Safety and Quality of Care Evaluation in Japanese Long-Term Facilities Focused on the Activity Condition and Cognitive Function of Residents 被引量:1

Objective: Currently, standards for evaluating long-term care facilities do not exist in Japan. This study aimed to evaluate the quality of healthcare services in Japan’s long-term care facilities and identify the structural and process indicators associated with the facilities’ outcome indicators. Methods: This retrospective study assessed changes in residents’ abilities to participate in physical activities, their cognitive function, and their vulnerability to injuries. From 2012 to 2013, we collected information on the healthcare services at 1067 long-term care facilities registered with Japan’s Welfare, Health and Medical Care Information Network in the Welfare and Medical Service Agency. We examined 12 structural indicators, 26 process indicators, and 7 outcome indicators. We used multivariate linear regression models adjusted to analyze relationships between outcome indicators and structural or process indicators. Results: Residents’ activity and cognitive function indicators either improved by 80% - 90% or were maintained for one year. The Geriatric Functional Independence Measures, the Barthel Index, and holding conferences related to care were all considered activities of daily living. Three adverse events—tumbles and falls, behavioral problems, and aimless wandering or leaving the facility without permission—were factors that restricted residents’ behavior and number of residents per care staff member. Conclusions: Maintaining or improving levels of independence and cognition in daily living requires a care process system that enables ongoing monitoring of residents’ activities of daily living and cognitive functioning. Ensuring the safety of residents and improving the quality of care in long-term care facilities without securing adequate care staff are not possible. 展开更多

Objective This study aimed to compare and analyze the clinical efficacy and safety of late-course and simultaneous integrated dose-increasing intensity-modulated radiation therapy(IMRT) for cervical cancer complicated with pelvic lymph node metastasis. Methods Sixty patients with cervical cancer complicated with pelvic lymph node metastasis who were admitted to our hospital from January 2013 to January 2015 were enrolled. The patients were randomly divided into the late-course dose-increasing IMRT group and the simultaneous integrated dose-increasing IMRT group, with 30 cases included in each group, respectively. All patients were concurrently treated with cisplatin. After treatment, the clinical outcomes of the two groups were compared. Results The remission rate of symptoms in the simultaneous integrated dose-increasing IMRT group was significantly higher than that in the late-course dose-increasing IMRT group(P < 0.05). The follow-up results showed that the overall survival time, progression-free survival time, and distant metastasis time of patients in the simultaneous integrated dose-increasing IMRT group were significantly longer than those in the late-course dose-increasing IMRT group(P < 0.05). The recurrent rate of lymph nodes in the radiation field in the simultaneous integrated dose-increasing IMRT group was significantly lower(P < 0.05) than in the late-course dose-increasing IMRT group. There was no significant difference in the incidence of cervical and vaginal recurrence and distant metastasis between the two groups(P > 0.05). The radiation doses of Dmax in the small intestine, D1 cc(the minimum dose to the 1 cc receiving the highest dose) in the bladder, and Dmax in the rectum in the simultaneous integrated dose-increasing IMRT group were significantly lower(P < 0.05) than in the late-course dose-increasing IMRT group. There was no significant difference in intestinal D2 cc(the minimum dose to the 2 cc receiving the highest dose) between the two groups(P > 0.05). The incidence of bone marrow suppression in the simultaneous integrated dose-increasing IMRT group was significantly lower(P < 0.05) than in the late-course dose-increasing IMRT group.Conclusion The application of simultaneous integrated dose-increasing IMRT in the treatment of cervical cancer patients complicated with pelvic lymph node metastasis can significantly control tumor progression, improve the long-term survival time, and postpone distant metastasis time with high safety. 展开更多

This study evaluated the impacts of food safety policies on Japan's Simultaneous Buy and Sell rice imports through measuring tariff equivalents of food safety policies.In order to construct an estimated model,a Japanese consumer's utility function is introduced and developed with consumer's preference parameters and elasticity of substitution.In the empirical study part,Japan's positive list system and rice traceability were analyzed and assessed as critical food safety policies.Results showed that after the implementation of the positive list system,consumers'preference for foreign rice and the substitution elasticity diminished.This decreasing tendency was quite similar to the results after the enforcement of rice traceability.The tariff equivalents of food safety policies on imported rice fluctuated around ¥50 yen/kg from fiscal year 2000 to 2005 and decreased because of the global grain price hike after 2006.The tariff equivalents soared in 2010,which was induced by the traceability regulation,and then dulled during Japan's earthquake and tsunami in 2011.Subsequently,after the recovery from natural disasters,the tariff equivalents of food safety policies became higher.Therefore,food safety policies had made imported rice less attractive,weakened the competitive power of rice exporting countries,and had statistically significant impacts on Japan's rice importation. 展开更多

