The aim of this study was to assess the prevalence of risk factors and cardiovascular diseases according to residents in town of Bougouni and its rural area (southern Mali). Methods: This was a retrospective descriptive study from February 2019 to March 2024. All patients who visited the “CENTRE MEDICAL DE BOUGOUNI” clinic with known or suspected heart disease during the study period were included. Data were collected from medical records. Incomplete or unusable records were excluded. Data were processed using SPS version 22 software. Results: we included 452 patients with an average age of 50 ± 19 years. High blood pression has been found in 42.70% of cases, with higher prevalence in urban areas (47.01%) than in rural areas (41.50%). Diabetes and smoking were found respectively 5.31% and 8.19% in our population. The prevalence of diabetes was higher in urban areas (5.97%) than rural area (5.35%). smoking prevalence was higher in rural areas (8.49%) than urban areas (6.72%). Alcoholism was found in 2.21% of cases with a little difference between rural area (2.24%) and urban area (2.20%). 31.63% of our patients had no cardiovascular factor. Dyspnea was the reason of consultation in 5.75% of cases and atypical precordialgia in 13.72%. Headaches and dizziness in 8.63% of cases and 21.90% of patients presented other non-specific symptoms. Diagnosis of idiopathic dilated cardiomyopathy was found in 23.45% of cases with higher prevalence in rural areas (28.30%) than urban areas (13.43%). Ischaemic heart disease accounted for 9.29% of cases with a higher prevalence in rural areas (10.38%) than in urban areas (6.71%). PPCMs accounted for 5.98% of cases, with a slight increase in urban areas (6.66%) compared with rural areas (5.68%). Rheumatic valvulopathy accounted for 2.21% of cases;2.84% in rural areas and 0.74% in urban areas. Other valvular diseases accounted for 3.76% of cases;4.73% in rural areas and 1.48% in urban areas. Right heart diseases accounted for 3.98% of cases and stroke for 1.77%. Conclusion: The prevalence of cardiovascular risk factors and heart diseases is high in the rural and urban population of the town of Bougouni and its surroundings (Mali). Excepted smoking, the prevalence of other CV risk factors is higher in urban areas, whereas the prevalence of idiopathic dilated cardiomyopathy and ischemic heart disease is higher in rural areas. These data could serve as a basis for developing tailored preventive strategies, depending on the place of residence.展开更多

Yet, sorghum productivity and profitability are impacted by diseases. In the 2022 growing season, 122 farmers’ fields across 7 regions, notably Diourbel, Fatick, Kaffrine, Kaolack, Kolda, Tambacounda, and Thies were surveyed for foliar and panicle diseases. During the survey, stops were made at 30 km intervals and at each stop, 2 - 5 fields were evaluated. In each field, 40 plants mostly at soft to early hard dough stages of development were assessed using a W-shaped pattern to cover the whole field. A total of 13 diseases, including leaf blight, anthracnose, Zonate leaf spot, Sooty stripe, rough leaf spot, oval leaf spot, long smut, grain mold, and covered kernel smut were documented. The most predominant diseases were leaf blight, anthracnose, and zonate leaf spot. The prevalence of leaf blight was 100%, while anthracnose and zonate leaf spot were found in 93 out of the 122 fields surveyed, indicating a 76% prevalence, respectively. Across the regions, the prevalence of rough leaf spot was 47%, covered kernel smut (32%), oval leaf spot (19%) and target leaf spot (19%). Mean incidence of leaf blight was high in all regions, ranging from 94% (Kaolack) to 100% (Fatick and Tambacounda). The highest mean incidence of anthracnose (62%) and covered kernel smut (16%) was noted in Tambacounda region. In the region of Thies, the highest mean incidence of zonate leaf spot (49%) was recorded. The mean severity of leaf blight (37%) was highest on plants assessed in the region of Kaffrine, followed by those in the regions of Kaolack, Tambacounda, and Kolda. Anthracnose infection was most intense on plants evaluated in Kolda, while zonate leaf spot was most severe in the region of Tambacounda. Fields with incidences of 85% and above were considered as “hotspots” to evaluate sorghum germplasm for disease resistance. This survey is significant because the information obtained will be beneficial to plant pathologists, sorghum producers, government officials, and funding agencies to prioritize research projects that ensure productivity and food security.展开更多

