状动脉病变特征对冠状动脉功能性狭窄的预测价值。方法回顾性分析2021年10月至2023年3月广东省人民医院收治的192例患者共计192条血管的相关数据。在接受干预措施之前,这些患者均已进行了血流储备分数(fractional flow reserve,FFR)检测,用基于默里定律的定量流量比(Murray-law quantita⁃tive flow ratio,μQFR)参数分析技术对冠状动脉病变段的特征进行评估,以FFR≤0.80定义为具有血流动力学显著性的冠状动脉缺血。结果研究结果显示,冠状动脉病变长度、直径狭窄率、微循环阻力、血流速度以及最小管腔直径在评估冠状动脉狭窄的功能性方面展现出优异的诊断效能。多因素Logistic回归模型分析结果显示,病变长度>19.39 mm[比值比(odds ratio,OR)=2.21,95%置信区间(confidence interval,CI):1.08~4.53,P=0.030]、直径狭窄率>43.7%(OR=2.16,95%CI:1.02~4.56,P=0.044)、微循环阻力≤1.91 mmHg^(*)s/m(1 mmHg=0.133 kPa)(OR=3.91,95%CI:1.89~8.10,P<0.001)、最小管腔直径≤1.78 mm(OR=2.16,95%CI:1.02~4.56,P<0.001)是冠状动脉功能性狭窄的独立影响因素。结论当无法进行FFR检测时,冠状动脉的病变长度、直径狭窄率、微循环阻力以及最小管腔直径等参数,在辨识冠状动脉功能性狭窄方面展现出了良好的预测价值。 展开更多

由于系统对线性元器件的要求非常严格,导致目前正交频分复用(OFDM)系统产生峰值平均功率比(PAPR)过高的问题,引起OFDM和基于OFDM的雷达一体化系统的信号失真,影响系统性能。针对该问题,该文提出了基于加权最小二乘法的子载波预留法(TR)... 由于系统对线性元器件的要求非常严格,导致目前正交频分复用(OFDM)系统产生峰值平均功率比(PAPR)过高的问题,引起OFDM和基于OFDM的雷达一体化系统的信号失真,影响系统性能。针对该问题,该文提出了基于加权最小二乘法的子载波预留法(TR)。该算法首先将OFDM设计方案中的子载波分为数据子载波和预留的空白子载波,数据子载波调制数据子载波,空白子载波调制空白数据。然后利用原始数据通过加权最小二乘法得到最佳削峰系数和削峰数据,并将削峰数据调制在空白子载波上面。最后将削峰数据叠加到原始数据上,完成PAPR的抑制。仿真表明,基于加权最小二乘法的TR算法能够在1~3次迭代下实现良好的PAPR抑制效果,并且它的收敛速度相比于传统算法有了明显的提高。 展开更多

【目的】探讨冠状动脉粥样硬化性心脏病(冠心病)临界病变患者残余胆固醇/高密度脂蛋白胆固醇比值水平对冠脉CT血流储备分数(FFRct)的影响。【方法】本研究回顾性分析2022年1月1日-2022年12月31日中山大学附属第三医院心内科收治住院并行... 【目的】探讨冠状动脉粥样硬化性心脏病(冠心病)临界病变患者残余胆固醇/高密度脂蛋白胆固醇比值水平对冠脉CT血流储备分数(FFRct)的影响。【方法】本研究回顾性分析2022年1月1日-2022年12月31日中山大学附属第三医院心内科收治住院并行CCTA显示存在冠状动脉临界病变(管腔狭窄50%~70%)的219例患者为研究对象,其中临界病变血管304条。对以上纳入的冠脉临界病变血管行FFRct检查。FFRct以0.8为界,分为缺血组(FFRct≤0.8,N=108例)与非缺血组(FFRct>0.8,N=111例)。采用多因素Logistic回归分析探究冠心病临界病变患者FFRct≤0.8的影响因素;绘制受试者工作特征(ROC)曲线评估RC/HDL-C对FFRct≤0.8的预测价值;并通过Pear-son相关分析评估RC/HDL-C与FFRct之间是否存在相关性。【结果】缺血组中有糖尿病病史患者比例显著增高(P<0.001)。与非缺血组相比,缺血组患者RC、RC/HDL-C比值、Non-HDL-C、APOB、HbA1c、FPG水平显著增高(P<0.05)。Pearson相关性分析显示,RC/HDL-C比值、RC、Non-HDL-C、TC、TG、LDL-C、HDL-C、LP(a)、HbA1c、FPG均与FFRct值呈显著负相关(P<0.05)。单因素Logistic回归分析结果显示,糖尿病、RC、RC/HDL-C比值、Non-HDL-C、TG、LP(a)、HbA1c、FPG均与FFRct≤0.8均呈显著相关(P<0.05)。多因素Logistic回归分析表明,RC/HDL-C比值是FFRct≤0.80的预测因子(OR=4.682,95%CI 1.197~18.316,P<0.05)。【结论】冠状动脉粥样硬化性心脏病临界病变患者RC/HDL-C比值水平与FFRct≤0.8独立相关,RC/HDL-C比值可能成为评估冠状功能性缺血的潜在指标。 展开更多

