BACKGROUND Myocardial bridge(MB)will compress the mural coronary artery(MCA)during the systole and cause myocardial ischemia.In the diagnosis of coronary heart disease(CHD),because the structure of MB is difficult to ...BACKGROUND Myocardial bridge(MB)will compress the mural coronary artery(MCA)during the systole and cause myocardial ischemia.In the diagnosis of coronary heart disease(CHD),because the structure of MB is difficult to be observed by coronary angiography(CAG),the clinical study of the influence of MB on CHD is lacking.With the advancement of computed tomography coronary angiography technology,detailed observations of the MB anatomy have realized.AIM To explore the main influencing factors of MB-related CHD and to find potential indicators for predicting MB-related CHD.METHODS A total of 1718 patients with suspected CHD due to the symptoms of myocardial ischemia were enrolled as subjects.Patients diagnosed with CHD were included in a CHD group,and patients with no significant abnormalities were included in a control group.In the CHD group,patients were divided into an MB-CHD subgroup if MB-related CHD was found.In the control group,patients were divided into a simple MB subgroup if MB was found.The patient's clinical data and MB-related indicators,including the branch of MB,MB type(superficial/deep type),MB length,MB thickness,systolic and diastolic compression of the MCA,and MCA systolic stenosis rate were recorded and compared.Logistic regression analysis was used to explore the independent influencing factors of MD-related CHD.ROC curve was used to analyze the diagnostic efficacy of potential indicators for MB-related CHD.RESULTS There were 1060 cases in the CHD group and 658 cases in the control group,and there were 236 cases in the MB-CHD subgroup and 52 cases in the simple MB subgroup.Multivariate logistic regression analysis showed that the combined MB had a significant effect on the occurrence of CHD(P<0.05).MB thickness,systolic compression,diastolic compression,and MCA systolic stenosis rate had significant effects on the occurrence of MB-related CHD(P<0.05).The area under the curve(AUC)of the combination of these influencing factors for the diagnosis of MB-related CHD was 0.959,which was significantly higher than the AUCs of the four indicators separately(P<0.05).The sensitivity was 97.06%and the specificity was 87.63%.CONCLUSION MB thickness,systolic compression,diastolic compression,and MCA systolic stenosis are independent influencing factors for MB-related CHD.The combination of these factors has potential diagnostic value for MB-related CHD.展开更多

Background:Spinal injuries are an urgent public health priority;nevertheless,no China-wide studies of these injuries exist.This study measured the incidence,prevalence,causes,regional distribution,and annual trends of...Background:Spinal injuries are an urgent public health priority;nevertheless,no China-wide studies of these injuries exist.This study measured the incidence,prevalence,causes,regional distribution,and annual trends of spinal injuries in China from 1990 to 2019.Methods:We used data from the Global Burden of Diseases,Injuries,and Risk Factors Study 2019 to estimate the incidence and prevalence of spinal injuries in China.The data of 33 provincial-level administrative regions(excluding Taiwan,China)provided by the National Center for Chronic and Noncommunicable Disease Control and Prevention,Chinese Center for Disease Control and Prevention(CDC)were use to systematically analyze the provincial etiology,geographical distribution,and annual trends of spinal injuries.The Bayesian meta-regression tool DisMod-MR 2.1 was used to ensure the consistency among incidence,prevalence,and mortality rates in each case.Results:From 1990 to 2019,the number of living patients with spinal injuries in China increased by 138.32%,from 2.14 million to 5.10 million,while the corresponding age-standardized prevalence increased from 0.20%(95%uncertainty interval[UI]:0.18-0.21%)to 0.27%(95%UI:0.26-0.29%).The incidence of spinal injuries in China increased by 89.91%(95%UI:72.39-107.66%),and the prevalence increased by 98.20%(95%UI:89.56-106.82%),both the most significant increases among the G20 countries;71.00%of the increase could be explained by age-specific prevalence.In 2019,the incidence was 16.47(95%UI:12.08-22.00,per 100,000 population),and the prevalence was 358.30(95%UI:333.96-386.62,per 100,000 population).Based on the data of 33 provincial-level administrative regions provided by CDC,age-standardized incidence and prevalence were both highest in developed provinces in Eastern China.The primary causes were falls and road injuries;however,the prevalence and specific causes differed across provinces.Conclusions:In China,the overall disease burden of spinal injuries increased significantly during the past three decades but varied considerably according to geographical location.The primary causes were falls and road injuries;however,the prevalence and specific causes differed across provinces.展开更多