E Method] The 6-week-old SPF chickens were respectively inoculated with five live attenuated Newcastle disease vaccines (A, B, C, D, and E) at the dose of 1-fold or 0.01-fold usage via intranasal, intraocular or oral route. After 14 d post immunization, the titers of HI antibody were detected. And all the chickens were chal- lenged by Newcastle disease virus (NDV) FEo standard strain or the isolated wild strains, NDV-2007-HB and NDV-2008-YB. The clinical symptoms of chickens were continuously observed, and the morbidity and mortality were determined. [ Resalt] After 14 d post immunization, antibodies were induced at a protective level in chickens immunized with the five vaccines. As shown by the animal experiment, the five vaccines at the dose of 1-fold or 0.01-fold usage protected all vaccinated chickens from the death caused by NDV strain, and more than 90% of vaccinated chick- ens from the death caused by NDV-2007-HB strain, while the vaccines, A, B, and C, at the dose of 0.01 -fold usage protected more than 90% of vaccinated chickens from the death caused by NDV-2008-YB strain. E Conclusion] Under laboratory conditions, the currently available Newcastle disease vaccines have better protective efficacy against the two currently prevalent NDV strains and prevent the occurrence of Newcastle disease.展开更多

The considerable financial and public health burden associated with chronic kidney disease can be lessened by raising awareness and adopting better practices for its impact, prevention, and early identification. Objective: In this study, individuals with hypertension and diabetes were evaluated for their knowledge of chronic kidney disease, its prevalence, and its risk factors. Method: It was a hospital-based cross-sectional study conducted on adult (>18 years) patients with diabetes mellitus and hypertension. Each participant provided written informed consent before having their data collected through interviews, medical information, and blood samples for CKD screening. The CKD epidemiology collaboration (CKD-EPI) equation was used to calculate the glomerular filtration rate (GFR) from serum creatinine, and CKD was determined using the estimated GFR (e-GFR). To find independent CKD factors, multivariate logistic regression was employed, with a p-value of 0.05 being regarded as statistically significant. This was accomplished using SPSS (Statistical Program for Social Sciences) version 22.0, IBM Corp., Armonk, NY. Result: A total of 156 participants took part in the study among which 95 (60.9%) were male, most of the participants 82 (52.6%) were aged between 51 - 70 years (mean 59.42 ± 11.007), 76 (48.7%) were unemployed and 97 (62.2%) were single. Overall, the knowledge score of participants on CKD was 65.4% for good knowledge and 34.6% for poor or inadequate knowledge of CKD. More than half of the participants (60%) had chronic kidney disease. Among these, the greatest proportion of CKD patients were those who were hypertensive (88.2%) followed by those who were both hypertensive and diabetic (70.7%). Conclusion: There is poor management of CKD in the South West Region of Cameroon which has contributed greatly to the progression of CKD and increases in the mortality rate.展开更多

METHODS: A total of 2356 subjects were randomly chosen for the sample population based on the database of the Turkish Statistical Agency in Eskisehir. Of the invited 2356 subjects, 2017 subjects participated, out of which 2009 were eligible for the study. Systemic diseases, drug use, smoking and body mass index were assessed using questionnaires. Measurements of central corneal thickness (CCT), anterior chamber depth (ACD) and intraocular pressure (IOP) were performed during June to December, 2015. After pupillary dilation, the anterior segment and lenses were evaluated using a slit lamp.RESULTS: Prevalence of PEX in this sample was 5.0% (n=100). Of patients with PEX, 26% also had glaucoma. Incidence of cataracts, and using drugs for hypertension, cardiac and psychiatric conditions was higher in patients with PEX compared to normal cases (P〈0.05). Hearing loss was more common in PEX cases (34.0% vs 5.4%; P〈0.001). The mean CCT, ACD and IOP were not significantly different between PEX and non-PEX cases.CONCLUSION: This study is the first population-based, randomized trial in Turkey. Prevalence of PEX in patients over 40 years old was found to be 5.0%. Besides glaucoma and cataract, hypertension, hearing loss, using drugs for cardiac and psychiatric diseases were associated with PEX.展开更多