鄂尔多斯盆地延长油田双河西区块长6油藏为低渗透油藏,常规开发指标计算方法并不利于油田进行油藏地质研究、开发政策制定与成本控制。对延长油田双河西区块长6油藏产量递减规律、水驱储量动用程度、注采比、含水率、注水利用状况及采... 鄂尔多斯盆地延长油田双河西区块长6油藏为低渗透油藏,常规开发指标计算方法并不利于油田进行油藏地质研究、开发政策制定与成本控制。对延长油田双河西区块长6油藏产量递减规律、水驱储量动用程度、注采比、含水率、注水利用状况及采收率进行分析。结果表明:双河西区块长6油藏产量递减规律为双曲递减模式;区块注水开发潜力较大,目前该区块水驱储量控制程度为74.54%,水驱储量动用程度为36.94%,注采连通率为27.27%;合理注采比约为2.5,随着采出程度增加,含水率上升速度先增大,后有所减小;根据存水率、耗水指数及水驱指数可知,开发后期,区块注水开发效果逐渐变好,最终采收率增加,研究区在开发过程中含水上升率应尽量小于6.1%,地层合理压力应大于9.1 MPa,在该开发条件下,研究区最终采收率约为23%。 展开更多

断控型碳酸盐岩油藏非均质性强,不同尺寸的孔、缝、洞交错发育,采用一般物质平衡方法计算的动态储量可能大于静态储量。在引入水油比的基础上,考虑孔缝洞介质不同时的岩石压缩系数,得到了适用于断控型油藏的综合压缩系数,并在此基础上... 断控型碳酸盐岩油藏非均质性强,不同尺寸的孔、缝、洞交错发育,采用一般物质平衡方法计算的动态储量可能大于静态储量。在引入水油比的基础上,考虑孔缝洞介质不同时的岩石压缩系数,得到了适用于断控型油藏的综合压缩系数,并在此基础上建立了断溶体流动物质平衡新方程,通过数值模拟,验证了新方程的准确性与适用性。结果表明:新方程计算的动态储量与数值模拟得到的静态储量误差仅为0.1099%,验证了新方程准确可靠;哈拉哈塘地区多口井用新方程计算的动态储量与地质建模雕刻的静态储量相对误差为-4.82%~-0.15%,远远低于物质平衡方程计算的动态储量与静态储量的相对误差,新方程计算结果更接近实际情况,更适用于哈拉哈塘地区断控型碳酸盐岩油藏的储量计算。 展开更多