Fatigue,corrosion,and bolt loosening are the main causes of structural performance degradation and collapse in steel bridges.Accurate monitoring of steel bridge diseases is a basic premise for ensuring high-quality op...Fatigue,corrosion,and bolt loosening are the main causes of structural performance degradation and collapse in steel bridges.Accurate monitoring of steel bridge diseases is a basic premise for ensuring high-quality operation and maintenance of steel bridges.In this regard,a summary and analysis were conducted on the classification of steel bridge diseases,monitoring and detection methods,application statuses,and major difficulties.The main causes,research status,and development trends of steel bridge diseases are discussed.The results showed that,for fatigue crack problems,fatigue crack initiation has a small scale,high difficulty in monitoring and detection,few methods,and low accuracy.As the cracks grow,the difficulty of monitoring and detection decreases,the number of methods increases,and the accuracy improves.Fatigue crack monitoring and detection are affected by the environmental and vehicular loads.Superficial corrosion features are evident in steel bridges,and corrosion identification methods and technologies are rapidly developing.Monitoring and detecting corrosion in concealed areas is difficult and requires further improvements in monitoring and detection technologies and their accuracy.Monitoring and detection methods and supporting equipment for bolt loosening in steel bridges are rapidly developing.The development of intelligent monitoring and detection technologies and supporting equipment is an important research topic that urgently needs to be addressed for the full-lifecycle operation and maintenance of steel bridges and the sustainable development of bridge engineering.Developing new intelligent sensing components based on high-performance materials and sensing element design theory to improve the monitoring and detection perception ability is an important development direction for steel bridge monitoring and detection.Research on intelligent monitoring and detection technologies,standardized indicators,and related topics based on intelligent operations and maintenance provide great support for the development of steel-bridge disease monitoring and detection.展开更多

Increases in experiences with the technique, in combination with advances in technolo...Pure laparoscopic hepatectomy is a less invasive procedure than conventional open hepatectomy for the resection of hepatic lesions. Increases in experiences with the technique, in combination with advances in technology, have promoted the popularity of pure laparoscopic hepatectomy. However, indications for usage and potential contraindications of the procedure remain unresolved. The characteristics and specific advantages of the procedure, especially for hepatocellular carcinoma(HCC) patients with chronic liver diseases,are reviewed and discussed in this paper. For cirrhotic patients with liver tumors, pure laparoscopic hepatectomy minimizes destruction of the collateral blood and lymphatic flow from laparotomy and mobilization, and mesenchymal injury from compression. Therefore, pure laparoscopic hepatectomy has the specific advantage of minimal postoperative ascites production that leads to lowering the risk of disturbance in water or electrolyte balance and hypoproteinemia. It minimizes complications that routinely trigger postoperative serious liver failure. Under adequate patient positioning and port arrangement, the partial resection of the liver in the area of subphrenic space, peri-inferior vena cava area or next to the attachment of retro-peritoneum is facilitated in pure laparoscopic surgery by providing good vision and manipulation in the small operative field.Furthermore, the features of reduced post-operative adhesion, good vision, and manipulation within the small area between the adhesions make this procedure safer in the context of repeat hepatectomy procedures.These improved features are especially advantageous for patients with liver cirrhosis and multicentric and/or metachronous HCCs.展开更多