The prevalence of G6PD deficiency in sickle cell patients is unknown in Benin. Objective: This study aimed to determine the prevalence of G6PD deficiency in sickle cell patients at the CNHU-HKM of Cotonou. Methods: This prospective study was conducted from April to November 2022 at the blood-related diseases teaching clinic and included sickle cell patients in the stationary phase. G6PD determination was performed using the enzymatic method on a Mindray BS 200 machine following the Herz method. Hematological parameters were determined using the XT 4000i analyzer and supplemented by a blood smear stained with May Grunwald Giemsa. Data were analyzed using Epi Info 3.5.4 software. Results: One hundred and sixty-four sickle cell patients (80 SS homozygotes and 84 SC heterozygotes) in the intercritical phase, with a mean age of 26.30 ± 10.76 years, were included. The prevalence of G6PD deficiency was 9.1% (15 cases found in 7 SS patients and 8 SC patients). In G6PD-deficient patients, the mean concentration of the enzyme was lower in Hb SC heterozygotes than in Hb SS homozygotes: 3.56 IU/g Hb versus 4.98 IU/g Hb. The mean reticulocyte count was 231.43 G/L in the deficient group, compared to 216.32 G/L in the non-deficient group. Conclusion: The preliminary results of our study reveal a high prevalence of G6PD deficiency in sickle cell patients. The impact of this association on hematologic and biological parameters should be evaluated for better management of sickle cell disease.展开更多

These data could serve as a basis for developing tailored preventive strategies, depending on the place of residence.
文摘Since 1990,China has made considerable progress in resolving the problem of“treatment difficulty”of cardiovascular diseases(CVD).The prevalent unhealthy lifestyle among Chinese residents has exposed a massive proportion of the population to CVD risk factors, and this situation is further worsened due to the accelerated aging population in China.CVD remains one of the greatest threats to the health of Chinese residents. In terms of the proportions of disease mortality among urban and rural residents in China,CVD has persistently ranked first. In 2021,CVD accounted for 48.98%and 47.35%of deaths in rural and urban areas, respectively. Two out of every five deaths can be attributed to CVD.To implement a national policy“focusing on the primary health institute and emphasizing prevention”and truly achieve a shift of CVD prevention and treatment from hospitals to communities, the National Center for Cardiovascular Diseases has organized experts from relevant fields across China to compile the“Report on Cardiovascular Health and Diseases in China”annually since 2005.The 2024 report is established based on representative, published, and high-quality big-data research results from cross-sectional and cohort population epidemiological surveys, randomized controlled clinical trials, large sample registry studies, and typical community prevention and treatment cases, along with data from some projects undertaken by the National Center for Cardiovascular Diseases. These firsthand data not only enrich the content of the current report but also provide a more timely and comprehensive reflection of the status of CVD prevention and treatment in China.

文摘BACKGROUND The prevalence of nonalcoholic fatty liver disease(NAFLD)in patients with chronic hepatitis B(CHB)has increased in recent clinical practice;however, the relationship between CHB and hepatic steatosis(HS)remains controversial.AIM To shed light on the potential association between NAFLD and hepatitis B virus(HBV)infection.METHODS We conducted a systematic literature search using multiple databases, including PubMed,the Cochrane Library,Web of Science, and EMBASE,to identify relevant studies.Predefined inclusion criteria were used to determine the eligibility of the studies for further analysis.RESULTS Comprehensive meta-analysis software was used for statistical analysis, which covered 20 studies. The results indicated a lower NAFLD susceptibility in HBVinfected individuals(pooled OR=0.87;95%CI=0.69-1.08;I2=91.1%),with diabetes(P=0.015),body mass index(BMI;P=0.010),and possibly age(P=0.061)as heterogeneity sources.Of note,in four studies(6197 HBV patients),HBV-infected individuals had a reduced NAFLD risk(OR=0.68,95%CI=0.51-0.89,P=0.006).A positive link between hyperlipidemia and metabolic syndrome emerged in hepatitis B patients, along with specific biochemical indicators, including BMI,creatinine, uric acid, fasting blood glucose, and homeostasis model assessment of insulin resistance.CONCLUSION HBV infection may provide protection against HS;however, the occurrence of HS in patients with HBV infection is associated with metabolic syndrome and specific biochemical parameters.