光学相干断层扫描定量血流分数预测冠状动脉支架植入后靶血管功能学的可行性

目的探讨基于光学相干断层扫描(optical coherence tomography,OCT)定量血流分数(quantitative flow ratio,QFR)——光学血流分数(optical flow ratio,OFR)中的虚拟支架植入技术预估经皮冠状动脉支架植入后血管功能学的可行性。方法本... 目的探讨基于光学相干断层扫描(optical coherence tomography,OCT)定量血流分数(quantitative flow ratio,QFR)——光学血流分数(optical flow ratio,OFR)中的虚拟支架植入技术预估经皮冠状动脉支架植入后血管功能学的可行性。方法本研究纳入广东省人民医院2019年6月至2022年2月期间因疑似冠状动脉粥样硬化性心脏病(冠心病)行冠状动脉造影术与OCT探查术且成功植入支架的患者,应用OFR系统衍生的虚拟支架仿真植入技术计算其血管残余OFR值,后与支架植入术后基于Murray定律的QFR(Murray law based quantitative flow ratio,μQFR)进行比较,通过Spearman相关系数反映相关性。绘制受试者工作特征曲线(receiver operating characteristic curve,ROC),计算曲线下面积(area under curve,AUC),同时可得出残余OFR预测经皮冠状动脉介入(percutaneous coronary intervention,PCI)治疗后μQFR≤0.89的准确度、敏感度、特异度、阳性预测值、阴性预测值。结果共有139例患者(141支血管)入选本研究。所有血管行PCI治疗前采用虚拟支架技术计算的残余OFR为0.96±0.37,支架术后血管的μQFR为0.97±0.36,两者比较差异无统计学意义(P=0.235),残余OFR与PCI治疗后的μQFR具有高度相关性(r=0.82,95%CI:0.74~0.88,P<0.001)和良好的一致性(平均差异=0.00;P=0.236);残余OFR预测PCI治疗后的次优结果(μQFR≤0.89)的准确度、敏感度、特异度、阳性预测值和阴性预测值分别为96.45%、66.67%、98.49%、75.00%、97.74%,AUC为0.96(95%CI:0.92~1.0)。结论残余OFR与术后μQFR高度相关,OFR虚拟支架植入技术及其衍生的残余OFR可有效预测PCI治疗后血管的功能学水平,有助于术者科学地选择血管兴趣段植入支架,进而优化手术策略、改善患者预后。 展开更多

基于默里定律的定量流量比诊断心脏结构异常和左心室舒张功能障碍患者的准确性

目的旨在从单一血管造影角度深入探讨基于默里定律的定量流量比(Murray-law quantitative flow ratio,μQFR)在诊断心脏结构异常和左心室舒张功能障碍患者中的准确性。方法回顾分析2018年1月至2018年12月期间广东省人民医院收治的90例... 目的旨在从单一血管造影角度深入探讨基于默里定律的定量流量比(Murray-law quantitative flow ratio,μQFR)在诊断心脏结构异常和左心室舒张功能障碍患者中的准确性。方法回顾分析2018年1月至2018年12月期间广东省人民医院收治的90例患者的90条血管数据。这些患者在接受干预前接受了血流储备分数(fractional flow reserve,FFR)和μQFR检测,同时通过超声心动图评估了心脏结构和功能。FFR≤0.80被定义为具有血流动力学意义的冠状动脉缺血。结果μQFR与FFR之间存在中等程度的相关性(r=0.75,P<0.001),但Bland-Altman图显示两者之间存在一定的差异(平均差异=0.00;标准差=0.08;P=0.69)。根据FFR标准,μQFR的诊断准确率、敏感度、特异度、阳性预测值(positive predictive value,PPV)和阴性预测值(negative predictive value,NPV)分别为95.56%(95%CI:89.01%~98.78%)、90.91%(95%CI:89.01~98.09)、98.25%(95%CI:90.61~99.96)、96.77(95%CI:81.04~99.53)和94.92%(95%CI:86.38~98.21)。进一步分析表明,μQFR/FFR的一致性与左心室舒张功能和心脏结构异常之间无显著相关性(P>0.05)。结论μQFR与FFR之间存在良好的一致性,且在心脏结构异常和左心室舒张功能障碍患者中,μQFR的诊断准确性并未受到显著影响。 展开更多