Most of the premature deaths from NCDs are attributed to modifiable risk factors. In Uttar Pradesh there has been a raise in prevalence of non-communicable diseases in the last two decades. Methodology: Mortality and morbidity data from 1st Jan 2014 to Dec 2016 was obtained from Medical Records Department, UPUMS, Saifai Etawah. The data, thus collected was analyzed by using SPSS 24. Results: Morbidity pattern in UPUMS Saifai reported Blindness and cardiovascular disease (CVD) was leading cause of admission and was increasing year after year. Road traffic acid (RTA), Stroke and CVD were the leading causes of mortality in last 3 years among the patients admitted in tertiary care hospital UPUMS, Saifai, Etawah, U.P., India. Conclusion: Even in rural area prevalence of NCD is at raising trend. RTA, CVD and Stroke were leading causes of death even in rural area.展开更多

Methods: Fifty-three idiopathic PD subjects were assessed on open roads. Prior to the driving assessment, participants were examined by a geriatrician with various clinical assessments. The caregivers filled out a questionnaire, the scores of which is a reflection of their concern on driving performanceof their PD relatives. The same measurements were collected for the subsequent two years. Hierarchical Poisson regression analysis, adjusting for gender, age and driving exposure (hours of driving per week), was then undertaken to determine whether the measures of driving assessment were associated with the score of the questionnaire. Results: During the three-year period, all PD participants were rated at least 3 questions positive in the caregiver’s questionnaire;the worst participant was rated positive eight times. Except the assessment criteria to gauge thetraffic rulesandregulations compliance,all other measures of the driving assessment were found to be significantly associated with the information provided by the caregivers. Conclusions: This study demonstrated that the information provided by caregivers was useful to identify PD patients with deteriorated driving performance. If adopted as part of the off-road driving assessment for PD patients, the questionnaire can provide reliable information to clinicians.展开更多
BACKGROUND Colorectal cancer(CRC)plays a significant role in morbidity,mortality,and economic cost in the Belt and Road Initiative(“B and R”)countries.In addition,these countries have a substantial consumption of pr...BACKGROUND Colorectal cancer(CRC)plays a significant role in morbidity,mortality,and economic cost in the Belt and Road Initiative(“B and R”)countries.In addition,these countries have a substantial consumption of processed meat.However,the burden and trend of CRC in relation to the consumption of a diet high in processed meat(DHPM-CRC)in these“B and R”countries remain unknown.AIM To analyze the burden and trend of DHPM-CRC in the“B and R”countries from 1990 to 2019.METHODS We used the 2019 Global Burden of Disease Study to collate information regarding the burden of DHPM-CRC.Numbers and age-standardized rates(ASRs)of deaths along with the disability-adjusted life years(DALYs)were determined among the“B and R”countries in 1990 and 2019.Using joinpoint regression analysis,the average annual percent change(AAPC)was used to analyze the temporal trends of age-standardized DALYs rate(ASDALR)from 1990 to 2019 and in the final decade(2010–2019).RESULTS We found geographical differences in the burden of DHPM-CRC among“B and R”countries,with the three highest-ranking countries being the Russian Federation,China,and Ukraine in 1990,and China,the Russian Federation,and Poland in 2019.The burden of DHPM-CRC generally increased in most member countries from 1990 to 2019(all P<0.05).The absolute number of deaths and DALYs in DHPM-CRC were 3151.15[95%uncertainty interval(UI)665.74-5696.64]and 83249.31(95%UI 15628.64-151956.31)in China in 2019.However,the number of deaths(2627.57-2528.51)and DALYs(65867.39-55378.65)for DHPM-CRC in the Russian Federation has declined.The fastest increase in ASDALR for DHPM-CRC was observed in Vietnam,Southeast Asia,with an AAPC value of 3.90%[95%confidence interval(CI):3.63%-4.16%],whereas the fastest decline was observed in Kyrgyzstan,Central Asia,with an AAPC value of-2.05%(95%CI:-2.37%to-1.73%).A substantial upward trend in ASR of mortality,years lived with disability,years of life lost,and DALYs from DHPM-CRC changes in 1990-2019 and the final decade(2010-2019)for most Maritime Silk Route members in East Asia,South Asia,Southeast Asia,North Africa,and the Middle East,as well as Central Europe,while those of the most Land Silk Route members in Central Asia and Eastern Europe have decreased markedly(all P<0.05).The ASDALR for DHPM-CRC increased more in males than in females(all P<0.05).For those aged 50-74 years,the ASDALR for DHPM-CRC in 40 members exhibited an increasing trend,except for 20 members,including 7 members in Central Asia,Maldives,and 12 high or high-middle social development index(SDI)members in other regions(all P<0.05).CONCLUSION The burden of DHPM-CRC varies substantially across“B and R”countries and threatens public health.Relevant evidence-based policies and interventions tailored to the different trends of countries in SDIs or Silk Routes should be adopted to reduce the future burden of CRC in“B and R”countries via extensive collaboration.展开更多