文摘Sorghum is a vital commodity and greatly contributes to the daily calorie needs for millions of the inhabitants in Senegal, West Africa. This survey is significant because the information obtained will be beneficial to plant pathologists, sorghum producers, government officials, and funding agencies to prioritize research projects that ensure productivity and food security.
文摘Ticks induce huge production and economic losses in the livestock industry and create serious environmental, animal and human health problems. The study was carried out to characterize tick species and determine the prevalence of tick infestation and tick-borne diseases in cattle in Cameroon. Tick and blood samples were collected from a total of 742 animals and analyzed to determine the type of tick and haemoparasites using standard procedures. Overall, four tick species namely Amblyomma variegatum (75.09%), Rhipicephalus microplus (19.43%), Rhipicephalus decoloratus (0.88%) and Hyalomma marginatum (0.18%) and six blood disease agents including Anapalasma marginale (11.29%), Ehrlichia ruminantium (3.52%), Babesia bovis (1.32%), Babesia major (0.44%), Anaplasma phagocytophilum (0.29%) and Dermatophilus congolensis (3.37%). Various co-infections were recorded and the predominant associations were Amblyomma variegatum-Rhipicephalus microplus (4.06%) and Amblyomma variegatum-Hyalomma marginatum (0.36%);Anaplasma-Ehrlichia (12%), Anaplasma-Dematophylus (8%), Babesia-Anaplasma-Ehrlichia (14%) and Babesia-Anaplasma-Ehrlichia-Dermato-phylus (5%). Breed, sex, age and locality significantly influenced the rate of tick infestation while locality, breed and age significantly influenced the detection of blood disease agents in the study.
文摘[Objective] To investigate the protective efficacy of currently available Newcastle disease vaccines against currently prevalent strains and thus provide guidance for the application of vaccines and the prevention and control of Newcastle disease. E Conclusion] Under laboratory conditions, the currently available Newcastle disease vaccines have better protective efficacy against the two currently prevalent NDV strains and prevent the occurrence of Newcastle disease.
文摘In the past 30 years, the accessibility and quality index of medical care have made remarkable progress in China, ranking the first among middle-income countries. Many cardiovascular technologies are at or near the world's leading level, and significant progress has been achieved in China solving the problem of“treatment difficulty”of cardiovascular diseases(CVD).However, due to the prevalence of unhealthy lifestyles among Chinese residents,a huge population with CVD risk factors, accelerated population aging, and other reasons, the incidence and mortality rate of CVD are still increasing, and the turning point of the decline in disease burden has not appeared yet in China. In terms of proportions of disease mortality among urban and rural residents,CVD still ranks the first. In 2020,CVD accounted for 48.00%and 45.86%of the causes of death in rural and urban areas, respectively;two out of every five deaths were due to CVD.It is estimated that the number of current CVD patients in China is around 330 million, including 13 million stroke,11.39 million coronary heart disease,8.9 million heart failure,5 million pulmonary heart disease,4.87 million atrial fibrillation,2.5 million rheumatic heart disease,2 million congenital heart disease,45.3 million peripheral artery disease, and 245 million hypertension cases.China has entered a new stage of transformation from high-speed development to high-quality development, and the prevention and control of CVD in China should also shift from previous emphasis on scale growth to strategies focusing more on strategic and key technological development in order to curb the trend of increasing incidence and mortality rates of CVD.
