文摘BACKGROUND While Crohn’s disease has been studied extensively in high-income countries, its epidemiology and care in low and lower-middle income countries(LLMICs)is not well established due to a lack of disease registries and diagnostic capacity.AIM To describe the published burden,diagnostic/treatment capacity, service utilization,challenges/barriers to individuals with Crohn’s in LLMICs and their providers.METHODS We conducted a scoping review utilizing a full search strategy was developed and conducted in PubMed,Embase and World Health Organization Global Index Medicus. Two independent reviewers screened the titles and abstracts of all of the publications found in this search, reviewed selected publications, and extracted relevant data, which underwent descriptive review and was analyzed in Excel.RESULTS The database search yielded 4486 publications,216 of which were determined to be relevant to the research questions.Of all 79 LLMICs,only 21(26.6%)have publications describing individuals with Crohn’s.Overall, the highest number of studies came from India, followed by Tunisia, and Egypt. The mean number of Crohn’s patients reported per study is 57.84 and the median is 22,with a wide range from one to 980.CONCLUSION This scoping review has shown that, although there is a severe lack of populationbased data about Crohn’s in LLMICs,there is a signal of Crohn’s in these settings around the world.
文摘Objective:The aim of this scoping review was to provide evidence for health practitioners to improve patient education practice for chronic pain management. Methods:A scoping review was guided by Arksey and O’Malley’s(2005)1 five-stage framework, investigated contemporary patient education programs(2007–2018)for chronic pain management in education content, formats of delivery, and tools used for evaluation.Content analysis and description were used for the outcome report. Results:Seven quantitative studies were included.Education content consisted of General information,Cognitive behavior therapy(CBT),Self-management,and Pain neurophysiology(PN).Education delivery formats varied from workbook to workbook,face-toface,online,when given for a group or individual or in a combined way.In total,19 tools were reported for the evaluation of the education programs. Conclusions:There is a variety in the education content and the delivery formats. The majority of programs showed effectiveness in patients’chronic pain management based on their selected evaluation tools. This review showed that patient education programs can be useful in chronic pain management. The effectiveness of patient education programs focuses on the improved patients’physical function and quality of life rather than the cessation of pain only.
文摘Objective:To ensure that only competent graduates are licensed to practice nursing, councils conduct licensing examinations, which may include among others clinical competency assessment. This review explored current practices in clinical competency assessment of nursing students as part of a larger study aimed at developing an evidence-based,context-specific framework for clinical competency assessment in a sub-Saharan African(SSA)country. Methods:A scoping guided by the Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews(PRISMA-ScR)was conducted. Results:Findings from 28 out of 1151 studies identified from Scopus,PubMed,CINAHL,Wiley Online Library, and ProQuest were included and synthesized. Results show that a good assessment system must be valid, reliable, transparent, feasible, fair, objective, and must provide feedback and continually improve to have an educational impact.Clinical competency assessment systems must be developed on sound empirical evidence, pilot tested, and involve thorough training and evaluation of the examiners.Continuous evaluation of the assessment system is also essential to ensure the quality and relevance of the assessment system.Only one of the included studies was conducted in Africa. Conclusions:The paucity of clinical competency assessment research in sub-Saharan Africa may lead to benchmarking assessment systems on research conducted outside the context.Sub-Saharan Africa has a set of circumstances that demand a context-specific clinical competency assessment framework to guide clinical competency assessment.
文摘Purpose: This review paper aimed to provide an overview of the published nursing research related to the COVID-19 pandemic over the last six months. Method: Online Google Scholar database was searched for COVID-19 related articles published between the 1st of Jan 2020 and 15th of June 2020. Findings: The initial search found 1930 results from combining the search between the words “nurse” and “nursing” with COVID-19. Only 48 articles met the eligibility criteria. Most of the studies focused on psychological factors (33.3%), followed by clinical practices (29.2%). The majority of the authors have an academic affiliation (70.8%), only (12.5%) have a clinical affiliation. About (21%) of the articles were funded, (90%) of this fund came from China. Most of the authors utilized the quantitative approach (27.1%);the qualitative approach has been used by (4.2%);only one article (2.1%) utilized the mixed methodology. No study used a theoretical or conceptual framework. Mainly, the nurses conducted their research independently (52.1%), while (35.4%) has been done in a multidisciplinary team;35.3% of them were done in collaboration with physicians. Conclusion: The results indicate that the nurses responded immediately to the COVID-19 pandemic by conducting researches mainly by utilizing a quantitative approach and writing a lot of editorials. To enrich the research results, nurses worked in research in collaboration with other disciplines, especially physicians. Still, the nursing research funding is under the limit. Stress and psychological factors are the primary concern for the nurses during the COVID-19 pandemic.