A long-span concrete-filled steel tubular(CFST)arch bridge suffers severe vehicle-induced dynamic responses during its service life.However,few quantitative studies have been reported on the typical diseases suffered ...A long-span concrete-filled steel tubular(CFST)arch bridge suffers severe vehicle-induced dynamic responses during its service life.However,few quantitative studies have been reported on the typical diseases suffered by such bridges and their effects on vehicle-induced dynamic response.Thus,a series of field tests and theoretical analyses were conducted to study the effects of typical diseases on the vehicle-induced dynamic response of a typical CFST arch bridge.The results show that a support void results in a height difference between both sides of the expansion joint,thus increasing the effect of vehicle impact on the main girder and suspenders.The impact factor of the displacement response of the main girder exceeds the design value.The variation of the suspender force is significant,and the diseases are found to have a greater effect on a shorter suspender.The theoretical analysis results also show that the support void causes an obvious longitudinal displacement of the main girder that is almost as large as the vertical displacement.The support void can also cause significant changes in the vehicle-induced acceleration response,particularly when the supports and steel box girder continue to collide with each other under the vehicle load.展开更多

文摘BACKGROUND Myocardial bridge(MB)will compress the mural coronary artery(MCA)during the systole and cause myocardial ischemia.In the diagnosis of coronary heart disease(CHD),because the structure of MB is difficult to be observed by coronary angiography(CAG),the clinical study of the influence of MB on CHD is lacking.With the advancement of computed tomography coronary angiography technology,detailed observations of the MB anatomy have realized.AIM To explore the main influencing factors of MB-related CHD and to find potential indicators for predicting MB-related CHD.METHODS A total of 1718 patients with suspected CHD due to the symptoms of myocardial ischemia were enrolled as subjects.Patients diagnosed with CHD were included in a CHD group,and patients with no significant abnormalities were included in a control group.In the CHD group,patients were divided into an MB-CHD subgroup if MB-related CHD was found.In the control group,patients were divided into a simple MB subgroup if MB was found.The patient's clinical data and MB-related indicators,including the branch of MB,MB type(superficial/deep type),MB length,MB thickness,systolic and diastolic compression of the MCA,and MCA systolic stenosis rate were recorded and compared.Logistic regression analysis was used to explore the independent influencing factors of MD-related CHD.ROC curve was used to analyze the diagnostic efficacy of potential indicators for MB-related CHD.RESULTS There were 1060 cases in the CHD group and 658 cases in the control group,and there were 236 cases in the MB-CHD subgroup and 52 cases in the simple MB subgroup.Multivariate logistic regression analysis showed that the combined MB had a significant effect on the occurrence of CHD(P<0.05).MB thickness,systolic compression,diastolic compression,and MCA systolic stenosis rate had significant effects on the occurrence of MB-related CHD(P<0.05).The area under the curve(AUC)of the combination of these influencing factors for the diagnosis of MB-related CHD was 0.959,which was significantly higher than the AUCs of the four indicators separately(P<0.05).The sensitivity was 97.06%and the specificity was 87.63%.CONCLUSION MB thickness,systolic compression,diastolic compression,and MCA systolic stenosis are independent influencing factors for MB-related CHD.The combination of these factors has potential diagnostic value for MB-related CHD.

The paper expounds the related matters of road and bridge engineering from three aspects. Firstly, it expounds the construction principles of road and bridge engineering, which are regarded as the theoretical basis of follow-up research. Secondly, it analyzes the common diseases of road and bridge engineering, including bridgehead damage, rein-forcement corrosion, and subgrade uneven settlement, etc. Finally, it puts forward the construction treatment technology of road and bridge engineering on the basis of the construction principles and taking the common diseases as reference.