文摘Background:Lifestyle plays an important role in preventing and managing gastroesophageal reflux disease(GERD).In response to the conflicting results in previous studies,we performed a systematic review and meta-analysis to investigate this association. Methods:Relevant studies published until January 2023 were retrieved from 6 databases, and the prevalence of symptomatic gastroesophageal reflux(GER)or GERD was determined from the original studies.A random effects model was employed to meta-analyze the association by computing the pooled relative risk(RR)with 95%confidence intervals(95%CIs).Furthermore,subgroup and dose-response analyses were performed to explore subgroup differences and the association between cumulative physical activity(PA)time and GERD.Results:This meta-analysis included 33 studies comprising 242,850 participants.A significant negative association was observed between PA and the prevalence of symptomatic GER(RR=0.74,95%CI:0.66-0.83;p<0.01)or GERD(RR=0.80,95%CI:0.76-0.84;p<0.01),suggesting that engaging in PA might confer a protective benefit against GERD.Subgroup analyses consistently indicated the presence of this association across nearly all subgroups, particularly among the older individuals(RR_(<40 years):RR_(≥40 years)=0.85:0.69,p<0.01)and smokers(RR_(smoker):RR_(non-smoker)=0.67:0.82,p=0.03).Furthermore,a dose-response analysis revealed that individuals who engaged in 150 min of PA per week had a 72.09%lower risk of developing GERD.Conclusion:Maintaining high levels of PA decreased the risk of GERD,particularly among older adults and smokers.Meeting the recommended PA level of 150 min per week may significantly decrease the prevalence of GERD.
文摘Background: Kidney failure, cardiovascular disease, and early mortality are just a few of the major negative effects of chronic renal disease, a serious global health issue. Conclusion: There is poor management of CKD in the South West Region of Cameroon which has contributed greatly to the progression of CKD and increases in the mortality rate.

文摘AIM: To investigate the prevalence of pseudoexfoliation syndrome (PEX) and its associations with ocular and systemic diseases in a population sample aged over 40y. Besides glaucoma and cataract, hypertension, hearing loss, using drugs for cardiac and psychiatric diseases were associated with PEX.
文摘BACKGROUND Population ageing is an important challenge for developed as well as developing countries due to the downward trends in mortality rates. The elderly population is increasing worldwide.Cardiovascular diseases(CVDs)are one of the most common diseases in the geriatric population. These diseases involve the heart or blood vessels and include hypertension, rheumatic heart disease, heart failure, and heart attack. An estimated 17.7 million people in India will die from CVDs representing 31%of all global deaths.AIM To perform a systematic review and meta-analysis of the gender prevalence of CVDs in the geriatric population of India.METHODS In the present study,we searched databases such as Google Scholar,PubMed and MEDLINE from the year 2003 to 2019 to identify the prevalence of CVDs in the Indian geriatric population.A meta-analysis was conducted using the statistical software R version 3.4.3 and the random effect model was used to determine the pooled estimate of the prevalence of CVDs in the geriatric population of India along with the 95%confidence interval rather than using the fixed effect model. The random effect model takes into consideration the heterogeneity across the various studies.RESULTS The prevalence of CVDs in the Indian geriatric population was determined in 6586 male subjects from 32 studies and 8164 female subjects from 32 studies, respectively. The overall prevalence of CVDs in the Indian geriatric population was 36.6%(95%CI:31.9%-41.3%).In addition, calculation of the various heterogeneity statistics(Cochran’s Q=3836.85,I2=98.6%,P<0.0001)indicated heterogeneity in the prevalence of CVDs in the elderly Indian population in these studies. The prevalence of CVDs in elderly males was 38.0%(95%CI:33.0%-43.0%)and the prevalence of CVDs in elderly females was 40.9%(95%CI:35.5%-46.2%).CONCLUSION The results indicate that the prevalence of CVDs in the female geriatric population was relatively higher than that in the male geriatric population.Policy makers must take immediate steps to prevent CVDs and improve geriatric health care services in India.