In recent years,the safety of oil and gas storage and transportation facilities has been paid more attention by the state and enterprises due to frequent accidents.The oil and gas storage and transportation facilities safety courses in China University of Petroleum(Beijing)includes“Engineering mechanics”,“Strength design of pipelines and tanks”and“Safety and integrity management of oil and gas storage and transportation facilities”.The three courses lack relevance and the teaching mode is too rigid,resulting in students losing their initiative in learning.If students can’t use the knowledge flexibly,it will affect the achievement of the objectives of the training program.Therefore,oil and gas storage and transportation facilities safety courses are reformed,training plans are adjusted and teaching methods are improved.The practice shows that the reform enriches the teaching content,improves the teaching quality,stimulates classroom activity and gets a good evaluation of students.The reform of safety courses has a certain significance for cultivating compound talents who have the ability to solve practical problems in engineering. 展开更多

The safety shutter of the SR (synchrotron radiation) facility located at the front end of the facility is an in- dispensable component for radiation protection. Its thickness is decided by the gas bremsstrahlung produced in the SR facility storage ring by the interaction of electrons with the residual gas molecules in the vacuum chamber of the storage ring. In the calculation, the 3.5 GeV, 300 mA electron beam and a 15 m long insertion-device straight section (0.133 μ Pa) were taken into account, and the safety shutter was assumed to be located 12 m away from the end of the straight section. The EGSnrc code based on the Monte-Carlo method and empirical formulas were used, respectively, to calculate the thickness to satisfy the shielding requirement of the safety shutter at the front end of the SR facility, and the results were compared and the availability of EGSnrc was proved. 展开更多

高速公路风险防控设施的合理应用能够显著降低交通运行风险。本文首先针对高速公路风险防控设施的研究现状进行了回顾与总结,将高速公路风险防控设施划分为非电气化防控设施和电气化防控设施。其次,归纳了高速公路非电气化和电气化防控... 高速公路风险防控设施的合理应用能够显著降低交通运行风险。本文首先针对高速公路风险防控设施的研究现状进行了回顾与总结,将高速公路风险防控设施划分为非电气化防控设施和电气化防控设施。其次,归纳了高速公路非电气化和电气化防控设施在风险防控中的作用以及研究中的关键问题,讨论了不同电气化风险防控设施的应用场景及其效用。然后,对由电气化设施构成的风险防控系统在多种典型场景中的应用效果进行了分析。最后,对高速公路风险防控设施应用综述结果及未来发展情况进行了总结。文献综述结果表明,高速公路风险防控设施和系统在不同场景中均能起到积极的效果;非电气化防控设施受限于自身物理特性,在应对动态信息发布和主动性、针对性风险防控上存在一定的不足;电气化风险防控设施以及风险防控系统的应用越来越广泛,对交通拥堵、不良天气等交通风险的防控愈加完善。同时,日渐成熟的车路协同技术和不断发展的智慧公路系统也是未来保障高速公路交通安全的重要一环。本文的研究成果可在一定程度上为今后高速公路风险防控设施的选型、设置及应用提供一定的借鉴。 展开更多

目的探究卡瑞利珠单抗联合同步放化疗对食管癌患者的治疗效果及安全性。方法回顾性分析2020年9月至2023年9月于运城市中心医院就诊的156例食管癌患者的临床资料。根据不同治疗方式将患者分为联合组(卡瑞利珠单抗联合同步放化疗,78例)和... 目的探究卡瑞利珠单抗联合同步放化疗对食管癌患者的治疗效果及安全性。方法回顾性分析2020年9月至2023年9月于运城市中心医院就诊的156例食管癌患者的临床资料。根据不同治疗方式将患者分为联合组(卡瑞利珠单抗联合同步放化疗,78例)和对照组(同步放化疗,78例)。比较两组临床疗效、程序性死亡受体1(PD-1)和细胞程序性死亡-配体1(PD-L1)水平、生活质量及不良反应发生情况。结果治疗后,联合组患者的客观缓解率和疾病控制率分别为79.49%(62/78)和96.15%(75/78),高于对照组的60.26%(47/78)和82.05%(64/78)(χ^(2)=6.851,7.988,均P<0.05)。治疗后,联合组患者的PD-1、PD-L1水平[分别为(201.81±16.12)ng·L^(-1)、(264.32±25.10)ng·L^(-1)]均低于对照组[分别为(232.23±19.58)ng·L^(-1)、(301.28±27.42)ng·L^(-1)](t=10.593、8.781,均P<0.05);联合组患者的生活质量改善率为85.90%(67/78),高于对照组患者的71.79%(56/78)(χ^(2)=4.650,P<0.05)。在不良反应方面,两组患者的消化道反应、白细胞减少、放射性食管炎、贫血及血小板减少发生率较高,但多为1~2级不良反应,大部分患者可耐受,经对症处理后未影响治疗进程。两组不良反应发生率比较差异均无统计学意义(均P>0.05)。结论卡瑞利珠单抗联合同步放化疗对食管癌患者具有良好的治疗效果,能降低血清PD-1、PD-L1水平,改善患者生活质量,相较于常规同步放化疗并未增加不良反应发生风险。 展开更多