文摘Background:Spinal injuries are an urgent public health priority;nevertheless,no China-wide studies of these injuries exist.This study measured the incidence,prevalence,causes,regional distribution,and annual trends of spinal injuries in China from 1990 to 2019.Methods:We used data from the Global Burden of Diseases,Injuries,and Risk Factors Study 2019 to estimate the incidence and prevalence of spinal injuries in China.The data of 33 provincial-level administrative regions(excluding Taiwan,China)provided by the National Center for Chronic and Noncommunicable Disease Control and Prevention,Chinese Center for Disease Control and Prevention(CDC)were use to systematically analyze the provincial etiology,geographical distribution,and annual trends of spinal injuries.The Bayesian meta-regression tool DisMod-MR 2.1 was used to ensure the consistency among incidence,prevalence,and mortality rates in each case.Results:From 1990 to 2019,the number of living patients with spinal injuries in China increased by 138.32%,from 2.14 million to 5.10 million,while the corresponding age-standardized prevalence increased from 0.20%(95%uncertainty interval[UI]:0.18-0.21%)to 0.27%(95%UI:0.26-0.29%).The incidence of spinal injuries in China increased by 89.91%(95%UI:72.39-107.66%),and the prevalence increased by 98.20%(95%UI:89.56-106.82%),both the most significant increases among the G20 countries;71.00%of the increase could be explained by age-specific prevalence.In 2019,the incidence was 16.47(95%UI:12.08-22.00,per 100,000 population),and the prevalence was 358.30(95%UI:333.96-386.62,per 100,000 population).Based on the data of 33 provincial-level administrative regions provided by CDC,age-standardized incidence and prevalence were both highest in developed provinces in Eastern China.The primary causes were falls and road injuries;however,the prevalence and specific causes differed across provinces.Conclusions:In China,the overall disease burden of spinal injuries increased significantly during the past three decades but varied considerably according to geographical location.The primary causes were falls and road injuries;however,the prevalence and specific causes differed across provinces.

文摘Fatigue,corrosion,and bolt loosening are the main causes of structural performance degradation and collapse in steel bridges.Accurate monitoring of steel bridge diseases is a basic premise for ensuring high-quality operation and maintenance of steel bridges.In this regard,a summary and analysis were conducted on the classification of steel bridge diseases,monitoring and detection methods,application statuses,and major difficulties.The main causes,research status,and development trends of steel bridge diseases are discussed.The results showed that,for fatigue crack problems,fatigue crack initiation has a small scale,high difficulty in monitoring and detection,few methods,and low accuracy.As the cracks grow,the difficulty of monitoring and detection decreases,the number of methods increases,and the accuracy improves.Fatigue crack monitoring and detection are affected by the environmental and vehicular loads.Superficial corrosion features are evident in steel bridges,and corrosion identification methods and technologies are rapidly developing.Monitoring and detecting corrosion in concealed areas is difficult and requires further improvements in monitoring and detection technologies and their accuracy.Monitoring and detection methods and supporting equipment for bolt loosening in steel bridges are rapidly developing.The development of intelligent monitoring and detection technologies and supporting equipment is an important research topic that urgently needs to be addressed for the full-lifecycle operation and maintenance of steel bridges and the sustainable development of bridge engineering.Developing new intelligent sensing components based on high-performance materials and sensing element design theory to improve the monitoring and detection perception ability is an important development direction for steel bridge monitoring and detection.Research on intelligent monitoring and detection technologies,standardized indicators,and related topics based on intelligent operations and maintenance provide great support for the development of steel-bridge disease monitoring and detection.
文摘Pure laparoscopic hepatectomy is a less invasive procedure than conventional open hepatectomy for the resection of hepatic lesions. Increases in experiences with the technique, in combination with advances in technology, have promoted the popularity of pure laparoscopic hepatectomy. However, indications for usage and potential contraindications of the procedure remain unresolved. The characteristics and specific advantages of the procedure, especially for hepatocellular carcinoma(HCC) patients with chronic liver diseases,are reviewed and discussed in this paper. For cirrhotic patients with liver tumors, pure laparoscopic hepatectomy minimizes destruction of the collateral blood and lymphatic flow from laparotomy and mobilization, and mesenchymal injury from compression. Therefore, pure laparoscopic hepatectomy has the specific advantage of minimal postoperative ascites production that leads to lowering the risk of disturbance in water or electrolyte balance and hypoproteinemia. It minimizes complications that routinely trigger postoperative serious liver failure. Under adequate patient positioning and port arrangement, the partial resection of the liver in the area of subphrenic space, peri-inferior vena cava area or next to the attachment of retro-peritoneum is facilitated in pure laparoscopic surgery by providing good vision and manipulation in the small operative field.Furthermore, the features of reduced post-operative adhesion, good vision, and manipulation within the small area between the adhesions make this procedure safer in the context of repeat hepatectomy procedures.These improved features are especially advantageous for patients with liver cirrhosis and multicentric and/or metachronous HCCs.
