目的观察妊娠期高血压疾病(hypertension disorder of pregnancy,HDP)患者血浆内皮素-1(ET-1)和一氧化氮(NO)水平变化及其对肾动脉血流动力学的影响。方法选取2013年6月至2015年6月于本院接受治疗的90例HDP患者为研究对象,根据病史、临...目的观察妊娠期高血压疾病(hypertension disorder of pregnancy,HDP)患者血浆内皮素-1(ET-1)和一氧化氮(NO)水平变化及其对肾动脉血流动力学的影响。方法选取2013年6月至2015年6月于本院接受治疗的90例HDP患者为研究对象,根据病史、临床表现及体征不同将其分为妊娠高血压组(34例)、轻度子痫前期组(30例)及重度子痫前期组(26例)。同时选取本院建卡的30例健康妊娠女性纳入对照组。检测并比较四组孕妇血浆ET-1与NO水平、肾功能与肾动脉血流动力学指标的差异,分析ET-1、NO水平与肾功能、肾动脉血流动力学的相关性。结果轻度子痫前期组与重度子痫前期组孕妇ET-1水平均显著高于妊娠高血压组与对照组(P<0.05),而NO水平均低于妊娠高血压组与对照组(P<0.05);轻度子痫前期组孕妇ET-1水平显著低于重度子痫前期组,而NO水平显著高于重度子痫前期组(P<0.05)。重度子痫前期组孕妇血浆BUN、Scr、血尿素及血尿酸水平均显著高于其他三组(P<0.05);轻度子痫前期组孕妇上述指标水平均显著高于妊娠高血压组和对照组;妊娠高血压组孕妇上述指标水平均显著高于对照组(P<0.05)。四组孕妇PSV比较差异均无显著性(P>0.05);轻度子痫前期组和重度子痫前期组孕妇EDV、RI、PI及S/D比较差异均无显著性(P>0.05),且均显著高于妊娠高血压组和对照组(P<0.05);妊娠高血压组和对照组孕妇各肾血流动力学指标比较差异均无显著性(P>0.05)。ET-1水平与RI、PI、S/D、BUN、Scr、血尿素、血尿酸水平呈正相关(P<0.05),与EDV呈负相关(P=0.035)。NO水平与RI、PI、S/D、BUN、Scr、血尿素、血尿酸水平呈负相关(P<0.05),与EDV呈正相关(P=0.021)。ET-1、NO水平与PSV均无明显相关性(P>0.05)。结论血浆ET-1、NO水平在HDP患者中表达异常,ET-1水平升高、NO水平下降提示HDP患者肾功能损伤。同时ET-1、NO水平与肾入球小动脉的血流动力学阻力指标等具有明显相关性,可能是导致HDP患者疾病加重时出现肾脏受累的重要原因之一。展开更多