文摘In order to understand the epidemic characteristics of major viral infectious diseases of pig farms in Guangxi from 2019 to 2022,the PCR and RT-PCR methods were applied to detect the pathogens of 1046 samples suspected CSFV(swine fever virus),PRRSV(porcine reproductive and respiratory syndrome virus),PCV-2(porcine circovirus virus type 2),PCV-3(porcine circovirus virus type 3),PRV(pseudorabies virus),TGEV(transmissible gastroeneritis of pigs virus),PEDV(porcine epidemic diarrhea virus)and PoRV(porcine rotavirus)infections in recent four years, and epidemiological analysis was carried out.The results showed that positive rates of CSFV,PRRSV,PCV-2,PCV-3,PRV,TGEV,PEDV and PoRV were 3.91%(21/537),20.46%(143/699),29.90%(116/388),16.10%(81/503),27.75%(121/436),2.10%(8/381),28.50%(61/214)and 13.74%(18/131).The infection rates of PCV-2 and PEDV were decreasing year by year, while the infection rate of TGEV was increasing year by year.In single infection, the infection rates of PRV and PEDV were higher, with an average infection rate of 13.53%and 18.22%.In the mixed infection, the infection rate of PRRSV and PCV-2 was the highest, with an average infection rate of 11.34%.The second mixed infection rate was PEDV and PoRV,and the average infection rate was 8.05%.The infection rate of PCV-3 and PEDV was higher in spring, with 18.80%and 40.00%respectively.The infection rate of PRRSV,PCV-2,PRV,TGEV and PoRV was higher in winter, with 24.40%,40.57%,37.30%,3.23%and 19.44%respectively.The results showed that PRRSV,PCV-2,PRV and PEDV were the main viral diseases with serious damage to pig farms in Guangxi. It was necessary to strengthen pathogen monitoring and improve comprehensive prevention and control ability,in order to reduce the incidence of disease and economic losses.
文摘BACKGROUND Gallbladder and biliary diseases(GABDs)are a major public health issue.AIM To analysis the cause-specific incidence, prevalence, and years lived with disability(YLDs)and its temporal trends of GABDs at the global, regional, and national level.Data on GABD were available from the Global Burden of Disease study 2019.METHODS The estimated annual percentage change(EAPC)was used to quantify temporal trend in GABD age-standardized incidence rates(ASIRs),age-standardized prevalence rate(ASPR),and age-standardized YLD rate(ASYR)by region, sex. We analyzed the relationship between the GABD burden and country development level using the human development index(HDI).RESULTS In 2019,the incident cases of GABD were 52003772,with an ASIR of 63432/100000 population.Globally, the number of incident cases and ASIR of GABD increased 97%and 58.9%between 1990 and 2019.Although,the ASPR and ASYR decreased from 1990 to 2019,the number of prevalent and YLDs cases increased. The highest ASIR was observed in Italy, and the highest ASPR and ASYR was observed in United Kingdom. The highest burden of GABD was found in low-SDI region, and the burden in female was significantly higher than males.A generally negative correlation(ρ=-0.24,P<0.05)of GABD with the EAPC and human development index(HDI)(in 2021)were observed for ASIR.What’s more,no correlation in ASPR(ρ=-0.06,P=0.39)and ASYR(ρ=-0.07,P=0.36)of GABD with the EAPC and HDI(in 2021)were observed, respectively.CONCLUSION GABD remain a major global public health challenge;however, the burden of GABD varies geographically.Globally, the number of incident cases and ASIR of GABD increased between 1990 and 2019.The results of our study provide insight into the global disease burden of GABD and may assist policymakers in formulating effective policies to mitigate modifiable risk factors.
文摘Background: Glucose-6-phosphate dehydrogenase (G6PD) deficiency and sickle cell disease are common genetic defects of red blood cells that lead to hemolytic anemia. The impact of this association on hematologic and biological parameters should be evaluated for better management of sickle cell disease.

文摘BACKGROUND Inflammatory bowel disease(IBD)was previously regarded as a Western disease;however, its incidence is increasing in the East. The epidemiology of IBD in Asia differs significantly from the patterns in the West.AIM To comprehensively investigate the epidemiology of IBD in South Korea,inclu-ding its incidence, prevalence, medication trends, and outcomes.METHODS We analyzed claims data from the Health Insurance Review and Assessment Service and Rare and Intractable Diseases(RIDs),operated by the National Health Insurance Service of South Korea.Patients with IBD were identified based on the International Classification of Diseases,Tenth Revision, and RID diagnostic codes for Crohn’s disease(CD)and ulcerative colitis(UC)from 2010 to 2018.RESULTS In total,14498 and 31409 patients were newly diagnosed with CD and UC,respectively, between 2010 and 2018.The annual average incidence of CD was 3.11 cases per 105 person-years,and that of UC was 6.74 cases per 10^(5) person-years.Since 2014,the incidence rate of CD has been stable, while that of UC has steadily increased, shifting the peak age group from 50-year-olds in 2010 to 20-year-olds in 2018.The CD and UC prevalence increased consistently over the study period;the use of 5-aminosali-cylates and corticosteroids gradually decreased, while that of immunomodulators and biologics steadily increased in both CD and UC.The clinical outcomes of IBD,such as hospitalization and surgery, decreased during the study period.CONCLUSION The CD incidence has been stable since 2014,but that of UC has increased with a shift to a younger age at peak incidence between 2010 and 2018.IBD clinical outcomes improved over time, with increased use of immunomodu-lators and biologics.