文摘Vaccine is a principal and highly cost-effective medical method on controlling infectious diseases and improving population health.Various vaccines are eagerly needed in the low-income countries along the Belt&Road.With the good quality to reach WHO prequalification standard,and the abundant capacity to fulfill the demand from market abroad,upon the platform of China-proposed"the Belt&Road Initiative",the vaccines manufactured in China are exporting to countries worldwide.The independent innovative vaccines'R&D system,which fruited a series of innovative infectious diseases vaccines(EV71 vaccine,sIPV,HEV,Ebola vaccine,etc.)to be launched in Chinese market,indicates that China has developed rapidly from"a great vaccine-production country"to"a powerful vaccine-innovation country".The implementation of National Innovation-driven Development Strategy would further push forward the developm ent and internationalization process of Chinese innovative vaccines.Therefore,the China-invented vaccines will make an important role in the prevention and control of infectious disease in various countries and become one of the most powerful weapons in fighting the global epidemic event in the future.
文摘Introduction: Non Communicable Diseases (NCD) has emerged as leading threat to mankind. It approximates to around 70% mortality burden worldwide. Its mortality burden is more in low & middle income countries. Most of the premature deaths from NCDs are attributed to modifiable risk factors. In Uttar Pradesh there has been a raise in prevalence of non-communicable diseases in the last two decades. Methodology: Mortality and morbidity data from 1st Jan 2014 to Dec 2016 was obtained from Medical Records Department, UPUMS, Saifai Etawah. The data, thus collected was analyzed by using SPSS 24. Results: Morbidity pattern in UPUMS Saifai reported Blindness and cardiovascular disease (CVD) was leading cause of admission and was increasing year after year. Road traffic acid (RTA), Stroke and CVD were the leading causes of mortality in last 3 years among the patients admitted in tertiary care hospital UPUMS, Saifai, Etawah, U.P., India. Conclusion: Even in rural area prevalence of NCD is at raising trend. RTA, CVD and Stroke were leading causes of death even in rural area.
文摘Objectives: This study aims to examine whether information provided by spouse or relatives can be employed to identify PD patients with deteriorated driving performance, using three-year caregiver’s reports on their driving ability as the outcome measure. Methods: Fifty-three idiopathic PD subjects were assessed on open roads. Prior to the driving assessment, participants were examined by a geriatrician with various clinical assessments. The caregivers filled out a questionnaire, the scores of which is a reflection of their concern on driving performanceof their PD relatives. The same measurements were collected for the subsequent two years. Hierarchical Poisson regression analysis, adjusting for gender, age and driving exposure (hours of driving per week), was then undertaken to determine whether the measures of driving assessment were associated with the score of the questionnaire. Results: During the three-year period, all PD participants were rated at least 3 questions positive in the caregiver’s questionnaire;the worst participant was rated positive eight times. Except the assessment criteria to gauge thetraffic rulesandregulations compliance,all other measures of the driving assessment were found to be significantly associated with the information provided by the caregivers. Conclusions: This study demonstrated that the information provided by caregivers was useful to identify PD patients with deteriorated driving performance. If adopted as part of the off-road driving assessment for PD patients, the questionnaire can provide reliable information to clinicians.