公路轻量化护栏材料选型与结构设计

传统碳钢护栏存在单位体积重量大、生产和运输过程碳排放量大、产品重复利用率低以及在碰撞事故中车辆容易冲入对向车道等缺点。文章优选一种新型不锈钢(QN1701)作为轻量化护栏材料,化学成分分析显示具有高锰、高铬和高PREN值特点,其抗... 传统碳钢护栏存在单位体积重量大、生产和运输过程碳排放量大、产品重复利用率低以及在碰撞事故中车辆容易冲入对向车道等缺点。文章优选一种新型不锈钢(QN1701)作为轻量化护栏材料,化学成分分析显示具有高锰、高铬和高PREN值特点,其抗拉强度达到740 MPa,屈服强度为430 MPa,延伸率为54%,比传统碳钢(Q235)相应力学指标提高了56%~72%,因此具有更高的结构安全性;经Newmark-β法理论计算,QN1701护栏结构体系可以有效减轻碰撞严重程度,保护车辆不冲出护栏,设计防护等级为SB级的护栏结构同比Q235护栏轻量化率达到了48.3%,数值仿真结果表明,新型不锈钢护栏的安全性能满足SB级防护等级要求。 展开更多

公路新型波形梁护栏结构仿真优化研究

为了解决开发公路新型护栏过程中需要实车足尺碰撞导致的经济代价高昂问题,论文采用有限元计算仿真设计了一种由轻量化不锈钢三波板和低合金高强度结构钢组合而成的公路新型SB级护栏结构。在深入分析材料特性基础上,构建了新型护栏立柱... 为了解决开发公路新型护栏过程中需要实车足尺碰撞导致的经济代价高昂问题,论文采用有限元计算仿真设计了一种由轻量化不锈钢三波板和低合金高强度结构钢组合而成的公路新型SB级护栏结构。在深入分析材料特性基础上,构建了新型护栏立柱和防阻块及相应的有限元模型。在验证整车模型可靠性基础上,设计了护栏板厚度、立柱厚度和防阻块厚度三因素四水平的正交实验仿真方案。碰撞仿真结果表明:三因素中防阻块厚度对车辆质心加速度的影响程度最大,且随着防阻块厚度的逐步增大,车辆质心加速度增大;护栏板厚度对护栏横向最大动态变形量的影响程度最大,且随着波形梁板的厚度逐步增大,护栏的最大动态变形量变小。最后,综合考虑碰撞指标和经济性,确定了护栏最优组合方案并验证了可行性。研究成果可为公路新型护栏结构优化提供参考。 展开更多

一期双侧膝关节内侧单间室置换的安全性研究

目的探讨一期双侧膝关节内侧单间室置换(OB-UKA)的安全性。方法回顾性分析2021年1月—2023年1月在南京鼓楼医院进行OB-UKA手术的35例膝关节骨性关节炎患者的临床资料,并在同期行单侧膝关节内侧单间室置换(UKA)和单侧全膝关节置换(TKA)... 目的探讨一期双侧膝关节内侧单间室置换(OB-UKA)的安全性。方法回顾性分析2021年1月—2023年1月在南京鼓楼医院进行OB-UKA手术的35例膝关节骨性关节炎患者的临床资料,并在同期行单侧膝关节内侧单间室置换(UKA)和单侧全膝关节置换(TKA)的患者中选择年龄、性别匹配者各35例作为对照,比较3组患者手术相关指标(手术时间、总失血量、住院时间)、营养状态[血红蛋白(Hb)、白蛋白(ALB)]、炎症指标[白细胞计数(WBC)、C反应蛋白(CRP)]、疼痛程度、膝关节功能评分及并发症。结果术前3组间一般资料的差异无统计学意义(P>0.05)。OB-UKA组手术时间较UKA组和TKA组长(P<0.05),总失血量与TKA组比较差异无统计学意义(P>0.05),明显高于UKA组(P<0.05);术后OB-UKA组和UKA组Hb、ALB比较差异无统计学意义(P>0.05);OB-UKA组和TKA组术后WBC、CRP比较差异无统计学意义(P>0.05);OB-UKA组术后视觉模拟评分(VAS)与UKA组比较差异无统计学意义(P>0.05),明显低于TKA组(P<0.05);OB-UKA组美国特种外科医院膝关节功能评分(HSS)与UKA组比较差异无统计学意义(P>0.05),但明显高于TKA组(P<0.05);UKA组、OB-UKA组并发症发生率均为2.9%,低于TKA组并发症发生率(5.7%)(P<0.05)。结论OB-UKA与UKA相比,手术时间延长,失血量增加,但术后营养相关指标与UKA水平相似,疼痛程度较低,膝关节功能评分与UKA相当。此外,OB-UKA术后炎症相关指标与TKA水平相似,围术期并发症发生率低于TKA,且与UKA相似。OB-UKA可以作为双膝内侧间室退变患者的安全有效的首选术式。 展开更多