文摘Heartburn is a common symptom shared by both gastroesophageal reflux disease(GERD)and functional heartburn(FHB),which can make it challenging to differentiate between the two conditions. However, examining oral manifestations of GERD can be a cost-effective and readily available method to aid in this differentiation process. It may serve as a valuable tool in distinguishing GERD from FHB.
文摘BACKGROUND Data regarding the worldwide gastrointestinal surgery rates in patients with Crohn’s disease(CD)remains limited.AIM To systematically review the global variation in the rates of surgery in CD.METHODS A comprehensive search analysis was performed using multiple electronic databases from inception through July 1,2020,to identify all full text, randomized controlled trials and cohort studies pertaining to gastrointestinal surgery rates in adult patients with CD.Outcomes included continent based demographic data,CD surgery rates over time,as well as the geoepidemiologic variation in CD surgery rates.Statistical analyses were conducted using R.RESULTS Twenty-three studies spanning four continents were included. The median proportion of persons with CD who underwent gastrointestinal surgery in studies from North America,Europe,Asia, and Oceania were 30%(range:1.7%-62.0%),40%(range:0.6%-74.0%),17%(range:16.0%-43.0%),and 38%respectively.No clear association was found regarding the proportion of patients undergoing gastrointestinal surgery over time in North America(R^(2)=0.035)and Europe(R^(2)=0.100).A moderate, negative association was seen regarding the proportion of patients undergoing gastrointestinal surgery over time(R^(2)=0.520)in Asia.CONCLUSION There appears to be significant inter-continental variation regarding surgery rates in CD.Homogenous evidencebased guidelines accounting for the geographic differences in managing patients with CD is prudent. Moreover,as a paucity of data on surgery rates in CD exists outside the North American and European continents, future studies, particularly in less studied locales, are warranted.
文摘BACKGROUND There are limited studies investigating the association between type 2 diabetes mellitus(T2DM)and non-alcoholic fatty liver disease(NAFLD)in the region of Bihar,India.AIM To estimate the prevalence of NAFLD in persons with newly diagnosed T2DM in the population of North Bihar,India.METHODS This single centre cross-sectional study was undertaken in the Research Centre for Diabetes Hypertension and Obesity,Samastipur,Bihar,India.Data were collected from persons newly diagnosed with T2DM or those diagnosed within 6 months of when the study was conducted between December 2022 to May 2023.RESULTS A total of 148 people with newly diagnosed T2DM were included(median age 47 years,46.6%female)and 109 patients with liver disease on ultrasound evaluation. The persons with liver disease consumed more fats and oils(88.1%vs 74.4%,P=0.042)and they had significantly greater body mass index(27.4 vs 23.0,P<0.001),waist circumference(37 vs 33,P<0.001),and waist-to-hip ratio(1.00 vs 0.70,P=0.025).Females were associated with greater liver disease[odds ratio(OR):3.09,95%confidence interval(CI):1.09-8.80,P=0.32].Waist circumference(OR:1.42,95%CI:1.22-1.66,P<0.001)and low-density lipoprotein cholesterol(OR:1.01,95%CI:1.01-1.02,P=0.048)were associated with any liver disease. The factors most associated with grade 2/3 liver disease was right upper quadrant pain or heaviness(OR:5.22,95%CI:1.40-19.41,P=0.14),greater income(OR:3.58,95%CI:1.28-10.04,P=0.015)and waist circumference(OR:1.31,95%CI:1.02-1.69,P=0.036).CONCLUSION NAFLD is common in new/recently diagnosed T2DM and disease burden is high and common among patients who are either high consumers of fats and oils or have obesity-associated markers.