文摘BACKGROUND Colorectal cancer(CRC)plays a significant role in morbidity,mortality,and economic cost in the Belt and Road Initiative(“B and R”)countries.In addition,these countries have a substantial consumption of processed meat.However,the burden and trend of CRC in relation to the consumption of a diet high in processed meat(DHPM-CRC)in these“B and R”countries remain unknown.AIM To analyze the burden and trend of DHPM-CRC in the“B and R”countries from 1990 to 2019.METHODS We used the 2019 Global Burden of Disease Study to collate information regarding the burden of DHPM-CRC.Numbers and age-standardized rates(ASRs)of deaths along with the disability-adjusted life years(DALYs)were determined among the“B and R”countries in 1990 and 2019.Using joinpoint regression analysis,the average annual percent change(AAPC)was used to analyze the temporal trends of age-standardized DALYs rate(ASDALR)from 1990 to 2019 and in the final decade(2010–2019).RESULTS We found geographical differences in the burden of DHPM-CRC among“B and R”countries,with the three highest-ranking countries being the Russian Federation,China,and Ukraine in 1990,and China,the Russian Federation,and Poland in 2019.The burden of DHPM-CRC generally increased in most member countries from 1990 to 2019(all P<0.05).The absolute number of deaths and DALYs in DHPM-CRC were 3151.15[95%uncertainty interval(UI)665.74-5696.64]and 83249.31(95%UI 15628.64-151956.31)in China in 2019.However,the number of deaths(2627.57-2528.51)and DALYs(65867.39-55378.65)for DHPM-CRC in the Russian Federation has declined.The fastest increase in ASDALR for DHPM-CRC was observed in Vietnam,Southeast Asia,with an AAPC value of 3.90%[95%confidence interval(CI):3.63%-4.16%],whereas the fastest decline was observed in Kyrgyzstan,Central Asia,with an AAPC value of-2.05%(95%CI:-2.37%to-1.73%).A substantial upward trend in ASR of mortality,years lived with disability,years of life lost,and DALYs from DHPM-CRC changes in 1990-2019 and the final decade(2010-2019)for most Maritime Silk Route members in East Asia,South Asia,Southeast Asia,North Africa,and the Middle East,as well as Central Europe,while those of the most Land Silk Route members in Central Asia and Eastern Europe have decreased markedly(all P<0.05).The ASDALR for DHPM-CRC increased more in males than in females(all P<0.05).For those aged 50-74 years,the ASDALR for DHPM-CRC in 40 members exhibited an increasing trend,except for 20 members,including 7 members in Central Asia,Maldives,and 12 high or high-middle social development index(SDI)members in other regions(all P<0.05).CONCLUSION The burden of DHPM-CRC varies substantially across“B and R”countries and threatens public health.Relevant evidence-based policies and interventions tailored to the different trends of countries in SDIs or Silk Routes should be adopted to reduce the future burden of CRC in“B and R”countries via extensive collaboration.
文摘In this letter,we comment on a recent case report by Sun et al in the World Journal of Cardiology.The report describes the successful management of a rare complication:The unloading or detachment of a bioresorbable stent(BRS)during percutaneous coronary intervention(PCI)in a male patient.The unloading of BRS was detected via angiography and intravascular ultrasound(IVUS)imaging of the left coronary artery and left anterior descending artery.Although this case is interesting,the authors’report lacked crucial details.Specifically,insufficient information about the type of BRS used,potential causes of BRS unloading,or whether optical coherence tomography(OCT)imaging for coronary arteries was performed before,during,or after PCI.The OCT imaging of coronary arteries before PCI can potentially prevent BRS unloading due to its higher resolution compared to IVUS.In addition,despite detecting myocardial bridging during the PCI,the authors did not provide any details regarding this variation.Here we discuss the various types of BRS,the importance of OCT in PCI,and the clinical relevance of myocardial bridging.

文摘A long-span concrete-filled steel tubular(CFST)arch bridge suffers severe vehicle-induced dynamic responses during its service life.However,few quantitative studies have been reported on the typical diseases suffered by such bridges and their effects on vehicle-induced dynamic response.Thus,a series of field tests and theoretical analyses were conducted to study the effects of typical diseases on the vehicle-induced dynamic response of a typical CFST arch bridge.The results show that a support void results in a height difference between both sides of the expansion joint,thus increasing the effect of vehicle impact on the main girder and suspenders.The impact factor of the displacement response of the main girder exceeds the design value.The variation of the suspender force is significant,and the diseases are found to have a greater effect on a shorter suspender.The theoretical analysis results also show that the support void causes an obvious longitudinal displacement of the main girder that is almost as large as the vertical displacement.The support void can also cause significant changes in the vehicle-induced acceleration response,particularly when the supports and steel box girder continue to collide with each other under the vehicle load.

文摘Digital cable-stayed bridge maintenance and management system (DCBMS) was developed for the need of maintenance and management of long-span cable-stayed bridges. In this paper, the major functions and theoretical application of eight modules were systematically stated with the background of Harbin Songhua River cable-stayed bridge, which include data management module, inspection and measurement module, assessment module, finite dement analysis module, disease diagnosis and prediction module, maintenance module, query module and help module. By analyzing and calculating the data from manual inspection database, basic database and health monitoring subsystem, DCBMS can accomplish the functions like life prediction, disease diagnosis, comprehensive assessment, maintenance and management of bridges. Therefore, the maintenance and management of long-span cable-stayed bridges can be made digital, professional and scientific. By running this system, a real-time and specific technical guidance can be provided for the maintainers and managers of long- span cable-stayed bridges.
